A Computed Tomography Analysis of the Success of Spinal Fusion Using Ultra-Low Dose (0.7 mg per Facet) of Recombinant Human Bone Morphogenetic Protein 2 in Multilevel Adult Degenerative Spinal Deformity Surgery

  • Study Design: Retrospective cohort study. Purpose: To report on spinal fusion assessment using computed tomography (CT) after adult spinal deformity (ASD) surgery using ultra-low dose recombinant human bone morphogenetic protein 2 (RhBMP-2). Overview of Literature: The reported dose of RhBMP-2 needed for successful spinal posterolateral fusion in ASD ranges from 10 to 20 mg per spinal level. This study reports the use of ultra-low dose of RhBMP-2 (0.07 mg per facet) to achieve spinal fusion in multilevel ASD surgery. Methods: Consecutive patients who underwent ASD surgery using ultra-low dose RhBMP-2 were recruited. Routine postoperative CT analysis for spinal fusion was performed by two spine surgeons. Inter-observer agreement was calculated for facet fusion (FF) and interbody fusion (IBF) at 6 and 12 months after the procedure. Results: Six consecutive ASD patients with a mean age of 62 years (28-72 years) were examined. Each patient received a total dose of 12 mg with an average dose of $0.69{\pm}0.2mg$ (0.42-1 mg) per single FF and $1.38{\pm}0.44mg$ (0.85-2 mg) for IBF. Total 131 FF and 15 IBF were examined in the study, with 88 FFs and nine IBFs being analyzed specifically at 6 months after the surgery. FF and IBF reported by surgeons A and B at 6 months were 97.7% vs. 91.9% FF, respectively (${\kappa}=0.95$) and 100% vs. 100% IBF, respectively (${\kappa}=1$). Two patients underwent longitudinal follow-up CT at 12 months, and the FF rates reported by surgeons A and B were 100% vs. 95.8%, respectively (${\kappa}=0.96$). Five out of nine facet (56%) non-unions were identified at the cross-links. The remaining four facet pseudarthrosis were noted at 1-2 spinal levels caudal to the cross-links. At the final clinical follow-up, there was no rod breakage, deformity progression, neurological deficit, or symptom recurrence. The Oswestry Disability Index improved by an average of $32.8{\pm}6.3$, while the mental component summary of the 36-item Short-Form Health Survey improved by an average of $4.7{\pm}2.1$, and physical component summary improved by an average of $10.5{\pm}2.1$. Conclusions: To our knowledge, this is the first study to report a CT that defined 92%-98% FF and 100% IBF using the lowest reported dose of RhBMP-2 in multilevel ASD surgery. The use of ultra-low dose RhBMP-2 reduces the RhBMP-2 related complications and healthcare costs.

Analysis of Surgical Treatment and Factor Related to Closed Reduction Failure for Patients with Traumatically Locked Facets of the Subaxial Cervical Spine (축추 이하 경추손상 환자에서 외상성 탈구에 의한 도수 정복의 실패 요인의 분석과 수술적 치료에 대한 분석)

  • Purpose: Cervical dislocations with locked facets account for more than 50% of all cervical injuries. Thus, investigating a suitable management of cervical locked facets is important. This study examined factors of close reduction failure in traumatically locked facets of the subaxial cervical spine patients to determine suitable surgical management. Methods: We retrospectively analyzed of the case histories of 28 patients with unilateral/bilateral cervical locked facets from Nov. 2004 to Dec. 2010. Based on MRI evaluation of disc status at the injury level, we found unilateral dislocations in 9 cases, and bilateral dislocations in 19 cases, The patients were investigated for neurologic recovery, closed reduction rate, factors of the close reduction barrier, fusion rate and period, spinal alignment, and complications. Results: The closed reduction failed in 23(82%) patients. Disc herniation was an obstacle to closed reduction (p=0.015) and was more frequent in cases involving a unilateral dislocation (p=0.041). The pedicle or facet fracture was another factor, although some patients showed aggravation of neurologic symptoms, most patients had improved by the last follow up. The kyphotic angle were statistically significant (p=0.043). Sixs patient underwent anterior decompression/fusion, and 15 patients underwent circumferential fusion, and 7 patients underwent posterior fusion. All patients were fused at 3 months after surgery. The complications were 1 case of CSF leakage and 1 case of esphageal fistula, 1 case of infection. Conclusion: We recommend closed reduction be performed as soon as possible after injury to maximize the potential for neurological recovery. Patients fot whom closed reduction of the cervical locked facets have a higher incidence of anatomic obstacles to reduction, including facet fractures and disc herniation. Immediate direct open anterior reduction or circumferential fixation/fusion of locked cervical facets is recommended as a treatment of choice for traumatic locked cervical facet patients after closed reduction failure.

Relationship between paravertebral muscle twitching and long-term effects of radiofrequency medial branch neurotomy

  • Background: To achieve a prolonged therapeutic effect in patients with lumbar facet joint syndrome, radiofrequency medial branch neurotomy (RF-MB) is commonly performed. The purpose of this study was to evaluate the prognostic value of paravertebral muscle twitching when performing RF-MB in patients with lumbar facet joint syndrome. Methods: We collected and analyzed data from 68 patients with confirmed facet joint syndrome. Sensory stimulation was performed at 50 Hz with a 0.5 V cut-off value. Patients were divided into 3 groups according to the twitching of the paravertebral muscle during 2 Hz motor stimulation: 'Complete', when twitching was observed at all needles; 'Partial', when twitching was present at 1 or 2 needles; and 'None', when no twitching was observed. The relationship between the long-term effects of RF-MB and paravertebral muscle twitching was analyzed. Results: The mean effect duration of RF-MB was 4.6, 5.8, and 7.0 months in the None, Partial, and Complete groups, respectively (P = 0.47). Although the mean effect duration of RF-MB did not increase significantly in proportion to the paravertebral muscle twitching, the Complete group had prolonged effect duration (> 6 months) than the None group in subgroup analysis. (P = 0.03). Conclusions: Paravertebral muscle twitching while performing lumbar RF-MB may be a reliable predictor of long-term efficacy when sensory provocation under 0.5 V is achieved. However, further investigation may be necessary for clarifying its clinical significance.

Developing Facets for Fiction Retrieval Based on User-generated Book Tags (이용자 생성 도서정보 태그에 기반한 소설 검색의 패싯 유형 개발)

  • The purpose of this study is to identify and systematize various facet elements required by users in fiction search situations from book tags to improve the fiction search environment. Based on the Ranganathan's PMEST formula, the basic facet system of the fiction was defined as 1) the personality that forms the fiction material, 2) the content and external characteristics that compose the fiction, 3) the reader interaction with books, 4) spatial information related to fiction and reading activities, and 5) time information related to fiction and reading activities. Out of approximately 310,000 tags assigned to 7,174 fiction, 3,730 core tags were selected and content-analyzed. As a result, various attributes were systematized around the top 25 categories of the fiction facets. The results of this study can be applied to facet navigation of OPAC and fiction DB in the future.

Descriptive Study for Sonographic Morphology of the 1st Facet of Subscapularis Footprint (견갑하건 부착부의 제1부착면에 대한 초음파 소견의 기술적 연구)

  • Purpose: The purpose of this study was to document the sonographic morphology of the subscapularis footprint, particularly the 1st facet, of the non-pathologic subscapularis tendon and footprint, and analyze the correlation between the size of the 1st facet and the demographic variables. Materials and Methods: Between March 2015 and December 2017, retrospectively data analysis was performed for the ultrasound (US) scans of 115 consecutive shoulder (mean age 53.4 years, range 23-74 years) with non-pathologic subscapularis tendon and footprint. The sonographic findings of the 1st facet of the subscapularis footprint was a very unique, flat, broad, and plane angle in the upward direction, which were distinguished from the other facets. On US, the transverse (medio-lateral) and longitudinal (superior-inferior) length of the 1st facet on axis of the humerus shaft were recorded. The demographic variables, including age, site, body height, weight, body mass index (BMI), and arm length, were reviewed. Results: On US, the mean transverse length of the 1st facet was 12.75 mm (range 10.54-14.50 mm, standard deviation [SD] 0.712) and the mean longitudinal length was 12.22 mm (range 9.20-13.30 mm, SD 0.888). The transverse and longitudinal length of the size of the 1st facet were significantly greater in males than in females (p<0.001, p=0.001). Of the demographic data (body height, weight, BMI, arm length) that showed a significant positive linear correlation, the correlation with body height (transverse r=0.749, p<0.001; longitudinal r=0.642, p<0.001) showed the strongest relationship, and the correlation with the BMI was weakly related. The relationships between the size of the 1st facet to site/age were not statistically significant or appeared to have no linear correlation. Conclusion: The structural and morphologic features of the 1st facet of the subscapularis footprint on the US were identified. This will provide anatomic knowledge of an US examination for subscapularis tendon pathology.

The Validation of Korean Version of Five Facet Mindfulness Questionnaire Short Form (한국판 5요인 마음챙김 단축형 척도 타당화 연구)

  • Objectives: To simplify the mindfulness scale and to ensure reliability and validity of the scale. Methods: To develop scale items using the original scale and to identify factor structure of the scale, exploratory factor analysis was conducted based on results of factor analysis, the short-form 5-factor mindfulness scale item was selected. Internal consistency coefficient was calculated and confirmatory factor analysis was conducted to verify reliability of the short-form scale. Finally, to verify validity of the abbreviated scale, we conducted correlation analysis with the abbreviated scale and the mindfulness scale (FFMQ). Results: As with the original scales, scaled scales were composed of 5 factors and consisted of 15 items in total. Factor analysis revealed factor loadings were adequate and reliability and validity were secured. Conclusions: It provided that mindfulness shortening scale validation can be used more simply to measure mindfulness in the clinical scene.

  • Due to big demand for satellite and communication service using personal handheld terminals, demand for satellites with huge antennas is increasing correspondingly. While such large antennas are realized by various types of deployable antennas, the reflecting surface is made by many facets irrespective of deploying mechanisms. In order to analyze the faceted-reflector more accurately, an existing ray-tracing method is improved. The algorithm allows the rays to cross each other, which is the main characteristic of the faceted-reflector, and takes unevenness of amplitude and phase over the aperture plane into consideration. For the study of the effect of facet configuration, facet generating algorithm is devised. From the analysis algorithm and the facet-generating algorithm, it has been found that the number of facets in a radial direction affects both directivity and sidelobe level. On the other hand, the number of facets in a circumferential direction affects sidelobe level only.

Biomechanical Effect of Total Disc Replacement on Lumbar Spinal Segment : A Finite Element Analysis (추간판 치환술이 요추분절에 미치는 생체역학적 영향 : 유한요소해석)

  • The artificial discs have recently used to preserve the motion of the treated segment in lumbar spine surgery. However, there have been lack of biomechanical information of the artificial discs to explain current clinical controversies such as long-term results of implant wear and excessive facet contact forces. In this study, we investigated the biomechanical effects of three artificial implants on the lumbar spinal segments by finite element analysis. The finite element model of intact lumbar spine(L1-S) was developed and the three implants were inserted in L4-L5 segment of the spine model. 5 Nm of flexion and extension moments were applied on the superior plate of L1 with 400 N of compressive load. Excessive motions and high facet contact forces at the surgical level were generated in the all three implanted models. In the flexion, the peak von-Mises stresses in the semi-constrained type implant was higher than those in the un-constrained type implant which would cause wear on the polyethylene core. The results of the study would provide a biomechanical guideline for selecting optimal surgical approach or evaluating the current design of the implants, or developing a new implant.

Single mode yield analysis of index-coupled DFB lasers above threshold for various facet reflectivity combinations (Index-coupled DFB 레이저의 여러 가지 양 단면 반사율 조합에 따른 문턱 전류 이상에서 단일 모드 수율 해석)

  • We have calculated the single mode yield of index-coupled (IC) DFB lasers above threshold for several kL, and facet reflectivity combinations, and investigated the correlation between those results and the single mode yield as a function of f number at the threshold. As a result, there is little correlation between the single mode yield above threshold and the single mode yield as a function of f number at the threshold. The single mode yields above threshold for kL of 0.8 and 1.25 is larger than those for kL, of 2 and 3 due to the spatial hole burning effect. Also, we have investigated the effect of the reflectivity of the AR facet on the single mode yield for AR-HR and AR-CL combinations. For AR-HR combinations, the single mode yield increases as the reflectivity of the AR facet decreases. However, for AR-CL combinations, the reflectivity of the AR facet for the largest single mode yield exists. In the single mode yield calculations for IC DFB lasers in this paper, the single mode yield for kL of 0.8 with AR(1%)-HR combination is largest above threshold.
