Effectiveness of virtual reality immersion on procedure-related pain and anxiety in outpatient pain clinic: an exploratory randomized controlled trial

  • Background: The study investigated virtual reality (VR) immersion in alleviating procedure-related pain in patients with chronic pain undergoing fluoroscopy-guided minimally-invasive intervention in a prone position at an outpatient clinic. Methods: In this prospective randomized controlled study, 38 patients undergoing lumbar sympathetic ganglion block were randomized into either the VR or the control group. In the VR group, procedure-related pain was controlled via infiltration of local anesthetics while watching a 30-minute VR hypnotic program. In the control group, the skin infiltration alone was used, with the VR device switched off. The primary endpoint was an 11-point score on the numerical rating scale, indicating procedure-related pain. Patients' satisfaction with pain control, anxiety levels, the need for additional local anesthetics during the procedure, hemodynamic stability, and any adverse events were assessed. Results: Procedure-related pain was significantly lower in the VR group (3.7 ± 1.4) than in the control group (5.5 ± 1.7; P = 0.002). Post-procedural anxiety was lower in the VR group than in the control group (P = 0.025), with a significant reduction from pre-procedural anxiety (P < 0.001). Although patients' satisfaction did not differ significantly (P = 0.158) between the groups, a higher number of patients required additional local anesthetics in the control group (n = 13) than in the VR group (n = 4; P = 0.001). No severe adverse events occurred in either group during the study. Conclusions: VR immersion can be safely used as a novel adjunct to reduce procedural pain and anxiety during fluoroscopic pain intervention.

Improving Children's Emotional Health through Installing Biowalls in Classrooms

  • Background and objective: The physical environment where children spend most of their time is closely associated with their emotional development. To improve the emotional health of children, the introduction of natural elements in the indoor space has been suggested, the benefits of which have been shown in preliminary studies. This study aims to examine the effects that a biowall - a wall installation - in a classroom has on the emotional health of children in kindergarten. Methods: A total of four biowalls were separately installed in four kindergarten classrooms at a school in Seoul, South Korea, and the 60 children in these classrooms participated in the study. We assessed the children's emotional intelligence (via an Emotional Intelligence Rating Scale), resilience (via the Devereux Early Childhood Assessment), and eco-friendly attitudes (via the Children's Attitudes Toward Scale) before the installation of the biowalls and then again 3 months later. Results: The children's emotional intelligence, resilience, and eco-friendly attitudes had been significantly improved after the installation of the biowalls (p = .01). The sub-categories of the children's emotional intelligence and resilience were also significantly improved (p < .001). Conclusion: This study demonstrates the potential of biowalls-as an indoor environmental factor-in promoting the healthy emotional development of children. By bringing natural elements into indoor classroom settings, biowalls appear to increase children's direct/indirect contact with nature. To extrapolate the results of the study to the general population, future studies should be conducted with broader age groups.

Development of Embedded Board for Integrated Radiation Exposure Protection Fireman's Life-saving Alarm (일체형 방사선 피폭 방호 소방관 인명구조 경보기의 임베디드 보드 개발)

  • In this paper, we propose the development of embedded board for integrated radiation exposure protection fireman's life-saving alarm capable of location tracking and radiation measurement. The proposed techniques consist of signal processing unit, communication unit, power unit, main control unit. Signal processing units apply shielding design, noise reduction technology and electromagnetic wave subtraction technology. The communication unit is designed to communicate using the wifi method. In the main control unit, power consumption is reduced to a minimum, and a high performance system is formed through small, high density and low heat generation. The proposed techniques are equipment operated by exposure to poor conditions, such as disaster and fire sites, so they are designed and manufactured for external appearance considering waterproof and thermal endurance. The proposed techniques were tested by an authorized testing agency to determine the effectiveness of embedded board. The waterproof grade has achieved the IP67 rating, which can maintain stable performance even when flooded with water at the disaster site due to the nature of the fireman's equipment. The operating temperature was measured in the range of -10℃ to 50℃ to cope with a wide range of environmental changes at the disaster site. The battery life was measured to be available 144 hours after a single charge to cope with emergency disasters such as a collapse accident. The maximum communication distance, including the PCB, was measured to operate at 54.2 meters, a range wider than the existing 50 meters, at a straight line with the command-and-control vehicle in the event of a disaster. Therefore, the effectiveness of embedded board for embedded board for integrated radiation exposure protection fireman's life-saving alarm has been demonstrated.

Optimization of Wool Dyeing with Yellow Dye from Carthamus Tinctorius L. (홍화 황색소를 이용한 모염색의 최적화)

  • This study investigated the adsorption of safflower yellow dye on wool protein fiber and the optimum dyeing conditions to test color reproducibility. In addition, the effects of mordants on dye adsorption, color, fastness, and photofading rate were also studied. The prepared dye in powder form was characterized with UV-vis spectroscopy and FT-IR spectrometric analysis. The color of dyed fabrics was characterized by CIE $L^*a^*b^*$ coordinates, H V/C, and K/S values. The color reproducibility of the dyed wool fabrics was examined. The amount of dye adsorption increased and also, the shade of the dyed wool fabrics became deeper and more saturated with increasing temperature, time, and dye concentration. The maximum color strength was obtained at pH 3.0. The shade of dyed wool fabrics ranged from light yellow to dark mustard yellow as the pH of the dye bath shifted from alkaline to acidic. Color reproducibility was reliable with color differences in the range of 0.53~1.75. Fastness to dry cleaning was relatively good at 4/5 rating irrespective of mordanting. Fe and Cu mordants showed the least color change of the dyed wool fabrics after exposure to light. Mordants did not contribute to improve dye uptake and color fastness, although they made variations in color tone. Safflower yellow dye can be used satisfactorily without mordants and will not cause damage to the environment.

Damage Characteristics of Korean Traditional Textiles by Acetaldehyde (아세트알데하이드에 의한 전통직물의 손상 특성)

  • Textiles damage caused by acetaldehyde($CH_3CHO$) is not clear as compared to other materials. Total 20 specimens were prepared using 4 different materials (silk, cotton, ramie, hemp) after dyed with 4 colors (undyed, red, yellow, blue, black). The specimens were exposed to $CH_3CHO$ gas in the test chamber. First, textile specimens' damage by differert concentration of acetaldehyde(0.1, 0.5, 1, 10, 100, 500, 1000 ppm) was tested. Second, accelerared damage to the textile specimens were tested according to the temperature and humidity conditions at the damage levels. Third, damage of deliberately degraded textile specimens were examined at the damage levels. After the exposure, optical, chemical, and physical evaluation was carried out. As a result, at 1000 ppm/day, the color difference of cotton_yellow has increased. At the condition of $25^{\circ}C-80%$, $30^{\circ}C-50%$, $30^{\circ}C-80%$, the color difference of yellow specimens has increased and grey scale rating has decreased. At $30^{\circ}C-80%$, acetate of cotton_undyed increased and the pH of silk_undyed decreased. In the case of deliberately degraded textile specimens, actetate concentration of black specimens increased. In conclusion, damage to the traditional fabric by acetaldehyde is not impact. However, it is expected that yellow specimens will be bleach and black specimens' actetate concentration will be increase.

The Verification of Korean Version Swallowing Disturbance Questionnaire (K-SDQ) (한국판 삼킴 곤란 척도(K-SDQ)의 번안본 검증)

  • Swallowing disorders that can affect nutrient intakes and quality of life are commonly shown among the elderly as well as patients with neurogenic disorder. This study verifies the reliability and validity of the Swallowing Disturbance Questionnaire (SDQ), a subjective swallowing disability assessment tool, modified for Koreans' eating habit and cultural sentiment, against 105 stroke patients, in order to help identify early swallowing problems of the elderly. Reliability of internal consistency in the Korean version of SDQ is .601, test-retest reliability is .97, and concurrent validity is .956. Based on 8 points of cut-off score, 46.8% of sensitivity and 81.6% of specificity. Comparing the results of video fluoroscopic study (VFSS), an objective swallowing disorder test with those of Korean version of SDQ, negative predictive value (NPV) and positive predictive value (PPV) was shown as 81% and 53%. The Korean version of SDQ is expected to be a useful testing tool to discriminate swallowing disorders in stroke patients. It has great clinical significance in that swallowing difficulties shown by subjects can be sorted out to request a diagnostic assessment before clinical evaluation by a rehabilitation therapist or ruling out unnecessary exposure to additional tests by accurately identifying stroke patients without swallowing problems.

Effects of Light Therapy on Stress, Depression, Quality of Sleep and Quality of Life in Middle-Aged Women: A Randomized Controlled Trial (빛 요법이 중년여성의 스트레스, 우울, 수면의 질 및 삶의 질에 미치는 효과: 무작위 대조군 연구)

  • This study is a randomized controlled trial to compare the effects of light therapy on stress, depression, sleep quality and quality of life in middle-aged women. The participants of this study were 56 middle-aged women who worked at the same office. Data collection period was from August 26 to September 13 for the control group and from September 16 to October 4 for the experimental group. Subjective stress, depression, quality of sleep and quality of life in middle-aged women were measured by Numeric Rating Scale (NRS), Center for Epidemiology studies of scale (CES-D), Verran & Synder-Halpern Sleep scale (VHS) and WHO Quality of Life Scale Abbreviated Version (WHOQOL-BREF). Stress index was measured using a Canopy9 RSA (IEMBIO, Gangwondo, Korea) equipment. The data were analyzed by mean, standard deviation, real number and percentage, χ2-test, t-test, repeated measures ANOVA and ANCOVA using IBM SPSS Statistics version 25.0. The results showed that the experimental group applying light therapy had significantly lower subjective stress (F=6.30, p<.001) and higher quality of life (F=2.80, p=.049) than the control group, There was no difference in objective stress index, depression, and sleep quality. As a result, light therapy is partially effective in subjective stress and quality of life for middle-aged women, and further research is needed on middle-aged women working in work environments with limited light exposure.

  • We serveyed the actual condition of business of the departments of radiology of 45 health conters (except 3) in the area of Seoul, Kyungki and Inchon from March, 1984 to November, 1984. The results are summarized as follows : 1. T.O. of the radiologic technologist is three persons in each health center of Seoul area, and one person in each one of Kyungki and Inchon area. P.O. is 2-5 persons in Seoul area, 1-2 persons in Kyungki or Inchon area. 2. The number of all the radiologic technologists employed now is 75 persons, and among all of them, when analized by position class 7th is 54.7%, class 8th 28.0%, class 9th is 13.3%, and class 6th is 2.7%, and by sex, female is 68.0%, male is 32.0%, by educational background, for the most part, junior college graduates come to 73.3%, by age group 60% of them is in their twenties, 16.0% is in their thirties and forties, 8.0% is in their fifties, and by career after certificate 60% have the career of 1-5 years, 13.3% have the one of 6-7 years or mor than 21 years, and 6.7% have the one of 11-15 years of 16-20 years. 3. All the diagnostic x-ray equipment being kept is 62, and among them flxing equipment is 71.0%, portable equipment is 29.0% and by rating of X-ray equipment, maximum tube current 100 mA is 46.8%, maximum KV 100KVP is 72.6%, the most part. 4. Photofluorographic camera and hood are equipped in every health center. While, as to the radiographic cassettes, $14{\times}14"$ cassetts are equipped in every health center, but cassettes of other sizes are in half of them. 5. Bucky's table is equipped in 11.9% health centers, the automatic processor is in 21.4%, the photofluorographic film changer is 9.5%, the grid is 73.8%, the protective apron is in 88.1%, and the protective glove is in 57.1% health centers. 6. The number of the people who got the x-ray examination for one year (by the year 1989) is the most, 1,000-6,000 in direct radiography of the chest, or 15,0001-45,000 in the health centers of Seoul area, 5,000-20,000 in Kyungki and Inchon area in photofluorography of the chest. Moreover, other radiographies are being taken extremely limitedly in all health centers. 7. In processing types of x-ray film, automatic processing is used in 9 health centers (21.4%), manual tank processing is in 30 (71.4%), and manual tray processing in 3 (7.2%). 8. As for collimation of x-ray exposure field, "continual using restricted by a subject size" has the most part, 78.6% "restricted using at every radiography" has 19%, and the case of "never considered" has 2.4% response. 9. As for the dosimeter used for radiation control, film badge (35.7%) and pocket dosimeter (26.2%) are used, and in 38.1% health centers the dosimeter is not equipped at all. Consideration of the previous radiation exposure is being done in only one health center. 10. Reading of radiographs is mainly depended on the radiologists electively (45.2%) or on the genral practitioners(45.2%).

Self-optimizing feature selection algorithm for enhancing campaign effectiveness (캠페인 효과 제고를 위한 자기 최적화 변수 선택 알고리즘)

  • For a long time, many studies have been conducted on predicting the success of campaigns for customers in academia, and prediction models applying various techniques are still being studied. Recently, as campaign channels have been expanded in various ways due to the rapid revitalization of online, various types of campaigns are being carried out by companies at a level that cannot be compared to the past. However, customers tend to perceive it as spam as the fatigue of campaigns due to duplicate exposure increases. Also, from a corporate standpoint, there is a problem that the effectiveness of the campaign itself is decreasing, such as increasing the cost of investing in the campaign, which leads to the low actual campaign success rate. Accordingly, various studies are ongoing to improve the effectiveness of the campaign in practice. This campaign system has the ultimate purpose to increase the success rate of various campaigns by collecting and analyzing various data related to customers and using them for campaigns. In particular, recent attempts to make various predictions related to the response of campaigns using machine learning have been made. It is very important to select appropriate features due to the various features of campaign data. If all of the input data are used in the process of classifying a large amount of data, it takes a lot of learning time as the classification class expands, so the minimum input data set must be extracted and used from the entire data. In addition, when a trained model is generated by using too many features, prediction accuracy may be degraded due to overfitting or correlation between features. Therefore, in order to improve accuracy, a feature selection technique that removes features close to noise should be applied, and feature selection is a necessary process in order to analyze a high-dimensional data set. Among the greedy algorithms, SFS (Sequential Forward Selection), SBS (Sequential Backward Selection), SFFS (Sequential Floating Forward Selection), etc. are widely used as traditional feature selection techniques. It is also true that if there are many risks and many features, there is a limitation in that the performance for classification prediction is poor and it takes a lot of learning time. Therefore, in this study, we propose an improved feature selection algorithm to enhance the effectiveness of the existing campaign. The purpose of this study is to improve the existing SFFS sequential method in the process of searching for feature subsets that are the basis for improving machine learning model performance using statistical characteristics of the data to be processed in the campaign system. Through this, features that have a lot of influence on performance are first derived, features that have a negative effect are removed, and then the sequential method is applied to increase the efficiency for search performance and to apply an improved algorithm to enable generalized prediction. Through this, it was confirmed that the proposed model showed better search and prediction performance than the traditional greed algorithm. Compared with the original data set, greed algorithm, genetic algorithm (GA), and recursive feature elimination (RFE), the campaign success prediction was higher. In addition, when performing campaign success prediction, the improved feature selection algorithm was found to be helpful in analyzing and interpreting the prediction results by providing the importance of the derived features. This is important features such as age, customer rating, and sales, which were previously known statistically. Unlike the previous campaign planners, features such as the combined product name, average 3-month data consumption rate, and the last 3-month wireless data usage were unexpectedly selected as important features for the campaign response, which they rarely used to select campaign targets. It was confirmed that base attributes can also be very important features depending on the type of campaign. Through this, it is possible to analyze and understand the important characteristics of each campaign type.
