만성폐쇄성폐질환에서 제지방량이 최대운동능력에 미치는 영향 (The Influence of Fat-Free Mass to Maximum Exercise Performance in Patients with Chronic Obstructive Pulmonary Disease)

  • 연구배경: 만성폐쇄성질환은 호흡곤란과 신체활동능력저하가 특징이며, 이들 환자의 체중감소는 예후를 결정하는 중요한 인자로 알려져 있다. 이러한 사실은 체성분의 변화가 환자의 운동능력에 영향을 미칠 수 있는 요인으로 작용할수 있음을 의미한다. 저자들은 최대산소섭취량에 영향을 미치는 요소들을 분석하여, 체성분의 변화가 환자의 최대운동능력에 미치는 영향을 알아보았다. 대상 및 방법: 만성폐쇄성질환으로 입원한 83명(남자 63명, 여자 20명)을 대상으로 급성기 증상이 호전된 후 퇴원하기 직전 안정시 폐기능 및 체성분 분석, 그리고 운동부하심폐기능을 측정하여 최대산소섭취량에 영향을 미치는 요소를 분석하였다. 결 과: 만성폐쇄성질환자의 최대산소섭취량은 영양상태와 관계없이 감소하였으나, 영양결핍환자들의 최대산소섭취량 및 최대운동부하량, 그리고 최대산소맥압은 영양상태가 정상인 환자보다 훨씬 낮았다. 최대산소섭취량은 최대흡기압 (r=0.71), 최대호기압(r=0.68), 노력성 흡기폐활량(r=0.55), 그리고 폐확산능(r=0.53)과 상관관계가 있고(p<0.01), 체성분 가운데 제지방량(r=0.55), 체질량지수(r=0.47), 제지방지수(r=0.41)와 상관관계가 있었다(p<0.01). 최대산소섭취량과 상관관계가 있는 지표들을 단계적 회귀분석으로 확인한 결과 제지방량 (t=2.31, p<0.05), 노력성 흡입폐활량(t=3.22, p<0.01), 폐확산능(t=3.41, p<0.01)이 유의수준 5% 이내에서 통계적인 의미가 있었다. 이들 세 가지 지표만으로 회귀모델을 만들 경우 최대산소섭취량의 68.8%를 설명할 수 있었다($r^2=0.688$, p<0.05). 결 론: 만성폐쇄성폐질환자의 제지방량은 안정시 폐기능과 더불어 최대운동능력에 영향을 미치는 중요한 인자이며, 환자의 활동능력을 유지하기 위하여 근육량을 보존할 수 있는 치료적인 접근이 반드시 필요할 것으로 생각한다.

표면 열과 심부 열의 생물학적 원리에 관한 고찰 (Biophysical Principles of Superficial Heating and Deep Heating Agents)

  • Heating of injured tissue has been used for centuries for pain relief and reduction of muscle spasm. In physical therapy locally applied heating gents are used not only to promote relaxation and provide pain relief, but they are also used to increase blood flow, to facilitate tissue healing, and to prepare stiff joints and tight muscles for exercise. Superficial heating agents primarily cause in increases in skin and superficial cutaneous tissue temperature. Superficial heating agents such as hot packs, paraffin wax, Deep heating agents, including shortwave diathermy and continuous-wave ultrasound, can increase tissue temperature at depths ranging from 3to 5cm without overheating the skim and subcutaneous tissue.

Pulmonic stenosis corrected by balloon valvuloplasty in a Maltese dog

  • A 6-month-old intact male Maltese dog (2.5 kg of body weight) was presented with the primary complaint of heart murmur and exercise intolerance. Diagnostic studies revealed severe pulmonic stenosis. Since the pressure gradient in stenotic pulmonic valve area was higher than 200 mmHg (maximum flow velocity = 7.1 m/s), the dog was underwent balloon valvuloplasty. With this procedure, the peak pulmonic velocity was reduced to 3.0 m/s (PG = 36 mmHg). Periodic re-evaluations performed in monthly interval revealed no further deterioration even after 4 month of procedure. This is the first case report of pulmonic stenosis fixed by non-surgical balloon valvuloplasty in Korea.

고공 강하용 수직풍동의 개념설계에 관한 연구 (A Study on the Concept Design of Vertical Wind Tunnel for Skydiver)

  • This paper describes a case study on the design factor analysis of vertical wind tunnel for skydiver's training or experiencing of paradropping exercise in the air. The case study of vertical wind tunnel design is to provide the knowledges on effects of parameter's variation when it is applied to overall or partial duct of tunnel circuit. The analysis of design parameters based on pressure loss are produced one by one through the tunnel components from the flight chamber because the wind tunnel must satisfy the requirement of flight chamber such as flow speed, quality and quantity. Results shows the various effects of parameter variation with pressure loss in the wind tunnel circuit. Pressure loss should be based on the determination of fan and power system which can be selected from market or new design.

Siberian Husky 견에서 팔로사징 진단 증례 (A Case of Tetralogy of Fallot in a Siberian Husky Dog)

  • A 7-month-old 8.2 kg female Siberian Husky with history of exercise intolerance, delayed growth, and tachypnea was presented to Veterinary Medical Teaching Hospital, Kyungpook National University. A holosystolic murmur was identified in cardiac ausculation. In electrocardiography, thoracic radiographs, and ultrasonography, a right axis deviation, enlarged right ventricle, stenotic main pulmonary artery, large ventricular septal defect, marked right ventricle hypertrophy, and overriding aorta with dilation were identified. The color Doppler examination showed the left-to-right shunting of blood via interventricular septal defect and the blood flow streams converging from the right and left ventricles into the aorta. The dog was diagnosed as tetralogy of Fallot. The current state of the dog is well-tolerated without any specific medication.

The Effect of Treadmill Training with Elastic Bands on the Chest Expansion and Pulmonary Functions of Young Adults

  • The purpose of this study was an determine whether elastic band on treadmill training might effect the chest expansion and pulmonary function of the 20's men. 40 subjects with experimental group(male: 20) and control group(male: 20) was participated in experiment. During four weeks, each group participated thirty minutes for three times per week. Subjects were assessed using pre-value and post-value measurement chest length(chest length for resting, chest expansion) and pulmonary function(forced vital capacity, forced expiratory volume at one second, FEV1/FVC, peak expiratory flow, vital capacity, tidal volume, expiratory reserve volume, inspiratory reserve volume) by the CardioTouch 3000S(BIONET, USA). These findings suggest that experimental group can be used to improve chest expansion, pulmonary function than control group. In comparison of both groups, post-test was more improved in experimental group. In conclusion, experimental group helped improving function of pulmonary volume and respiratory muscle, and thus it indicates that the functions will be more improved through the continued respiratory exercise program.

  • Analyzing the ECG signal, we can find heart disease. Myocardial ischemia is a disorder of cardiac function caused by insufficient blood flow to the muscle tissue of the heart. Myocardial ischemia is inscribed on ST-segment of the ECG during and after patient takes exercise or is under stress, but after long time past, the ECG pattern is return to steady state. Therefore, it is necessary to monitor and analyze the ECG signal continuously for patient or aged people. Our primary purpose is the detection of temporary change of the ST-segment of ECG automatically. In the signal processing, the wavelet transform decomposes the ECG signal into high and low frequency components using wavelet function. Mitral Dysplasia with Papillary Muscle Atrophy in a Dog

  • A 9-month-old, female, mixed-breed dog was presented with dyspnea, exercise intolerance and a left apical heart murmur. Echocardiography revealed mitral systolic regurgitant flow by morphological abnormalities of the mitral apparatus including dilated annulus, excessively asymmetric mitral leaflets, short and stout chordae tendinae, and atrophic papillary muscles. The dog was diagnosed with congenital mitral dysplasia based on the echocardiographic features. Two months later, the dog died and the echocardiographic findings were confirmed by necropsy. This case report describes the echocardiographic features of mitral dysplasia with papillary muscle atrophy, and compares the gross necropsy findings in a dog.

Transvenous occlusion of patent ductus arteriosus using an embolization coil in a Maltese dog

  • A 6-year-old female Maltese dog (body weight 2.0 kg) was referred to the Veterinary Teaching Hospital, Kangwon National University with primary complaints including exercise intolerance and heart murmur. Based on clinical and diagnostic findings including grade V/VI left basal continuous murmur, bounding femoral pulsation, left ventricular enlargement pattern in electrocardiogram, cardiomegaly with aortic bulging on the thoracic radiography, and shunt flow between aorta and pulmonary artery on the echocardiography, the dog was diagnosed as the left-to-right patent ductus arteriosus. The patent ductus arteriosus was successfully treated by lodging a single embolization coil with transjugular approach.

A Network Capacity Model for Multimodal Freight Transportation Systems

  • This paper presents a network capacity model that can be used as an analytical tool for strategic planning and resource allocation for multimodal transportation systems. In the context of freight transportation, the multimodal network capacity problem (MNCP) is formulated as a mathematical model of nonlinear bi-level optimization problem. Given network configuration and freight demand for multiple origin-destination pairs, the MNCP model is designed to determine the maximum flow that the network can accommodate. To solve the MNCP, a heuristic solution algorithm is developed on the basis of a linear approximation method. A hypothetical exercise shows that the MNCP model and solution algorithm can be successfully implemented and applied to not only estimate the capacity of multimodal network, but also to identify the capacity gaps over all individual facilities in the network, including intermodal facilities. Transportation agencies and planners would benefit from the MNCP model in identifying investment priorities and thus developing sustainable transportation systems in a manner that considers all feasible modes as well as low-cost capacity improvements.
