Clinical Analysis about Complications of Stroke Treated Intensively in the Intensive Care Unit (뇌졸중으로 중환자실에서 집중치료한 환자들의 합병증에 관한 임상적 고찰)

  • Purpose : Cerebrovascular disease is the most frequent cause of death in Korea and it remains severe disabilities disturbing normal life, According to the previous studies, mortality of the stroke in the first one week is up to the 20% and 95% of stroke patients in the acute stage are accompanied by more than one complications. These complications affect not only the acute stage mortality but also the late stage rehabilitations. In Korea the oriental medicine is preformed in the treatment of stroke. Therefore it is important to recognize thoroughly the complications in the acute stage of stroke and to prevent them. But studies about complications of acute stage stroke is rarely presented in the fields of oriental medicine. So this study is prepared for investigating the characteristics and frequency of complications in the acute stage of stroke. And we are to assess the importance of theses acute complications by systemic reviewing the previous studies. Methods : Fifty one patients are included who had been admitted to ICU(intensive care unit) of Chenju Hospital of Wonkwang Oriental Medicine. Twenty nine patients are diagnosed as ischemic stroke and twenty one patients are diagnosed hemorrhagic stroke. Medical and neurological complications were investigated retrospectively based on medical notes excluding primary symptoms of stroke Le. motor weakness, sensory disturbance and speech disorder. And risk factors of stroke such as D.M. or hypertension are excluded. Results : Medical complications are more frequent than neurological ones. Most frequent medical complication is dysuria(61%) and constipation(45%), fever(30%) and aspiration pneumonia(22%) are followed in order of frequency. In Neurological complication dysphagia(56%), the exacerbation of infarction due to increased intracranial pressure(24%), irritability or insomnia(21%) is most frequently complicated in order of frequency. Conclusions : These complications are mostly caused by bed rest state in acute stage stroke. It is supposed that more aggressive management can prevent theses ones. And it is possible to improve the medical and neurological conditions by sticking theses study results.

Analysis of 119 dispatch for patients with cardio-cerebrovascular and respiratory diseases according to particulate matter (미세먼지 농도에 따른 심·뇌혈관계 및 호흡기계 환자의 119 구급 출동 분석)

  • Purpose: The purpose of this study was to provide basic data for improving the response capacity of 119 EMS systems by analyzing the effects of particulate matter on cardio-cerebrovascular and respiratory symptoms in the pre-hospital stage. Methods: We examined 46,389 patients who transferred to the hospital with complaints of cardiopulmonary arrest and cardio-cerebrovascular and respiratory symptoms by 119 ambulances in Incheon from 2016 to 2018. Results: The probability of 119 emergency dispatch for patients with cardiopulmonary arrest increased 2.8-4.0% from the day of symptom onset until two days before hospital presentation as particulate matter 10㎛ or less in diameter(PM10) increased by 10㎍/㎥ (OR=1.028; 95% CI=1.014-1.041, p=0.000, lag 0), (OR=1.040; 95% CI=1.024-1.056, p=0.000, lag 1), (OR=1.032; 95% CI=1.016-1.049, p=0.000, lag 2). Meanwhile, emergency dispatch increased 3.6-6.1% for PM2.5 in creased by 10㎍/㎥ (OR=1.046; 95% CI=1.024-1.068, p=0.000, lag 0), (OR=1.061; 95% CI=1.035-1.088, p=.000, lag 1), and (OR=1.036; 95% CI=1.010-1.063, p=0.006, lag 2). Conclusion: Emergency medical technicians (EMTs) who respond to 119 calls should rapidly and accurately evaluate patients and provide professional emergency care by identifying the characteristics of the vulnerable groups relative to particulate matter size. To prevent the occurrence and exacerbation of symptoms caused by particulate matter, EMTs should be prepared and equipped with a response system for high particulate matter in the EMS system.

  • Objectives : The purpose of this study is to observe the effect of Coptidis Rhizoma (CCE-extract of C. chinensis Rhizome) in induction of immune protein on mouse macrophages. Methods : To analyze cytokines interleukin(IL)-$1{\alpha}$, IL-3, IL-9, IL-12p40, IL-13, IL-17, Monocyte Chemoattractant Protein(MCP)-1 induced by macrophages, mouse macrophages were incubated with CCE and was measured. Results : IL-$1{\alpha}$ measurement, CCE showed significant inhibition only at concentration level of 200 ${\mu}g/mL$. IL-3, MCP-1 measurement, CCE showed significant inhibition only at concentration level of 100, 200 ${\mu}g/mL$. IL-9 measurement, CCE showed significant inhibition only at concentration level of 50 ${\mu}g/mL$. IL-13 measurement, CCE showed significant inhibition only at concentration level of 50, 100, 200 ${\mu}g/mL$. The IL-12p40, IL-17 levels indicated no changes at 25, 50, 100, 200 ${\mu}g/mL$ on mouse macrophages. Conclusions : CCE did not significantly increased inflammatory cytokines IL-$1{\alpha}$, IL-3, IL-9, IL-12p40, IL-13, IL-17, Monocyte Chemoattractant Protein(MCP)-1 on mouse macrophages. It was verified CCE does not trigger cytokine related hypersensitivity reaction of organism or exacerbation of acute/chronic inflammatory disease.

Calcific Tendinitis of Shoulder Associated with Intraosseous Involvement -A Case Report- (골내 침범을 동반한 견관절 석회화 건염 - 증례 보고 -)

  • Purpose: We present an atypical case of calcific tendinitis of the shoulder with intraosseous loculation. Materials and Methods: A 59 year-old female complained of acute exacerbation of chronic left shoulder pain and restricted range of motion. Simple radiographs showed a subacromial calcific deposit and magnetic resonance imaging revealed cortical erosion with intraosseous extension of calcific material mimicking infection or tumor. She was managed with arthroscopic excision of the calcific deposit, curettage of the intraosseous lesion and subsequent rotator cuff repair with a suture anchor. Results: Her acute pain promptly subsided. Her rehabilitation was uneventful and she gained full range of motion. Radiographs five months after the operation showed no recurrence of calcific material. Conclusion: Calcific tendinitis of the shoulder can present with a variety of images involving the adjacent bone. The correct recognition of this disorder may avoid unnecessary investigation and treatment.

Physiological, Pharmacological and Toxicological Implications of Heterodimeric Amino Acid Transporters

  • The heterodimeric amino acid transporter family is a subfamily of SLC7 solute transporter family which includes 14-transmembrane cationic amino acid transporters and 12-transmembrane heterodimeric amino acid transporters. The members of heterodimeric amino acid transporter family are linked via a disulfide bond to single membrane spanning glycoproteins such as 4F2hc (4F2 heavy chain) and rBAT $(related\;to\;b^0,\;^+-amino\;acid\;transporter)$. Six members are associated with 4F2hc and one is linked to rBAT. Two additional members were identified as ones associated with unknown heavy chains. The members of heterodimeric amino acid transporter family exhibit diverse substrate selectivity and are expressed in variety of tissues. They play variety of physiological roles including epithelial transport of amino acids as well as the roles to provide cells in general with amino acids for cellular nutrition. The dysfunction or hyperfunction of the members of the heterodimeric amino acid transporter family are involved in some diseases and pathologic conditions. The genetic defects of the renal and intestinal transporters $b^{0,+}AT/BAT1\;(b^{0,+}-type\;amino\;acid\;transporter/b^{0,+}-type\;amino\;acid\;transporter\;1)$ and $y^+LAT1\;(y^+L-type\;amino\;acid\;transporter\;1)$ result in the amino aciduria with sever clinical symptoms such as cystinuria and lysin uric protein intolerance, respectively. LAT1 is proposed to be involved in the progression of malignant tumor. xCT (x-C-type transporter) functions to protect cells against oxidative stress, while its over-function may be damaging neurons leading to the exacerbation of brain damage after brain ischemia. Because of broad substrate selectivity, system L transporters such as LAT1 transport amino acid-related compounds including L-Dopa and function as a drug transporter. System L also interacts with some environmental toxins with amino acid-related structure such as cysteine-conjugated methylmercury. Therefore, these transporter would be candidates for drug targets based on new therapeutic strategies.

Implementation of PDA Based Personal Asthma Management System for Effective Management of Asthmatic (천식환자의 효과적인 관리를 위한 PDA 기반 개인용 천식관리 시스템 구현)

  • Self management of chronic asthma is of great importance, since the disease could lead the patient into an emergent situation. In the present study, we describe design and implementation of a personal digital assistant(PDA) based asthma management system for personal application including symptom and medication to prevent from the potential exacerbation of the disease. The software program was written by the Visual C++ tool in the mobile computing environment and Object Store was applied for data management. User friendly GUI environment was provided for the patient to input his/her daily condition and self treatment such as medication for successful management. The input screen design substituted for keyboard input to a mouse in order to easy to select an item and minimize the keyboard input. The implementation results of this system., Real-time data collection and process were possible and be able to have been carried effectively out a continuous symptom, a medication of asthma patients, risk management.

Experiences of Medication Misuse and Abuse Behaviors in Elderly People with Chronic Disease in Community (지역사회에 거주하는 만성질환 노인의 약물 오·남용행위 경험)

  • The purpose of this study was to understand the essential meanings in experiences of medication misuse and abuse behavior in elderly people with chronic disease in community. The participants were 10 patients with chronic disease over 65 ages who were selected by convenience sampling. Data were collected through individual in-depth interviews from February to May 2014, and were analyzed using Colaizzi (1978)'s phenomenological analysis method. As a results, the essential meanings and structure of medication misuse and abuse behavior experience in elderly people with chronic disease were derived into five clusters of themes as follows: 'Burden on the medications', 'Anxiety about the side effect of the medications or the disease exacerbation', 'There is no alternative other than the medications', 'Expectations of the medications effect for my criteria', 'Lack of understanding on the correct medication use'. The results of this study contributed to promoting a deeper understanding of medication misuse and abuse behaviors in elderly people with chronic disease, and provided for the basic data of strategic development for the prevention of medication misuse and abuse behaviors from the perspective of elderly people with chronic disease in community.

Asthma Worsening factors of Adolescent Asthma Patients in Korea -Associated with Intake of Antioxidant Food- (청소년 천식 환자의 천식 악화의 영향 요인 -항산화식이 섭취와 관련하여-)

  • The purpose of this study was to investigate the association between antioxidant food intake pattern and asthma control status in Korean adolescents. This study was conducted using data from 1,578 patients diagnosed with asthma among the participants in the 11th online health behavior survey. Statistical analyses were performed for sociodemographic variables, known risk factors for asthma worsening, and all variables which were related to food intake pattern. For final decision, logistic regression analysis was used to estimate the associations between antioxidant food intake and asthma worsening. Male gender and smoking experiences were associated with asthma exacerbation. Low intake of antioxidant foods status also significantly increased asthma exacerbations. Antioxidant food intake and smoking prevention education is important for adolescent asthma patients. It is highly needed to recommend taking fruits and vegetables properly.

Prescription Patterns and Factors Related to the Number of Medications in Chronic Obstructive Pulmonary Disease in Non-elderly Adults (비고령 성인환자의 만성폐쇄성폐질환 약물사용 현황과 영향인자)

  • Background: This study is to investigate the prescription patterns and factors related to the number of medications treating chronic obstructive pulmonary disease (COPD) in patients under 65 years old according to GOLD guidelines. Methods: We retrospectively analyzed the medical records of patients aged 40-64 years with a diagnosis of COPD from January to March 2016. Patients were classified by combined assessment of COPD (grades A, B, C, D) using spirometry, exacerbation history, mMRC, and/or CAT results. We analyzed prescribed medications, treatment options and factors related to the numbers of COPD medications. Results: The total number of prescriptions were 251. About 35.5% of patients were classified as GOLD A, 34.2% as GOLD B, 17.1% as GOLD C and 13.2% as GOLD D. Inhaled bronchodilator was prescribed for 86.9% of patients and the most frequent COPD medication was long-acting muscarinic antagonist (LAMA) followed by inhaled corticosteroids/long acting beta agonist (ICS/LABA). The majority of low risk patients (GOLD A/B) were prescribed a monotherapy with LAMA or LABA. For high risk patients (GOLD C/D), combination treatment with ICS+LAMA+LABA was mostly prescribed. The 21.2% of patients in GOLD D received systemic corticosteroid. The average number of medications per prescription was 3.7, and this number increased with increasing COPD grade, COPD duration and lung function reduction ($FEV_1$, $FEV_1/FVC$). Conclusion: Generally high adherence to GOLD guideline recommendations was reported. Given the progressive nature of the disease, results suggest that closer attention to respiratory symptoms for early detection, diagnosis, and appropriate treatment of COPD is warranted.
