An Outbreak of Epidemic Keratoconjunctivitis by Adenovirus Type 8 in a Neonatal Intensive Care Unit (신생아 중환자실에서의 아데노바이러스 8형에 의한 유행성 각결막염의 발생)

  • Purpose : Epidemic keratoconjunctivitis (EKC) caused by adenovirus is a highly contagious disease, which has been reported as outbreaks involving adults in the community. However, there has been no report on EKC outbreak by adenovirus in a neonatal intensive care unit (NICU) in Korea. Aims of this study were to investigate the EKC outbreak by adenovirus type 8 in NICU and to confirm an effectiveness of polymerase chain reaction (PCR) for diagnosis. Methods : Conjunctival swab or nasopharyngeal aspirate specimens were taken from all patients and tested by viral culture and PCR. Adenovirus serotype was determined by sequencing of PCR product of selected region of hexon gene using the virus isolates or specimens. Results : An outbreak of EKC occurred which was involving 12 preterm infants in the NICU of the Seoul National University Children's Hospital between July 12th and August 1st, 2005. Three hospital staffs and one family member of the neonate were also affected. Adenovirus was detected in 12/12 (100%), 6/11 (54.5%) by PCR and virus culture, respectively. Eleven PCR-positive neonates were identified as serotype 8 by sequencing. The first affected 4 babies have had routine ROP (retinopathy of prematurity) examinations one week ago. While previous outbreaks were sustained for a few months, the event in our unit was controlled without complications in 3 weeks. Conclusion : We analyzed the EKC outbreak by adenovirus type 8 in NICU. Adenovirus serotype was identified by PCR and sequencing with high sensitivity for the first time in Korea, so we suggest this method can be very useful for rapid diagnosis and infection control.

Legislative Study on the Mitigation of the Burden of Proof in Hospital Infection Cases - Focusing on the revised Bürgerliches Gesetzbuch - (병원감염 사건에서 증명책임 완화에 관한 입법적 고찰 - 개정 독일민법을 중심으로 -)

  • Owing to causes such as population aging, increased use of various medical devices, long-term hospitalization of various patients with reduced immune function such as cancer, diabetes, and organ transplant patients, and the growing size of hospitals, hospital infections are continuing to increase. As seen in the MERS crisis of 2015, hospital infections have become a social and national problem. In order to prevent damage due to such hospital infections, it is necessary to first strictly implement measures to prevent hospital infections, while, on the other hand, providing proper relief of damage suffered due to hospital infections. However, the mainstream attitude of judicial precedents relating to hospital infection cases has been judged to in fact shift responsibility over damages due to hospital infections on the patient. In light of the philosophy of the damage compensation system, whose guiding principle if the fair and proper apportionment of damages, there is a need to seek means of drastically relaxing the burden of proof on the patient's side relative to conventional legal principles for relaxing the burden of proof, or the theory of de facto estimation. In relation to such need, the German civil code (Burgerliches Gesetzbuch), which defines contracts of medical treatment as typical contracts under the civil code, and has presumption of negligence provisions stipulating that, in cases such as hospital infections which were completely under the control of the medical care providers, if risks in general medical treatment have been realized which cause violations of the life, body, or health of patients, error on the part of the person providing medical care is presumed, was examined. Contracts of medical treatment are entered into very frequently and broadly in the everyday lives of the general public, with various disputes owing thereto arising. Therefore, it is necessary to, by defining contracts of medical treatment as typical contracts under the civil code, regulate the content of said contracts, as well as the proof of burden when disputes arise. If stipulations in the civil code are premature as of yet, an option may be to regulate through a special act, as is the case with France. In the case of hospital infection cases, it is thought that 'legal presumption of negligence' relating to 'negligence in the occurrence of hospital infections,' which will create a state close to equality of arms, will aid the resolution of the realistic issue of the de facto impossibility of remedying damages occurring due to negligence in the process of occurrence of hospital infections. Also, even if negligence is presumed by law, as the patient side is burdened with proving the causal relationships, such drastic confusion as would occur if the medical care provider side is found fully liable if a hospital infection occurs may be avoided. It is thought that, alongside such efforts, social insurance policy must be improved so as to cover the expenses of medical institutions having strictly implemented efforts to prevent hospital infections in the event that they have suffered damages due to a hospital infection accident, and that close future research and examination into this matter will be required.

Efficient Application of Westgard Multi-Rules and Quality Control Implementation Improvement (Westgard Multi-Rules의 효율적 적용과 조치사항의 개선)

  • Purpose Westgard multi-rules application based on test quality improvement and commercialized international standard has been widely used in quality control. However, it is difficult to applicate the Westgard multi-rules in nuclear medicine in vitro tests due to the larger sample sizes and the simultaneous measurement of quality control material and patient sample. This study investigated the usefulness of Westgard multi-rules application in nuclear medicine in vitro tests. Materials and Methods A total of 282 systematic error multi-rules (22s, 101s) recorded in the samsung medical center computer system from January 2013 to June 2016 along with 117 cases of corrective measure record was analyzed. The Quality control implementation is recorded in Hospital information system were divided into 4 high-level areas including quality control material error, experimental procedural error, Kit lot number management error, and others. To prevent quality control material error, the existing method that each staff used their own method was changed. The staff who in charge of managing the quality control material was designated and daily consumption amount of every test was strictly controlled by one person. To prevent other errors, every test step was standardized so that the entire test procedures are identically implemented. Results The total quality control implementation was 117 cases; As a result, 62 quality control material errors were 62 cases, experimental process errors were 24 cases, Kit lot number control errors were 18 cases, and other errors were 13 cases. The quality control material error was corrected and could be used fresh materials within 2 days after thawing. The cases of systemic error were decreased to causes as quality control material error. The quality control materials were reduced above 10 vials to a monthly average. In addition, these errors of experimental processing and Kit lot number were improved by test standardization. Consequently, the cases of 101s and 22s in systematic error rules decreased at least 2 cases to a monthly average. Conclusion To confirm of systematic error through multi-rules application quickly, it is necessary to base on management of the QC material, target values and standard deviation. Moreover, in the event of a systematic error, it was found important to record measures based on test cause analysis. The experiment results are expected to contribute to internal quality control improvement and prompt and accurate result reporting through error recording and causal analysis based on Westgard multi-rules analysis.

Earthquake impacts on hydrology: a case study from the Canterbury, New Zealand earthquakes of 2010 and 2011

  • On 4 September 2010 an earthquake of magnitude 7.1 on the Richter scale occurred on the Canterbury Plains in the South Island of New Zealand. The Canterbury Plains are an area of extensive groundwater and spring fed surface water systems. Since the September earthquake there have been several thousand aftershocks (Fig. 1), the largest being a 6.3 magnitude quake which occurred close to the centre of Christchurch on 22February 2011. This second quake caused extensive damage to the city of Christchurch including the deaths of 189 people. Both of these quakes had marked hydrological impacts. Water is a vital natural resource for Canterburywith groundwater being extracted for potable supply and both ground and surface water being used extensively for agricultural and horticultural irrigation.The groundwater is of very high quality so that the city of Christchurch (population approx. 400,000) supplies untreated artesian water to the majority of households and businesses. Both earthquakes caused immediate hydrological effects, the most dramatic of which was the liquefaction of sediments and the release of shallow groundwater containing a fine grey silt-sand material. The liquefaction that occurred fitted within the empirical relationship between distance from epicentre and magnitude of quake described by Montgomery et al. (2003). . It appears that liquefaction resulted in development of discontinuities in confining layers. In some cases these appear to have been maintained by artesian pressure and continuing flow, and the springs are continuing to flow even now. In spring-fed streams there was an increase in flow that lasted for several days and in some cases flows remained high for several months afterwards although this could be linked to a very wet winter prior to the September earthquake. Analysis of the slope of baseflow recession for a spring-fed stream before and after the September earthquake shows no change, indicating no substantial change in the aquifer structure that feeds this stream.A complicating factor for consideration of river flows was that in some places the liquefaction of shallow sediments led to lateral spreading of river banks. The lateral spread lessened the channel cross section so water levels rose although the flow might not have risen accordingly. Groundwater level peaks moved both up and down, depending on the location of wells. Groundwater level changes for the two earthquakes were strongly related to the proximity to the epicentre. The February 2011 earthquake resulted in significantly larger groundwater level changes in eastern Christchurch than occurred in September 2010. In a well of similar distance from both epicentres the two events resulted in a similar sized increase in water level but the slightly slower rate of increase and the markedly slower recession recorded in the February event suggests that the well may have been partially blocked by sediment flowing into the well at depth. The effects of the February earthquake were more localised and in the area to the west of Christchurch it was the earlier earthquake that had greater impact. Many of the recorded responses have been compromised, or complicated, by damage or clogging and further inspections will need to be carried out to allow a more definitive interpretation. Nevertheless, it is reasonable to provisionally conclude that there is no clear evidence of significant change in aquifer pressures or properties. The different response of groundwater to earthquakes across the Canterbury Plains is the subject of a new research project about to start that uses the information to improve groundwater characterisation for the region.   • The purpose of this article is to bring about 10.26 accident by providing a fuse for the overthrow of the Park Chung-hee administration in the 10.18 Buma uprising. So, first of all, this article has a main purpose in analyzing the comparison between Masan's 3.15 uprising and 10.18 Buma uprising. The purpose of the study is to compare the 3.15 democratic movement in Masan with the 10.18 Buma uprising, given that the incident laid the foundation for the dictator to be overthrown in the event of an anti-dictatorial movement in Masan. The research method of this article is intended to be used as a research method in the 3.15 and 10.18 protests, given that if a person in power conducts election fraud or suppresses anti-government movements in order to maintain the system, it could lead to the destruction of those in power. In the end, the Masan 3.15 uprising and 10.18 uprising failed to reach a direct attempt to overthrow the regime due to unfinished democratic movements, but with the revolution of 4.19 and the massive political transformation of 10.26 Accident, he achieved the leading role theory that allowed the Rhee Syngman and Park Chung-hee administrations. In eradicating authoritarian regimes, however, the historic significance of the democratization movement was that the authoritarian regime eventually brought about the collapse of the regime by making a hard-line stance on election schemes or popular protests over the trap of power boomerang, which causes the regime to collapse.

Assessment of Environmental Impact on the Severely Soil-Eroded Area by heavy Rainfall (집중호우로 인한 토양침식 우심지역 환경영향평가)

  • The steep-sloped agricultural land was severely damaged by rainfall events during July and August every year. The objective of this study was to investigate an effects of intensive rainfall to the soil properties of the steep-sloped agricultural land. Survey sites including the Sacheon myeon area were located in Gangneung, those were severely damaged from a forest fire in April 2000. Surveys were taken at these sites after two years of forest fire and severe rainfall events in August 2002, which included an event that poured with 870 mm of rainfall in a day. After this storm, soil erosion, burying, and flooding were observed. Severe soil loss was found at lower soil-depths, greater slopes, longer slope lengths, and concave landscapes. Soil loss and land slides were often found at areas with having a coarser textures, higher bulk densities, lower water holding capacity, and lower rates of soil aggregation. Crop growth stagnation was found at the site of crop restoration because of low soil fertility and poor drainage caused by the abrupt textural changes. In conclusion, it is necessary to manage the steep-slope agricultural land based on environmental impact assessment data of macro morphological and physical characteristics by intensive rainfall.

Study on Tragic Characteristic in Lope's Drama La estrella de Sevilla (로뻬의 연극 『세비야의 별』에 나타난 비극성 연구)

  • La estrella de Sevilla, is a representative masterpiece of the Golden Age of Spain, and is a tragic work of Lope de Vega, who was a representative drama writer living in the 17th century. However, regardless of its great literary value, there are insufficient researches on this drama even in Spain, due to several controversies surrounding the truth about Lope's creation. Here, this research concretely investigated the tragic characteristic of this drama, by largely dividing it into the viewpoint of Aristotle and Shakespeare. Thanks to the noble social status of the characters and proper dialogue of the hero Sancho Ortiz in trouble, there seems to be no problem in performing the cathartic function that Aristotle mentioned regarding the work in his review. However, there are a few problems in the aspect of the 'hero's free will to resist the absolute fate,' which is fundamentally the essence of the associated Aristotelian tragedy. Because, there's no Hamartia in this drama, the core mechanism that forms the absolute fate, makes the statement that accordingly, no absolute fate or irresistible force of destiny is established. Rather, the tragic characteristic of this drama can be more properly investigated from the viewpoint of Shakespeare. As noted differently from Aristotle, Shakespeare considered that the tragic characteristic came from the hero's characteristic. According to him, tragedy starts from the hero's value view and personality, instead of the external factors such as an absolute fate. Actually, Busto's death and separation between Sancho Ortiz and Estrella, two tragic affairs of La estrella de Sevilla, were caused by the crooked selfishness of King Sancho IV, who abused his authority, and Sancho Ortiz' excessively blind loyalty to the King, rather than the irresistible event. In conclusion, in light of a lethargic and hopeless situation, with no eventual choice of options before the injustice of absolute power, is must have been a significant tragedy for the audience, witnessing the play's tragic ending.

Development of the National Integrated Daily Weather Index (DWI) Model to Calculate Forest Fire Danger Rating in the Spring and Fall (봄철과 가을철의 기상에 의한 전국 통합 산불발생확률 모형 개발)

  • Most of fires were human-caused fires in Korea, but meteorological factors are also big contributors to fire behavior and its spread. Thus, meteorological factors as well as topographical and forest factors were considered in the fire danger rating systems. This study aims to develop an advanced national integrated daily weather index(DWI) using weather data in the spring and fall to support forest fire prevention strategy in South Korea. DWI represents the meteorological characteristics, such as humidity (relative and effective), temperature and wind speed, and we integrated nine logistic regression models of the past into one national model. One national integrated model of the spring and fall is respectively $[1+{\exp}\{-(2.706+(0.088^*T_{mean})-(0.055^*Rh)-(0.023^*Eh)-(0.014^*W_{mean}))\}^{-1}]^{-1}$, $[1+{\exp}\{-(1.099+(0.117^*T_{mean})-(0.069^*Rh)-(0.182^*W_{mean}))\}^{-1}]^{-1}$ and all weather variables significantly (p<0.01) affected the probability of forest fire occurrence in the overall regions. The accuracy of the model in the spring and fall is respectively 71.7% and 86.9%. One integrated national model showed 10% higher accuracy than nine logistic regression models when it is applied weather data with 66 random sampling in forest fire event days. These findings would be necessary for the policy makers in the Republic of Korea for the prevention of forest fires.

Spatial and Temporal Distribution of Zooplankton Communities in Lake Paldang (팔당호 동물플랑크톤 군집의 시공간적 분포)

  • The zooplankton community and environmental factor were investigated on a weekly basis from March to November 2015 in Lake Paldang, Korea. The seasonal succession of zooplankton community structure was influenced by hydraulic and hydrological factors such as inflow, outflow and rainfall. However, the hydraulic retention time in 2015 (16.3 day) was affected by the periods of water shortage that had continued since 2014 and increased substantially compared to 2013 (7.3 day). Therefore, the inflow and outflow discharge were decreased, and the water quality (COD, BOD, TOC, TP, Chl-a) of Lake Paldang (St.1) was the same characteristics as the river type Bukhan river (St.3), compared with the lake type Namhan river (St.2) and Gyeongan stream (St.4). Zooplankton community dominated by rotifers (Keratella cochlearis, Synchaeta oblonga) in spring (March to May). However, Copepod (Nauplius) and Cladoceran (Bosmina longirostris) dominated in St.4. In summer (June to August), there was a few strong rainfall event and the highest number of individuals dominated by Keratella cochlearis (Rotifera) and Difflugia corona (Protozoa) were shown during the study period. In autumn (October to November), the water temperature was decreased with decrease in the total number of individuals showing Nauplius (Copepoda) as the dominant species. As a result of the statistical analysis about zooplankton variation in environmental factors, the continuous periods of water shortage increased the hydraulic retention time and showed different characteristic for each site. St.1, St.3 and St.2, St.4 are shown in the same group (p<0.05), showing the each characteristics of river type and lake type. Therefore, the water quality of catchment area and distribution of zooplankton community would be attributed to hydraulic and hydrological factors.

Spatio-temporal Distribution Patterns of Lotic Benthic Macroinvertebrate Communities in Namhan-River Weir Section (남한강 보 구간 유수성 저서성 대형무척추동물의 시·공간적 분포 특성)

  • Lotic organisms in streams are affected by natural and anthropogenic disturbances such as increase of heavy rainfall event caused by climate change and flow regime change caused by weir constructions. Based on domestic and foreign literature, 157 Korean benthic macroinvertebrate taxa were selected species as potential lotic candidates. Three shoreline sites (total 54 samples) were surveyed consecutively before ('08~'09), during ('10~'12) and after ('13~'16) the construction of the weirs (Gangcheon, Yeoju and Ipo weir) in the Namhan-River for tracing changes of lotic communities. As a result, water flow of the Ipo-wier and water quality variables such as T-N, T-P, BOD5, etc. of the weir section revealed no significant changes. Physical habitat conditions such as the flow velocity and streambed substrate evidently changed. Particulary, flow velocity measured at sampling points along with each microhabitat drastically decreased and particle size of streambed substrate steadily decreased after weir constructions. Lotic organisms also decreased after construction, especially Hydropsychidae (insecta: Trichoptera) acutely decreased from $3,526ind.\;m^{-2}$ to $2ind.\;m^{-2}$ As a result of CCA, lotic species such as Hydropsyche valvata, Hydropsyche kozhantschikovi, Cheumatopsyche brevilineata, Cheumatopsyche KUa, Macrostemum radiatum, etc. correlated with the flow velocity, streambed substrate. Therefore, the decrease of the flow velocity and substrate size after weir construction seemed to be closely related to the decrease of the individual abundances of the lotic organisms independently of water quality. In order to evaluate the influence of the ecosystem on the flow regime change more accurately, it is necessary to study the indicator species based on the resistance or preference of the flow.
