Digital intraoral impression for immediate provisional restoration of maxillary single implant: A case report (구강 내 디지털 인상채득을 통한 상악 전치부 임플란트 즉시 임시 보철 수복 증례)

  • Developing of digital technique, it is possible to fabricate implant prostheses for edentulous area using intraoral 3-dimentional information throughout implant diagnosis and treatment process. It is being changed that from the method using CAD/CAM, producing prostheses by model scanning after conventional impression and model processing, to the method of fabricating implant provisional restorations and customized abutments by digital impression after connecting digital impression copings (scanbody) and implant fixtures without models. But, this digital method has not been actively used for implant prostheses not yet. Specially, it is short of intraoral digital impression cases for immediate provisional restorations of the maxillary anterior implants. The gingival contour impression of maxillary anterior area is very important for esthetic restorations. Accordingly, in this case report, the using a digital impression coping (scanbody) and digital impression by CEREC Omnicam (Sirona, Bensheim, Germany) or Trios (3shape, Copenhagen, Denmark) were introduced for immediate provisional restorations in 3 cases needed a single implant restoration in maxillary anterior area. The clinical results were satisfactory on the convenience and accuracy of digital impression technique and the good esthetics of final restorations.

  • The difficult oral healthcare in intellectually disabled patients with poor behavioral control has led to debate over the cost-effectiveness and validity of implant treatment in these patients. The patient in the present report had schizophrenia that had led to poor oral care and severe dental caries in the full mouth. Tooth extraction and a removable prosthesis were planned, but the guardian wanted an implant procedure. Since the guardian showed strong will and cooperation with regard to the patient's oral healthcare, extraction followed by immediate implant placement was performed across two rounds of general anesthesia. Since the outcome appears successful, we present this case report. Immediate implant placement after tooth extraction requires fewer surgeries and rounds of general anesthesia, reduces horizontal bone resorption, and can achieve better esthetic results. Therefore, as long as a certain degree of oral care is possible, this can be a positive option for restoration of a partially edentulous mouth, even in intellectually disabled patients.

  • Rehabilitation of maxillectomy patients is challenging. The maxillary defects need to functional restoration because of mastication, speech, swallowing problems. The goal of making obturator is to restore maxillary defects and give patients comfortable, esthetic prosthesis. This case report presents acquired masticatory and esthetic results and improved retention resulting from the pharyngeal obturator prosthesis using implant.

  • In the case of single tooth replacement, a fixed prosthesis or a dental implant is the treatment option commonly selected as first choice. However, any amount of sound tooth structure should be removed to prepare the abutment teeth for full coverage retainer. The adjacent tooth damage can be avoided placing a dental implant. However, depending on the patient's oral condition and any other circumstances, it may be impossible or delayed. In this case resin bonded fixed partial denture was selected as an alternative that can restore single tooth loss without much tooth structure removal to the young patients and the patients with gingival recession. We report these patients were satisfied with esthetic and function.

  • With the introduction of dental implants, restoration of missing teeth with conventional fixed or removable partial dentures is being replaced with implants. Especially, with young patients, not only longevity but also esthetic factors need to be considered. Implant restorations provide long-term success functionally but, esthetic complications such as, marginal exposure due to gingival recession, loss of the papilla and dark color of metal abutments may occur. Recently, zirconia restorations with CAD/CAM technology provide functional, biocompatible and esthetic restorations possible. All-ceramic restorations using the pressed ceramic technique show better fracture toughness values than those of the conventional porcelain veneering technique. Pressed ceramic technique creates the veneer design in wax and the lost wax technique is used to create the restoration. The final contour of the restoration may be controlled during wax-up. A 22-year old female patient was restored with dental implants and zirconia restorations using the pressed ceramic technique presenting short-term but optimistic prognosis.

  • Prosthodontic treatment planning for various edentulous arches is not an easy issue for dentists. Especially, in case of removable partial denture, we must have a knowledge of overall treatment procedures, and a careful approach is needed. Recently, interest of dental implant and case reports are increasing, the decrease of the removable partial denture is true, but dental implantation takes longer treatment period and it is more expensive. Also, there are still some limitations like lack of available alveolar bone, patient's general condition, and chronic periodontitis. Therefore, sometimes implantation is impossible. Finally, implantation cannot be adapted to every single patient. Currently, the clasp type removable partial dentures are used routinely, giving patients many unesthetic and functional difficulties. With better laboratory technique, removable partial denture with attachment and removable partial denture in rigid type increase patient's happiness level much more than predicted. The case presented in this article, clinically demonstrate the efficiency of using a telescopic system to improve esthetic and functional recovery for patients who lose multiple teeth.

  • When treating partial edentulous patients, it is important to use an appropriate restorative materials. Inappropriate restorative materials can adversely affect the outcome of prosthetic restorations. Zirconia and dental metal, which are currently and widely used materials, have a higher elastic modulus than cortical bone, so when an external force is generated, a harmful force can be applied to the implant and the bone around the implant. Polyetherketoneketone (PEKK), a recently introduced material, has a elastic modulus similar to that of cortical bone, and has many advantages in terms of physical properties and biocompatibility. This case report describes that implant-supported fixed prosthetic treatment using PEKK was performed, and functional and esthetic satisfactory results were obtained.

  • To obtain better esthetic results when immediately placing a dental implant, the soft tissue surrounding the implant must be conditioned during healing of the extraction socket. To this end, the emergence profile can be customized through immediate restoration of the provisional prosthesis, and good clinical results can be obtained at the time of definitive restoration in the future, resulting in high patient satisfaction. In this case, horizontal root fracture occurred after trauma to both maxillary central incisors. Immediate implant placement and loading was planned considering aesthetics and alveolar bone condition. By taking an impression using a digital intraoral scanner, a digital diagnostic wax-up was performed to make a more aesthetic prosthesis without applying external force to the traumatized teeth. Based on this, the ideal placement location was determined and immediate implant placement was performed using a 3D printed surgical guide. The provisional prosthesis was restored 5 days after placement, and the definitive zirconia crown was restored through soft tissue conditioning and customization using the shape of the provisional prosthesis for 3 months.

  • Mini-implant has been reported as an alternative method to conventional diameter implants in the treatment of mandibular anterior region with narrow ridge. However, one-piece type mini-implant showed biological and technical complications, such as peri-implantitis by residual cement and prosthetic detachment. Recently, one-piece type mini-implant with additional retentive structure has been introduced to increase the retention and stability of the mini-implant. The mini-implant is based on spreading the upper retentive structure with a spreader and connected it with the inner surface of the zirconia prosthesis. In two cases, we tried to reduce these complications through the mini-implant on the narrow mandibular anterior region, and it was possible to reduce the overall treatment period through a digital system. In this case report, restoration using the digital mini-implant system showed improvement of patient satisfaction on the aspect of function and esthetic in the narrow mandibular anterior region.

Mechanical strength of Zirconia Abutment in Implant Restoration (지르코니아 임플란트 지대주의 기계적 강도에 관한 연구)

  • Purpose: As the esthetic demands of dental implant patients are increased, the demands of zirconia as implant abutment material are also increased. It has non-metalic color, good biocompatibility, high strength and high toughness. Even thought the advatage of zirconia abutment, there are a few studies about mechanical properties of zirconia abutment. This study evaluated the mecanical strength with compressive bending strength and endurance limit of implant-zirconia abutment assembly. Materials and Methods: Static and cyclic loading of implant-Zirconia abutment assembly were simulated under worst case condition according to ISO. Test groups were implants of external butt joint with straight regular diameter and angled regular diameter zirconia abutment, implant of external butt joint with narrow straight diameter zirconia abutment and implant of internal conical joint with straight narrow diameter zirconia abutment. All test group were evaluated the mecanical strength with compressive bending strength and endurance limit. After fatique testing, fracture surface were examined by SEM. Results: The compressive bending strengths exceed 927N. Regular diameter zirconia abutment were stronger than narrow diameter zirconia abutment(P<.05). The endurance limits ranged from 503N to 868N. Conclusion: Within the limitation of this study, zirconia implant abutment exceeded the estabilished values for maximum incisal biting forces reported in the literature.
