Risk Factors of Nosocomial Bacteremia of Extended-spectrum ${\beta}$-Lactamase Producing Escherichia coli (병원획득 Extended-spectrum ${\beta}$-Lactamase 생성 Escherichia coli 균혈증의 위험인자)

  • Background: The prevalence of extended-spectrum ${\beta}$-lactamase (ESBL)-producing Escherichia coli is increasing rapidly worldwide. Treatment options for ESBL-producing E. coli are limited, and infections caused by this organism are associated with improper antibiotic use, a long hospital stay, and increased mortality. Thus, the assessment and early recognition of the risk factors of nosocomial infections due to ESBL-producing E. coli are important for the infection control and proper treatment. Methods: A case-control study was performed that included nosocomial episodes of ESBL-producing E. coli bacteremia at a tertiary care hospital from January 2004 to December 2007. For each case patient, three controls were randomly selected and data on predisposing factors were collected. Results: Fifty-five cases of nosocomial ESBL-producing E. coli bacteremia were studied. Carbapenem usage (OR: 11.3, 95% CI: 1.1-115.9, p=0.041), quinolone usage (OR: 4.5, 95% CI: 1.1-18.8, p=0.042), biliary obstructive disease (OR: 11.8, 95% CI: 3.0-46.7, p<0.001) and the APACHE II score (OR: 1.3, 95% CI: 1.2- 1.5, p<0.001) were analyzed as independent risk factors of nosocomial ESBL-producing E. coli bacteremia. Conclusion: Our results showed that physicians caring for patients with risk factors of nosocomial bacteremia should consider ESBL-producing E. coli as the causative organisms of the disease.

Inactivation of Escherichia coli O157:H7, Salmonella and Listeria monocytogenes by Organic Acid (유기산이 Escherichia coli O157:H7, Salmonella 및 Listeria monocytogenes의 증식에 미치는 영향)

  • The inhibitory effect of the food processing agent on growth of Escherichia coli O157:H7, Salmonella Enteritidis, and Listeria monocytogenes was performed with organic acid, and combination of citric acid, acetic acid, propionic acid and vanillic acid. The minimun inhibitory concentration(MIC) of propionic acid was 5,000 ppm in E. coli O157:H7, 2,500 ppm in Salmonella Enteritidis and Listeria monocytogenes. MIC of citric acid was 10,000 ppm in E. coli O157:H7 and Salmonella Enteritidis, 2,500 ppm in Listeria monocytogenes. MIC of acetic acid was 2,500ppm, while in vanillic acid was 5,000 ppm in Escherichia coli O157:H7, Salmonella Enteritidis, and Listeria monocytogenes. MIC of combined organc acid in E. coli O157:H7 were 2,500ppm in PC, 1,250 ppm in PA, PV, CA, CV and AV. MIC of combined organc acid in Salmonella Enteritidis were 2,500 ppm in PC, PA, PV, CA, and CV, 1,250 ppm in AV. MIC of combined organc acid in Listeria monocytogenes were 1,250 ppm in all treatment group. MIC of combined treatment of three organc acid in E. coli O157:H7, S. Enteritidis and L. monocytogenes were 1,250 ppm in PCA, PCV, PAV and CAV. The inhibitory effect of organc acid in E. coli O157:H7, S. Enteritidis and L. monocytogenes could be confirmed from the result of this experiment. Therefore, it was expected that the food process would increase or maintain by using organic acid.

Survival of Escherichia coli O157:H7 and Salmonella ser. typhimurium in Fermented Milk Products (발효유제품내에서 Escherichia coli O157:H7과 Salmonella ser. typhimurium의 생존)

  • Escherichia coli O157:H7 and Salmonella ser. typhimurium are pathogens involved in food poisoning in numerous countries. This study aimed to obtain knowleges on the survival of E. coli O157:H7 KSC 109 and S. ser. typhimurium ATCC 14028 in fermentedmilk products which were on sale in Suwon Yakult supplier. To the final concentration of 103~104 cfu/$m\ell$ of E. coli O157:H7 KSC 109 or S. wer. typhimurium ATCC 14028 in the fermented milks, Metchnikoff, Ace, Yakult, Mastoni and Super 100 were inoculated with these pathogens and then were stored at 4$^{\circ}C$ and viable cells of these pathogens were periodically counted. The results showed that the survival of two pathogens differed in the different types of fermented milks tested. Number of suriviving E. coli O157:H7 KSC 109 and S. ser. typimurium ATCC 14028 cells (initial inoculum, 103~104 cfu/$m\ell$) were decreased to 101, 102 cfu/$m\ell$ in Ace after 100 hours, and were decreased gradually to 101 cfu/$m\ell$ in Yakult after 250 hours. In the other fermented milks, viable cells of E. coli O157:H7 KSC 109 was not drastically decreased but those of S. ser. typhimurium ATCC 14028 was decreased gradually to 102 (Mastoni), and to 101 cfu/$m\ell$ (Super 100) after 250 hours. It appeared that S. ser. typhimurium ATCC 14028 was more susceptible than E. coli O157:H7 KSC 109 at low pH. Vibale cells of E. coli O157:H7 KSC 109 was not drastically decreased in most of fermented milks tested except Ace and Yakult, but in general, S. ser. typhimurium ATCC 14028 was drastically decreased in most of the fermented milks. The major ingibition factor against these pathogens in the fermented milks during storage at 4$^{\circ}C$ appeared to be the acidity and the metabolites produced by the starters bacteria used in fermented milk products.

  • Purpose : Escherichia coli O157 can produce diarrhea as well as hemorrhagic colitis and hemolytic uremic syndrome. In many parts of North America, E. coli O157 often is the second or third most commonly isolated enteric bacterial pathogens. Recently, intakes of fast food, including hamburgers have increased in Korea. Therefore, E. coli O157 infection in Korea are likely to be increased. Methods : Stool samples from 317 pediatric diarrheal patients were analyzed by culture on sorbitol-MacConkey agar. Sorbitol-negative colonies were teated by E. coli O157 latex agglutination test. Results : Of the 317 specimens, one (0.3%) were E. coli O157:NM that not produced Shiga toxin. The 7 year old male patient who had complained of abdominal pain, vomiting and non-bloody diarrhea for 2 days. The patient was improved for 2 days after admission. Conclusions 1 Only one (0.3%) of all fecal samples were isolated E. coli O157 that not produced Shiga toxin. Therefore, routine stool culture for the isolation of E. coli O157 was not likely to be neccessary so far.

  • The emergence of extended spectrum ${\beta}$-lactamase producing bacteria is causing very serious problems in Korea. Although there have been many reports about these bacteria Isolated from patients and clinical specimens, there is no report of extended spectrum ${\beta}$-lactamase producing organisms isolated from natural environment in Korea. This study was conducted to investigate the biological characteristics and extended spectrum ${\beta}$-lactamase types of eighteen strains of extended spectrum ${\beta}$-lactamase producing Klebsiella pneumoniae and Escherichia coli isolated from a slaughterhouse in Pusan in Korea during 2002 to 2004. Extended spectrum ${\beta}$-lactamases were identified by double-disk synergy test, conjugation, isoelectric focusing values and gene sequencing. Eight strains of Klebsiella pneumoniae and two strains of Eschericha coli were isolated kom pigs and transferred extended spectrum ${\beta}$-lactamase genes to recipient Escherichia coli J53 (sodium azide resistant and ceftazidime senstive) strain by conjugation. The conjugants of extended spectrum ${\beta}$-lactamase genes were alignments and translated to amino acids by BCM and NCBI blast. Eight conjugants of Klebsiella pneumonae were typed TEM-52, and two strains of Escherichia coli, SHV-12, but CMY-1 type were not detected in this study.

Escherichia coli-Derived Uracil Increases the Antibacterial Activity and Growth Rate of Lactobacillus plantarum

  • Lactobacillus plantarum (L. plantarum) is a representative probiotic. In particular, L. plantarum is the first commensal bacterium to colonize the intestine of infants. For this reason, the initial settlement of L. plantarum can play an important role in determining an infant's health as well as their eventual health status as an adult. In addition, L. plantarum combats pathogenic infections (such as Escherichia coli (E. coli), one of the early pathogenic colonizers in an unhealthy infant gut) by secreting antimicrobial substances. The aim of this research was to determine how L. plantarum combats E. coli infection and why it is a representative probiotic in the intestine. Consequently, this research observed that E. coli releases uracil. L. plantarum specifically recognizes E. coli-derived uracil, which increases the growth rate and production of antimicrobial substance of L. plantarum. In addition, through the inhibitory activity test, this study postulates that the antimicrobial substance is a protein and can be considered a bacteriocin-like substance. Therefore, this research assumes that L. plantarum exerts its antibacterial ability by recognizing E. coli and increasing its growth rate as a result, and this phenomenon could be one of the reasons for L. plantarum settling in the intestine of infants as a beneficial bacterium.

Immunosensor for Detection of Escherichia coli O157:H7 Using Imaging Ellipsometry

  • Imaging ellipsometry (IE) for detection of binding of Escherichia coli O157:H7 (E. coli O157:H7) to an immunosensor is reported. A protein G layer, chemically bound to a self-assembled layer of 11-mercaptoundecanoic acid (11-MUA), was adopted for immobilization of monoclonal antibody against E. coli O157:H7 (Mab). The immobilization of antibody was investigated using surface plasmon resonance. To fabricate antibody spots on a gold surface, protein G solution was spotted onto the gold surface modified with an 11-MUA layer, followed by immobilizing Mab on the protein G spot. Ellipsometric images of the protein G spot, the Mab spot, and Mab spots with binding of E. coli O157:H7 in various concentrations were acquired using the IE system. The change of mean optical intensity of the Mab spots in the ellipsometric images indicated that the lowest detection limit was $10^3$CFU/ml for E. coli O157:H7. Thus, IE can be applied to an immunosensor for detection of E. coli O157:H7 as a detection method with the advantages of allowing label-free detection, high sensitivity, and operational simplicity.

Patterns of antibiotic resistance in Escherichia coli isolated from fresh and recycled poultry litter (깔짚 교체 및 재사용 육계농장 분리 대장균의 항생제 내성 양상)

  • The isolation rate of Escherichia (E.) coli in poultry litter was investigated at 44 broiler farms, 20 that used fresh litter and 24 that used recycled litter. The patterns of resistance to antibiotics of the E. coli isolates were compared. In litter sampled before the rearing period, the isolation rate of E. coli was higher at farms that used fresh litter; E. coli was present in the litter in 94.5% (35 out of 37 flocks tested) of the farms that used fresh litter vs. 51.2% (21 out of 41 flocks) of the farms that used recycled litter. The susceptibility of the 93 isolates of E. coli to 13 antibiotics was studied. Before the rearing period, E. coli isolates from the farms that recycled litter showed higher resistance rates than isolates from farms that replaced litter with fresh litter. Comparing the antibiotic resistance patterns of isolates from litter sampled before and at the end of the rearing period, the antibiotic resistance rates at the end of the rearing period increased dramatically compared with rates before the rearing period.

  • The N-acetylneuraminate lyase(NALase) from Escherichia coli was cloned and it was compared to that from Haemophilus influenzae and Clostridium perfringens. NALase from E. coli was expressed in very high level(about 6U/mg). The ManNAc Km value of three enzymes was almost the same. Pyruvate inhibited from H. influenzae was inhibited by GlcNAc in lower level than the others. The crude extract has about 30 times more activity than the cell for the substrate and product diffusion limit problem. The pH stability of three enzymes at pH 11 was also checked for its importance in the direct synthesis of Neu5Ac from GlcNAc and pyruvate at high alkaline condition.

Combination Effects of Benzoate, Sorbate and pH for Control of Escherichia coli O157:H7 (Escherichia coli O157:H7의 제어를 위한 Benzoate, Sorbate 및 pH의 병용처리 효과)

  • Effects of benzoate (0~0.6 g/$\ell$ ) and sorbate (0~2.0 g/$\ell$) on the growth of Escherichia coli O157:H7 in tryptic soy broth at various pH levels (4~8) and temperatures (4$^{\circ}C$, 37$^{\circ}C$) were investigated. Benzoate and sorbate were inhibited the growth of E. coli O157:H7 up to 12 hours cultivation at 4$^{\circ}C$, and 2.0 g/$\ell$ sorbate was only inhibited during 48 hours cultivation at 37$^{\circ}C$. Among the pH levels tested, pH 4 showed significant inhibitory effect against the E. coli O157:H7 on 4$^{\circ}C$ and at 37$^{\circ}C$, respectively. When used in combination 0.2 g/$\ell$ benzoate and sorbate were completely inhibited the growth of E. coli O157:H7 on pH 4 and at 37$^{\circ}C$. While on pH 5 at 4$^{\circ}C$, all of the concentration tested did not exert any inhibitory effect. The combined effects were retarded more than single treatment of E. coli O157:H7.
