Effects of Massage Therapy on Growth Parameters of Orphaned Infants during First Six Months of Life (마사지 요법이 시설 영아의 신체발달에 미치는 효과 - 생후 6개월간의 결과를 중심으로 -)

  • As social changes, there are growing number of instituted infants. Most of them were poor growth and developmental outcome due to situational and environmental vulnerability. Developmental interventions for this infants have been the subjects of debate for many years. The purpose of this study was to evaluate the effectiveness of massage intervention on the physical growth of orphaned infants during the first 6 month of life. This study utilized equivalent control group with repeated measuring by quasi experimental design. Fifty eight orphaned infants were randomly assigned to control(n=28), or an experimental(n=30) group. In addition to routine orphanage care, infants in the experimental group received 15 minutes of massage intervention twice a day, 5 days a week, for 4 weeks. Data has been collected from April 1st, 2000 to August 31st, 2002 and were analyzed using SPSS/PC(Version 10.0) with chi-square test, t-test, and repeated measures ANOVA to determine the effectiveness of massage intervention and to test the homogeneity of general characteristics between experimental and control group. The results were as follows; Compared to the control group, the experimental group has gained significantly more weight(F=33.52, p < .0001) and had larger increases in length(F=10.04, p < .0001) and head circumference(F=49.78, p < .000) after the 4-week intervention period and at 6 months of age. These data demonstrate that massage intervention may be effective in facilitating growth for newborn infants placed in orphanages during first 6 months of life.

Live birth outcomes of vitrified embryos generated under growth hormone stimulation are improved for women categorized as poor-prognosis

  • Objective: To determine the clinical pregnancy (CP) and live birth (LB) rates arising from frozen embryo transfers (FETs) that had been generated under the influence of in vitro fertilization (IVF) adjuvants given to women categorized as poor-prognosis. Methods: A registered, single-center, retrospective study. A total of 1,119 patients with first FETs cycle include 310 patients with poor prognosis (109 treated with growth hormone [GH], (+)GH group vs. 201 treated with dehydroepiandrosterone, (-)GH group) and 809 patients with good prognosis (as control, (-)Adj (Good) group). Results: The poor-prognosis women were significantly older, with a lower ovarian reserve than the (-)Adj (Good) group, and demonstrated lower chances of CP (p< 0.005) and LB (p< 0.005). After adjusting for confounders, the chances of both CP and LB in the (+)GH group were not significantly different from those in the (-)Adj (Good) group, indicating that the poor-prognosis patients given GH had similar outcomes to those with a good prognosis. Furthermore, the likelihood of LB was significantly higher for poor-prognosis women given GH than for those who did not receive GH (p< 0.028). This was further confirmed in age-matched analyses. Conclusion: The embryos cryopreserved from fresh IVF cycles in which adjuvant GH had been administered to women classified as poor-prognosis showed a significant 2.7-fold higher LB rate in subsequent FET cycles than a matched poor-prognosis group. The women with a poor prognosis who were treated with GH had LB outcomes equivalent to those with a good prognosis. We therefore postulate that GH improves some aspect of oocyte quality that confers improved competency for implantation.

  • In this paper I attempted to investigate the matters related with the clarification of the close relationship between writing system and pronunciation. On the way of pursuing the research on the subject I found the fact that the same topic has been the main academic target in Korea. There have been some remarks about English alphabets and pronunciation. Nevertheless, the relation between alphabet codes and pronunciation tokens wasn't considered as the main key to master the English pronunciation correctly and completely. As the main target of this paper I argue that it is necessary to comprehend the connection. Then, we can recognize the significant role of alphabetic structure for understanding the gist of pronunciation exercise. This paper is classified into four parts. Each part consists of the material to affirm the fact that writing system should be the inevitable equivalent of sound system, and vice versa. In the first section I show that the development of the way of pronouncing English words is closely related with the endeavors of the scholars. While performing the survey of the studies about the alphabetic structure of the age many scholars found that the spelling construction was recorded without any common denominator. Thus, they not only sought to stage the bedrock for the standard written form of words but also to associate the alphabet letters with phonetic features. Secondly I mention the negative aspect of the 'only spelling based English pronunciation education' for the educational goal of 'Phonics methodology.' In this part I suggest the essentiality of phonemic properties with the phonetic prospect: phonemic awareness. Thirdly I refer to the standardization of the spelling system of English. As the realm of application of the language is extended toward the various professional areas such as commercial, scientific, and cultural spheres, it is quite natural to assume that the usage of the language will be transformed according to the areas in the world. Fourthly I introduce the first English-Korean grammar book with the section of 'the introduction to English pronunciation.' At the chapter the author explained the sound features of English based on the regulation of 'Scientific Alphabet' of U.S.A. In the transcribing system all the symbols were postulated on the basis of the English alphabet form instead of the separate phonetic signs of IPA.

Effects of Life Review-Narrative Group Therapy Program to Increase Ego-Integrity in the Elderly (생애회고적 이야기치료 집단프로그램이 노인의 자아통합감 증진에 미치는 효과)

  • The purpose of this study was to verify the effectiveness of a life review-narrative group therapy program aimed at increasing ego-integrity in the elderly. The life review-narrative group therapy program was organized according to the tfe course from childhood to old age. The objectives of the program were for the elderly to enhance their self-worth, become aware of their individual self existence, accept aging and death, accept the past and present life, and to make the most of their experiences of overcoming difficulties. The counseling techniques used in the program were mainly reminiscence and re-authoring of the narrative therapy. Thirty-two elderly people aged 60 years and over (16 in each of experimental and control groups) were selected randomly from the Daegu Metropolitan City General Welfare Center for the research. The two groups were identified as equivalent for the study in a pre-test. The program of the experimental group was implemented twice a week for 90 minutes per session over a 6-week period (Sep. 8th - Oct. 14th. 2005). The pre (Sep. 8th 2005), the post (Oct. 14th. 2005) and follow-up (Nov. 14th. 2005) tests were implemented in ordo, to verify the effectiveness of the program. The instruments used in the study were the Ego-Integrity Scale. The data were analyzed using t-test, and Ore-Factor Repeated Measures ANOVA. The findings of the study were as follows. The experimental group evidenced significant effects on enhancing ego-integrity. The result of the post hoc multiple comparisons showed that the effects of the life review-narrative group therapy Program were maintained for one months. Meanwhile the control group did not show any significant change in ego-integrity. Based on these findings, the limitations of the study were discussed, and further research was suggested.

The Effect of Mother-fetus Interaction Promotion Program of Talking and Tactile Stimulation on Maternal-fetal Attachment (태담과 촉각 자극의 모-태아 상호작용 증진 프로그램이 모-태아 애착에 미치는 효과)

  • The purpose of this study is to investigate the effect on mother-fetus attachment through mother-fetus interaction promotion program of talking and tactual stimulation aimed at enhancing sensitivity of primipara Non-equivalent control group posttest design was used. Fifty primiparas (26 mothers for intervention group and 24 mothers for control group) were recruited from three general hospital and an OBGY clinic located in Gang Nung city. Data was collected from January 30th to December 20th in 2001. For the intervention group, programed education which focused on mother-fetus interaction promotion of talking and tactile stimulation in the 1st trimester. Telephone counselling was provided with interval of two weeks. For two groups, home visiting for data collection of mother-fetus attachment was conducted at 36 to 38 weeks of gestational age. The data were analyzed using chi-square test and Fisher' Exact test to test the equivalence of two groups, and the effect of intervention program was determined with t- test. The result was as follow: The significant difference was found in mother-fetus attachment between two groups(t= 2.772, P= 0.0079). It indicated that intervention program was effective in improving mother-fetus attachment. In conclusion, this study has shown that the applied nursing intervention aimed at enhancing sensitivity of primiparas to fetus's movement promoted mother-fetus attachment. Therefore, this study suggests that this nursing intervention to increase maternal sensitivity to the fetus's movement should be broadly applied to primiparas, which can be beneficial for formation of mother-infant relationship, and for promotion of the social, affective, and cognitive developments of their children.

Comparison of Single Agent Gemcitabine and Docetaxel in Second-Line Therapy for Advanced Stage Non-Small Cell Lung Cancer in a University Hospital in Turkey

  • Purpose: To compare the efficacy and toxicity of gemcitabine versus docetaxel in a second-line setting of nonsmall cell lung cancer (NSCLC) patients previously treated with platin-based combination chemotherapy. Materials and Methods: We retrospectively evaluated the medical records of 57 patients treated with single agent gemcitabine or docetaxel in second-line setting of advanced NSCLC who received one prior platinum-based therapy. Results: The mean age was $56.7{\pm}8.39$ years with 55 (96.5%) males and two (3.5%) females. Forty of them received docetaxel and 17 gemcitabine. The mean number of chemotherapy cycles was $6.8{\pm}4.0$ in the gemcitabine group, while it was $4.6{\pm}3.0$ in the docetaxel group. Overall response rates were 8% and 12% (P=0.02) for gemcitabine and docetaxel, respectively. The median survival time was 22 versus 21 months for gemcitabine and docetaxel, respectively. The median times to progression were 8 and 5 months. There was no difference between the two groups in terms of incidence of adverse affects (40% vs 47.1%). All of the hematological side effects were grade 1/2. No major toxicity was encountered necessitating stopping the drug for either group. Conclusions: Treatment with gemcitabine demonstrated clinically equivalent efficacy with a significantly improved safety profile compared with those receiving docetaxel in the second-line setting for advanced NSCLC in this study. Based on these results, treatment with gemcitabine should be considered a standard treatment option for second-line NSCLC.

A Petrological Study on the Southwestern Contact Zone of the Boeun Granodiorite, Ogcheon Zone (보은화강섬록암(報恩花崗閃綠岩) 서남부(西南部) 접촉대(接觸帶)에 관(關)한 암석학적(岩石學的) 연구(硏究))

  • Southwestern contact zone of the Boeun granodiorite occurs near the thrust fault between the Ogcheon Group and Majeonri Limestone Formation. Ogcheon Group, metasediments composed of the Munjuri Formation, Changri Formation, and unconformably overlying Hwanggangri Formation, belongs to greenschist facies of regional metamorphism accompanied with deformation of two fold axes, $N10^{\circ}E$ and $N45-65^{\circ}E$ directions. Basic metamorphic rocks occurring in the Changri and Limestone Formations are the meta-basalts and meta-diabases of tholeiitic basalt series. The meta-basalts intruded in the Changri Formation as sills, whereas the meta-diabases in the Changri and Limestone Formations as stocks in appearance. They are considered to have emplaced before the formation of two fold axes and related with the thrust fault, based on the geologic setting of the area. The metamorphic facies are identified to be greenschist facies to epidote-amphibolite facies for the meta-basalt, and epidote-amphibolite facies for the meta-diabases. It is interpreted that such a variety of facies was related from the combination of earlier deuteric alteration and later regional metamorphism. The metasediments in southwestern contact zont of the Boeun granodiorite which is a product of later syntectonic intrusion of middle Jurassic in age, show pyroxene-hornfels facies near the contact and amphibole-horenfels facies away from the contact to the mineral zoning in the contact metamorphic aureole of the Limestone Formation, based on the paragenetic analysis of mineral assemblages. The Limestone in the area appears to be considerably $SiO_2-CaO-MgO-CO_2-H_2O$ can be adopted to evaluate equilibrium conditions of the mineral assemblages in each mineral zone. It is revealed that a temperature gradient was existed accross the contact aureole ranging from the higher igneous side to lower sedimentary side, whereas no clear trend of $XCO_2$ variation appears but high mole fraction. The tremolite diopside-quartz-calcite assemblages occurs in common through the most mineral zones of contact aureole that is in good agreement with the equivalent reaction curve which extends over a wide range of $T-XCO_2$ conditions.

Microstructural, Mechanical, and Durability Related Similarities in Concretes Based on OPC and Alkali-Activated Slag Binders

  • Alkali-activated slag concretes are being extensively researched because of its potential sustainability-related benefits. For such concretes to be implemented in large scale concrete applications such as infrastructural and building elements, it is essential to understand its early and long-term performance characteristics vis-a'-vis conventional ordinary portland cement (OPC) based concretes. This paper presents a comprehensive study of the property and performance features including early-age isothermal calorimetric response, compressive strength development with time, microstructural features such as the pore volume and representative pore size, and accelerated chloride transport resistance of OPC and alkali-activated binder systems. Slag mixtures activated using sodium silicate solution ($SiO_2$-to-$Na_2O$ ratio or $M_s$ of 1-2) to provide a total alkalinity of 0.05 ($Na_2O$-to-binder ratio) are compared with OPC mixtures with and without partial cement replacement with Class F fly ash (20 % by mass) or silica fume (6 % by mass). Major similarities are noted between these binder systems for: (1) calorimetric response with respect to the presence of features even though the locations and peaks vary based on $M_s$, (2) compressive strength and its development, (3) total porosity and pore size, and (4) rapid chloride permeability and non-steady state migration coefficients. Moreover, electrical impedance based circuit models are used to bring out the microstructural features (resistance of the connected pores, and capacitances of the solid phase and pore-solid interface) that are similar in conventional OPC and alkali-activated slag concretes. This study thus demonstrates that performance-equivalent alkali-activated slag systems that are more sustainable from energy and environmental standpoints can be proportioned.

The Effect of Education for Prevention of Osteoporosis Patients with Bone Fracture (골다공증교육이 골절환자의 지식과 생활양식변화 수행에 미치는 영향)

  • The purpose of the study was to identify the effect of structured patients education had on prevention of osteoporosis, with fracture and the resulting of life style changes in patients. In this study, a non equivalent control group pre and a post test design was employed. Data were collected through an interview process using questionnaires from April to December of 1999. The subjects, consisting of 59 patients with fractures and over 40 years of age, were diagnosed in K University Hospital. This study tested the patients knowledge at three times. The times were before the program 2 weeks into the program, and 6 months after education program. Life style change related to prevention of osteoporosis was shown twice (before and 6 months after the education program) in the experimental group, and control group went without it. The instruments used for this study were developed by literature review according to a reliability test. Data was analyzed using X2 test and t test to determine similarities between the experimental and control groups. The hypothesis was tested using repeated measures of ANOVA, t-test and Pearson correlation coefficients. The results of the study were summarized as follows: 1. The first hypothesis was accepted: a higher level of knowledge about osteoporosis was found in experimental groups who received education than to the control group during the period (F=19.82, p=.0001). 2. The second hypothesis was accepted: a higher level of life style changes about osteoporosis on experimental group were recorded than as compared to control group (t=3.55, p=.001). 3. The third hypothesis was accepted: the higher the knowledge about osteoporosis the higher the level of performance of life style changes about prevention of osteoporosis (r=.600, p=.0001). In conclusion, structured patient education in patient with fractures improved the level of knowledge about osteoporosis and more likely undergo of life style changes 6 months after the education program. Also reeducation would be needed 6 months after program ends. That is structured patient education in pamphlet form would be very effective in nursing intervention that may to result in life style changes. Therefore further research is needed to reinforce the education material and to generalize the education effect.

The Effect of Dance Therapy on Pulmonary and Cognitive Function in the Elderly (무용요법이 노인의 폐기능과 인지기능에 미치는 효과)

  • This study was done to explore the effects of dance therapy on pulmonary and cognitive functions in the elderly. The design of this study was a non-equivalent pre-post test experiment. The subjects consisted of elderly persons living in a facility located in Kyoungi-Do. Fifty eight subjects had normal cognition, sensory function and resting blood pressure. They underwent tests of pulmonary and cognitive function as baseline data before dance therapy, and at 6th week and at the end of 12nd week after following dance therapy. Twenty seven elderly persons were assigned to the experimental group and participated with the dance therapy. This therapy was based on the Marian Chace's dance therapy and Korean traditional dance with music. The dance therapy consisted of 50 minutes session, 3 times a week for 12 weeks. One session consisted of warming-up, expression, catharsis, sharing and closing stage. the intensity of the dance therapy was at the 40% of age-adjusted maximum heart rates. Data were analyzed with mean, standard deviation, Chi-square test, unpaired t-test, repeated measures ANOVA, and Bonferroni multiple regression using SAS program. The results were as follows : 1. Pulmonary function(forced expiratory volume at one second and forced vital capacity) of the experimental subjects significantly increased over time more than that of the control subjects. 2. The experimental group had significantly higher score for pulmonary function than the control group at the 12nd week after dance therapy. 3. Cognitive function of the experimental subjects significantly increased over time more than that of the control subjects. 4. The experimental group had significantly higher score for cognitive function than the control group at the 6th week and 12nd week after dance therapy. The findings showed the dance therapy could be effective in improving the pulmonary and cognitive function of the elderly.
