The Synergistic Effect of Juglans sinensis, Psoralea corylifolia, and Their Combination on the Asthmatic Murine Model (천식모델을 이용한 호도, 파고지 및 두 배합약물의 실험적 연구)

  • Objectives : To clarify the possible effect of JS (Juglans sinensis), PCF (Psoralea corylifolia L.), and J+P(JS+PCF), we examined their influence on the development of pulmonary eosinophilic inflammation in the asthmatic murine model. Methods : All mice were immunized on two different days (21 days and 7 days before inhalational exposure) by intraperitonial injections of 0.2 ml alum-precipitated Ag containing 100 ${\mu}$g of OVA bound to 4 mg of aluminum hydroxide in PBS. Seven days after the second sensitization, mice were exposed to aerosolized ovalbumin for 30 minutes/day on 3 days/week for 8 weeks (at a flow rate of 250 L/min, 2.5% ovalbumin in normal saline) and, JS, PCF and J+P (200 mg/kg, 400 mg/kg) were orally administered 3 times per week for 8 weeks. Results : The suppressive effects of JS, PCF, and J+P were demonstrated by the accumulation of eosinophils into airways, with the reduction of eosinophils and lung leukocytes. These were correlated with the marked reduction of IL-4, IL-5, IL-13 levels in the BALF and serum. OVA-specific IgE levels were also decreased in serum and BAL from these mice. And also JS, PCF, and J+P decreased eosinophilic CCR3 and CD11b expression in lung tissue. Conclusions : These results indicate that JS, PCF, and J+P have deep inhibitory effects on airway inflammation and hyper-responsiveness in the asthmatic murine model. The suppression of IL-5, IgE, and eosinophilils and the increase of IFN-${\gamma}$ production in BALF seem to contribute to these effects. Specially, esosinophils and TNF-a in J+P combination group were significantly reduced in BALF and lung tissue. Hence, the results indicated that JS, PCF, and J+P could act as an immuno-modulator which possesses anti-inflammatory and anti-asthmatic property by modulating the imbalance between Th1 and Th2 cytokines.

  • Eosinophils play critical roles in the maintenance of homeostasis in innate and adaptive immunity. Although primarily known for their roles in parasitic infections and the development of Th2 cell responses, eosinophils also play complex roles in other immune responses ranging from anti-inflammation to defense against viral and bacterial infections. However, the contributions of pattern recognition receptors in general, and NOD-like receptors (NLRs) in particular, to eosinophil involvement in these immune responses remain relatively underappreciated. Our in vivo studies demonstrated that NLRC4 deficient mice had a decreased number of eosinophils and impaired Th2 responses after induction of an allergic airway disease model. Our in vitro data, utilizing human eosinophilic EoL-1 cells, suggested that TLR2 induction markedly induced pro-inflammatory responses and inflammasome forming NLRC4 and NLRP3. Moreover, activation by their specific ligands resulted in caspase-1 cleavage and mature IL-1β secretion. Interestingly, Th2 responses such as secretion of IL-5 and IL-13 decreased after transfection of EoL-1 cells with short interfering RNAs targeting human NLRC4. Specific induction of NLRC4 with PAM3CSK4 and flagellin upregulated the expression of IL-5 receptor and expression of Fc epsilon receptors (FcεR1α, FcεR2). Strikingly, activation of the NLRC4 inflammasome also promoted expression of the costimulatory receptor CD80 as well as expression of immunoregulatory receptors PD-L1 and Siglec-8. Concomitant with NLRC4 upregulation, we found an increase in expression and activation of matrix metalloproteinase (MMP)-9, but not MMP-2. Collectively, our results present new potential roles of NLRC4 in mediating a variety of eosinopilic functions.

  • Background : Asthma and eosinophilic bronchitis(EB) are eosinophilic inflammatory diseases of the airway. However, EB differs from asthma in that there is no variable airway obstruction or airway hyper-responsiveness. Pathologically, asthma is characterized by the accumulation of eosinophils and CD4+ T lymphocytes in the submucosa. A recent study showed that there was no significant difference between asthma and EB in terms of the submucosal eosinophil and T lymphocyte count. However, it is not known whether or not an infiltration of CD4+ and CD8+ T lymphocytes occurs in the airways of EB patients. The aim of this study was to identify the difference between the two conditions by measuring the submucosal CD4+ and CD8+ T lymphocyte count. Methods : Immunohistochemical analysis of bronchial-biopsy specimens was performed in 17 subjects with asthma and 24 subjects with EB. Results : The CD4+ T lymphocytes count in the asthma subjects and the EB subjects was similar (median, 58.6 vs 50.0 $cells/mm^2$, respectively; P=0.341). In contrast, the number of CD8+ T lymphocytes in the EB subjects was higher than that in the asthma subjects (median, 46.7 vs 11.8 $cells/mm^2$, respectively; P=0.003). Conclusion : The infiltration of submucosal CD8+ T lymphocytes may be associated with the pathophysiology of EB.

  • Laryngeal amyloidosis is a rare benign disease. It is characterized by extracellular deposition of homogeneous and eosinophilic protein material in the form of fibrils. Diagnosis of this disease is made by histologic examination of involved tissue. It is ususlly primary or localized but rarely associated with a systemic or generalized disease. We present a case of primary laryngeal amyloidosis that was treated with KTP 532 laser vaporization.

  • Langerhans cell histiocytosis (LCH), previously called histiocytosis X, refers to a spectrum of disease characterized by idiopathic proliferation of histiocytes producing focal or systemic manifestations. Causes and pathogenesis remain unclear The basic histopatholgic fndings are identical in the three well-established clinical syndromes (eosinophilic granuloma, Hand-Shuller-Christian disease, Letterer-Siwe disease). The disease has a predilection for children, although it may occur in adults. We experienced two cases of adult onset Langerhans cell histiocytosis in the ribs. One case was associated with diabetes insipidus and the other case had a solitary lesion.

  • The melanotic neuroectodermal tumor of infancy is a rare neoplasm arising in the first year of life. A 33-day-old female infant had an enlarged alveolar ridge on the right anterior maxilla. Intraoral examination revealed a nonulcerative swelling at the site. An intraoral radiograph showed an ill-defined radiolucency on the right anterior maxilla and displacement of primary incisors from their alveolar sockets. CT scans revealed an expansion of the surrounding bone and partial destruction of the anterior wall of the premaxilla. Histopathologic examination showed the cytoplasm of neuroblastic cells and eosinophilic, epithelioid cells frequently contained a dark brown granular pigment that stained positively to vimentin and HMB45, focally positive to NSE and cytokeratin. Four weeks after the operation, CT scans showed a rapidly growing soft tissue mass occupying right maxillary sinus encroaching to the orbit and nasal cavity. The final diagnosis was made as a malignant melanotic neuroectodermal tumor of infancy.

  • Warthin's tumor is composed of glandular and often cystic structures with a papillary cystic arrangement, lined by characteristic eosinophilic epithelium. The stroma contains lymphoid tissue. Between 5 and 10 percent of all parotid gland tumors are Warthin's tumor and occurs almost exclusively in the parotid gland. This tumor is much more common in men than in women (5:1). Histologically it is composed of epithelial and lymphoid tissue. The treatment of this tumor is surgical excision and seldom recur after removal. Recently, We had experienced a case of warthin's tumor of the parotid gland in a 66 year old male treated with surgical removal.

  • Pulmonary alveolar proteinosis(PAP) is a rare disease in which the alveolar spaces are filled with an eosinophilic, PAS-positive material, whereas the interstitial architecture of the lung usually remains unaffected. Although a definitive diagnosis is usually made by an open lung biopsy, bronchoalveolar lavage(BAL) cytology may play a decisive role in the diagnosis and therapy of these patients and may spare a patient a more invasive diagnostic procedure. The author presents a patient in whom BAL cytology specimen contained the characteristic globules of amorphous proteinaceous PAS-positive material accompanied by background of rare macrophages and inflammatory cells. Ultrastructural study using BAL specimen can confirm the diagnosis of PAP.

  • Throughout the studies the followings were obtained and summarized here. 1. Thickenings of plerua were due to the organized fibrin deposition on the pleural surface. 2. Thickenings of plerura were accompanied with increaseness of fibrous connective tissues in the interlobular and alveolar septa. 3. Eosinophilic inclusions seemed to LCL bodies were observed in the cytoplasms of epithelial cells of bronchi, bronchial glands and in the cytoplasms of mononuclear cells. 4. Histo-pathologically, there were some evidences influenced by viruses and the organisms of Psittacosis-Lymphogranuloma Venereum Group.

Oxyphilic Papillary Carcinoma of the Thyroid in Fine Needle Aspiration (갑상선의 호산성 유두상 암종의 세침흡인 세포학적 소견)

  • Oxyphilic (Hurthle) cells have abundant eosinophilic granular cytoplasms and occur in both benign and malignant neoplasms of the thyroid gland. Most published studies described mainly oxyphilic tumors of follicular type, and reports on oxyphilic papillary thyroid carcinomas are rare. The oxyphilic variant of papillary carcinoma differs from the classic papillary carcinoma in its more aggressive biological behavior. We report a case of oxyphilic papillary carcinoma exhibiting exclusively oxyphilic cells in a follicular pattern without papillary structures. The pattern of papillary carcinoma was confirmed by fine needle aspiration material and lymph nodal metastasis, both of which revealed typicaled papillary structures with characteristic nuclear features, psammoma bodies, and oxyphilic cytoplasm.
