Pediatric Kidney Transplantation

  • Pediatric kidney transplantation is the best option since it can achieve near normal glomerular filtration rate, adequate fluid balance, and autonomic endocrine function of the kidney in end-stage kidney disease. However, pediatric kidney transplantation is difficult because children are developing and growing, management and complications of pediatric kidney transplantation are different from those of adults. This review covers the current status of pediatric kidney transplantation in Korea, key considerations that must be taken before kidney transplantation in children, and management strategy of immunosuppression and common complications.

  • A 7 month old male shepherd was presented with anorexia, diarrhea, and salivation, and euthanized due to no clinical improvement. Grossly, ulcers were seen on ventral surface of tongue and stomach. The mineralization was obvious in the intercostal space, lung and inner surface of aorta. Kidney was pale, firm and irregular. Histopathology confirmed uremic pneumonitis, end stage kidney and hyperplasia of parathyroid. This case was the typical uremic condition of juvenile dog, which was characterized the diffuse soft tissue mineralization due to hyperparathyroidism secondary to chronic renal failure.

IgA 신증에 의한 말기 신질환에 대한 증례보고 (End Stage Renal Disease caused by IgA Nephropathy : One Case Report)

  • This case is to report the effect of renal function of chronic kidney disease(CKD) caused by IgA nephropathy. A 37-year-old man visited a Korean medicine hospital, who has been diagnosed with end stage renal disease(ESRD), 5 stage of CKD, caused by IgA nephropathy, has had no improvement of western medical treatment, and wanted to be treated using Korean medicine before renal transplantation. The decrease of creatinine value, the increase of glomerular filtration rate(GFR), and the decrease of CKD stage (5 to 4) was observed after combination treatment of Ikkigeonbiisuhwalhyeoltang and saam acupuncture was applied. This case report is suggested that combination treatment of acupuncture and herbal medicine could be effective to renal function of CKD in spite of a single case.

소아의 복막투석 (Peritoneal dialysis in children and adolescents)

  • Peritoneal dialysis is a preferred modality of replacement therapy in children and adolescents with end-stage renal disease waiting for kidney transplantation. Recent development of pediatric swan-neck catheters with cuffs, novel dialysis solutions, and cyclers for automated peritoneal dialysis enabled more flexible prescriptions of dialysis with less complication, and improved patients' activities as well as the dialysis adequacy. Principles and practical issues of chronic peritoneal dialysis in children and adolescents are reviewed and utility of a web-based Korean Pediatric CRF Registry is explained.

Pediatric kidney transplantation is different from adult kidney transplantation

  • Kidney transplantation (KT) is the gold standard for renal replacement therapy in pediatric patients with end-stage renal disease. Recently, it has been observed that the outcome of pediatric KT is nearly identical to that in adults owing to the development and application of a variety of immunosuppressants and newer surgical techniques. However, owing to several differences in characteristics between children and adults, pediatric KT requires that additional information be learned and is associated with added concerns. These differences include post-KT complications, donor-recipient size mismatch, problems related to growth, and nonadherence to therapy, among others. This review was aimed at elucidating the clinical characteristics of pediatric KT that differ from those observed in adults.

  • Renal transplantation is the treatment of choice for children with end-stage renal disease. The outcome of pediatric kidney transplantation has improved dramatically in recent years, with lower acute rejection rates, superior graft survival, and low mortality. These improvements have allowed increased attention to other aspects of care for long-term survivors. Taking this into consideration, this review article will focus on the key issues related to pediatric kidney transplantation such as growth, neurocognitive function, nonadherence, and posttransplantation infectious complications, including lymphoproliferative disease, to broaden the understanding of pediatricians who provide pre-and postoperative care to children with end-stage renal disease.

A rare case of childhood-onset systemic lupus erythematosus associated end-stage renal disease with cerebral abscess and hemorrhage

  • Systemic lupus erythematosus (SLE) is a chronic autoimmune inflammatory disease that affects multiple organs. More than half of the patients with SLE have kidney involvement, and up to 10% of patients with lupus nephritis develop end-stage renal disease (ESRD). Central nervous system (CNS) involvement in SLE occurs in 21% to 95% of patients. Severe neurological manifestations such as seizures, cerebrovascular disease, meningitis, and cerebrovascular accidents can develop in childhood-onset SLE, but cerebral infections, such as brain abscess and hemorrhage, are seldom reported in lupus nephritis, even in adults. Here, we report a rare case of childhood-onset SLE with ESRD, cerebral abscess, and hemorrhage. A 9-year-old girl diagnosed with lupus nephritis was administered high-dose steroids and immunosuppressant therapy to treat acute kidney injury (AKI) and massive proteinuria. The AKI deteriorated, and after 3 months, she developed ESRD. She received hemodialysis three times a week along with daily peritoneal dialysis to control edema. She developed seizures, and imaging showed a brain abscess. This was complicated by spontaneous cerebral hemorrhage, and she became unstable. She died shortly after the hemorrhage was discovered. In conclusion, CNS complications should always be considered in clinical practice because they increase mortality, especially in those with risk factors for infection.

  • A 3-year-old male Maltese dog(weighing 2.5 kg) was referred with an excessive movement of mandible and depression. Comprehensive diagnostic investigation revealed an end-stage of renal disease with secondary hyperparathyroidism. The renal allograft was performed after the condition of dog was stabilized by hemodialysis and medical treatment. After transplantation, the renal function of this dog was returned to normal. thereafter, the dog died suddenly without apparent clinical signs. The subsequent pathological studies revealed the actual etiology of death was not clearly identified However acute pancreatitis caused by abrupt introduction of food after prolonged luting might be involved in the etiology. This case study showed the necessity of pancreatic function test in postoperative management after renal transplantation.

말기 신부전 환자에서 혈액투석 치료가 성대 및 강도의 변화에 미치는 영향 (The Influence of Vocal Cords and Intensity by Hemodialysis in End Stage Renal Disease)

  • 신장 질환은 초고령화 사회로 접어들고 있는 현대 사회에서 발병율이 매우 높은 질환이다. 이러한 신장 질환을 초기에 발견하면 약물이나 식이요법으로 막을 수 있지만 악화될 경우 인공투석이나 신장이식을 받아야 한다. 따라서 본 논문에서는 혈액투석에 따른 성대 및 강도의 변화율을 측정, 분석하여 신장과 음성과의 상관성을 분석하였다. 이를 위해 혈액투석 치료를 받는 피실험자를 대상으로 혈액투석 전과 후의 음성을 수집하고 음성 분석학적 요소의 적용을 통해 비교, 분석하였다. 즉, 신장에 이상이 있을 때 목소리가 가라앉는다는 동양의학적 진단 이론에 기반하여 혈액투석 치료 전의 음성과 치료 후의 음성을 각각 수집하고 수집된 음성 자료에 성대의 떨림과 강도 분석을 적용하여 혈액투석 치료 전과 후의 음성 변화를 측정, 분석하였다.

  • Diabetes is the leading cause of end-stage renal disease worldwide. In Korea, diabetic kidney disease accounted for 39% of all new dialysis patients in 1998. Diabetic nephropathy is characterized by glomerular hyperfiltration, albuminuria, and expansion of glomerular mesangium. Since glomerular mesangial cells regulate glomerular filtration rates and are capable of producing extracellular matrix (ECM) proteins, the functional abnormalities of mesangial cells under diabetic milieu play an important role in the development and progression of diabetic nephropathy.(omitted)
