  • The present study was undertaken to investigate the crystal growth onto enamel mineral and synthetic hydroxyapatite seeds in media resembling the enamel fluid composition. Effects of fluoride at low concentrations on the precipitation were also examined in a benchtop crystal growth model adopting a miniaturized reaction column. X-ray diffraction and Fourier transform infrared spectroscopy(FTIR), as well as chemical analyses, were employed for characterization of both seed materials before and after experimentation. Remarkable findings were that (1) both biological and synthetic seeds at the same total surface areas yielded rather similar precipitation rates at all levels of fluoride concentration in solution and (2) the precipitation rate was accelerated in a manner depending on fluoride concentrations in media. FTIR differential analysis disclosed that the precipitating phase was characterized as poorly crystallized apatite, which incorporated subtle carbonate. Most of the fluoride ions in soution were readily incorporated into crystals. The overall results support the view that the seeded crystal growth model is of value to gain insight into the mechanism of enamel crystal growth under fluoride regimens.

  • The purpose of this study is to evaluate the effect of dentin bonding agents on marginal leakage. V-shaped cavities were prepared on the faciocervical area of 140 extracted human teeth. In Groups of twenty cavities, they were restored as follow: Group 1 with enamel bonding resin and Silux, Group 2 Scotchbond and Silux, Group 3 enamel bonding resin and Heliomolar, Group 4 Dentin Adhesit and Heliomolar, Group 5 enamel bonding resin and Durafill, Group 6 Dentin Adhesive and Durafill, Group 7 Chembond. All specimens were thermocycled alternatively at $4^{\circ}C$ and $60^{\circ}C$ of 2% methylene blue dye solution, and sectioned faciolingually with diamond disk under water spray. The sectioned specimens observed with stereo microscope. Following results were obtained: 1. The groups filled with Scotchbond-Silux or Dentin Adhesit-Heliomolar had appeared lesser marginal leakage compared with the groups with enamel bonding resin-Silux or enamel bonding resin-Helimolar. 2. The group filled with Dentin Adhesive-Durafill did not show the reduction of the marginal leakage compared with the group with enamel bonding resin-Durafill. 3. There was significant difference among the four dentin bonding agents. Scotchbond showed the greatest marginal sealing ability, and Dentin Adhesit was the next. The marginal sealing ability of Dentin Adhesive was the worst.

  • Enamel microabrasion is a means by which superficial enamel discoloration is removed using hydrochloric acid and fine pumice. As enamel microabrasion alone may not be sufficient in cases of deeper discoloration, composite resin restoration is recommended in areas where there is remaining discoloration. The purpose of this study was to investigate the effects of different methods and number of applications of enamel microabrasion on the shear bond strength of composite resin to enamel. Untreated control was designated as group 1. 5-second applications of a mixture of 18% HCl and fine pumice were performed 5 and 10 times on groups 2 and 3, respectively. A commercially available mixture of 10% HCl and abrasives(PREMA) was applied using a 10 : 1 gear reduction handpiece 5 and 10 times on groups 4 and 5, respectively, with each application lasting 20 seconds. After etching with 37% phosphoric acid, composite resin was bonded. Thermocycling was performed and shear bond strength was measured. The following results were obtained : 1. Group 2 showed the highest bond strength$(24.36{\pm}3.34)$, while group 3 showed the lowest$(19.35{\pm}3.43)$, Shear bond strength decreased in the following order: 2>4>5>1>3. 2. Group 2 showed bond strength significantly higher compared to groups 1 and 3(p<0.05). 3. There were no significant differences between groups 2 and 3, which had been microabraded using HCl and pumice, and groups 4 and 5, to which PREMA had been applied, when bond strengths were compared(p>0.05). 4. When modes of fracture were examined, adhesive failure was observed in groups 3 and 4, while cohesive failure was observed in groups 1, 2, 3 and 4. Only mixed failures were found group 5. 5. When viewed using a SEM, groups 2 and 3, which had been microabraded using HCl and pumice, showed surface appearances similar to that of enamel etched with phosphoric acid. Groups 4 and 5, treated with PREMA, exhibited a smooth surface similar to that of group 1. All oops showed similar, typical surface characteristics following phosphoric acid etching.

  • Objective: The purpose of this study was to investigate the relationship between the enamel thickness of proximal surfaces and the morphologic features of mandibular incisors. Methods: Mesiodistal/faciolingual (MD/FL) index, MD width, and height of contour width/cervical width ratio were measured in 40 incisors extracted from Koreans. For determining the height of contour width/cervical width ratio, the cervical width was measured as the distance between proximal cementoenamel junctions. Then, the labial surface was ground to the height of the contour level to measure enamel thickness. Pearson correlation analysis was used to investigate the correlation between enamel thickness and morphologic features. Results: Enamel thickness was $0.75{\pm}0.07mm$ per side, and MD width was $5.56{\pm}0.40mm$. Enamel thickness and MD width were significantly correlated. However, a significant relationship was not observed between enamel thickness and MD/FL index or the height of contour width/cervical width ratio. Conclusions: The results suggest that enamel thickness is affected only by MD width. Therefore, if the MD width is the same for mandibular incisors with a large MD/FL index or triangular shape and mandibular incisors with normal shape, then the limit of enamel reduction for reproximation will be the same.

  • Objective: The aim of this study was to evaluate the effect of remineralization and inhibition to demineralization after fluoride gel (acidulated phosphate fluoride, APF) or hydroxyapatite (HAp) paste application on interdentally stripped teeth. Methods: After interdental stripping, 1.23% APF or 5%, 10% HAp paste were applied for 7 days for remineralization. Afterwards, teeth were exposed to lactate carbopol buffer solution for demineralization. Scanning electron microscopy (SEM) and energy dispersive X-ray spectroscopy (EDS) were used to compare change in surface contents and crystal structures after remineralization, and then after demineralization. Results: EDS analysis indicated that calcium (p < 0.001) and phosphate (p < 0.01) contents were increased after 10% HAp paste application on stripped enamel, calcium (p < 0.05) and phosphate (p < 0.01) contents were increased after 5% HAp paste application, and fluoride (p < 0.01) contents were increased after 1.23% APF application. SEM image showed that enamel surfaces became smoother and crystal structures became small and compact after APF or HAp application. After demineralization, calcium (p < 0.05) and phosphate (p < 0.05) contents remained increased on the enamel remineralized with 10% HAp paste, and phosphate (p < 0.05) contents remained increased on the enamel remineralized with 5% HAp paste. After demineralization, surfaces looked less destroyed in the enamel remineralized beforehand than those of the control, and small pores between crystal structures, formed by remineralization were remained. Conclusions: Hydroxyapatite paste and fluoride gel were helpful to remineralize and inhibit deminerlization on stripped enamel.

  • Objectives : This study was carried out to examine the effect of varnish fluoride and APF gel on the acid resistance and the remineralization of the enamel. Methods : At first, the microhardness changes of enamel surface were measured after demineralizing the fluoride treated tooth surface. Next, the changes were measured after fluoride application to the demineralized enamel surface. Results : 1. Acid resistance was higher in varnish fluoride groups than APF gel groups and the difference was significant(p<0.001). 1) Varnish fluoride groups Microhardness of enamel surface showed $297.76{\pm}9.89$ after fluoride treatment and $260.90{\pm}28.67$ after drmineralization. The changes of Vickers hardness number(VHN) were $-36.86{\pm}27.30$. 2) APF gel groups Microhardness of enamel surface showed $298.79{\pm}17.28$ after fluoride treatment and $43.75{\pm}18.58$ after demineralization The changes of VHN were $-255.04{\pm}21.31$. 2. No significant changes were surveyed in both varnish fluoride groups and APF gel groups as for remineralization of enamel(p>0.05). 1) Varnish fluoride groups Microhardness of enamel surface showed $46.58{\pm}15.42$ after demineralization and $46.61{\pm}15.70$ after fluoride treatment. The changes of VHN were $0.02{\pm}3.75$. 2) APF gel groups Microhardness of enamel surface showed $47.13{\pm}19.31$ after demineralization and $42.59{\pm}16.12$ after fluoride treatment. The changes of VHN were $-4.54{\pm}5.06$. Conclusions : Varnish fluoride showed higher acid resistance than APF gel, however both of them were observed to have no effect on the remineralization of the enamel.

  • The aim of this in vitro study was to evaluate the effectiveness of experimental 2.26% fluoride-polyvinyl alcohol (F-PVA) tape in inhibition of enamel demineralization using enamel surface microhardness (SMH) analysis and scanning electron microscopy (SEM) examination. Enamel specimens (n=60) randomly assigned to four groups: control group, F-PVA tape group, fluoride varinish (F-varnish) group, casein phosphopeptide-amorphous calcium phosphate (CPP-ACFP) group. After topical application, pH-cycling was processed. Then, SMH was measured and the percentage loss of surface microhardness (%SML) was calculated. For the SEM examination, five sample specimens in each group were treated and the morphologic character was evaluated. After pH-cycling, the SMH values of the enamel specimens of F-PVA tape and F-varnish group were significantly higher than that of CPP-ACFP group, there was no significant difference between F-PVA tape and Fvarnish group. With SEM examination, enamel surfaces in the F-PVA tape group and F-varnish group showed numerous spherical and ovoid crystals formed on the enamel surface were also observed. The density of crystals was higher than that of both control group and CPP-ACFP group. F-PVA tape is effective in inhibition of enamel demineralization. Also, F-PVA tape's inhibition of enamel demineralization is comparable to that of F-vanish and greater than that of CPP-ACFP.

  • This study compared the microshear bond strength $({\mu}SBS)$ to end and side of enamel rod bonded by four adhesives including two total etch adhesives and two self-etch adhesives. Crown segments of extracted human molars were cut mesiodistally. The outer buccal or lingual surface was used as specimens cutting the ends of enamel rods, and inner slabs used as specimens cutting the sides of enamel rods. They were assigned to four groups by used adhesives: Group 1 (All-Bond 2), Group 2 (Single Bond), Group 3 (Tyrian SPE/One-Step Plus), Group 4 (Adper Prompt L-Pop). After each adhesive was applied to enamel surface, three composite cylinders were adhered to it of each specimen using Tygon tube. After storage in distilled water for 24 hours, the bonded specimens were subjected to ${\mu}SBS$ testing with a crosshead speed of 1 mm/minute. The results of this study were as follows: 1. The $({\mu}SBS)$ of Group 2 $(16.50\pm2.31 MPa)$ and Group 4 $(15.83\pm2.33 MPa)$ to the end of enamel prism was significantly higher than that of Group 1 $(11.93\pm2.25 MPa)$ and Group 3 $(11.97\pm2.05 MPa)$ (p<0.05). 2. The $({\mu}SBS)$ of Group 2 $(13.43\pm2.93 MPa)$ to the side of enamel prism was significantly higher than that of Group 1 $(8.64\pm1.53 MPa)$, Group 3 $(9.69\pm1.80 MPa)$, and Group 4 $(10.56 \pm1.75 MPa)$ (p<0.05), 3. The mean $({\mu}SBS)$ to the end of enamel rod was significantly higher than that to the side of enamel rod in all group (p<0.05).

  • This study aimed to determine the criteria for quantifying developmental defects of enamel in primary teeth in premature babies and to investigate the severity of developmental defects according to the gestational age, birth weight, systemic complications, and treatments received after preterm birth. Birth information, a history of complications, the duration of parenteral nutrition, and endotracheal intubation were investigated by retrospectively reviewing the admission and discharge records of premature babies in the neonatal intensive care unit. The Preterm Developmental Defects of Enamel (PDDE) index was designed by modifying the existing developmental defects of enamel index. Based on PDDE index, the evaluator scored developmental defects of enamel by classifying them as enamel hypomineralization and hypoplasia. The PDDE scores in the extremely preterm and extremely low birth weight groups were significantly higher than those in other groups. Furthermore, PDDE scores of premature babies with bronchopulmonary dysplasia, rickets, intraventricular hemorrhage, or necrotizing colitis were significantly higher than those in the control group. In addition, more than 50 days of endotracheal intubation and more than 20 days of parenteral nutrition were associated with significantly higher PDDE scores than those in the control group and were risk factors for developmental defects of enamel. This study provides basic information for identifying risk factors for developmental defects of enamel in premature babies.

  • This study was done to evaluate the changes of enamel surface by interproximal stripping and recovery of it by polishing. The number of 34 1st premolars which had extracted for orthodontic treatment were selected as samples. Interproximal stripping was performed by hand with metal strip and strip placer (Dentaurum Co., Germany) and low speed handpiece with diamond disk (Superdiaflex, Germany). Polishing was performed by hand with plastic strip (3M Col) and low speed handpiece with whip-mix, DCPA (Dicalcium Phosphate, Anhydrous, $CaHPO_4$) powder and Sof-lex (3M Co. U.S.A.) polishing kit. Each groups were examined under the scanning electron microscope (JEOL Co., JSM-840A, Japan) and the following results were obtained: 1. The stripped group performed by metal strip and diamond disk altogether showed deep furrow on the enamel surface as wide as about $10{{\mu}m}$. 2. There could be seen more irregular scratched line in the group stripped by metal strip than that by diamond disk. 3. The polished group performed by plastic strip and DCPA powder showed slight smoothening of the edge of stripped furrow on the enamel surface without relation to the stripping method. 4. The polished group performed by Sof-lex progressive polishing kit could not avoid the formation of the furrows on the enamel surface according to the particle size without relation to the stripping method. 5. The polished group performed by the superfine polishing wheel, the final stage of Sof-lex polishing method showed shallow scratched line as wide as within about $2{{\mu}m}$ on the enamel surface without relation to the stripping method. 6. The interproximal stripped enamel surface could not recover its original surface texture by any kind of polishing methods.
