Emergent Surgical Intervention for Embolization of Atrial Septal Defect Closure Device

  • The percutaneous transcatheter closure of secundum atrial septal defect has recently become an increasingly widespread alternative to surgical closure in many centers. Although immediate, short, and intermediate term results of percutaneous transcatheter septal closure are promising, the procedure is not free from inherent complications that could be lethal. We report a case of device embolization necessitating emergent surgical retrieval.

Study of vascular hydrophyte vegetation and biomass in Bigumdo, Shinangun, Korea (신안군 비금도의 관속수생식물의 식생 및 생산량(Biomass)에 관한 연구)

  • A study of the vascular hydrophyte communities and biomass was undertaken in the Bigumdo wetlands, Shinangun, from March to November, 2005. As a result, the vegetation was divided into 14 communities. Among them, emergent hydrophytes consisted of 6 communities, including Miscanthus sacchariflorus community, Phragmites communis community, Typha angustata community, Leersia japonica community, Paspalum disticum var. indutum community, and Persicaria thunbergii community, floating hydrophytes 5 communities including Hydrocharis dubia community, Nelumbo nucifera community, Euryale ferox community, Trapa japonica community and Nymphaea tetragona var. angusta community, free-floating hydrophytes 2 commuinties including Lemna paucicostata community and Spirodela polyrhiza community, and submergent hydrophyte 1 community, including Myriophyllum verticillatum community. Biomass was the highest at emergent hydrophytes and decreased along the life form, in the order like floating hydrophytes, submergent hydrophyte and free-floating hydrophytes. In addition, hydrophytes in the Bigumdo wetland showed the typical vertical zonation pattern like a natural swamp. These results were considered that the wetland of Bigumdo was characterized by the typical structure of aquatic plant ecosystem.

Simulation of Transient Flow in a Cold-Water Supply System with Pressure Reducing Valves for Cooling the Inside of a Coal Mining Pit (감압밸브를 사용한 탄광갱내 공조용 냉수 공급시스템의 과도현상 해석)

  • Transient flow in a cold-water supply system for cooling the inside of a coal mining pit was numerically simulated. Properly designed and presetted pressure reducing valves control the level of pressures of the piping system at normal or emergent conditions Quasi-steady relations to simulate the valve motion are obtained and the transient performance of the valve is investigated in the present paper. The present method reasonably simulate transient phenomena in the system including the pressure reducing valve. Excessive valve motion and column separation are simulated when the flow is abruptly reduced. A calculated example of the real system is also presented. The simulation can be used for the safety-check and the guidance for design and operation in emergent cases of the system.

Current Opinion on Endovascular Therapy for Emergent Large Vessel Occlusion Due to Underlying Intracranial Atherosclerotic Stenosis

  • For recanalization of emergent large vessel occlusions (ELVOs), endovascular therapy (EVT) using newer devices, such as a stent retriever and large-bore catheter, has shown better patient outcomes compared with intravenous recombinant tissue plasminogen activator only. Intracranial atherosclerotic stenosis (ICAS) is a major cause of acute ischemic stroke, the incidence of which is rising worldwide. Thus, it is not rare to encounter underlying ICAS during EVT procedures, particularly in Asian countries. ELVO due to underlying ICAS is often related to EVT procedure failure or complications, which can lead to poor functional recovery. However, information regarding EVT for this type of stroke is lacking because large clinical trials have been largely based on Western populations. In this review, we discuss the unique pathologic basis of ELVO with underlying ICAS, which may complicate EVT procedures. Moreover, we review EVT data for patients with ELVO due to underlying ICAS and suggest an optimal endovascular recanalization strategy based on the existing literature. Finally, we present future perspectives on this subject.
