A review of the qualification criteria for the national examinations for emergency medical technicians (응급구조사 국가시험 응시 자격기준에 관한 고찰)

  • Purpose: Emergency medical technicians (EMTs) have so far been trained as professionals under the same conditions, with no change in the 1995 Act. We aimed to find ways for them to secure expertise in accordance with social needs by strengthening the quality of the EMT education. Methods: This is a descriptive study comparing the operation status of the national emergency medical examination conducted by the Korea Health Personnel Licensing Examination Institute, and the national examinations of EMT paramedic and EMT basic. The scope of the national examinations for EMT was compared by subject and area. Results: The national written exam for EMT paramedic consists of five areas. EMT basic does not include basic medicine in three related subjects, 11 areas, and 18 detail areas. Paramedic care does not include advanced pediatric life support. In addition, nine areas and 20 detail areas are not included. Conclusion: The study suggests the need for institutional supplementation so that those who have completed EMT basic and the subjects prescribed by the ordinance of the Ministry of Health and Welfare at universities, etc., in the Higher Education Act can take the EMT paramedic national exam.

Prehospital care and knowledge, prehospital care confidence toward the infant and toddlers of the 119 emergency medical technicians (119 구급대원의 영유아에 대한 응급처치 현황 및 응급처치 지식과 수행 자신감 비교)

  • Purpose: The purpose of this study was to improve the quality of prehospital care provided to infants and toddlers (preschoolers) based on questionnaire findings. Methods: A total of 1,634 running sheets from 119 centers of D city from patients older than 1 month and less than 7 years of age from January 1, 2013 to December 31, 2013 were reviewed for 119 emergency medical technicians (EMTs) activity analysis. And a self-reported questionnaire about knowledge and competency in prehospital care of preschoolers was completed by 159 EMTs at fire stations in D city. Data were analyzed using IBM SPSS Statistics for Windows, Version 21.0. Results: Of 1,634 patients, 58.6% were call to 119 due to disease, 33.8% due to injuries and 7.5% due to traffic accident. Knowledge and confidence in treating preschoolers was significantly higher among paramedic EMTs than basic EMTs. The 119 EMTs surveyed indicated that training for preschooler prehospital care was most needed. Conclusion: In order to provide high quality prehospital care for preschoolers transported to emergency rooms, it is necessary to equip ambulances with the proper equipment. In addition, EMTs should be provided educational opportunities and clinical training.

  • Purpose: This study aimed to improve the quality of prehospital emergency care for elderly patients by determining current prehospital care status based on run sheets and level of knowledge, and prehospital care confidence toward the elderly through questionnaires answered by 119 emergency medical technicians (EMTs). Methods: This study was conducted with 4,946 elderly patients who were transferred to the hospital by 119 ambulances in D city from March 1, 2013 to February 28, 2014. This study was a descriptive study of 119 run sheets. The questionnaire was completed by 160 EMTs working in D city. All data were analyzed by using SPSS 21.0. Results: 74.2% was due to disease among the 4,946 elderly patients who were transferred by 119 ambulances, and a rest was due to injury. The mean knowledge score of the 119 EMTs regarding elderly patients was low. Their confidence on prehospital care of the elderly patients statistically significantly differed according to career at hospital and education. Conclusion: Theoretical knowledge, education on prehospital care of the elderly, and repeated technical prehospital care education and training accompanied by feedback assessment by 119 EMTs are needed.

Legal Reviews and Countermeasures against Violence to 119 Emergency Medical Technicians (EMT) (119구급대원의 폭행피해에 대한 법적인 고찰 및 대응방안)

  • There have been various discussions to prevent violence to 119 emergency medical technicians, and the aims of the were as follow ; first, the staff dispatched to the scene should be reinforced so that the patients committing violent acts can be subdued. Secondly, self-defense devices should be provided to 119 emergency medical technicians so that they can use them in a dangerous situation and escape from it. And finally, 119 emergency medical technicians should be allowed to refuse to transport the patients when they or their guardians do violence to them in order to prevent potential violent cases. When countermeasures after violence is committed, it is important to secure evidence and determine strongly to bring charges to the offenders rather than agree to overlook the violence in order to correct them right. It is necessary to introduce a support program to help the victim technicians concentrate on their physical and mental treatment. Violence to 119 EMT should be punished so that the society will think it is natural to punish such offenders. In a safer environment, 119 EMT can perform their duties and it is very important to draw this consensus.

  • This study aimed to develop a prehospital protocol on CPR to patients with nontraumatic cardiac arrest for emergency medical technicians. For the purpose, we prepared a protocol based on preliminary items and demonstrated its validity to confirm a final protocol from March 1, 2007- August 30. 2008. The practical CPR protocol on cardiac arrest victims can be used as means to support quick and exact decision-making on CPR, which will contribute to enhancement of emergency medical services.

  • Purpose: The study investigated the perception, report, and education of elder abuse in 119 emergency medical technicians (EMTs) in Korea. Methods: A self-reported questionnaire was distributed to 110 EMTs in K city, and 100 answers were collected from June 1 to 10, 2015. The study instrument was perception regarding elder abuse and education inventory developed by Kim and Kim. Data were analyzed using SPSS/WIN 18.0 version. Results: The elder abuse perception score was 3.81 on a Likert 4-point scale. Physical abuse was the most common type, followed by verbal, financial, negligence, and emotional abuse. Most EMTs recognized that is important for the prevention of elder abuse, but most of them had no experience in reporting abuse. A majority of the EMTs had attended an elder abuse program and recognized the importsnce of such program. Conclusion: This study suggests the necessity of more targeted education of elder abuse prevention combined with family counseling methods and practical approach toward awareness of elder abuse.

Defining core competencies for 119 emergency medical technicians based on the analysis of requirements and priorities of the profession (119구급대원의 직업기초능력 요구도 및 우선순위 분석 기반 직무기초역량 제안 연구)

  • Purpose: This study aimed to determine the core competencies for 119 emergency medical technicians (EMTs) and to provide evidence for the development and utilization of 119 EMTs, education and training programs. Methods: Data were collected with a questionnaire that consisted of items on the general characteristics of the subjects (8 items) and importance and satisfaction levels for 10 competencies, including 34 items on subfactors. The Borich Needs Assessment Model was used in designing the questionnaire. A locus for focus model was used to derive the top priority competencies for the improvement of core competency in the profession. Data were analyzed with by SPSS ver.21 (IBM, Armonk, NY, USA). Results: The core competencies for 119 EMTs were derived from technical competency, information competency, communication competency, problem-solving competency, self-development competency and interpersonal competency. In addition, among the sub-factors of these competencies, 16 abilities (including leadership), ability to apply technical knowledge, technical comprehension, conflict management ability, ability to create documents, problem handling ability, ability to think, technical selection ability, and computer literacy were included in the priority category. Conclusion: This study derived and presented the core competencies for 119 EMTs to enable them to meet the modern day requirements of their roles, which may contribute to enhancing their professionalism.

Influence of post-traumatic stress and compassion competence on the quality of life of emergency medical technicians (119 구급대원의 외상 후 스트레스와 공감역량이 삶의 질에 미치는 영향)

  • Purpose: This descriptive correlational study investigated the effects of post-traumatic stress and compassion competence on the quality of life of 119 Emergency medical technicians(EMTs). Methods: The data of 119 EMTs were collected between May 1 and September 30, 2020. Data analysis was performed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and hierarchical multiple regression with SPSS 24.0 program. Results: Post-traumatic stress was seen to be negatively correlated with the quality of life (r=-.456, p<.001) and compassion competence was positively correlated with quality of life (r=.250, p=.006). Participants' subjective health status (β=0.30, p<.001), post-traumatic stress (β=-0.39, p=.001) and compassion competence (β=0.29, p<.001) explained 38.5% of variance in the quality of life of the 119 EMTs. Conclusion: These findings indicate that post-traumatic stress and compassion competence are important factors to improve the quality of life of EMTs. Therefore, systematic programs for reducing post-traumatic stress and enhancing compassion competence levels should be developed.

Awareness of Caregivers, Emergency Medical Technicians, and Emergency Medical Doctors about the Storage Media for Avulsed Teeth (완전탈구된 치아의 보관매체에 관한 보호자, 구급대원, 응급의학과 의사의 인식도)

  • The aim of this study was to assess the awareness of caregivers, emergency medical technicians, and emergency medical doctors by investigating the storage media for avulsed teeth at the trauma site. This study was performed by reviewing medical records of 183 patients with the age under 17 years old, who visited Ajou University Hospital for dental trauma. Based on their medical records, storage media for avulsed permanent teeth were investigated by classifying them as dry, milk, saline, saliva and tap water. The storage media chosen by caregivers at the trauma site were classified as cases by parents and school nurse teachers. Storage media chosen by emergency medical technicians and emergency medical doctors were also investigated using the same classification criteria. The results show that both parents and school nurse teachers used milk the most as a storage medium for avulsed teeth. Dry storage was the second most chosen medium by parents, and saline by nurse teacher. As a storage media emergency medical technicians used saline the most, while emergency medical doctors used only saline when they failed to replant avulsed teeth. There is a need to improve the knowledge of caregivers, emergency medical technicians, and emergency medical doctors regarding the storage media for avulsed teeth.

Work Stress and Affecting Factors of Emergency Medical Technicians (응급구조사의 업무 스트레스와 그 영향 요인)

  • Objectives: To identify work stress and affecting factors of the emergency medical technicians(EMT) and to provide basic data for work circumstance improvement and emergency medical service and duty efficiency elevation. Methods: The data were collected by questionnaire from 635 emergency medical technicians throughout the whole country from August 5 to September 20 2013 and analyzed by using the SPSS(version 18.0) program. Factors affecting stress were analyzed by Multiple Linear Regression Analysis(${\alpha}=0.05$). Results: Mean level of job stress was markedly high in the spot situation(3.43 point), user awareness(3.31 point), burden related to work(3.21 point), role and conflict as a professional job(3.10 point), unsuitable treatment(2.95 point) but was lowest in specialty demand of knowledge and skill(2.76 point). Conclusions: To diminish work stress, understanding and cooperation of the residents and reduction of burden related to work must be improved. Above all, positive interest of 119 and related organ and duty environment improvement will and effort and continuous measure establishment and support and cooperation of the government are needed.
