급성 경항통 환자의 흉쇄유돌근에 대한 근 에너지 기법(MET) 시술 후의 경근전도 변화 연구 (The Clinical Study of Muscle Energy Technique (MET) Performed on Sternocleidomastoid Muscle of Acute Nuchal Pain Patients on Meridian Electromyography)

  • Objectives To evaluate the clinical utility of MET performed on sternocleidomastoid muscle of acute nuchal pain patients, we measured a change of meridian electromyography. Methods We compared electrical activity before and after MET performed on sternocleidomastoid muscle of acute nuchal pain patients in same group (n=20) in isometric exercise state during five seconds. We analyzed amplitudes and areas of electrical activity and asymmetry index (AI). Results 1. After MET performed on sternocleidomastoid muscle of acute nuchal pain patients, maximum voluntary isomeric contraction (MVIC) was significantly increased more than before MET performed (p<0.005). 2. Comparing with before MET performed on sternocleidomastoid muscle of acute nuchal pain patients, muscle fatigue after MET performed on sternocleidomastoid muscle of acute nuchal pain patients decreased but there was no significant difference. 3. Comparing with before MET performed on sternocleidomastoid muscle of acute nuchal pain patients, asymmetry index (AI) after MET performed on sternocleidomastoid muscle of acute nuchal pain patients decreased but there was no significant difference. Conclusions According to above results, after performing MET on sternocleidomastoid muscle of acute nuchal pain patients, maximum voluntary isomeric contraction (MVIC) increased significantly, so it is certain that performing MET on sternocleidomastoid muscle has a clinical effect.

Effect of repetitive wrist extension with electromyography-triggered stimulation after stroke: a preliminary randomized controlled study

  • Objective: The purpose of this study was to explore the effect of repetitive wrist extension task training with electromyography (EMG)-triggered neuromuscular electrical stimulation (NMES) for wrist extensor muscle recovery in patients with stroke. Design: Randomized controlled trial. Methods: Fifteen subjects who had suffered a stroke were randomly assigned to an EMG-triggered NMES group (n=8) or control group (n=7); subjects in both groups received conventional therapy as usual. Subjects in the experimental group received application of EMG-triggered NMES to the wrist extensor muscles for 20 minutes, twice per day, five days per week, for a period of four weeks, and were given a task to make a touch alarm go off by activity involving extension of their wrist. In the control group, subjects performed wrist self-exercises for the same duration and frequency as those in the experimental group. Outcome measures included muscle reaction time and spectrum analysis. Assessments were performed during the pre- and post-treatment periods. Results: In the EMG-triggered NMES group, faster muscle reaction time was observed, and median frequency also showed improvement, from 68.2 to 75.3 Hz, after training (p<0.05). Muscle reaction time was significantly faster, and median frequency was significantly higher in the experimental group than in the experimental group after training. Conclusions: EMG-triggered NMES is beneficial for patients with hemiparetic stroke in recovery of upper extremity function.

안면신경마비의 평가척도와 SEMG 검사 결과 간의 상관성 연구 (A Study on the Correlation between Surface Electromyography and Assessment Scale for Facial Palsy)

  • Objjectives : The purpose of this study is to investigate the correlation between surface electromyography(SEMG) and assessment scales for facial palsy. Methods : The subjects for this study were 21 patients who had peripheral facial nerve palsy. We analyzed about the correlation between the results of SEMG and facial nerve assessment scales that were carried out at the same time. The assessment scales used in this study were House-Brackmann scale, Yanagihara grading system, Sunnybrook facial grading system. Results : There are considerable correlation between the results of SEMG and the results of three kinds of assessment scales for facial nerve palsy. Conclusions : SEMG may be an objective examination for facial nerve palsy. Some research aimed at development of SEMG examination guideline for facial nerve palsy will be needed after this. And we expect that many researches for development of diagnostic equipments or treatment equipments that utilize SEMG will proceed.

단시간 하이힐 보행이 젊은 여성의 발목 주위근의 근 활성도에 미치는 영향 (Influences of Short-term High-heeled Walking on the Activities of Ankle-stabilizing Muscles in Healthy Young Females)

  • Background: The purpose of this study was to investigate the influence of short-term treadmill walking with high-heeled shoes on electromyography activities of the medial gastrocnemius, lateral gastrocnemius and tibialis anterior in healthy young females. Methods: Fifteen healthy females were recruited for this study. To measure muscle activation, the subjects were asked in random order to walk on a treadmill using either high-heeled shoes or barefoot conditions. The shoe heel height for high-heeled walking was 7 cm. The walking speed on the treadmill was 4 km/h, and the inclination rate of the treadmill was 10%. The subjects performed treadmill walking in the barefoot and high-heeled walking conditions for 5 minutes. Electromyography data were collected from the tibialis anterior, medial gastrocnemius, lateral gastrocnemius, and soleus on both firm and foam surfaces and during eyes-open and-closed conditions while standing. Results: Tibialis anterior activity was significantly different before and after the walking task while standing on a foam surface with eyes closed (p<.05). Conclusion: This finding suggests that the activity of the tibialis anterior may be lowered after high-heeled walking. Therefore, high-heeled shoes contribute to harmful effects at the ankle joints, increasing the risks of falling and musculoskeletal injury.

휴대용 요역동학 검사 시 근전도 신호를 이용한 복압측정 방법 (A Non-invasive Measurement of Abdominal Pressure on Ambulatory Urodynamics Study Using Surface Electromyography)

  • 복압 측정을 위한 기존의 카데터 방법은 배뇨압에 영향을 줄 수 있는 많은 오차를 유발할 수 있으며 환자에게 불편함을 야기한다. 이러한 문제점을 해결하기 위해, 본 연구에서는 표면 근전도를 이용하여 비침습적으로 복압을 측정할 수 있는 새로운 방법을 개발하였다. 연구 결과, 기존 직장 카테터를 이용하여 측정된 복압과 복압측정 알고리즘을 이용하여 추정된 복압 간의 상관계수는 $0.79{\pm}0.06$이었으며, 두 신호간의 오차는 $0.10{\pm}0.07$이었다. 본 연구에서 개발된 복압추정 알고리즘을 이용하면 휴대용 요역동학 검사 시 편리하게 간접적으로 복압을 측정할 수 있을 것으로 생각된다.

뇌졸중 환자에서 내측 비복근의 조직탄성 측정을 위한 Myotonometer의 유용성 (Usefulness of Myotonometer for Measurement of Tissue Compliance on Medialis Gastrocnemius in Patients with Stroke)

  • 본 연구의 목적은 뇌졸중 환자의 족관절 저측굴근 경직에 대하여 Myotonometer와 surface Electromyography(sEMG)를 이용하여 경직의 정량적 평가에 유용한 지표를 알아보고, 이 지표들과 임상적으로 평가되는 modified Ashworth scale(MAS)과의 관계를 알아보고자 하였다. 족관절 저측굴근의 경직평가는 물리치료사 5명(임상경력 5년이상)이 MAS를 이용하여 MAS 2, 3, 4 해당 군에 각 5명씩을 무작위로 15명씩 배정하였다. 각 군의 조직탄성과 근활성 측정은 Myotonometer와 sEMG로 이완(relaxed)상태와 최대 수의적 수축(contracted)상태에서 측정하였다. 연구 결과, MAS 등급이 높아짐에 따라 이완과 수축상태 간의 조직저항도 전위차와 근활성도의 차이는 작아졌고, 상관관계 분석에서도 MAS 등급이 높아질수록 이완 시보다 수의적 수축 시 실린더가 받는 낮은 강도에서의 상관성이 더 높아짐을 확인할 수 있었다. 따라서 Myotonometer는 경직을 평가하는 방법으로 다양한 인체의 관절에 비교적 쉽게 적용할 수 있으며 경직의 변화를 민감하게 반영할 수 있어 보다 객관적이고 정량적인 경직의 평가 도구로 사용될 수 있을 것이다.

Correlation between the Pressure Pain Threshold and Sonography and Spontaneous Electrical Activity in Myofascial Trigger Points

  • Purpose: This study was designed to investigate possibilities for quantitative analysis using the electromyography and sonography. For better understanding, we evaluated the correlation between the pressure pain threshold and sonography, spontaneous electrical activity in trigger points located in the upper trapezius muscle. Methods: Thirty three active subjects volunteered to participate in this study (n=33). They had a palpable taut band, exquisite spot tenderness of a nodule in a taut band, spontaneous pain, referred pain, jump sign, local twitch response, and a painful limit to full stretch range of motion. We measured Pressure pain threshold, density, white area index, root mean square, and reaction. Pearson’s correlation coefficient was calculated to estimate the relationship between the pressure pain threshold and other variables including density, white area index, root mean square, and reaction time. Results: There were significant correlations between pressure pain threshold and density (r=-0.75, p<0.01), and between pressure pain threshold and white area index (r=-0.74, p<0.01). A significant correlations between pressure pain threshold and root mean square (r=-0.59, p<0.01). The significant correlation was found between pressure pain threshold and reaction time (r=-0.64, p<0.01). Conclusion: These should indicate whether quantitative analysis can be done using the characteristics of electromyography and sonography.

A Study of the Electrical Properties of the Buccal Area using Facial Surface Electromyography

  • Objectives : The purpose of this study was to determine the electrical properties of the buccal area using facial surface electromyography (sEMG). Methods : This research was conducted on 44 healthy participants irrespective of their sex. Surface electrodes were attached to the midpoints of three imaginary lines connecting ST4 (Dicang) to ST6 (Jiache), ST4 to SI18 (Quanliao), and ST4 to the center point of SI18 and ST6. Then, the participants were trained in the movement that included a comprehensive action of buccal area. While the participants were performing the motion, sEMG values ($E_1$, $E_2$, $E_3$) and the distance change of the three imaginary lines ($D_1$, $D_2$, $D_3$) were measured. The data were statistically analyzed using SPSS ver. 22.0. Results : Significant differences were observed in the distance changes ($D_1$>$D_3$, $D_2$>$D_3$) and sEMG values ($E_1$<$E_2$<$E_3$). Moreover, there were positive correlations between $D_1$ and $E_1$, $D_2$ and $E_2$. Conclusion : We suggest that the measurement at ST4 to the center point of ST6 and SI18 with this motion would be adequate to check the electrical characteristics of the buccal area.

성대 부전 마비의 진단에서 후두 근전도 검사의 유용성 (Usefulness of the Laryngeal Electromyography in Diagnosis of Vocal Fold Paresis)

  • Background and Objectives : Vocal fold paresis is a clinical condition and considered as a continuum of neurologic dysfunction encompassing partial denervation and variable degrees and patterns of reinnervation. Its incidence, clinical presentation, significance are incompletely understood and still debated. This study describes the clinical, electromyographic findings in patients who presented with complaints of dysphonia and whose laryngoscopic finding revealed vocal fold paresis. Materials and Method : 47 patients (male : 25, female : 22) who referred to Ewha Womans University Medical Center Voice clinic for evaluation of vocal complaints were enrolled in this study. All patients had undergone a through history and physical examination including strobovideoscopic and laryngoscopic examination. Patients with in the history and/or laryngoscopic examination suggestive of vocal fold paresis were evaluated by laryngeal electromyography (LEMG). Results : Of these patients, 23 (48.9%) were found to have evidence of neuropathy on LEMG. There was no significant difference in voice symptoms and laryngoscopic findings between two groups of patients with evidence of neuropathy and who show normal findings on LEMG. Conclusion : LEMG can clinically help to guide the evaluation and management of vocal fold paresis. Due to some limitations of LEMG, laryngoscopic findings and clinical correlations should also be considered when diagnosing the vocal fold paresis.
