Care Workers' Perceptions of Professionalism and Job Satisfaction (요양보호사의 전문성 인식과 직무만족에 관한 연구 -노인요양시설을 중심으로-)

  • This study aims to examine care workers' role perceptions of their occupation and job satisfaction with their work. They are core human resources in elderly care work practices and their perceptions of their occupation are likely to affect their work performance and job satisfaction. A mail survey is conducted targeting to 400 care workers who are at work in long-term care facilities located in Seoul, Gyeonggi and Pusan. Research findings are as follows: firstly, the perception level of care workers' professionalism is 3.38, and job satisfaction is 3.41. Secondly, the correlation analysis between perception level of professionalism and job satisfaction, coefficient is .553, which shows positive relations between two variables. Thirdly, the study shows that care workers' perceptions of professionalism significantly affects their job satisfaction. This study concludes that it is necessary to take a variety of measures in contribution to improve the quality of service through reinforcement of perception of professionalism higher than present level of care workers who supply direct service in long-term care facilities.

The Effects of Nursing Hospital Nurses' Caring Efficacy and Empathy Competence on Human-Centered Care (요양병원간호사의 돌봄효능감, 공감역량이 인간중심돌봄에 미치는 영향)

  • This study is a descriptive research study to confirm the care efficacy and empathy competency of nursing hospital nurses and analyze the factors to improve person-centered care behavior, provide theoretical and practical information accordingly, and prepare basic data. The study collected data from March 20, 2023 to April 20, 2023, targeting 146 nurses at a nursing hospital for the elderly in C region located in G province, and finally analyzed a total of 144 copies. Using IBM SPSS/25, descriptive statistics t-test, ANOVA, and correlation multiple regression analysis were analyzed. As a result of analyzing the variables that affect the subject's person-centered care by hierarchical multiple regression, the higher the caring efficacy and empathy capacity, the more the person-centered care was affected, and the explanatory power was 31.5%. As a result of this study, empathy competency and care efficacy were found to have an effect on person-centered care. It is thought that various measures should be prepared to improve the care efficacy and empathy competency for the efficient nursing performance of person-centered care of nursing hospital nurses.

Model Development a Womens' Health Care Center in the Community (여성건강 간호센터를 위한 모형개발 - 일개 통합시를 중심으로-)

  • The purpose of this study was to analyze womens' health problems using Green & Kreuter's 1991 PRECEDE model and to develop a model for a womens' health care center located in the community. The subjects were recruited from Wonju City. 1. The results showed that 23% of the sample population felt there was a need for a womens' health care center. The mean number of health problems was 3.1. The prevalence rate, was 44.4%, and the rate for an artificial abortion, was 36.4%. Also 30.5% did not have a health examination in the past year. Women using the hospital for medical care accounted for 45% of the sample, while 40% used the drugstore. The average score on the HPLP was 2.41, and this was influenced by self-efficacy, family support, sexual role, and health locus of control. There are a few educational programs in the city provided by the Wonju Health Center and by community health nurse practitioners. 2. The nursing center, as defined in North America, is a nurse-anchored system of primary health care delivery or neighborhood health center. Centers offer various services ranging from primary care to the more traditional such as education, health promotion, wellness screening, and coordination of services by advanced practice nurses. For examples in Sweden MCH centers provide total services for childbearing women and their families, sexual counseling and education for adolescents, and screening by midwives for cervical cancer. 3. The developed model combines purpose, target population, organization, and services, and is related to health resources. The purpose is primary health care and promotion of the quality of life. The target population can be grouped according to the life cycle, (premarriaged age group, the childbearing/child rearing age group, and middle aged and elderly women) and focuses on self-help. The organization of the center includes an advisory committee to plan and evaluate, and a health services team that will be multidisciplinary to provide health care, counseling education, and research. The model development suggested that a variety of women's health care centers are needed to insure adequate management of women's health. Follow-up research using PROCEED is needed to analyze health outcomes. Also a health nursing specialist system is required to develop health promotion, and improve the quality of life of women.

A Study on Facilites Safety of Long-Term Care Facilities for Elderly :Focused on the Factual Survey of Seoul Metropolitan Area (장기요양시설의 노인보호를 위한 시설안전에 관한 연구 : 수도권 시설안전 실태조사를 중심으로)

  • This study examines factual facilites safety of long-term care facilities for eldery, for improvement of Korean policy. So, This study reviewed policies of facilites safety of long-term care facilities on main OECD countries. We analyzed factual survey of facilites safety of long-term care facilities in Seoul metropolitan area. The results of the study were as follows: First, There were differences of law and rule of facilites safety of long-term care facilities in comparison with those of social welfare facilities, child day-care center, hospital. Second, it was the result of location condition analysis. Long-term care facilities in the Seoul metropolitan area were located on more than six floors of buildings. The location conditions of private facilities are weak compared to those of corporations and local governments. Third, there were the result of internal structure of facilities. Overall, the placement status was insufficient. For instance, there are smokeproof masks/fireproof blankets, smoke windows(fire safety windows), smoke smothering. In addition, there were places where legal essential installation structures were insufficient. Private facilities were generally insufficient in facility structures compared to those of local governments and welfare corporations. Fourth, there were the result of safety management. It was confirmed that private facilities were insufficient compared to local government facilities. For instance, there are safety management analysis, response manuals, fire preparedness training, etc. Finally, implications and future directions policies of facilites safety of long-term care facilities were discussed based on the finding of the study.

A study on the knowledge and awareness of care workers on the geriatric oral health (요양보호사의 노인 구강보건에 대한 지식과 인식도 연구)

  • The study aims to examine the knowledge and awareness of care workers on oral health knowledge at the geriatric care facilities in order to improve and develop oral health services. For the research method, the study carried out a questionnaire survey to 163 care workers working at the geriatric care facilities (10) in D Metropolitan City from May 1~31, 2015 and then the collected data were statistically analyzed. As a result, the oral health knowledge of care workers was 20.09 points out of total 26 points (77.2%). The highest knowledge was 'proper toothbrushing method' with 93.0% and the lowest one was 'knowledge on the use of oral supplementary goods' with 58.1%. Those with experience of oral health education had higher demand on oral health education (P<0.001), and those with higher experience of oral health education had significantly higher oral health knowledge (P<0.001), demand (P<0.01) and awareness (P<0.05). As for factors influencing care workers' oral health awareness, if they had higher oral health knowledge points (P<0.01), oral health education demand (P<0.01), they had higher oral health awareness level. Accordingly, it is required to provide efficient, sustainable and practical oral health education of care workers considering oral characteristics of the long-term hospitalized patients at the care hospital. And, the role of dental hygienist as the person in charge of oral health education is necessary. Therefore, institutional support from the government is required to assign professional personnel.

Convergence Research on Oral Care and Subjective Oral Health of Elderly with Diabetes: Focus Group Interview (노인 당뇨병 환자의 구강관리 및 주관적 구강건강에 관한 융합연구: 포커스그룹 면담)

  • This study was intended to prepare the basic data required for the development of oral products for the patient with diabetes by analyzing the relation between diabetes and oral health and the actual condition of oral care from the senior citizens diagnosed with diabetes out of the senior citizens over 65. In this study, 8 senior citizens who visited the senior welfare center in D city, were organized into the group of 4 senior citizens and the interview approximately for 40 minutes per group was conducted. In the results of this study, 5 core topics of 'Diabetes and Oral Health", 'Oral Discomfort', 'Oral Care Method', 'Visit to Dentist' and 'Demand for Oral Care' were drawn. For the relation between diabetes and the oral health, the most of research subjects answered that they heard about it for the first time, They answered that for the oral health care, they brushed the teeth using toothpaste 3 ~ 4 times a day and for the visit to dentist, they visited the dentist only when they had oral problem. For what is necessary for oral care, they answered that they wish that the oral care education customized for patient with diabetes and diverse oral care products such as toothpaste, etc. would be developed. Considering the results of this study, the development of oral care management program and the development of oral products in future are deemed to be necessary.

Changes in concentration of VSCs after home oral care interventions based on community care in older adults (커뮤니티케어 기반 방문구강관리 중재에 의한 노인의 구취 농도 변화)

  • Objectives: This case study was conducted to assess changes in the concentration of volatile sulfur compounds (VSCs) of older adults following home oral health care interventions based on community care. Methods: The participants were three elderly people with the halitosis. An oral health intervention programs including oral massage, oral hygiene care, and oral muscle training strenghtening were conducted for 12 weeks. Halitosis was measured using an oral malodor-checking device. Results: The program showed positive effects on changes in halitosis. The concentration of VSCs of the first case decreased rapidly from 44.5 Refres Oral Volume (ROV) on pre-test to 15.5 ROV on the first post-test. In the second case, the score decreased from 14.5 ROV on pre-test to 12 ROV on 2nd post-test. In the third case, the score decreased slightly from 6.5 ROV on pre-test to 6 ROV on the first post-test. Conclusions: Oral health care interventions contributed to decreasing the concentration of VSCs and improving the quality of life of older adults. The active promotion and customization of these programs are required.

A STUDY ON THE REFERRAL PROGRAM FROM PRIMARY HEALTH CARE FACILITIES IN A RURAL AREA (일부 농촌지역의 의뢰환자 프로그램 운영에 관한 연구)

  • This study was conducted to follow the patient referral system operated from the primary health care facilities to the hospital in a rural area of Korea. The subject for this study was sampled from a community health development project carried out by the Korea University in Yeoju Kun, Kyonggi Province. The data of referred patients from primary health care facilities were collected during the period from January 1989 to December 1989. The data was sorted out by a computer system using Database package. The results of this study were summarized as follows: 1. Characteristics of the referred patients were: males $32.0\%$. and females $68.0\%$. The more elderly of the patients visited to the hospital after having been referred there by CHPs or public physicians, $25.9\%$ has been to hospital on at least one previous occasion as against $74.1\%$ for whom it was there first visit. 2. The majority of patients who were referred to a hospital where: medicine $44.3\%$ and orthopedics$16.4\%$, major diseases were : diseases of digestive system $(21.3\%)$ ; symptoms and ill defined conditions $(17.3\%)$ ; diseases of the muscular skeletal system and connective tissue$(14.2\%)$. chronic illness was $82.0\%$ and acute illlness was $18.0\%$. 3. From Community health practitioners more patients referred than the public physicians. Categoris of diseases of the referred patients were different between community health practitioners and public physicians. Due to the. respective differences between the medical restrictions put on the nursing staffs at the community health practitioners and public physicians. From this study it was recommended to define the reason of differences between ~he two groups in futher study. Study as to 1) why one group should be referring more for hospital treatment than the other. And 2) why the two agencies should be referring different diseases.

A study on the present status and improving management of the non-eligible people in Korean long-term care insurance system (노인장기요양보험 등급외 판정자의 관리현황과 개선방안)

  • To vitalize the link program of Korean long-term care insurance system to community-based services for non-eligible people, we analysed the claim data from the Korean National Health Insurance Corporation (NHIC), and conducted a questionnaire survey to charging employees of elderly service department at local governments. The subjects were all 81,377 people, 57,454 of them were arranged to community-based services. The link program was more necessary among the missed subjects rather than the arranged people due to the need for physical or psychological assistance. By the result of the survey to the local government employees, 59.5% of subjects responded their proportion of link service was over 10% and under 20%, and 54.3% of them responded their job boundary are not clear. Major type of linking was notification the subject list to local government, 91.4%; proportion of periodical notification on the status of their service link were 57.1%, only 7.1% were followed to manage after the link. Difficult factors at the link process were pointed out the overload by other side work, deficiency of resources, rigidity of priority of link, and so on. Considering these results, to vitalize the community-based services to the non-eligible people, it may be essential the active participation of the subjects, construction of parts working in coordination among the institutions including NHIC, local governments, and service providers; development of various services for maintenance or promotion of the non-eligible peoples' health and functional status; and active participation of institutions from the third sector, and so on.

Reforming the Rural Health Insurance Programs in Korea (농촌의료보험의 당면과제와 개선방향)

  • Despite its universal coverage of health insurance, the rural health insurance program(RHIP) stands at the crossroads in Korea. The RHIP has weaknesses in stability of financing, problems of inequities in the provision of health services and has suffered from high cost of running the program. The author has analyzed these problems from the perspective of health insurance policy and presented several options for improvement. First of all, this study urged the importance of a firm Governmental commitment of RHIP with the 50% subsidization of contributions as the Government had promised, instead of the current 40%. This can be justified from the 20% subsidization by the Government for the contributions of private school teachers and their dependents, who belong to richer segments of the population. Second, various cost containment measures ought to be sought curbing the rising demand for medical through strengthening health education and increasing individual responsibility, and tightening the claim review process. Third, this study requires the Government to run a demonstration project on the introduction of case payment system for primary health care. Fourth introducing an income-related cost sharing scheme is another possibility. Reforming the cost sharing formula for large medical expenditures is recommendable for a beginning. This measure can take the form of tax credit for medical expenditures of the poor. Fifth, the degree of financial adjustment among health insurance plans should be levelled up for enhancing stability of RHIP and social solidarity. Sixth, health policy should be redirected toward development of rural health resources and higher priority should be put on relieving difficulties in access to care. Seventh. the insurance plan owned-hospital needs to be developed or provision of health services in the medically underserved areas, and the need of such facilities is particularly acute for geriatric care, rehabilitation and renal dialysis, etc. Eighth, more generous insurance benefits are required of the elderly who are suffering the most : elimination of the maximum 180 days of benefit period and provision of glasses and artificial dentures, etc. Ninth. the economies of scale principle is working for the operating expenses of regional self-employed insurance plan. Thus, measures should be instituted to pursue an optimum size of health insurance plans. Lastly, excessive dependence on exclusion items is an evil so that some radical remedies are urgently required to cut them.
