  • Background: Detection and treatment of tuberculosis (TB) infection with contact investigation is a key component of TB control program. I evaluated the prevalence and risk factors for TB infection among contacts of recently diagnosed pulmonary TB patients in a tertiary hospital in Korea. Methods: 206 contacts of 90 adult pulmonary TB patients underwent tuberculin skin tests (TST) and chest radiography. The TST results were considered positive with an induration of 10 mm or more, suggesting TB infection. A standardized questionnaire was used to assess risk factors associated with TB infection. Results: TST was positive in 97 of 206 contacts of TB patients (47.1%) and positive rate of TST increased with age. The risk of TB infection was significantly associated with close contact with TB patients (sleeping in the same room) (odd ratio [OR], 4.94; 95% confidence interval [CI], 1.43~17.00). Conclusion: TB infection rate was higher in the elderly, and the risk of TB infection was significantly increased with close contact of TB patients.

  • Background: Notified tuberculosis (TB) cases in Korea have not decreased over the last decade (2001-2010). Methods: To clarify the reasons, we analyzed an annual report on notified tuberculosis patients and age-specific population drift in Korea. Results: Compared to the age-specific notified TB cases between 2001 and 2010, distinctive features in notified TB cases and new cases increased markedly in people aged 45-54 years and in patients over 65 years old, whereas those between 15-34 years in 2010 decreased drastically. In particular, notified TB individuals over 65 years old occupied 29.6% of the cases in 2010, which was 1.5 times higher than that in 2001. The main reason not to decrease in notified TB patients for the last decade (2001-2010) was due to the increasing elderly population as well as the aging of baby boomers, which have a higher risk of TB development. Conclusion: Korea needs to pay attention to the older population in order to successfully decrease the burden of TB in the future.

  • Pneumonia represents a spectrum of diseases that range from community-acquired to health care-associated pneumonia. Despite advances in diagnosis, antimicrobial therapy, and supportive care, pneumonia remains an important cause of morbidity and mortality, particularly in elderly patients and in those with significant comorbidities. Community-acquired pneumonia (CAP) is the leading cause of death from infectious disease in Korea. This article provides a synopsis of recent studies regarding various types of pneumonia, with a focus on CAP.

  • Background : The frequency of MOTT has risen as the prevalence of tuberculosis has been declining. Our country has been also. The most of MOTT was resistant to the major anti-tuberculous drugs. Method : To compare clinical characteristics and frequencies of MDR tuberculosis with MOTT, the author studied 65 patients showing AFB culture positive with sputum. The data were collected from 176 patients who had been admitted at the National Masan Tuberculosis Hospital from May to June, 1997 to April, 1998. Result : The frequency of MDR tuberculosis was 43.1% and that of MOTT was 9.2%. Among 65 isolated mycobacteria, 3 cases were M. intracellulare. 2 cases were M. fortuitum, and 1 case was unidentified MOTT. The most frequent age group in 65 culture positive patients was 4th decade and the mean age was 44. The mean age was 61 in MOTT and 42 in M. tuberculosis and had significant difference(p<0.01). The numbers with past history of treatment were 2.3 in MDR tuberculosis and 1.7 in non-MDR tuberculosis and had significant difference(p<0.05). At the time of admission, the most frequent regimen for the treatment of MDR tuberculosis was 24 months regimen(85.7%) with the 2nd line anti-tuberculosis drugs. For non-MDR tuberculosis, 9 or 12 months regimen (72.9%) with the 1st line anti-tuberculosis drugs and had significant difference (p<0.01). At the time of admission, the symptom of weight loss was shown in 84.7% of M. tuberculosis and 50.0% in MOTT and there was significant difference(p<0.05) between them. All of the MOTT were identified to be resistant against INH and PAS. Drug resistance rates to INH, OFX(p<0.01) and PAS(p<0.05) in MOTT were higher than in MDR. All of three M. intracellulare strains were resistant to INH, RFP, PAS and OFX. All of two M. fortuitum strains were resistant to most anti-tuberculosis drugs. And the other MOTT was resistant to INH, EMB and PAS. Conclusion : MOTT was more common in elderly patients than M. tuberculosis. MOTT cases should be considered to be the probability of multiple drug resistance and treatment failure during the 1st treatment because they showed more resistance to anti-tuberculosis drugs than M. tuberculosis cases. Therefore, there should be more careful investigations for clinical characteristics, natural history of disease, and efficient management for MOTT.

  • Background: Methicillin-resistant Staphylococcus aureus (MRSA) infection is a severe and life-threatening disease in patients with community-onset (CO) pneumonia. However, the current guidelines lack specificity for a screening test for MRSA infection. Methods: This study was retrospectively conducted in elderly patients aged ${\geq}65years$, who had contracted CO-pneumonia during hospitalization at the Jeju National University Hospital, between January 2012 and December 2014. We analyzed the risk factors of MRSA in these patients and developed a scoring system to predict MRSA infection. Results: A total of 762 patients were enrolled in this study, including 19 (2.4%) with MRSA infection. Healthcare-associated pneumonia (HCAP) showed more frequent MRSA infection compared to community-acquired pneumonia (4.4% vs. 1.5%, respectively; p=0.016). In a multivariate logistic regression analysis, admissions during the influenza season (odds ratio [OR], 2.896; 95% confidence interval [CI], 1.022-8.202; p=0.045), chronic kidney disease (OR, 3.555; 95% CI, 1.157-10.926; p=0.027), and intensive care unit admission (OR, 3.385; 95% CI, 1.035-11.075; p=0.044) were identified as predictive factors for MRSA infection. However, the presence of HCAP was not significantly associated with MRSA infection (OR, 1.991; 95% CI, 0.720-5.505; p=0.185). The scoring system consisted of three variables based on the multivariate analysis, and showed moderately accurate diagnostic prediction (area under curve, 0.790; 95% CI, 0.680-0.899; p<0.001). Conclusion: MRSA infection would be considered in elderly CO-pneumonia patients, with three risk factors identified herein. When managing elderly patients with pneumonia, clinicians might keep in mind that these risk factors are associated with MRSA infection, which may help in selecting appropriate antibiotics.

  • In 2021,the incidence of newly notified tuberculosis in Jeju Province decreased by 23.0% compared to that in the previous year. This was the largest decline among the 18 municipalities studied. This study aimed to examine trends in the incidence of tuberculosis among different age and sex groups in Jeju Province between 2017 and 2021. This study observed the number of new cases in the annual report from 2017 to 2021, published by the Jeju Center for Infectious Diseases Control and Prevention. The average annual percentage change (AAPC; %) was calculated to determine the trend in crude incidence rates over the past 5 years by sex and age. While a P-value of <0.1 was considered marginally significant, aP-value of <0.05 was considered statistically significant. The AAPC of all age groups in Jeju Province had negative values. In men, there was a statistically significant decrease in ages 30-34, 40-44, 45-49, and 50-54 years and a marginally significant decrease in ages 35-59, 55-59, 60-64, 65-69, 75-79, and ≥80 years. In women, a statistically significant decrease was observed among patients aged 30-34, 35-39, and ≥80 years, and a marginally significant decrease was observed among patients aged 45-49, 50-54, 60-64, 65-69, and 75-79 years. A statistically significant decrease in tuberculosis was observed among patients in their 30s and 40s, and elderly patients aged ≥80 years comprised the group vulnerable to tuberculosis. Based on these findings, tuberculosis prevention and screening programs conducted in Jeju Province were effective.

  • Background: Community-acquired pneumonia (CAP) is one of the leading causes of death among the elderly. Several studies have reported the clinical usefulness of serum procalcitonin, a biomarker of bacterial infection. However, the association between the levels of procalcitonin and the severity in the elderly with CAP has not yet been reported. The aim of this study was to evaluate usefulness of procalcitonin as a predictor of severity and mortality in the elderly with CAP. Methods: This study covers 155 CAP cases admitted to Pusan National University Hospital between January 2010 and December 2010. Patients were divided into two groups (${\geq}65$ years, n=99; <65 years, n=56) and were measured for procalcitonin, C-reactive protein (CRP), white blood cell, confusion, uremia, respiratory rate, blood pressure, 65 years or older (CURB-65) and pneumonia severity of index (PSI). Results: The levels of procalcitonin were significantly correlated with the CURB-65, PSI in totals. Especially stronger correlation was observed between the levels of procalcitonin and CURB-65 in the elderly (procalcitonin and CURB-65, ${\rho}$=0.408 with p<0.001; procalcitonin and PSI, ${\rho}$=0.293 with p=0.003; procalcitonin and mortality, ${\rho}$=0.229 with p=0.023). The correlation between the levels of CRP or WBC and CAP severity was low. The existing cut-off value of procalcitonin was correlated with mortality rate, however, it was not correlated with mortality within the elderly. Conclusion: The levels of procalcitonin are more useful than the levels of CRP or WBC to predict the severity of CAP. However, there was no association between the levels of procalcitonin and mortality in the elderly.

  • Purpose: Tuberculosis(TB) care unit in public health center should be carefully considered to be re-designed as an infection safety environment for both patient and healthcare workers. So, for the enhancement, this study analyses the facility requirements for co-using the screening clinic as a TB and other respiratory disease care unit. Methods: Not only screening clinic facility guidelines from "A Study for Standard Triage Design and Construction Document" but also the guidelines of TB care and related medical facility are reviewed; KDCA, CDC, ECDC and WHO as a TB care, and FGI and NHS for facility. The facility requirements are summarized space, approach, and mechanical requirement in order. By comparing the summary and screening clinic facility guidelines, supplementations are proposed for TB care unit setting. Results: The result of this study shows that both the space program and mechanical requirement of the screening clinic and that of TB care unit are almost identical and could be share, which include direct airflow or negative air pressure in an exam room. To increase functional and economical efficiency, however, it is necessary to consider a multi-functional negative pressured room, So care process may be re-designed based on a room type; face-to-face room or glass wall inbetween. Implications: The facility guidelines for TB care unit of a public health center should be developed to build a safe environment for infection control by reflecting its medical plan and budget.
