목적: 3년이 경과된 진구성 주관절 탈구 증례를 경험하였기에 보고하고자 한다. 대상 및 방법: 45세 여자가 3년이 경과된 진구성 주관절 탈구로 수술적 소견상 주관절 외측 및 내측 측부 인대는 구축되어 있어 박리술을 시행하였다. 후방 관절낭을 완전히 유리시키고 전방 관절낭을 절개한 이후 수동적 조작을 가하여 요상완 및 척상완 관절을 정복시켰다. 변형된 Morrey형의 경첩형 외고정 장치를 주관절부에 장착하고 조기 주관절 운동을 시행하였다. 결과 및 결론: 저자들은 3년이 경과된 진구성 주관절 탈구에서 개방적 정복과 경첩형 외고정 장치를 이용한 고정술의 만족스런 결과를 경험하여 문헌고찰과 함께 보고하는 바이다.
The purpose of this study was to identify the difference and correlation in elbow joint maximal flexion strength according to measurement methods and characteristics of muscular contraction, and to develop the predictive equation of elbow joint maximal flexion strength for the optimal exercise intensity setting and accurate measurement. Subjects were 30 male university students. Elbow joint maximal flexion strength of isokinetic contraction, isometric contraction at $75^{\circ}$ elbow joint flexion position, isotonic concentric 1RM, manual muscle strength (MMT) were measured with isokinetic dynamometer, dumbbell, and manual muscle tester. Pearson's r, linear regression equation, and multiple regression equation between variables were calculated. As a result, the highest value was isometric contraction. The second highest value was MMT. The third highest value was isokinetic contraction. 1RM was the lowest. Predictive equations of elbow joint maximal flexion strength between isometric and isokinetic contraction, between isometric contraction and 1RM, among isometric contraction, 1RM, and body weight were developed. In conclusion, 1RM and isokinetic elbow joint maximal flexion strength could be seemed to underestimate the practical elbow joint maximal flexion strength. And it is suggested that the developed predictive equations in this study should be useful in criteria- and goal-setting for resistant exercise and sports rehabilitation after elbow joint injury.

Background: In elbow fracture-dislocation, partial excision of the comminuted radial head fracture that is not amenable to fixation remains controversial considering the accompanying symptoms. This study was undertaken to evaluate the results of radial head partial excision when the comminuted radial head fracture involved <50% of the articular surface in all-arthroscopic repair of elbow fracture-dislocation. Methods: Patients were divided into two groups based on the condition of the radial head fracture. In Group A, the patients had a radial head comminuted fracture involving <50% of the articular surface, and underwent arthroscopic partial excision. Group B was the non-excision group comprising patients with stable and non-displacement fractures. Follow-up consultations were conducted at 6 weeks and at 3, 6, 12, and 24 months after surgery. Results: In all, 19 patients (Group A: 11; Group B: 8) met the inclusion criteria and were enrolled in the study. At the final follow-up, all 19 patients showed complete resolution of elbow instability. No significant differences were observed in the range of motion, visual analogue scale score, and Mayo elbow performance score between groups. Radiological findings did not show any complications of the radiocapitellar joint. However, nonunion of the coracoid fracture was observed in 3 patients (Group A: 1; Group B: 2), without any accompanying instability and clinical symptoms. Conclusions: Considering that the final outcome is coronoid fracture fixation and lateral collateral ligament complex repair for restoring elbow stability, arthroscopic partial excision for radial head comminuted fractures involving <50% of articular surface is an effective and acceptable treatment for elbow fracture-dislocation.

Background: The Discovery Elbow System (DES) utilizes a polyethylene bearing within the ulnar component. An exchange bearing requires preoperative freezing and implantation within 2 minutes of freezer removal to allow insertion. We report our outcomes and experience using this technique. Methods: This was an analysis of a two-surgeon consecutive series of DES bearing exchange. Inclusion criteria included patients in which exchange was attempted with a minimum 1-year follow-up. Clinical and radiographic review was performed 1, 2, 3, 5, 8 and 10 years postoperative. Outcome measures included range of movement, Oxford Elbow Score (OES), Mayo Elbow Performance Score (MEPS), complications and requirement for revision surgery. Results: Eleven DESs in 10 patients were included. Indications were bearing wear encountered during humeral component revision (n=5); bearing failure (n=4); and infection treated with debridement, antibiotics and implant retention (DAIR; n=2). Bearing exchange was conducted on the first attempt in 10 cases. One case required a second attempt. One patient developed infection postoperatively managed with two-stage revision. Mean follow-up of the bearing exchange DES was 3 years. No further surgery was required, with no infection recurrence in DAIR cases. Mean elbow flexion-extension and pronosupination arcs were 107°(±22°) and 140° (±26°). Mean OES was 36/48 (±12) and MEPS was 83/100 (±19). Conclusions: Our results support the use of DES bearing exchange in cases of bearing wear with well-fixed stems or acute infection. This series provides surgeons managing DES arthroplasty with management principles, successful and reproducible surgical techniques and expected clinical outcomes in performing DES polyethylene bearing exchange. Objective : The objective of this study is to observe the effect of Pharmacopuncture of Anti-inflammatory Herbal compound(AiC) theraphy on elbow pain Methods : The patients diagnosed as lateral epicondylitis and treated mainly with Anti-inflammatory Herbal compound(AiC). After the application of herbal acupuncture, The NRS and ROM of elbow were assessed. Results & conclusion : Symptoms of the patient such as elbow pain and dysfunction were improved after above treatments. so, it is suggested that oriental medical treatment(Pharmacopuncture of Anti-inflammatory Herbal compound(AiC)) is effective on Tennis elbow.
The purpose of this study is to evaluate the effect of local wall thinning on the collapse of elbow subjected to internal pressure and bending moment. Thus, the nonlinear 3D finite element analyses were performed to obtained collapse moment of elbow containing various wall thinning defects under two loading; modes (closing and opening modes) and defect locations (intrados and extrados). From the results of analyses, the influence of wall thinning defect on the global moment-rotation behavior of elbow was discussed, and the dependance of collapse moment of elbow on wall thinning depth, length, and circumferential angle was investigated under different loading mode and defect location.
Ratcheting behavior of $90^{\circ}$ elbow piping subject to internal pressure 20 MPa and reversed bending 20 kN was investigated using experimental method. The maximum ratcheting strain was found in the circumferential direction of intrados. Ratcheting strain at flanks was also very large. Moreover, the effect of temperature on ratcheting strain of $90^{\circ}$ elbow piping was studied through finite element analysis, and the results were compared with room condition ($25^{\circ}$). The results revealed that ratcheting strain of $90^{\circ}$ elbow piping increased with increasing temperature. Ratcheting boundary of $90^{\circ}$ elbow piping was determined by Chaboche model combined with C-TDF method. The results revealed that there was no relationship between the dimensionless form of ratcheting boundary and temperature.
In a broad sense, the healthcare system refers to the conventional medical service for the field of treatment. From head to toe, our whole body corresponds to the subject of the medical service. In this paper, we discuss the abnormality of flexion and extension in general elbow disease. Flexion and extension refer to flexing and extending of the arm while it is set to be at 90 degrees. In this case, if the angle of the arm is remarkably small or is accompanied with pain, there is an abnormality that occurs in the elbow. We tested the flexion and extension of the elbow for 100 people in their 50s and calculated the number of people for each case. Afterwards, we classified people with abnormalities in flexion and extension and presented the respective treatment methods. In this paper, a system was constructed for the treatment of musculoskeletal disorders.
In the present study, three-dimensional finite element analysis was performed to investigate the effects of internal wall thinning defect on the failure pressure of elbow in the piping system and to develop the failure pressure evaluation model. From the results of finite element analysis, the failure pressure was derived by employing local stress criteria, and the effects of thinning location, bend radius, and defect geometry on the failure pressure of internally wall thinned elbow were investigated. Also, based on these investigations and previous model developed to estimate the failure pressure of elbow with an external pitting defect, the failure pressure evaluation model to be applicable to the elbow containing an internal thinning defect was proposed and compared with the results of finite element analysis. The failure pressure calculated by the model agreed well with the results of finite element analysis.
