팬텀연구에서 경직장 전단파탄성초음파의 가변성 (Variability of Transrectal Shear Wave Elastography in a Phantom Model)

  • 목적 본 연구는 제작한 팬텀을 사용해 경직장 전단파탄성초음파의 가변성을 알아보았다. 대상과 방법 아가로즈와 실리콘에멀전을 각각 1, 2, 3 cm 크기의 둥근 모양과 사각 모양의 팬텀 물질로 제작하였다. 1, 2, 3 cm의 깊이에 팬텀을 놓고, 크기, 깊이, 모양에 따른 굳기값(coefficient variant)의 차이를 중심부/주변부에서 확인하였다. 두 명의 영상의가 경직장 초음파 탐촉자를 이용해 각각 3회씩, 두 개의 초음파기계로(기계 A, B), 굳기값을 확인하였다. 가변성은 변동계수로 표현하였다. 결과 팬텀의 크기가 커질수록 변동계수는 감소하였다. 크기에 따른 굳기값은, 아가로즈 팬텀은 기계 A 3 cm 깊이(p < 0.001), 기계 B 1 cm 깊이에서(p = 0.010), 실리콘에멀전 팬텀은 2 cm 깊이에서 두 기계 모두 유의한 차이를 보였다(p = 0.047, p = 0.020). 깊이가 깊어질수록 변동계수는 증가하였다. 깊이에 따른 굳기값은, 1 cm 크기 아가로즈 팬텀은 두 기계 모두(p = 0.037, p = 0.021), 2 cm 크기 아가로즈 팬텀은 기계 A에(p = 0.047) 유의한 차이를 보였다. 기계 A 실리콘에멀전에서만 모양에 따른 굳기값의 유의한 차이를 보였고(p = 0.032) 기계 B는 두 물질 모두 관심영역에 따른 굳기값의 유의한 차이가 보였다. 굳기값은 두 기계 간 유의한 차이가 있었고(p < 0.05), 시술자 내/시술자 간 일치도는 높았다(급내상관계수 > 0.9). 결론 팬텀의 크기, 깊이, 사용된 기계가 전단파탄성초음파 가변성에 영향을 주는 요소로 나타났다.

Quantitative Evaluation of Gastrocnemius Medialis Stiffness During Passive Stretching Using Shear Wave Elastography in Patients with Parkinson's Disease: A Prospective Preliminary Study

  • Objective: To prospectively investigate the feasibility of shear wave elastography (SWE) as a new quantitative and objective method for evaluating the stiffness of the gastrocnemius medialis (GM) muscle during passive stretching in patients with Parkinson's disease (PD). Materials and Methods: SWE of the GM muscle was performed in 28 patients with PD [13 female and 15 male; mean age ± standard deviation (SD): 63.0 ± 8.5 years] and 12 healthy controls (5 female and 7 male; mean age ± SD: 59.3 ± 6.4 years) during passive ankle rotation. A Young's modulus-ankle angle curve was constructed. The GM slack angle and baseline Young's modulus (E0) were compared between the markedly symptomatic and mildly symptomatic sides of patients with PD, and healthy controls. Additionally, the correlation between the GM slack angle and the severity of rigidity, and the observer reproducibility of SWE in determining the GM slack angle were evaluated. Results: The GM slack angle was smaller on both the markedly and mildly symptomatic sides in patients with PD than in healthy controls (mean ± SD of -29.13° ± 3.79° and -25.65° ± 3.39°, respectively, vs. -21.22° ± 3.52°; p < 0.001 and p = 0.006, respectively). Additionally, in patients with PD, the GM slack angle on the markedly symptomatic side was smaller than that on the mildly symptomatic side (p = 0.003). The E0 value was lower on both the markedly and mildly symptomatic sides in patients with PD than in healthy controls (mean ± SD of 10.11 ± 2.85 kPa and 10.08 ± 1.88 kPa, respectively, vs. 12.23 ± 1.02 kPa; p = 0.012 and p < 0.001, respectively). However, no significant difference was found between the markedly and mildly symptomatic sides in patients with PD (p = 0.634). A negative linear relationship was observed between the GM slack angle and lower limb rigidity score on the markedly symptomatic side in patients with PD (r = -0.719; p < 0.001). The intraclass correlation coefficients for observer reproducibility of SWE ranged from 0.880 to 0.951. Conclusion: The slack angle determined by SWE may be a useful quantitative and reproducible method for evaluating muscle stiffness in patients with PD.

Diagnostic Potential of Strain Ratio Measurement and a 5 Point Scoring Method for Detection of Breast Cancer: Chinese Experience

  • Aim: To evaluate the differential diagnostic potential of lesion stiffness assessed by the sonoelastographic strain index ratio (SR) and elastographic color scoring system (UE) for breast lesions. Materials and Methods: Three hundred and forty two breast masses (158 benign and 184 malignant) from 325 consecutive patients (mean age 44.2 years; range 16-81)who had been scheduled for a sonographically guided core biopsy were examined proposed by Itoh et al, with scoring 1-3=benign and 4-5=malignant. Strain and area ratios of each lesion were calculated within the same machine. Histological diagnosis was used as the reference standard. The area under the curve (AUC) and cut-off point were obtained by receiver operating curve and the cross table Fischer Test was carried out for assessing diagnostic value. Sensitivity, specificity, PPV, NPV, accuracy and false-discovery rates were compared. Results: The mean strain ratios for benign and malignant lesions were 1.87 and 7.9 respectively. (P<0.0001). When a cutoff point of 3.54 was used, SR had a sensitivity of 94.6%, a specificity 94.3%, a PPV of 95.1%, an NPV of 93.7% and an accuracy of 94.4%. The AUC values were 0.90 for the 5 point scoring system (UE) and 0.96 for the strain index ratio. The overall diagnostic performance was SR method was better (P<0.05). Conclusions: Strain ratio measurement could be another effective predictor in elastography imaging besides 5 the point scoring system for differential diagnosis of breast lesions.

Clinical Utility of Liver Stiffness Measurements on Magnetic Resonance Elastrography in Patients with Hepatocellular Carcinoma Treated with Radiofrequency Ablation

  • Purpose: To determine whether liver stiffness (LS) measured by magnetic resonance elastography (MRE) can predict the outcome of radiofrequency ablation (RFA) in hepatocellular carcinoma (HCC) patients. Materials and Methods: A total of 107 patients with Child-Pugh class A liver function who were treated with RFA for single HCC and who had undergone a gradient-echo MRE within 6 months before RFA were included. We evaluated the relationship between the LS values and the ablation volume, local tumor progression (LTP), and intrahepatic distant recurrence (IDR). We also constructed receiver operating characteristic (ROC) curves to examine the role of LS in predicting liver function deterioration, which was defined as an increase of Child-Pugh score by one point or more at 1 year after RFA. Results: There was no significant correlation between LS and ablation volume, and neither time to LTP nor IDR was associated with LS. Among the 66 patients who did not have recurrence 1 year after RFA, 5 patients (7.6%) developed liver function deterioration. A high LS value was significantly associated with development of liver function deterioration after RFA and the area under the ROC curve was 0.764 (95% CI 0.598-0.929, P = 0.003). Conclusion: LS measured by MRE could not predict ablation volume and tumor recurrence. However, high LS values were significantly associated with development of liver function deterioration.

초음파 탄성 영상 알고리듬 (Algorithms for Ultrasound Elasticity Imaging)

  • 초음파를 사용해 음속도, 감쇠 계수, 밀도, 비선형 B/A 파라미터 등을 측정하여 인체 조직의 특성을 정량적으로 영상화하고자 하는 연구가 1980년대부터 많이 진행되어 왔으나 아직 상용화 단계에는 도달하지 못했다. 하지만 1990년대 초에 시작된 탄성 영상법은 최근 들어 초음파 진단기에 상용화되어 임상에서 B-모드 영상법과 함께 전립선, 유방, 갑상선, 간, 혈관 등을 진단하기 위한 보완적이며 더 정량적인 모드로 사용되고 있는 단계에 진입하였다. 본고에서는 주로 준정적 또는 정적탄성 영상법에 사용되는 여러 가지 알고리듬을 소개하고 특성을 비교하고자 한다. 대부분의 알고리듬은 상호상관함수 또는 자기상관함수 방법에 그 기반을 두고 있으며 전자는 래그를 변화시켜가면서 시간 이동량을 찾지만 후자는 보간 과정 없이 고정된 래그에서의 위상차로부터 시간 이동량을 바로 구해 변위를 추정하는 점이다.

Correlating two-dimensional shear wave elastography of acute pancreatitis with Spec cPL in dogs

  • Background: Pancreatitis is a common disease in which 37% of dogs had evidence of acute or chronic pancreatitis at necropsy. Although biopsy is still the gold standard to diagnose acute pancreatitis, clinical data including ultrasonographic findings and measurement of canine serum pancreatic lipase immunoreactivity (cPLI) are used in routine. However, it may be insufficient in the diagnostic approach to acute pancreatitis. Objectives: To evaluate the clinical diagnostic feasibility of two-dimensional shear wave elastography (2D SWE) on canine acute pancreatitis for enhanced diagnostic confidence. Methods: 2D SWE was used to assess pancreatic stiffness and determine the correlation between pancreatic shear wave velocity (SWV) and Spec cPL concentration in 31 dogs with healthy pancreas and 10 dogs with acute pancreatitis. Results: The pancreatic SWV was significantly higher in the acute pancreatitis group (2.67 ± 0.20 m/s) than in the healthy pancreas group (2.30 ± 0.26 m/s; p < 0.05). The moderate positive correlation was found between the pancreatic SWV and Spec cPL concentration (95% confidence interval, 0.214-0.693; r = 0.489; p < 0.05). Conclusions: These results indicated that 2D SWE was feasible for assessing pancreatic stiffness in acute pancreatitis, and that pancreatic SWV using 2D SWE correlated with Spec cPL concentration. SWE could provide a quantitative measure of pancreatic stiffness, which can increase the accuracy of diagnosing acute pancreatitis in dogs. The 2D SWE can be used as a complementary imaging modality for diagnosing acute pancreatitis in dogs.

Ultrasonographic evaluation of the masseter muscle in patients with temporomandibular joint degeneration

  • Purpose: Sonographic elastography can be used to evaluate the hardness of muscle tissue through the application of compression. Strain elastography gauges hardness through the comparison of echo sets before and after compression. This study utilized ultrasonography to measure the thickness and hardness of the masseter muscle in individuals with temporomandibular joint(TMJ) osteoarthritis. Materials and Methods: This study included 40 patients who presented with joint pain and were diagnosed with TMJ osteoarthritis via diagnostic cone-beam computed tomography, along with 40 healthy individuals. The thickness and hardness of each individual's masseter muscle were evaluated both at rest and at maximum bite using ultrasonography. The Mann-Whitney U test and the chi-square test were employed for statistical analysis, with the significance level set at P<0.05. Results: The mean thickness of the resting masseter muscle was 0.91 cm in patients with osteoarthritis, versus 1.00 cm in healthy individuals. The mean thickness of the masseter muscle at maximum bite was 1.28 cm in osteoarthritis patients and 1.36 cm in healthy individuals. The mean masseter elasticity index ratio at maximum bite was 4.51 in patients with osteoarthritis and 3.16 in healthy controls. Significant differences were observed between patients with osteoarthritis and healthy controls in both the masseter muscle thickness and the masseter elasticity index ratio, at rest and at maximum bite (P<0.05). Conclusion: The thickness of the masseter muscle in patients with TMJ osteoarthritis was less than that in healthy controls. Additionally, the hardness of the masseter muscle was greater in patients with TMJ osteoarthritis.

Quantification of Nerve Viscosity Using Shear Wave Dispersion Imaging in Diabetic Rats: A Novel Technique for Evaluating Diabetic Neuropathy

  • Objective: Viscoelasticity is an essential feature of nerves, although little is known about their viscous properties. The discovery of shear wave dispersion (SWD) imaging has presented a new approach for the non-invasive evaluation of tissue viscosity. The present study investigated the feasibility of using SWD imaging to evaluate diabetic neuropathy using the sciatic nerve in a diabetic rat model. Materials and Methods: This study included 11 diabetic rats in the diabetic group and 12 healthy rats in the control group. Bilateral sciatic nerves were evaluated 3 months after treatment with streptozotocin. We measured the nerve cross-sectional area (CSA), nerve stiffness using shear wave elastography (SWE), and nerve viscosity using SWD imaging. The motor nerve conduction velocity (MNCV) was also measured. These four indicators and the histology of the sciatic nerves were then compared between the two groups. The performance of CSA, SWE, and SWD imaging in distinguishing the two groups was assessed using receiver operating characteristic (ROC) analysis. Results: Nerve CSA, stiffness, and viscosity in the diabetic group was significantly higher than those in the control group (all p < 0.05). The results also revealed a significantly lower MNCV in the diabetic group (p = 0.005). Additionally, the density of myelinated fibers was significantly lower in the diabetic group (p = 0.004). The average thickness of the myelin sheath was also lower in the diabetic group (p = 0.012). The area under the ROC curve for distinguishing the diabetic neuropathy group from the control group was 0.876 for SWD imaging, which was significantly greater than 0.677 for CSA (p = 0.030) and 0.705 for SWE (p = 0.035). Conclusion: Sciatic nerve viscosity measured using SWD imaging was significantly higher in diabetic rats. The viscosity measured using SWD imaging performed well in distinguishing the diabetic neuropathy group from the control group. Therefore, SWD imaging may be a promising method for the evaluation of diabetic neuropathy.

유방의 결절성 근막염의 인공지능 유방촬영술과 탄성초음파를 포함한 영상 소견: 증례 보고 (Imaging Findings of Nodular Fasciitis in Breast including Artificial Intelligence Mammography and Shear Wave Elastography: A Case Report)

  • 결절성 근막염은 양성 섬유모세포 증식으로 유방에는 드물게 보고된다. 저자들은 악성 영상 소견을 보인 55세 여자 환자의 증례를 보고한다. 유방촬영술에서 오른쪽 유방에 유방조직과 비슷한 밀도를 보인 부분적으로 경계가 불명확한 결절이 보였고 인공지능 점수는 75%였다. 초음파에서 미세 소엽상 경계, 주변부 고에코를 가진 타원형의 저에코 결절이 보였고 증가된 혈류와 부드러운 탄성도를 보였다. 조직검사와 수술 후 결절성 근막염으로 진단되었다. 결절성 근막염은 유방에서 드물지만, 악성과 비슷하게 보일 수 있다. 따라서 불필요한 중재시술을 막기 위해 감별진단으로 고려되어야 한다.

Technical Performance of Two-Dimensional Shear Wave Elastography for Measuring Liver Stiffness: A Systematic Review and Meta-Analysis

  • Objective: To assess the technical performance of two-dimensional shear wave elastography (2D-SWE) for measuring liver stiffness. Materials and Methods: The Ovid-MEDLINE and EMBASE databases were searched for studies reporting the technical performance of 2D-SWE, including concerns with technical failures, unreliable measurements, interobserver reliability, and/or intraobserver reliability, published until June 30, 2018. The pooled proportion of technical failure and unreliable measurements was calculated using meta-analytic pooling via the random-effects model and inverse variance method for calculating weights. Subgroup analyses were performed to explore potential causes of heterogeneity. The pooled intraclass correlation coefficients (ICCs) for interobserver and intraobserver reliability were calculated using the Hedges-Olkin method with Fisher's Z transformation of the correlation coefficient. Results: The search yielded 34 articles. From 20 2D-SWE studies including 6196 patients, the pooled proportion of technical failure was 2.3% (95% confidence interval [CI], 1.3-3.9%). The pooled proportion of unreliable measurements from 20 studies including 6961 patients was 7.5% (95% CI, 4.7-11.7%). In the subgroup analyses, studies conducting more than three measurements showed fewer unreliable measurements than did those with three measurements or less, but no intergroup difference was found in technical failure. The pooled ICCs for interobserver reliability (from 10 studies including 517 patients) and intraobserver reliability (from 7 studies including 679 patients) were 0.87 (95% CI, 0.82-0.90) and 0.93 (95% CI, 0.89-0.95), respectively, suggesting good to excellent reliability. Conclusion: 2D-SWE shows good technical performance for assessing liver stiffness, with high technical success and reliability. Future studies should establish the quality criteria and optimal number of measurements.
