We studied the solar proton events (SPEs) associated with coronal mass ejections (CMEs) during the solar cycle 23 (1997-2006). Using 63 SPE dataset, we investigated the relationship among SPE, flare, and CME, and found that (1) SPE rise time and duration time depend on CME speed and the earthward direction parameter of the CME, and (2) the SPE peak intensity depends on CME speed and X-ray Flare intensity. While inspecting the relation between SPE peak intensity and the direction parameter, we found there are two groups: first group consists of large six SPEs (> 10,000 pfu at > 10 MeV proton channel of GOES satellite) and shows strong correlation (cc = 0.65) between SPE peak intensity and CME direction parameter. The second group has a weak intensity and shows poor correlation between SPE peak intensity and the direction parameter (cc = 0.01). By investigating characteristics of the first group, we found that all the SPEs are associated with very fast halo CME (> 1400km/s) and also they are mostly located at central region and within ${\pm}20^{\circ}$ latitude and ${\pm}30^{\circ}$ longitude strip.
Background: Although operative outcome is progressing due to the development of operative techniques and myocardial protection, some patients face an increased morbidity and mortality. Therefore, it has become increasingly important to predict the operative morbidity and mortality. Material and Method: This retrospective study reports the results of risk factor analysis of morbidity and mortality of 137 consecutive patients who were underwent coronary artery bypass graft surgery(CABG). Preoperative variables were age, sex, preoperative myocardial infarction, operative priority, left ventricular ejection fraction, obesity and triple vessel disease. Postoperative morbidities were arrhythmia, wound infection, cerebral infarction, prolonged postoperative hospitalization, pneumonia, acute renal failure, prolonged use of ventilator and operative death. Result: The mean age of total patients was 56.7 years, from 27 to 74. The overall mortality was 6.6%(9 of 137) with the mortality of 3.9%(5 of 128) for elective operation, and 44.4%(4 of 9) for emergent or urgent cases. The morbidity of patients over 65 years was stastistically higher than that of under 65 years. Sex distribution showed no difference in morbidity, however operative mortality rate was slightly higher in women (5/41, 12.19%) than in men(4/96, 4.17%). Morbidity of emergent or urgent operation was 100%, much higher than that of the elective operation. Mortality of the patients whose left ventricular ejection fraction was under 50% was higher than that of those over 50%. Conclusion: We concluded that the risk factors of morbidity after CABG were old age above 65 years and emergent or urgent operation, and that risk factors of mortality were low left venticular ejection fraction under 50% and emergent or urgent operation.
This study was to analysis what factors could affect left ventricle ejection fraction(LVEF) using 64-slice multidetector CT. 164 patients(84 men, 80 women) had a cardiac CT in this study, and their blood pressure, body mass index(BMI), heart rate(HR) measured. LVEF was 52.00${\pm}$18.95% in below 25kg/$m^2$, 59.50${\pm}$16.05% in above 25kg/$m^2$ of BMI. LVEF was 57.26${\pm}$17.84% in normal blood pressure group(NBPG), 49.95${\pm}$17.63 in hypertension group(HG). LVEF was 60.76${\pm}$17.26 in below 60 beats/min, 54.14${\pm}$16.56 in 60-70 beats/min, 50.83${\pm}$20.56 in above 70 beats/min of HR. LVEF was negatively correlated with age, HR(r=-0.283 p<0.05, r=-0.231 p<0.05. respectively). And LVEF was positively correlated with BMI(r=0.228 p<0.05). A measurement of LVEF at cardiac CT by using MDCT may be considered to age, blood pressure, heart rate and BMI.
Purpose: We compared estimates of ejection fraction (EF) determined by gated Tl-201 perfusion SPECT (g-Tl-SPECT) with those by gated blood pool (GBP) scan. Materials and Methods: Eighteen subjects underwent g-Tl-SPECT and GBP scan. After reconstruction of g-Tl-SPECT, we measured EF with Cedars software. The comparison of the EF with g-Tl-SPECT and GBP scan was assessed by correlation analysis and Bland Altman plot. Results: The estimates of EF were significantly different (p<0.05) with g-Tl-SPECT ($40%{\pm}14%$) and GBP scan ($43%{\pm}14%$). There was an excellent correlation of EF between g-Tl-SPECT and GBP scan (r=0.94, p<0.001). The mean difference of EF between GBP scan and g-Tl-SPECT was +3.2% Ninety-five percent limits of agreement were ${\pm}9.8%$. EF between g-Tl-SPECT and GBP scan were in poor agreement. Conclusion: The estimates of EF by g-Tl-SPECT was well correlated with those by GBP scan. However, EF of g-Tl-SPECT doesn't agree with EF of GBP scan. EF of g-Tl-SPECT can't be used interchangeably with EF of GBP scan.
Congenital aortic stenosis in children is characterized by "excessive" left ventricular hypertrophy with reduced left ventricular systolic wall stress that allows for supernormal ejection performance. We hypothesized that left ventricular wall stress was decreased immediately after surgical correction of pure congenital aortic stenosis. Also measuring postoperative left ventricular wall stress was a useful noninvasive measurement that allowed direct assessment for oxygen consumption of myocardium than measuring the peak systolic pressure gradient between ascending aorta and left ventricle for the assessment of surgical results. Material and Method: Between September 1993 and August 1999, 8 patients with isolated congenital aortic stenosis who underwent surgical correction at Yonsei cardiovascular center were evaluated. There were 6 male and 2 female patients ranging in age from 2 to 11 years(mean age, 10 years). Combined Hemodynamic-Ultrasonic method was used for studying left ventricular wall stress. We compared the wall stress peak systolic pressure gradient and ejection fraction preoperatively and postoperatively. Result: After surgical correction peak aortic gradient fell from 58.4${\pm}$17.6, to 23.7${\pm}$17.7 mmHg(p=0.018) and left ventricular ejection fraction decreased but it is not statistically significant. In the consideration of some factors that influence left ventricular end-systolic wall stress excluding one patient who underwent reoperation for restenosis of left ventricular outflow tract left ventricular end-systolic pressure and left ventricular end-systolic dimension were fell from 170.6${\pm}$24.3 to 143.7${\pm}$27.1 mmHg and from 1.78${\pm}$0.4 to 1.76${\pm}$0.4 cm respectively and left ventricular posterior wall thickness was increased from 1.10${\pm}$0.2, to 1.27${\pm}$0.3cm but it was not statistically singificant whereas left ventricular end-systolic wall stress fell from 79.2${\pm}$24.9 to 57.1${\pm}$27.6 kdynes/cm2(p=0.018) in 7 patients. For one patient who underwent reoperation peak aortic gradient fell from 83.0 to 59.7 mmHg whereas left ventricular end-systolic wall stress increased from 67.2 to 97.0 kdynes/cm2 The intervals did not change significnatly. Conclusion ; We believe that probably some factors that are related to left ventricular geometry influenced the decreased left ventricular wall stress immediately after surgical correction of isolated congenital aortic stenosis. Left ventricular wall stress is a noninvasive measurement and can allow for more direct assesment than measuring peak aortic gradient particularly in consideration of the stress and oxygen consumption of the myocardium therefore we can conclude it is a useful measurement for postoperative assessment of congenital aortic stenosis.
Left ventricular dysfuction is common in immediate postoperative periods after surgical correction of heart diseases with chronic left ventricular volume overload. We speculated postoperative changes of left ventricular volume and unction in patients with patent ductus osus(PDA) who had underwent surgical repair at ages older than 16 years. Factors influencing postoperative left ventricular volume and function were also analyzed. Material and Method: From August 1989 to August 1999 thirty-siz adult patients with PDA 28 females and 8 males. were enrolled in this study. Their age ranged from 16 years to 57 years(mean :32 years). Types of surgical repair were division with primary closure in 22, division with patch closure in 6, internal obliteration using cardiopulmonary bypass in 4 and ligation in 4. Aortic clamping was combined during surgical repair in 22(61%) and cardiopulmonary bypass was used in 8(22%) Two-dimensional echocardiography studies were performed in 34(94%) preoperatively and in 25(66%) immediate postoperatively to assess postoperative changes of left ventricular internal dimensions. left ventricular volume and ejection fraction. Duration of postoperative follow-up ranged from 1 month to 99 months (mean:22 months) and 10 patients underwent 16 echocardiographic evaluation during this period Result : Preoperative and postoperative left ventricular systolic dimensions(LVIDs) were 42$\pm$8.0mm and 42$\pm$8.3mm left ventricular diastolic dimensions(LVIDd) were 64$\pm$10.0mm and 56$\pm$7.4mm left ventricular end systolic volumes(LVESV) were 62$\pm$19cc (z=1.87$\pm$0.06) and 59$\pm$24cc(z=1.78$\pm$0.08) left ventricular end diastolic volumes(LVEDL) were 169$\pm$40cc(z-1.17$\pm$0.1) and 112$\pm$29cc(z=0.85$\pm$0.1) and ejection fractions(EF) were 66$\pm$6.7% and 48$\pm$12.6% respectively. There were statistically significant differences between preoperative and postoperative values in LVDIDd(p=0.001) LVEDV(p=0.001) and EF(p=0.0001) while no significant difference is LVIDs and LVESV. Postoperative depression of ejcection fraction was significantly related with z-score of preoperative LVESV and LVEDV by univariateanalysis while LVEDV only was significant risk factor for postoperative LV dysfunction by multiple regressioin analysis ($\Delta$LVEF=-13.3-4.62$\times$LVEDV(z), p=0.001) During the follow-up periods ejection fractions become normalized in all except one patients. Conclusion ; Left ventricular function is usually deteriorated after the surgical correction of PDA in adult age and preoperative LVEDV is a major determinant of postoperative LV function.

Background : The occurrence of atrial fibrillation after ablation of atrial flutter is clinically important. We investigated variables predicting this evolution in ablated patients without a previous atrial fibrillation history. Materials and Methods : Thirty-six patients (Male=28) who were diagnosed as atrial flutter without previous atrial fibrillation history were enrolled in this study. Group 1 (n=11) was defined as those who developed atrial fibrillation after atrial flutter ablation during 1 year follow-up. Group 2 (n=25) was defined as those who has not occurred atrial fibrillation during same follow-up term. Echocardiogram was performed to all patients. We measured left atrial size, left ventricle end diastolic and systolic dimension, ejection fraction and left atrial volume index before and after ablation of atrial flutter. The differences of each variables were compared and analyzed between two groups. Results : The preablation left ventricular ejection fraction (preLVEF) and postablation left ventricular ejection fraction (postLVEF) are $54{\pm}14%$, $56{\pm}13%$ in group 1 and $47{\pm}16%$, $52{\pm}13%$ in group 2. The differences between each two groups are statistically insignificant ($2.2{\pm}1.5$ in group 1 vs $5.4{\pm}9.8$ in group 2, p=0.53). The preablation left atrial size (preLA) and postablation left atrial size (postLA) are $40{\pm}4mm$, $41{\pm}4mm$ in group1 and $44{\pm}8mm$, $41{\pm}4mm$ in group 2. The atrial sizes of both groups were increased but, the differences of left atrial size between two groups before and after flutter ablation were statistically insignificant ($0.6{\pm}0.9mm$ in group 1 vs $-3.8{\pm}7.4mm$ in group 2, p=0.149). The left atrial volume index before flutter ablation was significantly reduced in group 1 than group 2 ($32{\pm}10mm^3/m^2$, $35{\pm}10mm^3/m^2$ in group 1 and $32{\pm}10mm^3/m^2$, $29{\pm}8mm^3/m^2$ in group 2, p<0.05). Conclusion : The difference between left atrial volume index before and after atrial flutter ablation is the robust predictor of occurrence of atrial fibrillation after atrial flutter ablation without previous atrial fibrillation.
Rest gated blood pool scan (Rest GBP scan) and dipyridamole $^{99m}Tc-MIBI$ SPECT were Performed in 34 patients with or suspected coronary artery disease. Both studies were performed within $2\sim32$ days (mean 8.1 days). A significant correlation was present between left ventricular ejection fraction (r= -0.7356, p<0.001) and peak ejection rate and peak filling rate in rest GBP scan and perfusion defect in MIBI SPECT. This study was carried out to get the optimum conditions for manufacturing high refractive glass beads from waste fluorescent lamp glass. Chemical composition, X-ray diffraction pattern, particle size distribution, refractive index of glass beads, and the effect of air mixing ratio and ejection rate were investigated. The obtained results are as follows. The X-ray diffraction pattern and chemical composition of glass beads made of waste fluorescent glass are similar to common glass except ReO2 0.0108 wt%, BaO 0.071 wt%, NiO 0.0039 wt% and CaO 7.8 wt% but 11.7 wt% of common glass. The glass beads made of waste fluorescent lamp glass have the narrower particle size distribution of and the higher refractive index than the glass beads made of common glass. The optimal conditions of kiln operation for manufacturing glass beads from waste fluorescent lamp glass are 20 m/sec of ejection rate, 1.7 of air mixing ratio, and 940℃ of temperature.
