What was the main factor in successful control of ascariasis in Korea?

  • In the l950s, under the legacy of traditional agriculture, Ascaris lumbricoides, spread epidemically in the war-bitten society of Korea. Consensus on the parasite control was drafted in the Parasite Disease Prevention Act, which passed a parliamentary agreement in 1966, and established safe disposal of feces and mass chemotherapy as control strategies. Biannual stool examinations and treating infected schoolchildren were basic scheme of the control activity through which revenue could be secured for organized business. In the 27 years following 1969, a maximum of 16 million stool examinations had been done every year. Cellophane thick smear enabled the task. The infection declined remarkably in the 1970s when industrialization and green revolution proceeded. A population study of A. lumbricoides in the late 1970s helped us better understand its epidemiology. The data also settled down the understandable protest of teachers against the repeated stool examinations. In the 9 years following 1987, the target population was gradually reduced when the egg positive rate was below 0.1%. An article in the Korean Law, stipulating obligatory stool examinations, was made optional. Although the long-term Korean effort of Ascaris control was a success, the effect of mass chemotherapy was not as succinct in terms of lowering reinfection. In the period of control, Korean agricultural technology changed, and the economy grew and supplied sanitary facilities by which the vicious cycle was disconnected. Reduction of morbidity was a benefit of mass chemotherapy, which is the only control method feasible in economically difficult countries. The most important hurdle of parasite control in the 1960s was poverty of general population and limited financial resources in Korea but the society formed a consensus on the priority of intestinal helminthiasis control during the ordeal period. The national consensus in the 1960s was the critical milestone for Ascaris control in Korea. Under the social agreement, application of timely technical and research advancements in parasitology achieved the success of ascariasis elimination. The successful experience of ascariasis elimination in Korea can be a benchmark for countries where neglected tropical diseases are endemically recycled.

Determination of Flunixin and 5-Hydroxy Flunixin Residues in Livestock and Fishery Products Using Liquid Chromatography-Tandem Mass Spectrometry (LC-MS/MS)

  • Flunixin is a veterinary nonsteroidal anti-inflammatory agent whose residues have been investigated in their original form within tissues such as muscle and liver. However, flunixin remains in milk as a metabolite, and 5-hydroxy flunixin has been used as the primary marker for its surveillance. This study aimed to develop a quantitative method for detecting flunixin and 5-hydroxy flunixin in milk and to strengthen the monitoring system by applying to other livestock and fishery products. Two different methods were compared, and the target compounds were extracted from milk using an organic solvent, purified with C18, concentrated, and reconstituted using a methanol-based solvent. Following filtering, the final sample was analyzed using liquid chromatography-tandem mass spectrometry. Method 1 is environmentally friendly due to the low use of reagents and is based on a multi-residue, multi-class analysis method approved by the Ministry of Food and Drug Safety. The accuracy and precision of both methods were 84.6%-115% and 0.7%-9.3%, respectively. Owing to the low matrix effect in milk and its convenience, Method 1 was evaluated for other matrices (beef, chicken, egg, flatfish, and shrimp) and its recovery and coefficient of variation are sufficient according to the Codex criteria (CAC/GL 71-2009). The limits of detection and quantification were 2-8 and 5-27 ㎍/kg for flunixin and 2-10 and 6-33 ㎍/kg for 5-hydroxy flunixin, respectively. This study can be used as a monitoring method for a positive list system that regulates veterinary drug residues for all livestock and fisheries products.

Asthma predictive index in children with recurrent wheezing (반복성 천명을 가진 소아에서 천식 발생 예측 지표의 적용)

  • Purpose : We compared the asthma predictive index(API) and the modified asthma predictive index (mAPI) of the Tuscon Children's Respiratory Study Group in Korean children with recurrent wheezing. We investigated the atopic profiles and presence of allergen sensitization of each risk group, and ascertained the significant clinical risk factors. Methods : Two hundred and sixty two children, who visited for recurrent wheezing from 1998 to 2005, were enrolled and divided into groups by API and mAPI. We investigated the history of the patients and their families, atopic profiles, and sensitization to aeroallergen and food allergens. Twenty nine children were followed up to 6 years of age and we evaluated the sensitivity, specificity and positive and negative predictive value of both indices. Results : The high risk group of API were of older age, were more likely to be sensitized to aeroallergen(P=0.001) and food allergen(P=0.034) and had higher levels of total eosinophil count, eosinophil percent, serum ECP, total IgE, and D.p-, D.f-specific IgE. High risk group of mAPI showed higher levels of atopic markers such as egg-, milk-, D.p- and D.f-specific IgE. Even though API did not include allergen sensitization, the high risk group was more significantly sensitized to common allergens than the low risk group. Twenty nine children were followed up until 6 years of age; therefore 15 children were diagnosed as asthma, clinically. The sensitivity, specificity, positive and negative predictive values of mAPI were higher than API. Conclusion : Both high risk groups of API and mAPI had higher levels of atopic markers and were more sensitized to common allergens. These findings suggest that sensitization to aeroallergens and food allergens are more objective markers as asthma predictive indices. In addition, mAPI is a more reliable index in predicting asthma in Korean children with recurrent wheezing than is API. But only 29 patients were followed until the age of 6, so we need to include more children with long term follow up for future study.

  • This study was undertaken to understand the seasonal changes of hormone levels such as luteinizing hormone (LH), follicle stimulating hormone (FSH) and estradiol-$17{\beta}$, serum components such as albumin, total protein and triglycerides, and GSI of rainbow trout (Onco-rhynchus mykiss) under the normal-light circumstances. LH levels were the highest in November whereas FSH levels were decreased progressively from October to February. Serum GTH, such as LH and FSH, levels were low immediately after egg-stripping and were significantly increased in the summer in association with the initiation of vitellogenesis. Serum albumin, total protein, and triglycerides were significantly increased in August in association with initiation of vitellogenesis and were peak in November prior to egg-stripping. Oocyte growth, measured by oocyte diameter and GSI, was significantly increased in August and showed the highest value in January. Correlation coefficients of serum LH with FSH and estradiol-$17{\beta}$ were +0.844 and +0.947, respectively. Correlation coefficients of estradiol-$17{\beta}$ with albumin and total protein were +0.634 and +0.859, respectively. Correlation coefficients of serum estradiol-$17{\beta}$ with triglycerides and GSI were +0.673 and +0.694, respectively. Probably by positive feedback mechanism, LH, FSH and estradiol-$17{\beta}$ appeared to stimulate to liver to secrete albumin, total protein and triglycerides for vitellosenesis.

A new endemic focus of Gymnophalloides seoi infection on Aphae Island, Shinan-gun, Jeollanam-do

  • A new endemic focus of Gymnophalloides seoi infection has been discovered on Aphae Island(Shinan-gun, Jeollanam-do), Republic of Korea. This area, which is referred to as Bokyong-ri, is a small seashore village located in the northern portion of the island. Fecal samples were collected from a total of 57 residents and examined by the Kato-Katz and formalin-ether sedimentation techniques. Helminth eggs were detected in 37 samples(64.9%); 33 samples were positive for G. seoi eggs(57.9%), 4 for Pygidiopsis summa(7.0%), 13 for other heterophyids(22.8%), 1 for Clonorchis sinensis(1.7%), and 1 for Trichuris trichiura(1.7%). Women(70.4%) revealed higher rates of G. seoi infection than did men(46.7%), and individuals older than 50 years of age also evidenced higher rates of G. seoi infection than in other age groups(P<0.05). In worm collection from 13 G. seoi egg positive cases, G. seoi (total 86,558 specimens), Heterophyes nocens(278), Stictodora sp.(10), Heterophyopsis continua(3), P. summa(3), and unidentified flukes(42) were collected. Oysters, the source of G. seoi infection, were collected from an area proximal to the village and 50 were examined for metacercariae; 47(94%) were found infected and the observed metacercarial density was $9.5{\pm}8.9$ per oyster. The results of this study indicate that Bokyong-ri is a new endemic area of G. seoi infection, with high rates of infection in humans and oysters.

  • In order to evaluate the present status of hook-worm infection in the rural areas in Korea, stool examinations were undertaken in several selected rural villages in each year of 1970 and 1977. The methods employed were cellophane thick smear technique and brine floatation technique in 3,454 specimens collected from 7 different rural villages for the prevalence rate of hookworm infection and Stoll's dilution egg counting technique for the intensity of hookworm infection. The incidences of Ancylostoma duodenale and Necator americanus infections were determind by polyethylene tube coproculture and by the evacuated adult worms after treatment with anthelmintics to the patients who were infected by hookworms. The results are as follows : 1) In 1970, 323(18.6%) out of 1,737 specimens collected from 4 different rural areas were positive for hook worm infection and the ranges of prevalence rates in those suveyed areas were shown as 6.9 to 38.2 per cent. On the other hand, 144(8.2%) out of 1,717 specimens collected from 3 different rural areas in 1977 were shown as positive for hookworm infection and 7.5 to 10.6 per cent of prvalence rates were shown in the surveyed areas. 2) In the age group under 10 years, there was no infection of hookworm in the studied areas in 1977, however in 1970 the prevalence of the hookworm infection was shown as very low in the age group of 5~9. On the whole, the hookworm prevalence steadily increased from the age of 15 in the studied areas in 1970, however in 1977 the prevalence increased from the age of 20 to the older age groups. 3) There was not observed much differences in the intensity of hookworm infection among the studied areas in 1970 and 1977. The mean EPG in each studied areas were shown under 1,000 EPG and mean numbers of evacuated worms per infected persons were counted as 9.4 to 15. 1. 4) Both A.duodenale and N.americanus have been found in all studied areas but the incidences and st-ate of distribution of two species of hookworn were shown as different in each localities.

High Prevalence of Enterobius vermicularis Infection among Schoolchildren in Three Townships around Yangon, Myanmar

  • In order to determine the status of Enterobius vermicularis infection among schoolchildren in suburban areas of Myanmar, 761 primary schoolchildren in 3 different townships around Yangon City were subjected to a survey using cello-tape anal swabs. The subjected schoolchildren were 383 boys and 378 girls who were 5-7 years of age. Only 1 anal swab was obtained from each child. The overall egg positive rate of E. vermicularis was 47.2% (359 positives), and sex difference was not remarkable (48.6% in boys and 45.8% in girls). However, the positive rate was the highest in South Dagon (54.6%) followed by Hlaing Thayar (43.8%) and North Dagon (34.8%). This difference was highly correlated with the living standards of the people in each township. Nucleotide sequence of the 5S rDNA from the eggs on the cello-tape (2 children) revealed 99.7% identity with that of E. vermicularis reported in GenBank. The results indicated that E. vermicularis infection is highly prevalent among primary schoolchildren around Yangon, Myanmar.

Development of Eggs, Larvae and Juvenile of the Korean Bullhead, Pseudobagrus fulvidraco (Richardson) reared in the laboratory (동자개의 형태발달과 성장특성)

  • The early developmental stages, growth and morphological changes of the Korean bullhead, Pseudobagrus fulvidraco, were studied from a series of reared specimens. Details of the early developmental stages are illustrated with special reference to morphological transformations. Egg and sperm of Korean bullhead were obtained from mature adults under hormonal treatment, fertilized artificially, and incubated in the aquarium. The incubation period of fertilized eggs was 55 to 66 hours at a temperature of 24.9${\pm}$0.34$^{\circ}$. Larvae were fed successively with Artemia salina and Daphnia magna for 2 to 15 days and artificial food after 20 days. Fertilized eggs were adhesive and spherical with a diameter of 2.04mm(n = 100). The mean total length of newly hatched larvae was about 4.92${\pm}$0.33 mm. Mouth opening occurred on one-day-old yolk-sac larvae, and initial feeding was observed on the third day after hatching. The morphological transitions from larvae to juvenile and juvenile to young stages occurred when the fish reached about 17 mm in total length (about 13days after hatching) and about 32 mm in total length (about 30 days after hatching), respectively. Many changes in proportion of body parts to total length were observed at about 7~8 mm and 30~32 mm, corresponding to the transformations from larvae to juvenile and from juvenile to young, respectively. In comparing relative growth of each body part against total length, those characteristics related to head parts showed positive growth in the pre-larval stages, while those concerning mobile abilities showed positive growth in the post-larval stage.

Prevalence, Worm Burden and Other Epidemiological Parameters of Ascaris lumbricoides Infection in Rural Communities in Korea (한국 농촌지역의 회충 감염율, 감염량 및 역학적 변수)

  • The epidemiological status of ascariasis was analyzed in 8 rural villages in Korea, through observation of its epidemiological parameters such as prevalence, worm burden and basic reproductive rate. Total 978 inhabitants were subjected to stool examination and recovery of worms after chemotherapy with pyrantel pamoate. The results were as follows: 1. The worm positive rate in each village was 16.5~79.5%, while the egg positive rate was 9~18% lower, 3.3~66.7%. The average worm burden (among all inhabitants) ranged from O. 21 to 8. 44 by villages and the frequency of cases with each worm burden showed negative binomial distributions with 'k' values of O. 38-0. 54. 2. The prevalence rates (worm) in each village was almost identical with the theoretical ones from Anderson and May's equation; $p=1-(1+M^*/k)^{-k}$, where 'p' is worm prevalence and '$M^*$' equilibrium average worm burden. The basic reproductive rate 'R' was calculated from 1.03 to 2.11. lt is suggested that, although 'R' in lower endemic areas is approaching to the breakpoint of reinfection (R=1), control programs of ascariasis in Korea should be continued until it becomes below the level nationwidely.

Immunohistochemieal study on the antigenicity of body compartments of Payugonimus westermani (폐흡충 충체 부위별 항원성에 대한 면역 조직화학적 연구)

  • Production of circulating specific antibodies to the lung fluke (Paragenimus westermani) by its host is well known and used in various kinds of immunodiagnostic methods, However, it has not been well documented which compartments (or structures) of the lung fluke are most responsible for the production of specific antibodies. The present immunohistochemical study was undertaken to demonstrate the antigenicity of each body compartment of p. westermani such as suckers, tegument, spines, vitelline glands, intestine, reproductive organs(male and female), and eggs. Indiret immunoperoxidase(IP) stain technique was applied, using formalin-fked, paraffin- embedded lung tissues of P westermani-infected cats sectioned in 4 Um thickness as the antigen and cat antisera (11~20 weeks of infection) as the primary antibody. Peroxidase-conjugated goat anti-cat IgG was used as the secondary antibody and diaminobensidine(DAB) as the coloring agent. Strong yellow or yellowish brown staining was regarded positive. The primary and secondary antibody dilutions were made at 1 : 500~1 : 2, 000 and 1 : 200~1 : 500 respectively, and IP stain was repeated 10 times for each dilution. A consistent result obtained was that the intestinal epithelial border, intestinal content, vitelline glands, and eggs scattered around the worm capsule showed strong positive staining, while uterine eggs and some parenchymal portions showed weak positive reaction. On the other hand, the suckers, tegument, spines, subtegumental cells, cytoplasm of intestinal epithelial cells, male reproductive organs, and ovary revealed negative staining. The body compartments showing higher antigenicity were, in the decreasing order, the intestinal epithelial border, intestinal content, eggs in the worm capsule, vitelline glands, uterine eggs, and parenchymatous portions. The intestinal epithelial border and luminal contents revealed positive staining even at a few concentration of 1 : 4, 000 primary antibody(secondary ab., 1 : 200) whereas the parenchymatous portion showed positive reaction only at higher concentrations than 1'500 (secondary ab., 1 : 200). The results suggest that the specific antibody responses of the host to p. westermani occur most strongly upon the excretes from the intestinal epithelium of the worm and e99s Produced around the worm capsule,
