Case Study on Advanced Thymic Cancer Treated with Wheel Balanced Therapy (수레바퀴 암치료법을 시행한 흉선암 환자에 대한 증례보고)

  • Background : Advanced thymic cancer still remains as an intractable disease. The survival rate of advanced thymic cancer could be increased through chemotherapy and radiation, but the results have not been satisfactory. Objectives : To see whether wheel balanced therapy (WBT) has the therapeutic effects or not on advanced thymic cancer patient. Methods : A patient diagnosed with progression of thymic carcinoma with pleura metastasis visited the East-West Cancer Center (EWCC) on Feb 9th, 2011. The patient was treated with WBT for a period of 9 weeks from Feb 9th to Apr 16th. She stayed 6 weeks in hospital and took oriental medicine prescribed by EWCC. Computed tomography (CT) and blood test were used to evaluate the disease progression of the patient. Results : Mass of chest CT was stable for 2 months. The patient's quality of life improved during her hospital stay. Conclusions : This case study supports WBT may have efficacy in treating advanced thymic cancer patients.

The effect of sucrose on infants during a painful procedure

  • Purpose: The purpose of this study was to test the efficacy of treating the pain among newborn infants associated with a medical procedure with sucrose with regard to overall physiological and behavioral stability. Methods: 103 newborn infants were enrolled in this study. The control group (n=63) did not receive any treatment. The experimental group (n=40) received 2 mL of 24% sucrose solution two minutes before a routine heel stick. The pain was assessed by measurements of physiological changes [e.g. pulse rate, oxygen saturation, salivary cortisol (hydrocortisone)] and behavioral changes [e.g. crying time, and the neonatal infant pain scale (NIPS) for neonates]. Results: There were no differences among the groups with respect to physiological changes associated with the pain from the procedure. However, there were significant group differences in behavioral changes to the pain. In the control group, the median crying time was 13 seconds, while in the experimental group, the median crying time was 3.5 seconds ($P$=.000). In the control group the median NIPS score was 4, while in the experimental group the median NIPS score was 2 ($P$=.000). Conclusions: These findings suggest that sucrose can be an effective method for the management of stress responses in infants with regard to behavior. However, this treatment had no significant physiological effects.

Escherichia coli Can Produce Recombinant Chitinase in the Soil to Control the Pathogenesis by Fusarium oxysporum Without Colonization

  • Fusarium wilt of cucumbers was effectively controlled by Escherichia coli expressing an endochitinase gene (chiA), and the rate was as effective (60.0%) as the wild-type strain S. proteamaculans 3095 (55.0%) where the gene was cloned. However, live cells of soil inoculated E. coli host harboring the chiA gene did not proliferate but declined 100-fold from $10^8$ CFU during the first week and showed less than 10 cells after day 14, suggesting that E. coli was able to express and produce the chitinase enzyme to the soil even as the population was gradually decreasing. Because the majority of the strains was alive for only a short period of time and the Fusarium-affected seedlings showed symptoms of wilting within 7-10 days, it seems that the pathogen control was decided early after the introduction of the biocontrol agent, eliminating the survival of the antagonist. These results indicated that soil inoculated E. coli could sufficiently express and produce the recombinant protein to control the pathogen, and root or soil colonization of the antagonist might not be a significant factor in determining the efficacy of biological control.

Development and Application Performance of a Granular Herbicide Applicator Attached to Rice Transplanter (승용 이앙기 부착형 제초입제 살포기 개발과 살포성능)

  • A herbicide applicator attached to a rice transplanter was developed to perform herbicide application and transplanting simultaneously. The prototype machine was composed of herbicide tank, discharge device, spinner, scattering plate and power drive. The application rate of the machine could be adjusted from 10 to 30 kg/㏊ and the application swath was 1.8m. According to the field test, application uniformity showed the range of 13.6~43.9% in terms of CV(coefficient of variation) depending on the spinner speed, application height and shape of diffuser. The best uniformity could be achieved with the spinner speed of 30.8m/s and application height of 20cm. Field efficiency of the prototype was 4.7hr/㏊. Transplanting machine has field efficiency of 4.6hr/㏊ in Korea. By attaching the herbicide applicator, field efficiency became lowed only by 0.1hr/㏊, which was counted for filling time of herbicide. Simultaneous operation of transplanting and herbicide application had a of labor saying. The weed control efficacy was measured to be 96% in field when AC140 + Stomp and Londax + YRC was applied at the 65th day after transplanting. Nonanmae was treated by manual application to be compared to.

  • Mammogram is one of the important techniques for mass detection, which is the early diagnosis stage of a breast cancer. Especially, the CAD(Computer Aided Diagnosis) using mammogram improves the working performance of radiologists as it offers an effective mass detection. There are two types of CAD systems using mammogram; automatic and semi-automatic CAD systems. However, the automatic segmentation is limited in performance due to the difficulty of obtaining an accurate segmentation since mass occurs in the dense areas of the breast tissue and has smoother boundaries. Semi-automatic CAD systems overcome these limitations, however, they also have problems including high FP (False Positive) rate and a large amount of training data required for training a classifier. The proposed system which overcomes the aforementioned problems to detect mass is composed of the suspected area selection, the level set segmentation and SVM (Support Vector Machine) classification. To assess the efficacy of the system, 60 test images from the FFDM (Full-Field Digital Mammography) are analyzed and compared with the previous semi-automatic system, which uses the ANN classifier. The experimental results of the proposed system indicate higher accuracy of detecting mass in comparison to the previous systems.

Dexamethasone Release from Glutaraldehyde Cross-Linked Chitosan Microspheres: In Vitro/In Vivo Studies and Non-Clinical Parameters Response in Rat Arthritic Model

  • The Dexamethasone (DEX) loaded chitosan microspheres were prepared by thermal denaturation and chemical cross-linking method using a dierent concentration of glutaraldehyde as chemical cross-linking agent. The prepared microspheres were evaluated for the percentage of Drug Loading (DL), Encapsulation Efficiency (EE) and surface morphology by Scanning Electron Microscopy (SEM). DL and EE were found to be maximum range of 10.0 to 10.79 % and 58.19 to 64.73 % respectively. The SEM Photographs of the resultant microspheres exhibited fairly smooth surfaces and predominantly spherical in appearance. In addition, Fourier Transform Infrared Spectroscopy (FTIR) and Differential Scanning Calorimetry (DSC) shown that there was no interaction between the drug and polymer. In vitro and in vivo release studies revealed that the release of dexamethasone was sustained and extended up to 63 days and effectively controlled by the extent of cross-linking agent. Non-clinical parameters such as paw volume, hematological parameters like Erythrocyte Sedimentation Rate (ESR), Paced Cell Volume (PCV), Total Leucocytes Count (TLC), Hemoglobin (Hb), Differential Cell Count (DCC) were investigated in Fruend's Complete Adjuvant (FCA) induced arthritic rats. Radiology and histopathological studies were also performed in order to evaluate the therapeutic efficacy of the DEX-loaded microspheres in extenuating the rat arthritic model.

Study on the Anti-tumor Effect of Gekko (천룡(天龍)의 항암효과에 대한 고찰)

  • Gekko has been used for several diseases including cancer in Oriental medicine and fork herbalogy. Nevertheless, its origin as herbal medicine and its efficacy and mechanism as anti-tumor drug have not yet been thoroughly reported in Korea. This study aimed to investigate anti-tumor effect of Gekko through selected articles from cqvip database in China. In vitro and In vivo, Gekko could obviously inhibit tumor growth, induce tumor cells apoptosis, reduce micro-vessel density in tumor tissue through down regulating VEGF & bFGF protein expression, promote cytotoxicity of lymphocyte. Gekko could improve survival rate, relive clinical symptoms, improve quality of life, and relieve anti-tumor treatment reaction, suggesting that Gekko might be a effective anti-tumor drug.

Metabolism and excretion of novel pulmonary-targeting docetaxel liposome in rabbits

  • Our study aims to determine the metabolism and excretion of novel pulmonary-targeting docetaxel liposome (DTX-LP) using the in vitro and in vivo animal experimental models. The metabolism and excretion of DTX-LP and intravenous DTX (DTX-IN) in New Zealand rabbits were determined with ultra-performance liquid chromatography tandem mass spectrometry. We found DTX-LP and DTX-IN were similarly degraded in vitro by liver homogenates and microsomes, but not metabolized by lung homogenates. Ultra-performance liquid chromatography tandem mass spectrometry identified two shared DTX metabolites. The unconfirmed metabolite $M_{un}$ differed structurally from all DTX metabolites identified to date. DTX-LP likewise had a similar in vivo metabolism to DTX-IN. Conversely, DTX-LP showed significantly diminished excretion in rabbit feces or urine, approximately halving the cumulative excretion rates compared to DTX-IN. Liposomal delivery of DTX did not alter the in vitro or in vivo drug metabolism. Delayed excretion of pulmonary-targeting DTX-LP may greatly enhance the therapeutic efficacy and reduce the systemic toxicity in the chemotherapy of non-small cell lung cancer. The identification of $M_{un}$ may further suggest an alternative species-specific metabolic pathway.

Use of Neighborhood Facilities and Perception of Walking Environment in Older Rural Women - Focused on the Chungnam Province - (농촌 여성노인의 근린시설 이용 및 보행환경 인식 연구 - 충남지역을 중심으로 -)

  • The objective of this study was to investigate the perception of walking environment and use of neighborhood facilities of rural female elderly, who are the most susceptible to ambulatory problems. The study showed that parks, senior centers and hospitals were among the most frequently used neighborhood facilities by rural female elderly, while bus stations and senior centers were recognized as the closest facilities. Frequency of use was higher for paid sports facilities rather than parks. Hospitals and health centers were facilities that were desired to be in close proximity. Social support, self-efficacy, and attitude were the factors that affected walking environment. Street trees, accessibility to parks, and street lights were factors that showed a positive relationship with total time of walking activity while perceived crime rate and slopes were perceived as environmental challenges for groups that walked under 150 minutes per week. One factor that negatively affected walking was the lack of sidewalks on streets. Providing parks and walking trails within neighborhood, pleasant walking environments, and safe sidewalks would contribute to improved perception of neighborhood and walking of older rural women.

Research Trends of Herbal Medicine Treatment for Uterine Leiomyoma - Research using CNKI (자궁근종의 한약치료에 대한 최신 임상 연구 동향 - CNKI 검색을 중심으로)

  • Objectives: This study was conducted to find out research trends for uterine leiomyoma and to recognize the efficacy and safety of herbal medicine treatment for uterine leiomyoma and to help in the clinical research of herbal medicine treatment for uterine leiomyoma in the future. Methods: Based on eight domestic and foreign databases, We analyzed the Randomized Controlled Trials using herbal medicine for uterine leiomyoma. Data retrieval was carried out on September 10, 2019 and the latest five-year papers were included. The treatment group was treated with herbal medicine and the control group was treated with western medicine. Results: 473 studies were searched in domestic and foreign databases, and 12 studies were finally selected. Although the indicators varied, all studies used total efficiency rate as indicators. In all 12 studies, the treatment group treated with herbal medicine was more effective than the control group treated with Western medicine.
