죽음준비교육이 일반인의 연명치료중단 및 호스피스·완화의료에 대한 인식에 미치는 효과 (Effects of Death Preparation Education on Awareness of Hospice Palliative Care and Withdrawing Life Sustaining Treatment in City Dwellers)

  • 목적: 본 연구에서는 일반인을 대상으로 죽음준비교육 프로그램이 연명치료중단 및 호스피스 완화의료에 대한 인식에 미치는 효과를 규명하기 위해 수행되었다. 방법: 본 연구의 죽음준비교육 프로그램은 총 5주에 걸쳐 진행되었으며 연구 대상자는 서울시 소재 일개구청과 대학교의 협력사업으로 진행한 죽음준비교육 참여자로 실험군 35명, 대조군 40명으로 총 75명이었고 사전 사후 측정은 자가 보고식 설문지를 이용하여 자료를 수집하였다. 결과: 죽음준비교육 후 호스피스 완화의료에 대한 인식은 실험군과 대조군에서 유의한 차이를 보이지 않았으나 연명치료중단 인식정도는 실험군이 대조군보다 유의하게 높은 결과를 보였다. 결론: 인간은 의미를 추구하는 존재로서 늘어나는 기대수명에 맞춰 삶의 질을 높이려는 욕구 또한 증가하고 있으며 자신의 삶의 마지막을 결정할 권리가 있는데, 본 연구결과는 인생의 마지막 순간에 자신의 삶을 결정하는 것에 대한 이론적 근거가 되었다고 본다. 또한, 일반인들에게 호스피스 완화의료에 대한 인식에 긍정적으로 영향을 줄 수 있는 죽음준비교육 프로그램 개발과 적절한 교육과정과 기간 및 교육방법에 대한 평가연구가 계속되어야겠다.

한국인의 음식인지도 : 맛과 영양에 대한 조사 (The Perceptions of Food in Korea : An Investigation of Taste and Nutrition)

  • The present study investigated the perceptions of food adopted by Koreans using the characteristics of taste and nutrition. Two hundred and nine Korean subjects were surveyed including University students and adults from many diverse social backgrounds. The subjects sorted pictures of foods according to two conditions, how tasty and then how nutritions they thought the foods were using the Q-method. These pictures included two sets, one for individual food items and the second depicting typical Korean meals, yielding a total of four conditions, This methodology effectively addressed any language barrier as pictures and words in both Korean and English were used. It was an effective tool for allowing the identification of perceptual structures and indicating how prevalent they were across samples. The data were factor analysed and the resulting factor scores interpreted. Meat was regarded positively in all four conditions. Individual sweet foods were preferred by younger subjects while individual traditional foods were preferred by older subjects. Traditional Korean meals were also favoured while processed meals were not. Nutritionally, proteins were more valued than carbohydrates, fibre and vitamins. Meals that contained protein and were regarded as filling and sustaining were regarded positively. The implications for nutrition education were discussed.

노인간호에 대한 바람직한 태도형성을 위한 교육방안 고찰 (Review of educational strategies to facilitate desirable attitudes toward gerontological nursing)

  • 연구목적: 본 연구는 미국의 학부 간호교육에서 노인간호에 대한 바람직한 태도를 강화하기 위해 시도된 새로운 교육적 접근법을 고찰하고 국내 간호교육에서의 적용방안을 모색하고자 수행되었다. 연구방법: PubMed, CINAHL의 검색엔진을 사용해 미국 John A. Hartford 재단의 지원으로 노인간호교육을 위한 획기적인 전략개발 프로젝트를 수행한 간호대학의 연구 8편과 그 외 다양한 교육적 방안을 고찰한 논문 4편을 포함한 12편의 연구를 고찰하였다. 연구결과: 노인간호에 대한 올바른 태도형성을 위해 제시된 교육적 접근법은 다음의 네 가지로 구분할 수 있다: 1) 노인간호에 대한 간호전공교과 간의 연계교육, 2) 지역사회 노인의 교육적 멘토 역할, 3) 시청각 매체의 활용, 4) 자기성찰을 통한 토론학습. 향후 응용: 국내 간호교육에서의 효과적인 적용을 위해서는 연계교육을 위한 간호전공교과목 간의 긴밀한 협의 및 교육과정의 탄력적 운영을 위한 교육행정부서의 협조가 수반되어야 한다. 또한, 교육기관과 지역사회와의 긴밀한 파트너십을 구축함으로써 지역사회 노인이 교육적 멘토로 참여하는 방안을 적용해 나갈 수 있다. 무엇보다, 노인간호에 대한 바람직한 태도형성을 위한 간호교육의 중요성을 인식하고 교수자의 교육역량강화와 새로운 교수법 개발을 위한 지속적 노력이 요구된다.

성인의 골다공증에 대한 지식과 실천에 관한 조사연구 (A Study on Knowlege and Practices about Osteoporosis in Adults)

  • Osteoporosis is a major health problem in countries with aging populations, resulting in excess morbidity and mortality. This study was conducted to investigate knowledge and practices about osteoporosis in adults and to identify some factors which were influenced to that. 56-item questionnaire was developed including five domains(general characteristics, osteoporosis-related characteristics, knowledge of risk factors, knowledge of prevetion and practices). Subjects of this study were 368 adult ranged from 30 to 59. Data were collected during the period from June 15 to July 10, 1998 by means of a structured questionnare. The data were analyzed using descriptive statistics, t-test, ANOVA, Post Hoc, Pearson Correlation by SPSSWIN program. The results were as follows: 1. The mean knowledge score of risk factors for osteoporosis was 7.46(full score=15) and that of prevention of osteoporosis was 8,79(full score=12). 2. The mean practice score of osteoporosis was slightly higher than median value. 3. Women had better knowledge about osteoporosis risk factors and practiced more osteoporosis-prevention measures than men. Although the data demonstrated fairly good general knowledge about osteoporosis in the subjects, the older group(age 50-59), those at the highest risk of developing the disease, knew less about osteoporosis than the younger group(age 30-39) did. 4. The persons who had heard about osteoporosis and bone mineral density though mass media medical pratitioners and who didn't take any medicine for osteoporosis had better knowledge about osteoporosis and preventive measures. The practice score was significantly higer in the postmenopausal women and persons who were already diagnosed as osteoporosis patients, or who underwent bone mineral density measurements. 5. The relation between knowledge and practice was significant. In conclusion, there was a modest degree of general knowledge about osteoporosis and its consequences in adults. Further randomized studies are needed to evaluate the relationship between osteoporosis and risk factors. However, these results support the importance of education to prevent osteoporosis.

초, 중, 고등학생들의 비만에 영향을 미치는 도시 특성 (Urban Characteristics Affecting Obesity of Elementary, Middle and High School Students)

  • 성인을 중심으로 건강도시를 논의한 기존 연구와 달리, 본 연구는 청소년 비만에 영향을 미치는 도시 특성을 규명하고자 했다. 도시특성이 청소년 비만에 영향을 미치는 기제(mechanism)에 대한 엄격한 의학적 이론이 드물기 때문에 본 연구는 선행연구 검토를 통해 청소년 비만에 영향을 미칠 것으로 판단되는 도시특성을 도출하고, 이를 토대로 회귀분석을 했다. 교육부의 학생 신체검사 자료를 이용하여 서울 시내 각 학교별 비만학생 비율을 종속변수로 설정했다. 선행연구를 통해 파악한 도시특성을 각 학교를 중심으로 하는 일정 반경(500m, 1000m, 1500m) 내에서 버퍼분석(buffer analysis)으로 측정한 뒤, 독립변수로 설정했다. 회귀분석한 결과, 성인교육수준이 낮을수록, 단독 연립 다세대 다가구 주택이 많을수록, 노후 주택이 많을수록, 토지이용혼합도가 낮을수록 비만청소년이 많았다. 인구밀도, 대중교통 접근성, 도시공원 등은 유의하지 않았다. 이러한 결과를 토대로 청소년 비만율을 낮추거나 관리하기 위한 도시계획 방안이 앞으로 더욱 논의되어야 할 것으로 보인다.

우리나라 성인의 비타민.무기질 보충제 섭취여부에 따른 인구.사회학적, 생활습관 및 영양섭취상태에 관한 연구 (Sociodemographic Characteristics, Lifestyle Factors, and Nutrient Intake by Taking Vitamin/mineral Supplements)

  • Taking vitamin and mineral supplements is increasingly common with the rapid economic growth. The aim of this study was to determine the prevalence of vitamin and mineral supplement use among adults aged 20 or older from the third Korean National Health and Nutrition Survey data and to explore the effect of sociodemographic and lifestyle factors as well as nutrient intake on taking supplements. People who had participated in both a health questionnaire and a nutritional survey were selected, and 2,871 men and 3,555 women were finally included in this analysis. Both men and women with a higher level of education, those residing in a metropolitan area, and those with higher income were more likely to take supplements. Health behaviors were not significantly associated with taking supplements. Mean nutrient intake of all nutrients except energy intake was not significantly different in men or women taking supplements after adjusting for age, education, marital status, resident area, smoking, and energy intake. Compared to Dietary Reference Intake (DRI) for Koreans, most vitamins and minerals, except vitamin $B_2$ and calcium, were consumed at higher than the Recommended Intake (RI) without supplements. In conclusion, taking supplements such as vitamin $B_2$ and calcium may promote health and prevent disease. However, the type and frequency of other vitamin and mineral supplements consumed should be considered with caution.

저소득층 노인의 건강증진을 위한 지역사회 참여형 연구에서 지역사회 주민 조직의 구성과 운영 (Operation of Community Resident Groups in a Community-Based Participatory Health Promotion Program for Low-income Older Adults)

  • 목적: 지역사회 참여형 보건 연구에서 현장의 지식과 경험을 토대로 주요 보건문제를 파악하고 대응하기에 중요한 역할을 하는 지역사회 주민 조직의 구성과 운영상의 특성을 논의하고자 한다. 방법: 미국 펜실베이니아 주 12개 저소득층 정부임대 아파트의 노인 입주자들을 대상으로 각 아파트마다 자발적인 주민조직을 구성하여 사회생태학 모형을 활용한 6단계 지역사회 역량강화 과정을 수행하였다. 주민조직의 과정과 성과를 기록한 200여건의 문건에 대해 질적 연구 분석을 실시하였다. 결과: 2년간 주민조직 월례회의를 통하여 낙후된 아파트 건물상태, 신선한 식재료 마련, 주민 간 관계개선 등을 지역사회 건강증진의 우선순위로 선정하고, 자체적인 해결방안을 구상하여 추진하였다. 주민조직 내의 리더십, 주민조직에 꾸준히 참여하는 핵심 회원, 주민조직 회의 개최의 일관성이 주민들에 의한 자치적인 지역사회 건강증진 활동의 주요 특성으로 드러났다. 리더십이 형성되고 회원의 참여와 회의의 개최가 꾸준한 주민조직일수록 지역사회 건강증진 목표 및 관련활동이 집중적인 경향이 있었다. 결론: 리더십, 참여, 일관성 등은 참여적이고 자치적인 지역사회 건강증진을 위한 역량요인이며, 이러한 역량을 개발하고 강화하는 과정에 대한 모니터링과 과정평가의 중요성이 강조된다.

공유진찰제: 만성질환 관리를 위한 혁신적 의료서비스 전달방식 (Reviews of the Shared Medical Appointments: Adopting Innovations in Care Delivery for Patients with Chronic Diseases)

  • Chronic diseases as well as a growing population of older adults are currently the leading cause of ill health and economic burden worldwide. Managing those diseases in one-on-one medical consultations poses substantial challenges due to limited time and resources in the current health care system. Various approaches have been taken to manage these conditions, most with limited success. Shared medical appointments (SMAs) are an innovative care delivery option to make the testing of alternative care modalities a prime concern. SMAs are individual medical consultations carried out in a group of patients with similar diseases by providing education, medication management, and disease monitoring. SMAs, since their initial conceptualization in 1998, have gained much popularity and adopted as one of the standard processes in many countries. Accumulated evidence-based studies show outcomes for increasing access to care, behavioral change facilitated through self-management education, maintained/better outcomes, physician productivity, and enhanced resource management. This review summarizes current evidence regarding the existing status of SMAs abroad. An extensive literature search was conducted on major electronic databases including PubMed and Google Scholar. This study suggests to explore and exploit the SMAs which have unique potential as a healthcare delivery innovation in Korea.

Nutrient Intakes and Dietary Habits of Single Living Korean Adults by Age Group

  • In order to investigate the differences in nutrient intakes and eating habits between people living alone and people living together with family or others by age group, dietary survey data of the subjects aged 20 years or older from 2001 Korean National Health and Nutrition Survey were analyzed. Living status of the subjects was defined as 'single' when the subjects' household member was one person. Age, gender, income, education were adjusted during the comparative analysis. The subjects living alone had diets with lower score of nutritional adequacy ratio and lower quality, and drank more alcoholic beverages when compared with the subjects living together. Females were more greatly affected in dietary intakes by living alone situation than males. Of the four age groups, a group with ages from 30 to 39 years showed less nutrient intake patterns in persons living alone than in persons living together, but the rest three groups with ages from 20 to 29, from 50 to 64, and 65 or older did not show any significant differences. Eating habits of the subjects living alone, such as skipping meals, kinds of snacks, dining-out, were worse as a whole than the other. In conclusion, single living particularly of females or of 30 to 39 years of age group had negative influences on dietary intakes and behavior. There may be statistical errors if socioeconomic and demographic factors such as age, gender, income, and education are not controlled in the population study investigating the effect of living alone on dietary intakes. Further studies will be needed to know the age-specific reasons for the worse nutrient intakes of single living persons.

A comparison of food and nutrient intake between instant noodle consumers and non-instant noodle consumers in Korean adults

  • Instant noodles are widely consumed in Asian countries. The Korean population consumed the largest quantity of instant noodles in the world in 2008, However, few studies have investigated the relationship between instant noodles and nutritional status in Koreans. The objective of this study was to examine the association between instant noodle consumption and food and nutrient intake in Korean adults. We used dietary data of 6,440 subjects aged 20 years and older who participated in the Korean National Health and Nutrition Examination Survey III. The average age of the instant noodle consumers (INC) was 36.2 and that of the non-instant noodle consumers (non-INC) was 44.9; men consumed more instant noodles than women (P<0.001), With the exception of cereals and grain products, legumes, seaweeds, eggs, and milk and dairy products, INC consumed significantly fewer potatoes and starches, sugars, seeds and nuts, vegetables, mushrooms, fruits, seasonings, beverages, meals, fishes, and oils and fats compared with those in the non-INC group. The INC group showed significantly higher nutrient intake of energy, fat, sodium, thiamine, and riboflavin; however, the INC group showed a significantly lower intake of protein, calcium, phosphorus, iron, potassium, vitamin A, niacin, and vitamin C compared with those in the non-INC group. This study revealed that consuming instant noodles may lead to excessive intake of energy, fats, and sodium but may also cause increased intake of thiamine and riboflavin. Therefore, nutritional education helping adults to choose a balanced meal while consuming instant noodles should be implemented, Additionally, instant noodle manufacturers should consider nutritional aspects when developing new products.
