Analysis of Factors Affecting Occupational Health Nurses' Early Breast Cancer Screening (산업간호사의 유방암 조기검진행위에 영향을 미치는 요인 분석)

  • Purpose: The aims of this study were to evaluate early breast cancer screening rate of occupational health nurses and to analyze factors affecting their behavior. Method: We reviewed and analyzed the structured questionnaires of III respondents from 230 participants in continuing education of occupational health nurses. Result: Breast self examination was performed in 72.1%, clinical breast examination was received in 30.6% and mammography was received in 40.5% significant factors affecting breast self examination were age and the age of menarche. significant factors affecting clinical breast examinations for breast cancers were age and the place of residence, and factors affecting mammography were age, monthly income, alcohol intake, the age of menarche, and attitude about early screening for breast cancers. Conclusion: Early breast cancer screening rate of occupational health nurses was higher than that of general population of women. But the rate of clinical breast examination and mammogaphy were lower of less than 50% of respondents. Therefore occupational health nurses who are responsible for health management of working women need more education for themselves to promote the motivation and to increase the rate of participation in early breast cancer screening.

Effect of fiber reinforcing on instantaneous deflection of self-compacting concrete one-way slabs under early-age loading

  • The Early-age construction loading and changing properties of concrete, especially in the multi-story structures can affect the slab deflection, significantly. Based on previously conducted experiment on eight simply-supported one-way slabs this paper investigates the effect of concrete type, fiber type and content, loading value, cracking moment, ultimate moment and applied moment on the instantaneous deflection of Self-Compacting Concrete (SCC) slabs. Two distinct loading levels equal to 30% and 40% of the ultimate capacity of the slab section were applied on the slabs at the age of 14 days. A wide range of the existing models of the effective moment of inertia which are mainly developed for conventional concrete elements, were investigated. Comparison of the experimental deflection values with predictions of the existing models shows considerable differences between the recorded and estimated instantaneous deflection of SCC slabs. Calculated elastic deflection of slabs at the ages of 14 and 28 days were also compared with the experimental deflection of slabs. Based on sensitivity analysis of the effective parameters, a new model is proposed and verified to predict the effective moment of inertia in SCC slabs with and without fiber reinforcing under two different loading levels at the age of 14 days.

  • Neonatal diabetes mellitus (NDM) is a rare disease that occurs at less than 6 months of age and is presumably caused by a mutation in the gene that affects pancreatic beta-cell function. Approximately 80% of NDM cases reveal a known genetic mutation, and mutations in potassium inwardly rectifying channel subfamily J member 11 (KCNJ11) and ABCC8 affecting the pancreatic beta-cell adenosine triphosphate-sensitive potassium channel may be treated with oral sulfonylurea. Early recognition of mutations in KCNJ11 and ABCC8 is important because early administration of sulfonylurea can not only control blood glucose levels but also improve neurodevelopmental outcomes. In the present study, we report a case of NDM that initially presented as diabetic ketoacidosis at the age of 1 month, accompanied by seizures during hospitalization. After confirmation of the KCNJ11 gene mutation (c.989A>C), we started administering oral sulfonylurea (glimepiride) at the age of 2 months. After gradually increasing the dosage of glimepiride, insulin was discontinued at the age of 3 months. To date, the infant's blood glucose levels have been well controlled without significant hypoglycemic events. No further episodes of seizures have occurred, and his developmental status is favorable.

Timing of Menarche and Physical Growth during Childhood and Adolescence : The Kangwha Study (초경시기와 아동기 및 청소년기의 신체성장의 변화 : 강화연구)

  • Objectives : To assess height, weight and body mass index from childhood to adolescence according to the age at menarche and hence to study the influence of childhood growth on the menarche age. Methods : $\lceil$The Kangwha Study$\rfloor$was a community-based prospective cohort study which included the entire population of 219 female first graders in Kangwha county in 1986. Among the 219 girls, 119 girls who had received complete follow up checks during the study period$(1986\sim1997)$, were included in this study, except one for whom menarche age information was unavailable. The remaining 118 girls were divided into three groups according to the timing of menarche : early(<25 percentile), intermediate and late($\geq75$ percentile) maturers. Results : The average age at menarche was 12.7 years . early 11.3 years, intermediate 12.6 years and late 13.7 years. The early maturers were taller and heavier between $6\sim8$ years. But, the mean weight and body mass index at the menarche age did not differ statistically among the three groups. The weight and body mass index of the early maturers were consistently higher than those of the late maturers over the entire period of the study. Conclusions : Critical body weight and body mass index must be attained for menstruation to be attained and the age at menarche is largely determined by the childhood growth. In addition, it seems that childhood growth and age at menarche are associated with adolescent weight and body mass index.

Autogenous shrinkage of ultra high performance concrete considering early age coefficient of thermal expansion

  • The recently developed Ultra High Performance Concrete (UHPC) displays outstanding compressive strength and ductility but is also subjected to very large autogenous shrinkage. In addition, the use of forms and reinforcement to confine this autogenous shrinkage increases the risk of shrinkage cracking. Accordingly, this study adopts a combination of shrinkage reducing admixture and expansive admixture as a solution to reduce the shrinkage of UHPC and estimates its appropriateness by evaluating the compressive and flexural strengths as well as the autogenous shrinkage according to the age. Moreover, the coefficient of thermal expansion known to experience sudden variations at early age is measured in order to evaluate exactly the autogenous shrinkage and the thermal expansion is compensated considering these measurements. The experimental results show that the compressive and flexural strengths decreased slightly at early age when mixing 7.5% of expansive admixture and 1% of shrinkage reducing admixture but that this decrease becomes insignificant after 7 days. The use of expansive admixture tended to premature the setting of UHPC and the start of sudden increase of autogenous shrinkage. Finally, the combined use of shrinkage reducing admixture and expansive admixture appeared to reduce effectively the autogenous shrinkage by about 47% at 15 days.

Children's relationships with teachers and peers, and their early school Adaptation (유아의 교사, 또래관계와 유아교육기관 일과적응)

  • The study aims to investigate the children's adaptability to early school circumstances and their relationships to teachers and to peers. The examination was done on 110 children aged 4 or 5 at a kindergarten and two child care centers in Seoul. Their 'adaptability to early school' and their 'relationships to teachers and to peers' were rated by their teachers. On the whole, the children subjects are on good terms with their teachers and peers, and they adapt well to early schools. But according to age groups, there were significant differences in the teacher-to-child relationships. Compared to boys, girls show more' adaptability to the early schools', more 'closeness to teachers' and more 'prosociality in peer group'. The 'closeness to teachers' has a positive relation to the 'sociability', 'prosocial behaviors' and 'leadership of peer group', but the 'conflict with teachers' has a negative relation to the peer group relationships. And the' children's relationships with teachers and with peers' were significantly related to their early school adaptability. The 'prosociality', the 'closeness' and the' dependency' were significantly related to the' adaptability to early school' in order.

Secular Trends and Influencing Factors for the Early Menarche among Korean Middle and High School Girls (우리나라 중고등학교 여학생의 조기 초경 경향과 영향요인)

  • The objective of this study is to identify the secular trend in age at menarche and to investigate the factors influence the early menarche(<12 years old) among Korean middle and high school girls. We analyzed data from the Korean Youth Risk Behavior Web-based Survey(KYRBWS) 2006-2014. This study was a descriptive study of 216,917 Korean middle and high school girls born between 1988 and 2002. Linear trends test performed to assess the trend age at menarche and percentage of early menarche. Multiple logistic regression analysis was to assess the risk factors influence the early menarche. Mean age at menarche decreased from $12.61{\pm}1.32$ years for middle and high school girls born 1988 to $11.88{\pm}0.75$ years for those born 2002(p for trends<.001). Percentage of early menarche increased from 19.7% to 25.2% between 1988 and 2002(p for trends<.001). Living in city, higher stress level, short sleep duration, and higher body mass index were associated with an early menarche among middle and high school girls(all p<.001). We found that age at menarche is still falling in the Korean adolescents, and it need intervention strategies to control the early menarche.

Colon Cancer Screening-Is It Necessary to Start under the Age of 50? (대장암 검진-50세 이하부터 시작하는 것은 필요한 것인가?)

  • Colorectal cancer is the third most common cancer worldwide. The occurrence of colon cancer can be prevented by removing precursor lesions. Several countries are making efforts to prevent the occurrence of colon cancer via screening programs. Korea is also following suite by screening individuals < 50 years of age. Currently, the incidence of colon cancer among the young is increasing globally, and Korea has a high colon cancer incidence rate among individuals in their 20s and 40s. Therefore, it may be necessary to start the screening individuals < 50 years of age to detect the cancer's manifestation early. Moreover, advanced adenomas associated with poor prognosis can be detected early, the overall screening rate can be increased, and death from colon cancer at a young age can be prevented. Although the period of colorectal cancer screening has been expanded, the overall colorectal cancer screening rate has not. Additionally, increasing the screening rate among individuals > 50 years could reduce the mortality rate at a lower cost. Although the incidence of colon cancer is increasing in younger individuals, the evidence to conclude that screening at the age of < 50 years has a meaningful impact on the incidence and survival rates of colon cancer remains insufficient. Therefore, rather than following the trend and performing screening early, an approach that actively selects cases where tests for colorectal cancer diagnosis are required is warranted.

The Rate of Conversion from Immune-tolerant Phase to Early Immune-clearance Phase in Children with Chronic Hepatitis B Virus Infection

  • Purpose: The spontaneous seroconversion rate of hepatitis B e antigen (HBeAg)-positive chronic hepatitis B (CHB) virus infection in children is lower than that in adults. However, few studies have investigated the rate of transition from the immune-tolerant to the early immune-clearance phase in children. Methods: From February 2000 to August 2011, we enrolled 133 children aged <18 years who had visited the Department of Pediatrics, Kyungpook National University Hospital. All subjects were in the immune-tolerant phase of HBeAg-positive CHB virus infection. The estimated transition rate into the early immune-clearance phase was calculated using the Kaplan-Meier method. Results: Among the 133 enrolled pediatric CHB virus infection patients in the HBeAg-positive immune-tolerant phase, only 21 children (15.8%) had converted to the early immune-clearance phase. The average age at entry into active hepatitis was $10.6{\pm}4.8$ years. The incidence of transition from the immune-tolerant to the early immune-clearance phase in these children was 1.7 episodes/100 patient-years. When analyzed by age, the estimated transition rate was 4.6%, 7.1%, and 28.0% for patients aged <6, 6-12, >12 years, respectively. Conclusion: In children with CHB virus infection, the estimated rate of entry into the early immune-clearance phase was 28.0% for patients aged 12-18 years, which was significantly higher than that observed for children aged <12 years (11.7%; p=0.001).

Comparison of Forensic Demographic Characteristics, Interpersonal Problems, and Personality Disorders between Early and Late Onset Criminal Patients with Schizophrenia (범법 조현병 환자에서 발병연령에 따른 범죄인구특성과 대인관계문제 및 성격장애의 차이)

  • Objectives : The purpose of this study was to investigate the differences in forensic demographic characteristics, interpersonal problems and personality disorder between early and late onset criminal patients with schizophrenia. Methods : The participants included 187 inpatients with schizophrenia who had committed crimes. They filled out the Korean Inventory of Interpersonal Problem Personality Disorder Scales and Psychopathic Personality Inventory-Revised. They were divided into early onset group and late onset group according to onset age of schizophrenia at 26 years old(under 26 versus 26 and older) and forensic demographic characteristics, interpersonal problem and personality disorders including psychopathy were compared between two groups. Results : There were no differences in gender, education years and family history between the two groups. Early onset group was associated with lower age, earlier onset of age and earlier age at the time of the crime. Duration from onset to diagnosis was not different between the two groups. Duration from onset to crime and after diagnosis to crime was shorter in late onset group. There were no differences between the two groups in the interpersonal problems, personality disorder and psychopathic personality evaluation. Conclusion : These results suggested that there may be forensic demographic differences related to crime between early and later onset schizophrenia. Psychiatrists should consider the age at onset of schizophrenia when assessing the risk of violence in patients with schizophrenia. In the future. it will be needed other study of age classification such as admixture analysis.
