Immunopathogenesis of COVID-19 and early immunomodulators

  • The novel coronavirus disease 2019 (COVID-19) is spreading globally. Although its etiologic agent is discovered as severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), there are many unsolved issues in COVID-19 and other infectious diseases. The causes of different clinical phenotypes and incubation periods among individuals, species specificity, and cytokine storm with lymphopenia as well as the mechanism of damage to organ cells are unknown. It has been suggested that in viral pneumonia, virus itself is not a direct cause of acute lung injury; rather, aberrant immune reactions of the host to the insults from viral infection are responsible. According to its epidemiological and clinical characteristics, SARS-CoV-2 may be a virus with low virulence in nature that has adapted to the human species. Current immunological concepts have limited ability to explain such unsolved issues, and a presumed immunopathogenesis of COVID-19 is presented under the protein-homeostasis-system hypothesis. Every disease, including COVID-19, has etiological substances controlled by the host immune system according to size and biochemical properties. Patients with severe pneumonia caused by SARS-CoV-2 show more severe hypercytokinemia with corresponding lymphocytopenia than patients with mild pneumonia; thus, early immunomodulator treatment, including corticosteroids, has been considered. However, current guidelines recommend their use only for patients with advanced pneumonia or acute respiratory distress syndrome. Since the immunopathogenesis of pneumonia may be the same for all patients regardless of age or severity and the critical immune-mediated lung injury may begin in the early stage of the disease, early immunomodulator treatment, including corticosteroids and intravenous immunoglobulin, can help reduce morbidity and possibly mortality rates of older patients with underlying conditions.

  • This paper is intended to verify the efficiency of anti-freeze agent and insulating form by analyzing the temperature history and the property of strength-increase about the concrete that is placed in the insulating form and normal form, using new type anti-freeze agent in batcher plant According to the results about the temperature history, while the lowest temperature shows 3$^{\circ}C$ in case of normal concrete + euroform, 4$^{\circ}C$ in case of normal concrete + insulating form, it shows 6$^{\circ}C$ in anti-freeze agent + the insulating form, so the effect is most favorable. The compressive strength with mixing anti-freeze agent or not, shows high in order of standard curing, structure-managing and open air-placed specimen and the concrete mixing anti-freeze agent shows the highest compressive strength-increase.

  • This paper is to study the response of flat plate slab-column connections consisting of various types of shear reinforcement and steel plate subjected to gravity loadings, mainly punching shear forces using the non-destructive testing, spectral analysis of surface waves and structural experiments. The base specimen failed due to punching shear generated from the gravity. The three other types of slab shear reinforcement and steel plate showed effective in resisting punching shear for these types of connections under gravity loading. This study has focused in evaluating the velocity response of a Surface wave during the early age as the poured concrete specimens have been hardened, the possibility of damage detection in the slab-column connection and the relationship between the punching shear forces and the surface wave velocities under the condition that the punching shear forces had gradually increased until the flat plate slab in slab-column connection had been failed.

  • Mass concreting in cold weather environment should be focused on the control of thermal crack caused by high hydration heat rather than curing method for protecting from frost damage at early age because the thermal stress have much influence on the quality of structure placed in cold weather. Therefore, in this paper, the control of thermal crack of mass concrete in cold weather environment are dealt with preparing the practice plan for mass concrete and placing the concrete according to the plactice plan. According to the results, we can obtain good quality mass concrete without thermal crack caused by the difference of temperature between inner part and outer part.

Evaluation of Renal Function Using the Level of Neutrophil Gelatinase-Associated Lipocalin is Not Predictive of Nephrotoxicity Associated with Cisplatin-Based Chemotherapy

  • Background: For early detection of renal damage during the usage of cisplatin based chemotherapy, changes in renal function should be monitored carefully. In recent years, neutrophil gelatinase-associated lipocalin, a small polypeptide molecule, has shown promise as a marker of acute renal failure. The aim of this present study was to assess possible risk prediction of cisplatin-induced nephrotoxicity using serum NGAL. Materials and Methods: A total of 34 consecutive patients with documented serum creatinine at least 24 hours before every cycle of cisplatin-based chemotherapy were included in the study. Demographic and medical data including age, performance status, tumor characteristics and comorbid diseases were collected from medical charts. Renal function was evaluated at least 48 hours before the treatment and at the end of the treatment based on the Modification of Diet in Renal Disease (MDRD) formula. Before and after cisplatin infusion serum NGAL levels were measured for the first and 3rd cycles of chemotherapy. Results: The median age of the study population was 54 (32-70) years. Fifteen patients (41.1%) were treated on an adjuvant basis, whereas 19 patients (58.9%) were treated for metastatic disease. There was no correlation of serum NGAL levels with serum creatinine (r=0.20, p=0.26) and MDRD (r=-0.12, p=0.50) and creatinine clearance-Cockcroft-Gault (r=-0.22, p=0.22) after cisplatin infusion at the end of the 3rd cycle of chemotherapy. Conclusions: In our study, serum NGAL levels were not correlated with the cisplatin induced nephrotoxicity. Further prospective studies are needed to conclude that serum NGAL level is not a good surrogate marker to predict early cisplatin induced nephrotoxicity.

Comparison of Penetrating and Blunt Traumatic Diaphragmatic Injuries

  • Purpose: Traumatic diaphragmatic injury (TDI) is no longer considered to be a rare condition in Korea. This study investigated differences in the prevalence of accompanying injuries and the prognosis in patients with traumatic diaphragmatic damage according to the mechanism of injury. Methods: We retrospectively reviewed the medical records of patients with TDI who were seen at a regional emergency medical center from January 2000 to December 2018. Among severe trauma patients with traumatic diaphragmatic damage, adults older than 18 years of age with a known mechanism of injury were included in this study. Surgery performed within 6 hours after the injury was sustained was defined as emergency surgery. We assessed the survival rate and likelihood of respiratory compromise according to the mechanism of injury. Results: In total, 103 patients were analyzed. The patients were categorized according to whether they had experienced a penetrating injury or a blunt injury. Thirty-five patients had sustained a penetrating injury, and traffic accidents were the most common cause of blunt injuries. The location of the injury did not show a statistically significant difference between these groups. Severity of TDI was more common in the blunt injury group than in the penetrating injury group, and was also more likely in patients with respiratory compromise. However, sex, the extent of damage, and the initial Glasgow coma scale score had no significant relationship with severity. Conclusions: Based on the findings of this study, TDI should be recognized and managed proactively in patients with blunt injury and/or respiratory compromise. Early recognition and implementation of an appropriate management strategy would improve patients' prognosis. Multi-center, prospective studies are needed in the future.

Urinary Tract Infection in Febrile Infants with Pyuria (발열과 농뇨가 있는 영아에서 요로감염에 관한 연구)

  • Objective : Urinary tract infection(UTI) is a frequent serious bacterial infection in young infants. The clinical presentation may be non-specific and variable, depends on factors such as the age and the level of infection. Children with renal involvement may be at risk of permanent renal damage. Experimental studies have shown that renal lesions caused by acute febrile UTI may be prevented or diminished by early diagnosis and treatment. Therefore, it is important to find a method that can permit early diagnosis and identification of patients who are at risk for progressive renal damage. We designed this study to identify related factors in culture positive UTI infants, and also to identify related factors in culture negative UTI infants, who are febrile with pyuria, by using renal imaging and functional studies including renal sonography, DMSA scan and VCUG. Methods : Retrospectively analyzed the medical records of 136 febrile infants with pyuria over 2 years(from January 2001 to February 2003). Urine culture was done in all cases, and regardless of urine culture findings, renal imaging study was done if symptomatic UTI suspected. Results : Total 57 organisms were isolated in 53 patients. E. coli was the most common organism(86%), followed by E. faecalis, M. morganii, Proteus species, P. aeruginosa, S. aureus and E. fergusonii. Most of the isolates had high sensitivity on cephalosporins or amikacin and had low sensitivities on aminopenicillins. Abnormal acute phase DMSA scan or VCUG findings were seen in both urine culture positive and negative group without statistical differences(P>0.05). In febrile infants with pyuria, fever over 48 hours, older age and high CRP related to abnormal acute phase DMSA scan findings regardless urine culture results. Conclusion : 1st or 3rd generation cephalosporins with amikacin could be the first choice of treatment for UTI. Febrile infants with positive urine culture dose mean urinary tract infection but not acute pyelonephritis which directly relates to cortical damage which could be confirmed by acute phase DMSA scan. Even cases with negative urine culture findings, acute pyelonephritis should be concerned in febrile infants with pyuria who are older than 3 months of age, has fever over 48 hours or high CRP level. And in such cases, acute phase DMSA scan and VCUG should be evaluated for early treatment and long term prognosis.

On the Audible Band Hearing Measurement of Both Ears Considering Age (연령이 고려된 양쪽 귀의 가청대역 청력측정에 관한 연구)

  • Since humans live while feeling rich in their five senses, care must be taken not to damage their hearing in order to lead a happy life. Hearing damage suffered in a living environment is common in a partial band where a specific frequency band is first damaged. However, since the initial pain of hearing damage does not appear much, it is difficult to determine the symptoms in the early stages. In this paper, we proposed a high-speed measurement method that can easily and quickly self-measure hearing damage through a simple app. In this method, 9 pure tones compensated for gain for each voice soub-band are alternately heard in both ears, so that one's own hearing damage can be detected at high speed. When 18 tone pulses were heard for 30 seconds per person, 1 out of 12 subjects was identified as suspected of partial hearing damage. And when the test subjects were informed of the hearing determination method for the number of hearings, the participants themselves immediately determined whether their hearing was damaged. This measurement method simply judges the hearing of both ears within 30 seconds with a smart phone, so that hearing measurements that took more than 10 minutes in hospitals are relatively accurate, and it was found that hearing health can be maintained while reducing time and costs.

Cognition and Experience about Sexual Harassment in Elementary School Students (초등학생의 성희롱에 대한 인식과 경험)

  • Purpose : Sexual abuse especially in younger group have been growing. This study was performed to propose the fundamental data for developing early education program to prevent damage from sexual harassment in elementary school students. Method : Questionnaire survey on cognitions and experiences about sexual harassment was done on 3125 elementary school students from seven elementary schools in Busan area from January to March, 2002. Data analysis was done with SPSS (ver 10.0). Results : Though 82% of the students have ever herd about sexual harassment previously, but self assessment of 'well-known' was only 5%. About 40% of them have ever experienced audio-visual sexual materials mainly through PC (18%), media (14%), etc. And, 60% of them have ever educated about sexual harassment. In cognitions, they agreed 'unwanted touch', 'intentional showing his (her) own body', 'send lewd mails and pictures', but disagreed 'ask massage from aged', 'gaudy joke' as one of sexual harassment. In attitudes, 'no response', 'defense with friends and family', 'express unpleasantness apparently' were desirable pattern, instead, 'his (her) own responsibility', 'problem only between person concerned', 'neglect is the best way' were undesirable. About 24% of them have ever experienced at least one of the fifteen types of sexual harassment with major types of lewd mails pictures (11%), hugging (5%), ask massage (4.7%) and unwanted touch (3.3%). The experience by gender was higher in girls (25.0%) than boys (22.4%) with no statistical significance. The mean level of cognition and attitude out of one hundred was $70.3{\pm}28.1$ and $73.5{\pm}15.9$, respectively. The mean frequency of experience was $0.4{\pm}1.1$ times. The level of cognition and attitude was higher in girls, 6th graders, and in ever educated group (p<0.001). Conclusions : The cognition level of elementary school students about sexual harassment assessed considerable. And, they also experienced several types of sexual harassment, though not too much. Therefore, compatible education program let them able to protect themselves from sexual harassment should be developed and applied even in early school age.

Discordant findings of dimercaptosuccinic acid scintigraphy in children with multi-detector row computed tomography-proven acute pyelonephritis

  • Purpose: The diagnosis of acute pyelonephritis (APN) is often difficult, as its clinical and biological manifestations are non-specific in children. If not treated quickly and adequately, however, APN may cause irreversible renal damage, possibly leading to hypertension and chronic renal failure. We were suspecting the diagnostic value of $^{99m}Tc$-dimercaptosuccinic acid (DMSA) scan by experiences and so compared the results of DMSA scan to those of multi-detector row computed tomography (MDCT). Methods: We retrospectively selected and analyzed 81 patients who were diagnosed as APN by MDCT during evaluation of their acute abdomen in emergency room and then received DMSA scan also for the diagnostic work-up of APN after admission. We evaluated the results of imaging studies and compared the diagnostic value of each method by age groups, <2 years (n=45) and ${\geq}$2 years (n=36). Results: Among total 81 patients with MDCT-proven APN, DMSA scan was diagnostic only in 55 children (68%), while the remaining 26 children (32%) showed false negative normal findings. These 26 patients were predominantly male and most of them, 19 (73.1%) were <2 years of age. Conclusion: DMSA scan holds obvious limitation compared to MDCT in depicting acute inflammatory lesions of kidney in children with APN, especially in early childhood less than 2 years of age. MDCT showed hidden lesions of APN, those were undetectable through DMSA scan in children.
