Effects of an Inpatient Pulmonary Rehabilitation Program on Dyspnea, Exercise Capacity, and Health Related Quality of Life for Patients with Chronic Lung Disease (입원 호흡재활 프로그램이 만성 폐질환자의 호흡곤란, 운동능력과 건강관련 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of an inpatient pulmonary rehabilitation program on dyspnea, exercise capacity, and health related quality of life in inpatients with chronic lung disease. Method: This quasi experimental study was designed with a nonequivalent control group pre-post test time series. Twenty three patients were assigned to the experimental group and nineteen to the control group. The inpatient pulmonary rehabilitation program was composed of upper and lower extremity exercise, breathing retraining, inspiratory muscle training, education, relaxation and telephone contacts. This program consisted of 4 sessions with inpatients and 4 weeks at home after discharge. The control group was given a home based pulmonary rehabilitation program at the time of discharge. The outcomes were measured by the Borg score, 6MWD and the Chronic Respiratory Disease Questionnaire(CRQ). Results: There was a statistically significant difference in dyspnea between the experimental group and control group, but not among time sequence, or interaction between groups and time sequence. Also significant improvements in exercise capacity and health related quality of life were found only in the experimental group. Conclusions: An Inpatient pulmonary rehabilitation program may be a useful intervention to reduce dyspnea, and increase exercise capacity and health related quality of life for chronic lung disease patients.

A Historical Study of the Acupoints for Using Acupuncture and Moxibustion on Wheezing and Dyspnea (효천증 침구치료 혈위 및 치법에 관한 고찰)

  • Objectives : The purpose of this study is to investigate acupoints that appear in ancient records and use with regard to treatment to the wheezing and dyspnea. Methods : We investigated 10 ancient records from Song Dynasty to the Qing Dynasty that had medical references to wheezing and dyspnea. Results and Conclusions : Out of acupoints that found out to be used for treatment of wheezing and dyspnea in this study, Conception and Governor channels account for 38.9%, the three yang meridians of the foot account for 22.2% and the three tin meridians of the Hand account for 13.9%. In 10 ancient records, CV22(Tian tu) appears 5 times, CV12(Zhongwan) and ST36(Zusanli) appear 4times. A total number of acupoints that appear in the ancient records were 35. In compendium of medicine, the number of acupoints was 21. In cmpendium of acupuncture and moxibustion, the number of acupoints was 10. In the treatment of wheezing and dyspnea, not only acupuncture but also moxibustion has a great importance.

Improvement of Dyspnea and Palpitations of Cardiac Origin Using Comprehensive Traditional Korean Medicine Treatment Including Bangibokryeong-tang (Fangji Fuling Decoction) : A Case Report (심장성 원인의 호흡곤란 및 두근거림을 방기복령탕가미방(Fangji Fuling Decoction)을 포함한 복합 한의치료로 호전시킨 증례 1례 - 증례보고)

  • This case report demonstrates the effect of modified Bangibokryeong-tang (Fangji Fuling Decoction), a traditional herbal prescription in treating dyspnea and palpitations, symptoms that often lead to anxiety and reduced quality of life of cardiac patients. A female patient in her late 70s with congenital valve malformation and atrial fibrillation presented with dyspnea and palpitations, each rated at a severity of 8 on a numeric rating scale (NRS). After voluntarily discontinuing Western medication, she received modified BGBRT for 25 days, leading to significant symptom relief and NRS improvement to 0-2. The patient reported maintaining an improved condition and showed a significant increase in vitality. This improvement was sustained for 7 months, but the symptoms recurred; thus, modified BGBRT was restarted. Modified BGBRT showed substantial effects on persistent cardiac-origin dyspnea and palpitations. Thus, this TKM has the potential to manage symptoms and enhance the quality of life of cardiac patients.

A Clinical Report of Worsened Dyspnea after Medical Treatment for Thymic Cancer by FulingXingrenGancao-Tang (흉선암 수술 후 악화된 호흡곤란을 복령행인감초탕으로 치료한 증례보고 1례)

  • Objective : This study is intended to demonstrate the effect of FulingXingrenGancao-Tang on worsened dyspnea after medical treatment for thymic cancer. Method : We diagnosed this case as XiongbiXintongDuanqi(胸痺心痛短氣病) in Jinguiyaolue and treated this patient with FulingXingrenGancao-Tang. Results : By using ZhishiXiebaiGuizhi-Tang, LingGuiWeiGan-Tang, Fuling XingrenGancao-Tang, the patient's symptoms were significantly decreased. Conclusion : This study shows that FulingXingrenGancao-Tang is effective on dyspnea after medical treatment of thymic cancer. However further clinical studies are needed.

A Case Report of Congestive Heart Failure Treated with Acupotomy Therapy (심부전으로 인한 호흡곤란 환자에게 시행한 침도침 시술 1례 보고)

  • Objectives : The purpose of this report is to report the effect of acupotomy for patients with congestive heart failure. Methods: We treated 1 patient who has dyspnea due to congestive heart failure with acupotomy. To check the effect and satisfaction of acupotomy we used New York Heart Association class(NYHA class), Modified Borg Scalw Dyspnea Index(Borg Index), Baseline Dyspnea Index(BDI), Five-point Likert scale. Results : After 1 month of treatment, the patient felt much better in breathing, and had better score in NYHA class, Borg Index, BDI. Conclusions : This report demonstrates that acupotomy therapy has useful effect on congestive heart failure, but the more cases and researches are needed.

  • Pleural metastasis from breast cancer is a common manifestation. While pleural effusion is the most frequent finding, it is relatively rare for pleural nodularity and plaque that do not accompany pleural effusion. We report a patient with a rapidly growing huge pleural mass without pleural effusion caused by breast cancer. The patient was treated for severe dyspnea caused by the pleural mass. Along with the case report, we performed a systematic review of management of dyspnea in terminal cancer patients.

Huge Subglottic Polyp Treated with Tracheotomy and Laryngofissure (기관절개술과 후두절개술로 치료한 거대 성문하 용종 1예)

  • Vocal polyps are benign laryngeal lesions which arise from the Reinke's space abd hoarseness is the most common symptom. However, airway compromised is rarely presented in the vocal polyp. A rare case of large subglottic polyp causing dyspnea is reported. Tracheostomy was performed under local anesthesia and then the mass was resected under general anesthesia using a laryngofissure approach. The dyspnea and hoarseness disappeared after surgery immediately. The histopathological findings indicated a diagnosis of vocal cord polyp with chronic inflammatiuon. We consider that tracheostomy is the safest and most useful procedure to guarantee the upper airway in cases of large vocal polyp showing dyspnea. We hereby report a case of huge subglottic polyp in which a tracheostomy and laryngofissure was required for removing the subglottic mass successfully.

  • Double aortic arch is the most common among vascular structure anomalies in infancy. It's clinical manifestations vary from incidental discovery without symptoms to symptoms of tracheal or esophageal compression such as stridor, wheezing, excessive secretion, dyspnea and dysphagia. Characteristically many patients show little sign of respiratory difficulty during sleep and quiet monents but this symptom is frequently exacerbated by crying or exertion, which may be difficult to distinguish from bronchial asthma. CT and MRI are believed to be the most valuable methods of diagnosis and surgical intervention is necessary in severe cases. Recently, we experienced a case of dyspnea due to double aortic arch. So we report this case with review of literatures.

  • Objectives To report the improvements after applying modified Sibjeondaebo-tang and Hyangsayangwi-tang on Soeumin patient with dyspnea and arrhythmia as sequelae of aspiration pneumonia. Methods The 88-year-old female patient had the symptoms of dyspnea, sputum, cough, dysphagia, arrhythmia and general weakness after antibiotics for an aspiration pneumonia. Soeumin Sibjeondaebo-tang and Hyangsayangwi-tang were applied to treat these symptoms. Results The respiratory symptoms improved and the cardiac abnormalities, including arrhythmia showed improvement on the electrocardiogram and in cardiac-related biomarkers. Conclusions This case report suggests that Sasang constitutional herbal medicine could improve respiratory and cardiac symptoms as sequelae of aspiration pneumonia.

The Effect of Acupuncture Therapy of Tian-zong (SI11) on Pulmonary Function Test of Patients with Dyspnea (천종혈(SI11) 자침이 자각적 호흡곤란 환자의 폐기능변화에 미치는 영향)

  • Objectives : Dyspnea is one of the most common symptoms that we meet, but so far there has been no research about the effects of acupuncture therapy on it. This study was performed in order to evaluate the effect of acupuncture therapy on dyspnea by means of using objective indexes. Methods : The effect of acupuncture therapy was assessed by analyzing the pulmonary function test (PFT) in 10 patients, who had suffered from dyspnea and had pain at the acupuncture point Tian-zong (SI11) when it was compressed by a finger, at our outpatient department. Each patient was treated with acupuncture therapy at both sides of Tian-zong. We compared the results of pre-treatment and post-treatment PFTs, including forced vital capacity (FVC), forced expiratory volume in one second (FEV 1.0), and peak expiratory flow rate (PEFR). Results : FVCs of pre-treatment and post-treatment PFTs were $2.23{\pm}0.34(L)$ and $2.40{\pm}0.36(L)$ respectively (P<0.01). FEV1.0s of pre-treatment and post-treatment PFTs were $1.54{\pm}0.37(L)$ and $1.65{\pm}0.41(L)$ respectively (p<0.05). PEFRs of pre-treatment and post-treatment PFTs were $3.41{\pm}1.26(L)$ and $3.93{\pm}1.42(L)$ respectively (p<0.05). Conclusions From the above data, we conclude that FVC, FEV1.0, and PEFR in dyspneic patients have been improved after acupuncture therapy of Tian-zong, so acupuncture therapy of Tian-zong may relieve the dyspnea symptom.
