고유수용성 신경근 촉진법이 편마비 환자의 보행에 미치는 영향 (Effects of Proprioceptive Neuromuscular Facilitation Techniques on the Gait for Hemiplegic Patients)

  • The purpose of this study was to investigate the effects of pelvic and lower extremity exercise on the gait of 25 patients with hemiplegia. 11 men and 14 women were studied, with a mean age of 61.2 years and a means duration post-onset of 8.7 months. Each participant recieved a total of 15 sessions of PNF(five times per week), and each procedure lasting for 40 minutes with affected side pelvic anterior elevation and posterior depression pattern, lower extremity flexion adduction with knee flexion pattern had 20 minutes respectively. Results showed significant improvement in gait speed and cadence in after 1st, 2nd, 3rd week(5th, 10th, 15th session, respectively) compared with the before treatment(p<0.05). And improvement in gait speed and cadence accompanied with longer stride length.

How Does 12-weeks of Taekwondo Training Effect Older Persons' Functional Fitness: A Preliminary Study

  • PURPOSE: This pre-post intervention study aimed to examine the effects of a 12-week supervised modified Taekwondo exercise program on the functional fitness of community-dwelling older adults. METHODS: 10 participants (mean age: 72.3 ± 4.6 years) completed the program over a 12-week period. The intervention consisted of Taekwondo-based exercises modified for older persons. Changes to functional fitness were measured using the finger-to-nose test, functional reach test, timed up and go test, 30-second sit-to-stand test, 30-second arm curl test, Chester step test, chair sit-and-reach test, and back scratch test. Participants were assessed prior to the training and after the final training session, changes were measured using descriptive statistics and paired sample 't' tests. RESULTS: Effect sizes ranged from small to large (Cohen's d = .22 to 1.23). The exercise program was well- tolerated by participants, with a high level of engagement and no attrition for the duration of the program. Results showed significant improvements in most measures of functional fitness (p < .05) except for the back scratch test (p = .051). CONCLUSION: These findings suggest a well-designed, supervised, modified Taekwondo exercise program can significantly improve functional fitness in older adults.

신경계 질환자 대상의 국내 음악중재연구에 대한 체계적 문헌고찰 (Systematic Review of the Literatures on Music Intervention for Neurological Patients in Korea)

  • Purpose: The purpose of this study was to review the literature on music intervention for neurological patients in Korea and to explore the effect of music intervention on functional rehabilitation of neurological patients. Methods: This systematic review examined literature from 2005 to 2010. The existing literature was searched in the electronic databases of RISS and KISS using the following keywords: music, rhythmic movement, stroke, brain injury, dementia, Parkinson, spinal cord injury. Seventy-seven studies were founded through the database. After The exclusion of duplicates, case studies, literature review, and irrelevant studies, nine studies were included for analysis. Results: They included nine quasi-experimental studies. The medical diagnoses of the study participants were stroke, dementia, brain injury, and spinal cord injury. Among nine studies, only two studies were conducted in the nursing area. Eight studies used comprehensive music programs composed of health education, gait training, vocal training, or relaxation therapy, and these comprehensive programs were effective in the areas of physical and psychosocial treatment. Seventeen sessions were provided on average throughout intervention, and the average duration of each session was 49.4 minutes. Conclusion: Comprehensive music intervention can be implemented in nursing care for patients with neurological problem.

뇌졸중 환자에게 적용한 로봇보행 재활훈련의 효과: 메타분석 (The Effects of Robot-Assisted Gait Training for the Patient With Post Stroke: A Meta-Analysis)

  • Robot-assisted rehabilitation therapy has been used to increase physical function in post-stroke patients. The aim of this meta-analysis was to identify whether robot-assisted gait training can improve patients' functional abilities. A comprehensive search was performed of PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), Physiotherapy Evidence Database (PEDro), Academic Search Premier (ASP), ScienceDirect, Korean Studies Information Service System (KISS), Research Information Sharing Service (RISS), Korea National Library, and the Korean Medical Database up to April, 2014. Fifteen eligible studies researched the effects of robot-assisted gait training to a control group. All outcome measures were classified by International Classification of Functioning, Disability, and Health (ICF) domains (body function and structures, activity, and participation) and were pooled for calculating the effect size. The overall effect size of the robot-assisted gait training was .356 [95% confidence interval (CI): .186~.526]. When the effect was compared by the type of electromechanical robot, Gait Trainer (GT) (.471, 95% CI: .320~.621) showed more effective than Lokomat (.169, 95% CI: .063~.275). In addition, acute stroke patients showed more improvement than others. Although robot-assisted gait training may improve function, but there is no scientific evidence about the appropriate treatment time for one session or the appropriate duration of treatment. Additional researchers are needed to include more well-designed trials in order to resolve these uncertainties.

수중운동치료가 만성 뇌졸중 환자의 균형에 미치는 효과 : 국내연구의 메타분석 (The Effect of Aquatic Exercise Therapy on Balance in Patients with Chronic Stroke : Meta-Analysis of Studies in Korea)

  • Purpose : This meta-analysis was aimed at guiding future research in stroke treatment and to provide real-world data relating to the effects of aquatic exercise therapy on balance in patients with chronic stroke. Methods : We performed a meta-analysis comprising 22 studies involving aquatic exercise therapy performed between 2006 and 2017. A meta-analysis software program was used to calculate the mean effect size, effect size by intervention, and effect size by outcome. We also performed a meta-regression analysis and an analysis of publishing bias. Results : The mean effect size was 0.563. The effect size by outcome was observed to be the largest for the functional reach test, followed by the Berg balance scale, balance equipment, the Timed Up and Go test and one leg standing. Meta-regression analysis showed that effect size increased with an increase in the duration, number, length of exercise session. Conclusion : Aquatic exercise therapy appears to show a moderate effect on balance in patients with chronic stroke. A meta-analysis is warranted for further research to determine the effects of aquatic exercise on walking, muscle strength, and range of motion.

음악요법에 관한 국내 간호학 연구 분석 (The Analysis of Nursing Research on Music Therapy in Korea)

  • Purpose: The purpose of this study was to review and analyze the trend and issues of nursing research on music therapy. This study was aimed to help direct developing better music therapy programs in the future. Method: We reviewed 57 dissertations and published articles from 1983 to Feb. 2005 that were related to music therapy in nursing. Result: Among the 57 published researches on music therapy, 47 (82.5%) were dissertations and 10 (17.5%) were published articles. Among the 57 researches, 55 (96.5%) were experiential researches and 2 (3.5%) were non-experimental researches. Surgical patients were the most popular subject for researches. The number of subject was a minimum of 7 people and maximum of 143 people. Listening to music 41 (74.5%) was the most popular program for music therapy. Among 55 researches, 33 (60.0%) had a music therapy session of 25-60 minutes and this was the most popular time duration of therapy. Among all the 108 dependent variables, 57 (52.8%) were psychological variables, 40 (37.0%) were physical variables and 11 (10.2%) were social-cognitive variables. Conclusion: There is an urgent need to develop systematic music therapy programs with different nursing interventions, as based on theories and principles while considering the physical, psychological, and social characteristic of the subjects.

The effect of aromatherapy on pain in individuals with diabetes: a systematic review and meta-analysis

  • Purpose: This study systematically analyzed the impact of aromatherapy on pain in individuals with diabetes. Methods: A search was performed in seven electronic databases based on the PICO-SD (Population, Intervention, Comparison, Outcome, Study Design) framework. The population (P) of interest was individuals with diabetes, and the intervention (I) included aromatherapy targeting pain reduction. The comparison (C) consisted of control groups that received no intervention, another intervention, or usual care. The outcome (O) measured was pain. The quality of the selected literature was assessed using the Joanna Briggs Institute checklist. In MIX 2.0 Pro, the pooled overall effect of pain was calculated using Hedge's g and a random-effects model, and heterogeneity was calculated using the Q statistic and Higgin's I2 values. Meta-regression and exclusion sensitivity analyses were performed. Results: Five articles and seven studies were included, showing a significant pooled overall effect of aromatherapy on diabetes-related pain (Hedge's g = -1.83, 95% CI: -2.76 to -0.91). Meta-regression demonstrated that effectiveness in reducing pain was associated with studies conducted in West Asia, those with IRB approval, and those receiving funding. Additionally, interventions involving subjects under 60, lavender oil (vs. turpentine oil or blended oils), massage therapy (vs. topical application), fewer hours per session, and more repeated measurements (vs. pre/post measurements) were associated with pain reduction. Conclusion: Aromatherapy, especially with lavender oil, effectively manages diabetes-related pain. Short-duration massage application is also effective. A personalized selection of oil type and application method could optimize therapeutic outcomes for individuals with diabetes.

Therapeutic Effect of Low-Energy Nitrogen Plasma Pulses on Tinea Pedis

  • Superficial fungal infections with dermatophytes, nondermatophyte molds, or yeasts are treated primarily with topical and/or systemic antifungal agents. Additional or alternative treatment modalities, particularly energy-delivering modalities, however, are used widely to induce fungicidal effects via selective photothermal reactions. In addition to light- or laser-based devices, plasma therapy also has antifungal properties. This report describes a Korean male patient with mycologically confirmed tinea pedis that was treated effectively with two sessions of nitrogen plasma treatment at one-week intervals using a plasma delivering system. Nitrogen plasma was prepared by loading a 0.28-ml inert nitrogen gas/pulse that was activated by a microwave generator. The other treatment settings were a nozzle diameter of 5 mm, pulse energy of 0.75 J, pulse duration of 7 msec, and two passes. One week after the first session of nitrogen plasma treatment, the patient exhibited marked reductions in scale and inflammation. One month after the final treatment, no clinical features of recurrence were found, and successive potassium hydroxide testing revealed negative results.

The development of new electromyographic parameters to diagnose low-back pain patients during sagittal flexion/extension motion

  • The Electomyographic (EMG) signals of flexor-extensor muscle pairs were investigated to identify the neural excitation pattern of low-back pain (LBP) patients during a repetitive bending motion. New parameters and EMG normalization technique were developed to quantitatively represent the difference of temporal EMG patterns between ten healthy subjects and ten LBP patients. Flexor-extensor muscle pairs such as rectus abdominis(RA)-erector spinae (ES at LS), external oblique(EO)-internal oblique(IO), rectus femois (quadriceps: QUD)-biceps femoris(hamstrings:HAM), and tibialis anterior(TA)-gastrocnemius(GAS) pairs of muscles were selected in this study. Results indicated that the temporal EMG pattern such as the peak timing difference of QUD-HAM muscle pair and the duration of coexcitation of ES-RA muscle pair showed a statistically isgnificant difference between healthy subjects and LBP patients. These results indicated that the new technique and parameters could be used as a diagnostic tool especially for LBP patients with soft tissue injuries that are rarely dentified by traditional imaging techniques such as X-ray, CT scan or MRI. Improtantly, the new EMG technique did not require the maximal volutary contraction(MVC) measure for normalization that helped patients minimize the pain experience during and after the session. Further study needs to be made to validate and refine this method for clinical application.

타이치 운동프로그램이 골관절염 환자의 체력, 통증, 자기효능감에 미치는 효과 (Effects of Tai Chi Exercise Program on Physical Fitness, Pain, and Self-efficacy in Patients with Osteoarthritis)

  • Purpose: The purpose of this study was to investigate the effects of Tai Chi exercise program on physical fitness, pain, and self-efficacy in patients with osteoarthritis. Method: A preexperimental research design was applied to 22 subjects who dwelled at a welfare institution in C-city and agreed to participate in this study. The Tai Chi exercise was conducted with a duration of 60 minutes per session twice a week for 9 weeks. Outcome measures were physical fitness($VO_{2}max$, strength, flexibility, foot power, agility, balance), pain, and self-efficacy. Data were analyzed by descriptive statistics and paired t-test. Result: After participating in the Tai Chi exercise program, the subjects showed significant improvements in strength, agility, balance, pain and self-efficacy. But there were no significant differences in $VO_{2}max$, flexibility and foot power. Conclusion: The results showed that Tai Chi exercise would partially improve physical fitness and be effective for pain reduction and self efficacy as well. Further studies are needed to confirm the effects of Tai Chi exercise on physical fitness.
