신생아 골밀도에 관한 연구 - 재태 연령과 출생 체중에 따른 골밀도의 차이 - (A Study on the Bone Density in Newborn Infants - Difference of the Bone Mineral Density according to the Gestational Age and the Birth Weight -)

  • 미숙아를 포함한 신생아에서의 재태 연령에 따른 골밀도의 차이를 관찰하여 향후 대사성 골질환의 진단을 기초 자료로 삼기 위하여, 1995년 3월 1일부터 1997년 2월 28일까지 영남대학교 의과대학 부속병원에서 출생한 신생아 중 자궁내 성장 발육 이상 소견이 없고, 생후 1주일이내에 골밀도 측정이 가능하였던 53명을 대상으로 이중에너지 X-선 흡수계측법 (dual energy X-ray absorptiometry, DXA, X-R 26, Norland, U.S.A.)을 사용하여 생후 1주일내의 요추에서의 골밀도를 측정하여 다음과 같은 결과를 얻었다. 1. 골밀도는 성별에 따른 차이는 없었고, 재태 연령군별 골밀도는 재태 연령 28-30주군의 $0.149{\pm}0.009g/cm^2$에서부터 39-41주군의 $0.229{\pm}0.034g/cm^2$까지 재태 연령의 증가에 따라 함께 증가를 보여 33-34주군과 35 - 36주군 사이를 제외한 재태 연령군별 골밀도는 유의한 차이를 보였으며 (p<0.05), 재태 연령이 28주 3일에서 41주 3일로 증가함에 따라 골밀도는 직선적인 상관관계를 나타내었다 ($Y=7.5{\times}10^{-3}X-0.082$, r=0.7018, p<0.001). 2. 출생체중별 골밀도는 1,000-1,499그람군의 골밀도는 $0.158{\pm}0.020g/cm^2$을 나타내었고, 3,500-4,000그람군의 골밀도는 $0.251{\pm}0.021g/cm^2$를 나타내어 출생 체중군별 골밀도는 1,000-1,499그람군과 1,500-1,999그람군 사이를 제외한 모든 체중군별로 유의한 차이를 나타내었다 (p<0.05). 출생체중이 1,250그람으로부터 3,880그람까지 증가하는데 따라 골밀도는 유의하게 증가하며 직선적인 상관관계를 나타내었다 ($Y=3.9{\times}10^{-5}X+0.093$, r=0.7296, p<0.001). 재태연령이 증가할수록, 출생체중이 증가할수록 골밀도가 증가하는 것을 관찰하였으며, 이는 향후 미숙아를 포함하는 신생아에 대한 골밀도 변화를 관찰하는 기초 자료가 될 수 있다고 생각된다.

이중 에너지 엑스레이 흡수기의 가동 시간에 따른 골밀도 값의 평가 (The Bone Mineral Density Value According to the Operating Time of the Dual Energy X-ray)

  • Purpose: Recently, the performance of the X-ray tube was very much improved by the power generation of the technology. However, the overload of equipment is occurred by the increment of the equipment operating time according to the increment of the examination number of cases. The X-ray dose can change by heat occurrence of the X-ray tube due to this. Moreover, the change of the bone mineral density value is possible to occur. Therefore, We tries to whether the change of the bone mineral density value of each equipment according to the difference of the examination number of cases and operating time occur or not. Materials and Methods: The BMD value was measured by the Aluminum Spine Phantom and the European Spine Phantom in each equipment, in order to find out about the difference of the time general classification bone mineral density value by using the Dual energy X-ray absorptiometry. And after scanning each phantom by using X-ray dose meter (Unfors Mult-O-Meter), a dose was measured by the same condition. As to, an average and standard deviation were found and the change of each equipment much BMD value was compared and it evaluated. Results: $Mean{\pm}SD$ of each equipment by using the Aluminum Spine Phantom, A equipment was $1.174{\pm}0.002$, $1.171{\pm}0.005$, $1.173{\pm}0.005$, B equipment was $1.186{\pm}0.003$, $1.187{\pm}0.003$, $1.185{\pm}0.003$, C equipment was $1.180{\pm}0.003$, $1.182{\pm}0.004$, $1.183{\pm}0.002$, D equipment was $1.188{\pm}0.004$, $1.185{\pm}0.003$, $1.185{\pm}0.004$. By using the European Spine Phantom, A equipment was $1.143{\pm}0.006$, $1.153{\pm}0.009$, $1.161{\pm}0.003$, B equipment was $1.134{\pm}0.004$, $1.13{\pm}0.008$, $1.127{\pm}0.015$, C equipment was $1.143{\pm}0.006$, $1.134{\pm}0.01$, $1.133{\pm}0.006$, D equipment was $1.14{\pm}0.001$, $1.122{\pm}0.002$, $1.131{\pm}0.008$, altogether included in the normal range. Conclusion: There was no significant change of the BMD value of using a phantom by time zones. Therefore, if the quality control is made to use the extent management method of the equipment for beginning in the present application, the reliability of the BMD equipment will be able to be enhanced.

가돌리늄조영제가 이중에너지 X-선 흡수법을 이용한 골밀도검사에 미치는 영향 (Effects of Gadolinium Contrast agent on Bone Mineral Density Measurement using Dual Energy X-ray Absorptiometry)

  • 골밀도검사에서 측정값에 영향을 주는 요인 중 하나인 방사선 조영제는 진단 목적으로 사용되어 조직 등에 흡수되어 골밀도 측정값을 높이는 효과가 있어, 골밀도검사 시행 전에는 될 수 있으면 피하고 있다. 이러한 방사선 조영제와는 다른 물리적 특성과 작용기전을 갖는 MRI 조영제는 금속원소인 가돌리늄 기반으로 방사선 불투과성 특징을 가지고 있어 일반적으로 방사선을 이용한 검사 전에 MRI 검사를 선 시행하고 있다. 본 연구에서는 MRI 조영제가 이중에너지 X-선 흡수법을 이용한 골밀도검사에 미치는 영향을 알아보고자 하였다. 가돌리늄 기반의 MRI 조영제 2종을 아크릴 수조에 용량별로 주입하여 인체모형 척추 팬텀을 넣고, 각 주입량 종류별로 5회씩 총 30회 scan 하여 L1-L4 번까지의 골밀도와 T-score를 분석하였다. 측정된 2종의 조영제별 Total(L1-L4) 골밀도의 평균값은 가도테레이트 메글루민성분 0mL, 7.5mL, 15mL일때, 각각 0.952±0.052, 0.957±0.050, 0.956±0.05 g/㎠로 나타났으며, 가도부트롤성분 0mL, 5mL, 10mL일 때, 각각 0.953±0.001, 0.954±0.001, 0.945±0.001g/㎠로 모든 부위에서 통계적으로 유의한 차이는 없었다(p>0.05). Total (L1-L4) T-score의 평균값은 가도테레이트 메글루민성분은 0mL, 7.5mL, 15mL일 때, 각각 -0.46±0.05, -0.4±0, -0.42±0.04로 나타났으며, 가도부트롤성분 0mL, 5mL, 10mL일 때, 각각 -0.46±0.05, -0.46±0.05, 0.5±0.00으로 모든 부위에서 통계적으로 유의한 차이는 없었다(p>0.05). 본 실험에서는 MRI 조영제는 이중에너지 X-선 흡수법을 이용한 골밀도검사에 영향이 없는 것으로 나타났으나, 임상 대상이 아닌 제한적 조건의 인체 모형 팬텀을 사용하여 조영제의 배출시간에 영향을 주는 신장 기능, 신체 상태 등이 고려되지 못한 제한점이 있어 추후 추가적인 임상연구 진행이 필요할 것으로 사료된다.

한국 과체중 및 비만 여성의 휴식대사량 측정 및 예측값의 비교 (Comparison of Predicted and Measured Resting Energy Expenditure in Overweight and Obese Korean Women)

  • Objectives: The purpose of this study was to compare predictions and measurements of the resting energy expenditure (REE) of overweight and obese adult women in Korea. Methods: The subjects included 65 overweight or obese adult women ranging in age from 20~60 with a recorded body mass index (BMI) of 23 or higher. Their height, weight, waist-hip ratio, and blood pressure were measured. The investigator also measured their body fat, body fat percentage, and body composition of total weight without fat using Dual energy X-ray absorptiometry (DXA) and measured resting energy expenditure by indirect calorimetry. Measured resting energy expenditures were compared with predictions from six methods: Harris-Benedict, Mifflin, Owen, WHO-WH, Henry-WH, and KDRI. Results: Harris-Benedict predictions showed the smallest differences from measured resting energy expenditure at an accurate prediction rate of 70%. The study analyzed regression between measured resting energy expenditure and body measurements including height, weight and age. The formula proposed by this research is as follows: Proposed REE equation for overweight and obese Korean women = $721-(1.5{\times}age)+(0.4{\times}height)+(9.9{\times}weight)$. Conclusions: These findings suggest that age is a significant variable when predicting resting energy expenditure in overweight and obese women. Therefore, prediction of resting energy expenditure should consider age when determining energy requirements in overweight and obese women.

타 인종에 있어 체질과 체성분의 상관성에 관한 연구 (Association between Sasang Constitutions and Body Composition in African-American, Asian, and Caucasian Race Groups)

  • Background: Sasang Constitutional Medicine (SCM) is a component of Korean traditional medicine that classifies humans into four categories, according to their constitution (Taeyangin, Taeumin, Soyangin and Soeumin). We are unaware of previous assessments of SCM in non-Asian race groups. Objectives: To quantify body composition components in three race groups and to determine whether SCM has an independent effect on body composition components. Design: A cross-sectional evaluation of 76 adults (28 Caucasian, 21 African-American, 27 Asian). Body composition was estimated using dual energy x-ray absorptiometry. Results: SCM component had a significant effect on weight, fat, and lean mass regardless of race group (p <0.001).

식이단백질량이 성장기 흰쥐가 골밀도에 대한 칼슘효율에 미치는 영향 (Effect of Dietary Protein Level on Ca Efficiency in Bone Mineral Density in Growing Rats)

  • This study was performed to evaluate the effect of dietary protein level on Ca efficiency in bone mineral density in growing male rats. Twenty male rate were divided into two groups. The rats in one group were fed on casein 20% diet as control group and the others were fed on casein 40% diet as protein group. All rats were fed on experimental diet and deionized water ad libitum for 9 weeks. The total body, spine and femur bone mineral density and bone mineral content were measured using dual energy-x ray absorptiometry. Urinary calcium, phosphate, pyridinoline and creatinine, serum calcium, phosphate, total protein, albumin, alkaline phosphatase(ALP) and osteocalcin were measured. Urinary Ca excretion, pyridinoline and crosslinks value and serum ALP content seem to be increased in high protein group. It appears that the growing rats in high protein group had a higher bone resprption and bone formation than those in control group. Animal fad a high protein diet had a siginficantly higher Ca efficiency in BMD, BMC of total body, spine and femur. The results of this show that increasing of dietary protein level (40%) is beneficial of improvement of Ca efficiency during growing period.

골밀도가 저하된 폐경 후 성인여성의 뇨중 불소농도와 치주조직과의 관계 (The Urinary Fluoride Concentration and Periodontal Condition in Postmenopausal Osteoporotic Women)

  • There were few reports about the fluoride concentration in bone and osteoporotic women. This study was designed to evaluate the relationship between the urinary fluoride concentration and periodontal condition in osteoporotic old women. Twentyeight postmenopausal women(Test group) and twenty-one premenopausal women(Control group) were examined. Bone mineral density(BMD) of lumbar spine(L2-L4) was measured by dual energy X-ray absorptiometry(DEXA). The urine samples were collected at early morning and determined with the help of a fluoride-specific electrode and Tisabbufferd samples. The results were as follows. 1. The mean urinary fluoride concentration in test and control group showed statistically no difference. 2. The bone mineral density(BMD) of the spine in test group was significantly lower than control group(p<0.05). 3. The significant negative correlation was found between BMD level and age after menopause $(p<0.001,\;{\gamma}=-0.526$. 4. The urinary fluoride concentration was not correlated with age, age after menopause and bone mineral density. 5. The urinary fluoride concentration was not correlated with periodontal condition.

골다공증의 진단시 파노라마 방사선사진 유용성에 대한 연구 (Usefulness of panoramic radiography in the diagnosis of osteoporosis)

  • Purpose: To assess the possibility of using panoramic indices as an indicator of jaw osteoporosis. Methods: Mandibular cortical width (MCW), degree of mandibular alveolar bone resorption (ABR) and morphology of mandibular inferior cortex (MIC) on panoramic radiograph were used as panoramic indices. These panoramic indices were compared with bone mineral density (BMD) of lumbar (L1-L4) and femoral neck measured by dual energy X-ray absorptiometry. We also compared MCW and ABR of young men with those of postmenopausal women. Results : There was a significant correlation between ABR and BMD of lumbar and femoral neck. And also significant correlation between MIC and BMD of lumbar and femoral neck. ANOVA test of BMD of lumbar and femoral neck showed significant differences according to morphologic classification of inferior cortex. There was significant difference in MCW and ABR between young men and postmenopausal women. Conclusion: Our results suggested that ABR and MIC on panoramic radiograph could be reliable in screening of osteoporosis.

장년기 여성의 칼슘영양상태와 골다공증 예방을 위한 영양교육의 효과 (A Study of Calcium Status and Effect of Nutrition Education of Prevention Osteoporosis in Middle - Women)

  • This study was conducted to investigate the relationship of calcium intake , serum calcium level to calcium supplementation and the effect of nutritional education in middle-aged women. The age distribution of the subjects was 49-55 years. Daily nutrient intakes and dietary sources of calcium were analyzed by convenient method and food frequency questionnaire. At the same time, fasting blood samples were collected and serum calcium and magnesium concentrations were measured before and after nutritional education. The BMD of subjects were measured by Dual Energy X-ray Absorptiometry(DEXA). Serum calcium and magnesium concentrations were significantly(p<0.05) different between pre and post nutritional education. After nutritional education, nutrient intakes were significantly increased(p<0.05), especially calcium. This results suggested that dietary calcium intake as well as nutritional education seem to be important in preventing in middle-aged women.

남성의 골밀도 영향요인 (Influencing Factors of Bone Mineral Density in Men)

  • Purpose: Osteoporosis is often considered as a women's disease, but men's osteoporosis is also increasingly recognized as an important public health problem. This study was carried out to identify the important modifiable risk factors for osteoporosis in men. Methods: Two hundred and fourteen men were selected among those who checked bone mineral density by dual energy x-ray absorptiometry in lumbar vertebrae from June to September, 2009 at 3 general hospitals in Busan. Self-administrated questionnaires and BMD results of lumbar vertebrae were used for data. Collected data were analyzed by using SPSS 14.0 program. Results: The sports activity level in 30's, body weight, and fracture history of mother were the most powerful influencing factors on the bone mineral density of lumbar vertebrae. Conclusion: These results highlight the need to participate vigorously in sports activity during 30s as a means of increasing peak bone mass in lumbar vertebrae in order to prevent osteoporosis and fracture in men.
