마우스의 기도 내 점적을 통한 가습기살균제 CMIT/MIT와 사망 간의 원인적 연관성에 관한 연구 (Intra-tracheal Administration of the Disinfectant Chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT) in a Mouse Model to Evaluate a Causal Association with Death)

  • Objectives: The deaths of Korean victims exposed to the disinfectant CMIT/MIT have remained unresolved. This is mainly due to a lack of concordance between the few available toxicity tests and the abundant epidemiological data, making it difficult to establish a cause-and-effect relationship. Therefore, this study was carried out to investigate any potential associations between CMIT/MIT exposure and death. Methods: Groups of experimental and control C57BL/6 mice were instilled (in the trachea) with chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT) using a visual instillobot. CMIT/MIT was instilled over a period of three days and eight weeks, respectively, to achieve acute and chronic exposures. A threshold dose-response model was applied for estimating the threshold level as one line of evidence for a causal association between CMIT/MIT and death. Results: An acute exposure of 1.2 mg ai/kg/day of CMIT/MIT was estimated to reflect the threshold for death. The dose-response curve with this threshold showed a very steep slope and a narrow range of CMIT/MIT exposures. The narrow range of CMIT/MIT exposures, in particular, indicated an evident boundary between survival and death, thus implicating a strong causal association. A similar threshold dose-response relationship observed following acute exposure was also seen following chronic exposure to CMIT/MIT. Airborne disinfectant exposure was visible as minimal or mild lung damage with no fibrosis, as shown by histopathological tests. However, many observations are considered to be functional respiratory tract or lung failure due to death, as observed in necropsies of the mice that died due to CMIT/MIT exposures. Conclusions: There are two strong lines of evidence for a causal association between death and CMIT/MIT exposure: 1) The threshold dose-response curve, with a very steep slope and a narrow range of CMIT/MIT exposures showing a visible boundary between survival and death; and 2) many cases of functional respiratory or lung failure.

Breast Density and Risk of Breast Cancer in Asian Women: A Meta-analysis of Observational Studies

  • Objectives: The established theory that breast density is an independent predictor of breast cancer risk is based on studies targeting white women in the West. More Asian women than Western women have dense breasts, but the incidence of breast cancer is lower among Asian women. This meta-analysis investigated the association between breast density in mammography and breast cancer risk in Asian women. Methods: PubMed and Scopus were searched, and the final date of publication was set as December 31, 2015. The effect size in each article was calculated using the interval-collapse method. Summary effect sizes (sESs) and 95% confidence intervals (CIs) were calculated by conducting a meta-analysis applying a random effect model. To investigate the dose-response relationship, random effect dose-response meta-regression (RE-DRMR) was conducted. Results: Six analytical epidemiology studies in total were selected, including one cohort study and five case-control studies. A total of 17 datasets were constructed by type of breast density index and menopausal status. In analyzing the subgroups of premenopausal vs. postmenopausal women, the percent density (PD) index was confirmed to be associated with a significantly elevated risk for breast cancer (sES, 2.21; 95% CI, 1.52 to 3.21; $I^2=50.0%$). The RE-DRMR results showed that the risk of breast cancer increased 1.73 times for each 25% increase in PD in postmenopausal women (95% CI, 1.20 to 2.47). Conclusions: In Asian women, breast cancer risk increased with breast density measured using the PD index, regardless of menopausal status. We propose the further development of a breast cancer risk prediction model based on the application of PD in Asian women.

심장혈관 조영술에서 씨네(cine)촬영의 프레임변화에 따른 ESD와 DAP 및 영상의 SNR·CNR 관계 분석: 10f/s과 15f/s을 중심으로 (Analysis of the Relationships Between ESD and DAP, and Image SNR·CNR According to the Frame Change of Cine Imaging in CAG : With Focus on 10 f/s and 15 f/s)

  • X선 장비를 이용한 심장혈관 조영술 시 프레임 변화에 따른 입사 표면 선량과 흡수선량을 비교 분석하여 피폭의 차이를 알아보고자 하였고 image J를 통한 촬영 영상의 SNR과 CNR을 측정 분석하여 프레임 변화가 영상 화질에 주는 영향을 비롯해 검사에 있어 적절한 프레임 선택의 방안을 모색하고자 하였다. 2017년 6월부터 2017년 10월까지 본원에서 CAG를 시행한 30명(남19, 여11)을 대상으로 하였고, 환자들의 연령대는 49-82세(평균 $65{\pm}9$세), 몸무게45-91kg (평균 $67{\pm}8.9kg$), 키 150-179cm (평균 $165.1{\pm}8.9kg$), BMI 19.5-30.5(평균 $24.5{\pm}2.9$)이었다. 입사표면선량 및 흡수선량은 후향적으로 Air kerma값과 DAP를 획득하여 비교 분석 하였고 SNR과 CNR은 Image J를 통하여 측정 분석 한 후 공식에 대입하여 결과 값을 도출하였다. 통계분석을 통한 상관관계 확인을 위해 통계프로그램은 SPSS를 사용하여 프레임 변화에 따른 입사표면선량과 흡수선량 및 SNR CNR의 상관관계 등을 분석하였다. 10프레임으로 촬영할 때와 15프레임으로 촬영 할 때 모두 입사표면선량과 흡수선량관계는 통계적으로 유의하지 않았다(p>0.05). SNR과 CNR의 관계에선 10프레임으로 촬영했을 때의 SNR($3.374{\pm}2.1297$)과 CNR($0.234{\pm}0.2249$)이 15프레임에서의 SNR($4.929{\pm}2.8532$)과 CNR($0.391{\pm}0.3025$)보다 SNR $1.43{\pm}0.4861$, CNR $0.132{\pm}0.0555$로 낮았으나 통계적으로는 유의하지 않았다(p>0.05). 상관관계 분석결과에서는 BMI와 air kerma, DAP간에, air kerma와 DAP간에, SNR과 CNR간에 통계적으로 유의한 결과를 얻었다(p<0.001, p<0.001). 결론적으로 심장혈관 조영술 시 10프레임과 15프레임으로 변화를 주어 촬영하여도 입사표면선량과 흡수선량은 큰 차이를 나타내지 않았으며, 10프레임보다 15프레임 촬영에서의 SNR과 CNR이 증가하였으나 통계적으로는 유의하지 않았기에 본 논문을 통해 환자와 시술자 모두 10프레임과 15프레임 촬영으로 인한 X선 피폭 문제뿐 아니라 영상의 화질감소에 관한 우려 역시 줄어들 수 있을 것으로 사료된다.

Hormesis as a Confounding Factor in Epidemiological Studies of Radiation Carcinogenesis

  • Biological mechanisms for ionizing radiation effects are different at low doses than at high doses. Radiation hormesis involves low-dose-induced protection and high-dose-induced harm. The protective component is associated with a reduction in the incidence of cancer below the spontaneous frequency, brought about by activation of defensive and repair processes. The Linear No-Threshold (LNT) hypothesis advocated by the International Commission on Radiological Protection (ICRP) and the Biological Effects of ionizing Radiation (BEIR) Report VII for cancer risk estimations Ignores hormesis and the presence of a threshold. Cancer incidences significantly less than expected have been found in a large number of epidemiological studies including, airline flight personnel, inhabitants of high radiation backgrounds, shipyard workers, nuclear site workers in scores of locations throughout the world, nuclear power utility workers, plutonium workers, military nuclear test site Participants, Japanese A-bomb survivors, residents contaminated by major nuclear accidents, residents of Taiwan living in $^{60}Co$ contaminated buildings, fluoroscopy and mammography patients, radium dial painters, and those exposed to indoor radon. Significantly increased cancer was not found at doses <200 $mSv^*$. Evidence for radiation hormesis was seen in both sexes for acute or chronic exposures, low or high LET radiations, external whole- or partial body exposures, and for internal radionuclides. The ubiquitous nature of the Healthy Worker Effect (HWE)-like responses in cellular, animal and epidemiological studies negates the HWE as an explanation for radiation hormesis. The LNT hypothesis is wrong and does not represent the true nature of the dose-response relationship, since low doses or dose-rates commonly result in thresholds and reduce cancer incidences below the spontaneous rate. Radiation protection organizations should seriously consider the cost and health implications of radiation hormesis.

Mesothelioma in Sweden: Dose-Response Analysis for Exposure to 29 Potential Occupational Carcinogenic Agents

  • Background: There is little information on the dose-response relationship between exposure to occupational carcinogenic agents and mesothelioma. This study aimed to investigate this association as well as the existence of agents other than asbestos that might cause mesothelioma. Methods: The Swedish component of the Nordic Occupational Cancer (NOCCA) study consists of 6.78 million individuals with detailed information on occupation. Mesothelioma diagnoses recorded in 1961-2009 were identified through linkage to the Swedish Cancer Registry. We determined cumulative exposure, time of first exposure, and maximum exposure intensity by linking data on occupation to the Swedish NOCCA job-exposure matrix, which includes 29 carcinogenic agents and corresponding exposure for 283 occupations. To assess the risk of mesothelioma, we used conditional logistic regression models to estimate hazard ratios and 95% confidence intervals. Results: 2,757 mesothelioma cases were identified in males, including 1,416 who were exposed to asbestos. Univariate analyses showed not only a significant excess risk for maximum exposure intensity, with a hazard ratio of 4.81 at exposure levels 1.25-2.0 fb/ml but also a clear dose-response effect for cumulative exposure with a 30-, 40-, and 50-year latency time. No convincing excess risk was revealed for any of the other carcinogenic agents included in the Swedish NOCCA job-exposure matrix. Conclusion: When considering asbestos exposure, past exposure, even for short periods, might be enough to cause mesothelioma of the pleura later in life.

노출지수를 이용한 요추 X선 촬영의 조사야 유효성 평가 (Effect of Field Size on the Clinical Exposure Index for Lumbar Spine X-ray Examination)

  • The field size of the lumbar spine X-ray examination, which belongs to the most frequent examination in general radiography, is 5 times wider than the width of the lumbar spine. Exposure index (EI) as per International Electrotechnical Commission has a proportional relationship with the dose incident on the image receptor for clinical protocols in addition to RQA5, which is a calibration beam quality. In this study, the effectiveness of the set field size was evaluated through the change of EI according to the size of field during lumbar spine X-ray examinations. Lumbar anterior-posterior and lateral examinations was performed using a whole-body phantom, and the national average exposure conditions of Korea investigated in 2017 were introduced for the X-ray exposure. As a result of comparing the EI displayed on the console of digital radiography system for the three field size in ① 18 × 36 cm2 ② 25 × 36 cm2 ③ 36 × 36 cm2, the EI values showed a tendency to increase as the field size increased. Since the patient dose, such as organ dose around the lumbar spine, increases as the field size becomes larger, thus, if the EI obtained from the field size at a level that does not interfere with diagnosis is set as a reference, the effectiveness of the field size can be evaluated through the EI displayed on the console when the lumbar spine X-ray examination is conducted.

  • Chemical mixtures of components, each of which are present at less than guidance concentrations, may be hazardous due to additivity, interactions, or both. Toxicological interactions may increase the health hazard above what would be expected from an assessment of each component singly, or all components additively. So chemical mixture are a particular issue in public health. There are several approach to assess whether there are additivity or interaction in assessing toxicological effects, such as, components-based approach, physiologically-based pharmacokinetic /pharmacodynamic(PBPK/PD) models, hazard index method, and weight-of evidence method. If we consider interaction or additivity effects in assessing the health effects of chemcial mixtures, we can get more accurate information about toxicological effects and dose-response relationship in chemical mixtures.

Effect of a Serial Irradiation of Low Dose Gamma Rays on the Growth and Photosynthesis of Red Pepper (Capsicum annuum L.) Plants

  • To reveal the relationship between the changes in the growth and photo- synthesis induced by low dose radiation, red pepper (Capsicum annuum L.) plants were serially irradiated three times with gamma rays of 0.5, 1, 2, 3, and 4 Gy. The plant growth was monitored by the fresh weight, the stem length, and the leaf length & width. All the irradiation groups (0.5-4 Gy) were stimulated in growth at 1 day after the $1^{st}$ irradiation (DA1I), but rather inhibited at 3 days after the $3^{rd}$ irradiation (DA3I). The maximum photochemical efficiency (Fv/Fm), the photochemical quenching (qP), the non-:photochemical quenching (NPQ) and the apparent rate of the photosynthetic electron transport (ETR) were used to represent the changes in the photosynthesis by the serial irradiation. The irradiation groups except 0.5 Gy had higher Fv/Fm values at 3 DA3I than the control one. After the 3$^{rd}$ irradiation, the qP values appeared to be a little lower in the 1-4 Gy groups than in the control and 0.5 Gy ones. In contrast, the NPQ values were rather higher in the irradiation groups except 0.5 Gy. During the whole experimental period, the ETRs decreased in the control group but remained relatively constant in the 4-Gy one. In conclusion, the results obtained indicate that the stimulatory effect of ionizing radiation on the plant growth was determined by the incident dose of the single irradiation rather than by the cumulative one of the serial irradiation. They also demonstrate that the growth stimulation induced by a low dose radiation could not be positively correlated with an alteration in the photosynthesis. Additionally, we discuss in text that an ionizing radiation may partly protect the leaf senescence by delaying the development of the plants.

대두 사포닌이 Aflatoxin B1으로 유도된 세포돌연변이에 미치는 영향에 관한 연구 (Effect of Soybean Saponins on Aflatoxin B1-induced Mutagenicity)

  • Free radicals formed during the metabolism of environmental chemicals are known to induce mutagenicity, while different types of antioxidants suppress this event. The purpose of this study was to determine the antioxidative and antimutagenic effects of soybean saponins, and to examine the relationship between these two effects for the elucidation of mechanisms involved in the anticarcinogenicity of soybean saponins. Also, antioxidative and antimutagenic effects of soybean saponins were compared with those of kinown antioxidants. For the measurement of antioxidative capacity, soybean saponins, L-ascorbic acid, $\alpha$-tocophoerol, and BHT at concentrations between 005 and 1.0mg/ml were tested for their ability to donate hydrogens and to reduce the formation of thiobarbituric substances(TBARS). Antimutagenic activity was examined using the Ames salmonella test system at concentrations of 600, 900 or 1200ug/ml. Study results showed soybean saponins and all of the other antioxidants tested possessed dose-dependent antioxidative activities. The ability of hydrogen-donation to DPPH was in the order of L-ascorbic acid>$\alpha$-tocopherol=>BHT>soybean saponins. TBARS formation was also inhibited by these compounds, in the order of BHT>$\alpha$-tocopherol=L-ascorbic acid>soybean saponins. Soybean saponins and other antioxidants also showed antimutagenicity in a dose-dependent manner. Especially, soybean saponins and BHT were excellent antioxidants compounds, inhibiting near 80% of the mutagenic effects at a concentration of 1200ug/ml. The correlation coefficients between antioxidative capacity and antimutagenicity for each compund was statistically significant at p<0.05. These results indicate that soybean saponins possess antioxidative and antimutagenic capacities. Also, antimutagenicity of saponins and other antioxidats is partly due to their antioxidative activities.

ICR 마우스의 태아(胎兒)에 대(對)한 방사선(放射線)과 초음파(超音波)의 공동효과(共同效果)에 관한 실험적(實驗的) 검토(檢討) (Combined Effects of Radiation and Ultrasound on Embryonic Development in Mice)

  • The combined effect of radiation and ultrasound has been studied in mouse embryos. Radiation and/or ultrasound were adminstered to ICR mice on day 8 of gestation. Intrauterine death, gross malformation, and fetal body weight were selected as indicators of effects. Does of whole-body ${\gamma}-irradiation$ were 0.5 to 2.5 Gy and those of ultrasound were $0.5\;W/cm^2$ to $3\;W/cm^2$. Intrautrine mortality increased with increasing radiation dose ; this trend was more remarkable in combination with ultrasound. Gross malformations such as exencephaly and anophthalmia/microphthalmia appeared frequently in the fetuses treated with both radiation and ultrasound. Decreased fetal weight was observed even in mice treated with 1.5 Gy of radiation or $1\;W/cm^2$ of ultrasound. There was a linear relationship between dose and reduction of fetal weight. The fetal weight was sensitive, precise and easy-to-handle indicator for the effects of growth retardation. Intrauterine mortality and frequencies of exencephaly and anophthalmia/microphthalmia were higher than the sum of those induced by radiation and by ultrasound. The results indicatied that the combined action of radiation and ultrasound on intrauterine death and malformations was synergistic.
