  • Objects : The daily dose of Pinelliae Tuber in ≪Treatise on Cold Damage Diseases≫ is half seung in volume, two and half ryang in weight, and fifteen in total number. But the daily dose should be a whole number. So I found out the background of this setting and correct solution. Methods : I searched Classics of Traditional Medicine, found out the background of the daily dose setting, solution. Results : The daily dose of Pinelliae tuber in ≪Hangdi's Internal Classic Miraculous Pivot≫, ≪Bohenggyuljangbuyongyakbeobyo≫ is half seung. ≪Treatise on Cold Dameage Diseases≫ followed the same daily dose of that because it referred to these books. In ≪Synopsis of Prescription of the Golden Chamber≫, the daily dose of that is half seung, one or two seung. The half seung of the Pinelliae Tuber is thirty three mL, but the diameter is 1~1.5 cm that accurate measurement by volume is difficult. The daily dose by weight is correct considering the unity of marking of the daily dose, accuracy of measurement, the fact that Pinelliae Tuber is currently distributed by cutting. So, two ryang is correct which is thirteen gram. Conclusions : Considering the traditionality, the convenience of measurement, the daily dose of Pinelliae Tuber in the ≪Treatise on Cold Damage Diseases≫ is half seung, but considering the unity, accuracy, current state of distribution, it is correct that the daily dose of it is two ryang. It corresponds to thirteen gram.

  • Radiation treatment for skin cancer has recently increased in tomotherapy. It was reported that required dose could be delivered with homogeneous dose distribution to the target without field matching using electron and photon beam. Therapeutic beam of tomotherapy, however, has several different physical characteristic and irradiation of helical beam is involved in the mechanically dynamic factors. Thus verification of skin dose is requisite using independent tools with additional verification method. Modified phantom for dose measurement was developed and skin dose verification was performed using inserted thermoluminescent dosimeters (TLDs) and GafChromic EBT films. As the homogeneous dose was delivered to the region including surface and 6 mm depth, measured dose using films showed about average 2% lower dose than calculated one in treatment planning system. Region indicating about 14% higher and lower absorbed dose was verified on measured dose distribution. Uniformity of dose distribution on films decreased as compared with that of calculated results. Dose variation affected by inhomogeneous material, Teflon, little showed. In regard to the measured dose and its distribution in tomotherapy, verification of skin dose through measurement is required before the radiation treatment for the target located at the curved surface or superficial depth.

  • To obtain site-specific values of the Derived Concentration Guideline Levels (DCGLs) for decommissioning of KRR-1&2, the soil density and distribution coefficient values for Cs-137, a major contaminant radionuclide, were determined. The soil density was evaluated according to the test method established by the Korean Agency for Technology and Standards of the Ministry of Trade, Industry, and Energy (KATS). The distribution coefficient was evaluated using a batch test. The validity of using the evaluated soil density and distribution coefficient as site-specific values was assessed through radiation dose assessment reflecting these values. Average soil density value obtained was 1.738 g/cm3, which was within the typical range of normal soil density, 1.0-1.8 g/cm3. The average distribution coefficient value was 7,754 mL/g. Applying the maximum, average, and minimum values of the evaluated soil density and distribution coefficient showed similar radiation dose results, thus suggesting that it is reasonable to use the average values of each parameter as site-specific values. Findings of this study can help determine DCGLs that reflect the characteristics of the research reactor site.

  • We investigated the influence of photon energy, couch and collimator angle differences between arcs on dose distribution of RapidArc treatment planning for prostate cancer. RapidArc plans were created for 6 MV and 10 MV photons using 2 arcs coplanar and noncoplanar fields. The collimator angle differences between two arcs were $0^{\circ}$, $15^{\circ}$, $30^{\circ}$, $45^{\circ}$, $60^{\circ}$, $75^{\circ}$ and $90^{\circ}$. The plans were optimized using same dose constrains for target and OAR (organ at risk). To evaluate the dose distribution, plans were analyzed using CI (conformity index), HI (homogeneity index), QOC (quality of coverage), etc. Photon energy, couch and collimator angle differences between arcs had a little influence on the target and OAR. The difference of dosimetric indices was less than 3.6% in the target and OAR. However, there was significant increase in the region exposed to low dose. The increase of V15% in the femur was 6.4% (left) and 5.5% (right) for the 6 MV treatment plan and 23.4% (left), 24.1% (right) for the noncoplanar plan. The increase of V10% in the Far Region distant from target was 54.2 cc for the 6 MV photon energy, 343.4 cc for the noncoplanar and 457.8 cc for the no collimator rotation between arcs.

  • This paper presents dose distributions in water around Fletcher-Suit colpostat containing $^{137}Cs$ tube, and shielding effect of Internal lead shield. Using ready packed film, author measured dose distribution in water around the colpostat containing cesium source. Nine sheets of films on one side of the colpostat are packed with acryl frames cut out so as to fill water, and irradiated in water by cesium source in the colpostat. Dose distributions on transverse plane and upper plane 0.5cm from upper surface of the colpostat were measured. Shielding effect was greater in upper medial direction than in lower medial direction. And that was the greatest around $30^{\circ}$ from the axis of the colpostat on upper side and around $50^{\circ}$on lower side. In the region 7cm from the center of the colpostat, shielding efficiency was 0.23 to 0.35 on the lower $50^{\circ}$ and 0.26 to 0.42 on the upper $30^{\circ}$, and decreased with increase of distance.

  • This study aims to analyze dose distribution and treatment time of endobronchial brachytherapy (EBBT) by changing the position step size of the dwell position. A solid water phantom and an intraluminal catheter were used in the treatment plan. The treatment plans were generated for 3, 5, 7, and 10 cm treatment lengths, respectively. For each treatment length, the source position step sizes were set as 2.5, 5, and 10 mm. Three reference points were set 1 cm away from the central axis of the catheter, along the axis, for uniform dose distribution. Volumetric dose distribution was calculated to evaluate the dosimetric effect. The total radiation delivery time and total dwell time were estimated for treatment efficiency, which were increased with position step sizes. At half-life time, the differences between the position step sizes in the total radiation delivery time were 18.1, 15.4, 18.0, and 24.0 s for 3, 5, 7, and 10 cm treatment lengths, respectively. The dose distributions were more homogenous by increasing the position step sizes. The dose difference of the reference point was less than 10%. In brachytherapy, this difference can be negligible. For EBBT, the treatment time is the key factor while considering the patient status. To reduce the total treatment time, EBBT can be performed with 2.5 mm position step size.

  • Purpose: To evaluate and compare the feasibilities of magnetic resonance (MR) image-based planning using synthetic computed tomography (sCT) versus CT (pCT)-based planning in helical tomotherapy for prostate cancer. Materials and Methods: A retrospective evaluation was performed in 16 patients with prostate cancer who had been treated with helical tomotherapy. MR images were acquired using a dedicated therapy sequence; sCT images were generated using magnetic resonance for calculating attenuation (MRCAT). The three-dimensional dose distribution according to sCT was recalculated using a previously optimized plan and was compared with the doses calculated using pCT. Results: The mean planning target volume doses calculated by sCT and pCT differed by 0.65% ± 1.11% (p = 0.03). Three-dimensional gamma analysis at a 2%/2 mm dose difference/distance to agreement yielded a pass rate of 0.976 (range, 0.658 to 0.986). Conclusion: The dose distribution results obtained using tomotherapy from MR-only simulations were in good agreement with the dose distribution results from simulation CT, with mean dose differences of less than 1% for target volume and normal organs in patients with prostate cancer.

  • Purpose: The purpose is to evaluate the accuracy of correcting the targeting error through the Target Location System (TLS) for the location change error of the reference point which arises from the movement or motion of patient during the treatment using the CyberKnife. Materials and Methods: In this test, Gafchromic MD-55 film was inserted into the head and neck phantom to analyze the accuracy of the targeting, and then the 6 MV X-ray of CyberKnife (CyberKnife Robotic Radiosurgery System G4, Accuray, US) was irradiated. End to End (E2E) program was used to analyze the accuracy of targeting, which is provided by Accuray Corporation. To compute the error of the targeting, the test was carried out with the films that were irradiated 12 times by maintaining the distance within the rage of $0{\pm}0.2\;mm$ toward x, y, z from the reference point and maintaining the angle within the rage of $0{\pm}0.2^{\circ}$ toward roll, pitch, yaw, and then with the films which were irradiated 6 times by applying intentional movement. And the correlation in the average value of the reference film and the test film were analyzed through independent samples t-test. In addition, the consistency of dose distribution through gamma-index method (dose difference: 3%) was quantified, compared, and analyzed by varying the distance to agreement (DTA) to 1 mm, 1.5 mm, 2 mm, respectively. Results: E2E test result indicated that the average error of the reference film was 0.405 mm and the standard deviation was 0.069 mm. The average error of the test film was 0.413 mm with the standard deviation of 0.121 mm. The result of independent sampling t-test for both averages showed that the significant probability was P=0.836 (confidence level: 95%). Besides, by comparing the consistency of dose distribution of DTA through 1 mm, 1.5 mm, 2 mm, it was found that the average dose distribution of axial film was 95.04%, 97.56%, 98.13%, respectively in 3,314 locations of the reference film, consistent with the average dose distribution of sagittal film that was 95.47%, 97.68%, 98.47%, respectively. By comparing with the test film, it was found that the average dose distribution of axial film was 96.38%, 97.57%, 98.04%, respectively, at 3,323 locations, consistent with the average dose distribution of sagittal film which was 95.50%, 97.87%, 98.36%, respectively. Conclusion: Robotic CyberKnife traces and complements in real time the error in the location change of the reference point caused by the motion or movement of patient during the treatment and provides the accuracy with the consistency of over 95% dose distribution and the targeting error below 1 mm.

  • In proton therapy, verification of proton dose distribution is important to treat cancer precisely and to enhance patients' safety. To verify proton dose distribution, in a previous study, our team incorporated a vertically-aligned one-dimensional array detection system. We measured 2D prompt-gamma distribution moving the developed detection system in the longitudinal direction and verified similarity between 2D prompt-gamma distribution and 2D proton dose distribution. In the present, we have developed two-dimension prompt-gamma measurement system consisted of a 2D parallel-hole collimator, 2D array-type NaI(Tl) scintillators, and multi-anode PMT (MA-PMT) to measure 2D prompt-gamma distribution in real time. The developed measurement system was tested with $^{22}Na$ (0.511 and 1.275 MeV) and $^{137}Cs$ (0.662 MeV) gamma sources, and the energy resolutions of 0.511, 0.662 and 1.275 MeV were $10.9%{\pm}0.23p%$, $9.8%{\pm}0.18p%$ and $6.4%{\pm}0.24p%$, respectively. Further, the energy resolution of the high gamma energy (3.416 MeV) of double escape peak from Am-Be source was $11.4%{\pm}3.6p%$. To estimate the performance of the developed measurement system, we measured 2D prompt-gamma distribution generated by PMMA phantom irradiated with 45 MeV proton beam of 0.5 nA. As a result of comparing a EBT film result, 2D prompt-gamma distribution measured for $9{\times}10^9$ protons is similar to 2D proton dose distribution. In addition, the 45 MeV estimated beam range by profile distribution of 2D prompt gamma distribution was $17.0{\pm}0.4mm$ and was intimately related with the proton beam range of 17.4 mm.
