Objectives : The purpose of this study was to show the effectiveness of Chuna Manual Therapy applied to a osteoporosis patient with gait disturbance due to lower limb fracture surgery. Method : We focused on Chuna therapy accompanied with acupuncture, herbal medicine, moxibustion, cupping treatment and rehabilitation exercise. This case was assessed using the Visual Analogue Scale (VAS), Pain Free Walking Distance (PFWD), Berg Balance Scale (BBS), and Modified Barthel Index (MBI). Result : This patient showed improvement on the VAS from 6 to 3, PFWD from 0 m to 2000 m using a walker and 50 m by self-walking, BBS from 2 to 28, and MBI from 56 to 89. Moreover the walking state improved from gait disturbance to self-walking. Conclusion : This study suggests that Chuna therapy could be used to improve gait disturbance in patients with osteoporosis and lower limb fracture surgery.

In this study, an optimal improvement method of stream channel structure is presented for the enhancement of fish habitat suitability by genetic algorithm. The correlation between fish habitat suitability and physical disturbance in stream is analyzed according to the changes of hydraulic characteristics by channel structure rehabilitation. Zacco koreanus which is an indicator fish of the soundness of aquatic ecosystem was selected as a restoration target species by investigating the community characteristics of fish fauna and river environments in Wonju stream. The habitat suitability is investigated by PHABSIM with the habitat suitability index of Zacco koreanus. Hydraulic analysis by HEC-RAS and physical disturbance evaluation in stream are carried out. The optimal channel width modified for the enhancement of fish habitat suitability is provided. The correlation analysis between habitat suitability and physical disturbance with the change of hydraulic characteristics by channel modification showed that the proper channel modification enhanced fish habitat suitability and mitigated physical disturbance in the stream. The improvement of physical disturbance score by the channel structure rehabilitation for the enhancement of fish habitat suitability was confirmed in this study.

Purpose: The purpose of this study was to examine symptoms, mood and sleep disturbance in patients with hemodialysis. Method: A sample of 96 hemodialysis patients participated. Data were collected using symptom scale of hemodialysis patients, profile of mood state and the sleep scale. Statistical analytic methods included t-tes, ANOVA and Pearson correlation analyses. Results: Patients with spouse/partners experienced significantly less symptoms than those with no spouse (F=6.29, p=.003). Mood disturbance was not significantly correlated with age; but older patients experienced higher sleep disturbance (F=4.88, p=.010). Symptoms, mood and sleep disturbance are significantly related with each other. Conclusion: It is important to assess symptoms, mood and sleep disturbance in older hemodialysis patients with no spouse. It is needed to repeat extensive study and comparative study with other population in order to define clearly.

Piriformis muscle syndrome is a condition that causes direct muscle pain around piriformis muscle or sciatica from irritated sciatic nerve and the diagnosis remains debatable. The main treatment is symptomatic relief from conservative therapy such as medication and piriformis stretching exercise, and various therapeutic injections including local anesthetic, corticosteroid, botulinum toxin can be considered for diagnostic and therapeutic purposes. In this case, a 54-year-old male who had sciatica and gait disturbance showed piriformis muscle hypertrophy in the pelvis MRI. From imaging studies, electrodiagnostic study and physical examination, he was diagnosed with piriformis muscle syndrome. He underwent trigger point injection and botulinum toxin injection into the piriformis muscle, and pain and gait disturbance significantly improved. This case reports a case of piriformis muscle syndrome with clinical symptom of gait disturbance, which was improved by botulinum toxin injection.

The purpose of this study is to report the effect of oriental medical treatments with exercise therapy on the frail elderly patient with gait disturbance and cognitive decline. The frail elderly patient with gait disturbance and cognitive decline was treated by oriental medical treatments combined with exercise therapy during 3 months. The improvement of clinical symptom was evaluated by Manual Muscle Test (MMT), Passive Range Of Motion (PROM), Mini Mental State Examination-Korea (MMSE-K). After 3 months, all of both lower extremity Manual Muscle Test (MMT) improved to Grade 4. Both knee extension Passive Range Of Motion (PROM) and Mini Mental State Examination-Korea (MMSE-K) also increased from -30/-30 to 0/-5, from 7 to 25 respectively. It could be suggested that oriental medical treatments with exercise therapy were effective on the frail elderly patient with gait disturbance and cognitive decline. And further research is encouraged to confirm the effectiveness of this treatment with a larger number of patients.

Purpose: The purpose of this study is to find out the effects of the aroma massage on shoulder pain, depression, and sleep disturbance in hemiparesis patients. Method: The research design was a nonequivalent control group nonsynchronized design. The data was collected from July 1 to August 31, 2003 at D medical Center in Busan. The subjects were 37 patients that they were assigned to two groups, nineteen subjects in the experimental group and seventeen subjects in the control group. The oil made from a blend of lavendar, clarysage, bergamot at a ratio 3:1:2 used for relieving shoulder pain, depression, sleep disturbance. The instruments used for this study were Visual Analogue Scale(VAS) for shoulder pain, Zung' depression scale, 4 score's scale for sleep disturbance. The data were analyzed using SPSS and hypotheses were examined with ANCOVA. Results: Shoulder pain, depression, and sleep disturbance were significantly lower in the experimental group compared than the control group. Conclusion: The aroma massage using selected essential oils is an effective intervention for relieving shoulder pain, depression, sleep disturbance in hemiparesis patients. However differences between aroma massage and massage were not clear, further study needs to be done.

The study reports the clinical case of a patient with lower extremity weakness and gait disturbance treated with Korean medicine treatments including Chuna therapy. The patient suffered lower extremity weakness and gait disturbance with diagnosis of lumbar disk herniation and spinal stenosis. As a treatment, the doctor applied Chuna therapy, herbal medicine, acupuncture, pharmacopuncture, cupping and moxa. The effect of treatment was evaluated by numeral rating scale (NRS), self-walkable distance, functional independence measure (FIM), Oswestry disability index (ODI) and manual muscle test (MMT). NRS decreased from 6 to 4 at the leg. Self-walkable distance increased from 0 m to 10 m, FIM increased 85 to 96 points while ODI decreased 64% to 54%. MMT of hip flex and knee extension improved from grade 3+, grade 3 to grade 4, respectively. Korean medicine treatment can be effective for patients who suffer lower extremity weakness and gait disturbance due to lumbar disk herniation and spinal stenosis. Further clinical studies are required to verify these findings.

Purpose: This study was to evaluate the effects of aromatherapy and foot reflex massage on emotion, sleep disturbance and wandering behavior in older adults with dementia. Methods: This is a quasi-experimental study with a non-equivalent control group. A total of 39 participants were divided into two groups. Aromatherapy and foot reflex massage was administrated to experimental group (N=19). There was no treatment for the control group (N=20). Data were analyzed using the SPSS/Win 18.0 program. Results: Experimental group showed significant differences in emotion, sleep disturbance and wandering behavior in comparison to the control group. Conclusion: The results showed that aromatherapy and foot reflex massage program was effective on emotion, sleep and decreasing a wandering behavior in older adults with dementia. This research suggests that aromatherapy and foot reflex massage could be recommended as a nursing intervention for enhancing quality of life in older adults with dementia.

l. Objectives To analyse gait disturbance of patient who was operated Total Hip Replacement Arthroplasty(THRA). 'This study is reported to emphasize on psychological stability and balance of vital dynamics. 2. Methods This patient was treated by the remedy of Soyangin, who was diagnosed as Soyangin. We used visual analogue scale(V AS) for the assessment of Rt. femoral pain. 3. Results The patients operated THRA are needed not only rehabilitation of gait, but also psychological stability and balance of vital dynamics. 4. Conclusions We suggest that patients operated THRA are needed not only rehabilitation of gait, but also psychological stability and balance of vital dynamics.

Purpose: The purpose of this study was to compare the effects of hand massage provided with different intervals and periods on pain and sleep disturbance after orthopedic surgery. Methods: A non-equivalent control group pretest- posttest design was used. The subjects were admitted in an orthopedic hospital to get a surgery. They were divided into three groups. Group I (n=30) had hand massage every day. Group II (n=30) had hand massage every other day. Control group (n=31) had usual care. Data of all three groups were collected on the day before operation, POD (postoperative day) 6 and POD12. Hand massage was given for 2 and half minutes per hand. Results: Pain on POD6 of experimental group II was reduced more than those of control group. Pains on POD12 of both experimental groups were reduced more than those of control group. On POD6, only perceived sleep disturbance (PSD) was significantly different among groups. On POD12, PSD, total sleeping time, and sleep efficiency were more improved in the experimental groups. Conclusion: Hand massage was effective on the reduction of pain and sleep disturbance after orthopedic surgery. Applying hand massage on alternate day was effective enough. Also the effects were more obvious after 12 days.
