수종(數種) 근관소독제(根管消毒劑) 및 근관충전재(根管充塡材)의 세포독성(細胞毒性)에 관(關)한 실험적(實驗的) 연구(硏究) (Evaluation of the Cytotoxicity of Root Canal Disinfectants and Root Canal Sealers on L Cells in Vitro)

  • This study was to evaluate the cytotoxic effect of three root canal disinfectants (formocresol, camphorated phenol and eugenol) and ten root canal sealers(Cavitec, Hypo-cal, Vitapex, AH26, Canals, Mynol, $N_2$, $N_2$-Medical, Z. O. E. and Calvital) in vitro. The experiments were performed in four differrent modes. In the first and second experiment, the "long-distance" cytotoxicity of three root canal disinfectants were tested on L cells. In the third exeriment, ten root canal sealers were tested for cytotoxicity by means of the tissue culture-agar overlay method immediately, 4 and 24 hours after the experiment. In the fourth experiment, the study with radioactively labeled L cells were employed to determine the relative cytotoxicity of ten root canal sealers. The results were as follows; 1. Every vapors from disinfectants showed more or less cytotoxicity. Of the three disinfectats, formocresol appeared to be the highest cytotoxic effect and camphorated phenol was the lowest. 2. Root canal sealers tested in tbis study showed cytotoxicity at every stage of time intervals. 3. The highest cytotoxic effect was freshly mixed $N_2$ meaical and $N_2$ also revealed the highest cytotoxic effect after 4 or 24 hours among these materials. Vitapex was found the lowest cytotoxic effect at all experimental stage. 4. Root canal sealers except N2 and Mynol showed cytotoxic effect were decreased cytotoxicity according to the time elapsed.

제주도 손바닥선인장 추출물의 표면소독제로써의 항균효과 (Effect of Antimicrobiotic of Opuntia ficus-indica on Surface Disinfection)

  • Purpose: To verify the anti-microbiotic activity and to develop eco-friendly surface disinfectants using the Opuntia ficus-indica extract. Methods: The stem and fruits of the Opuntia cultivated in Jeju Island were extracted with hot water, 70% EtOH and 100% MeOH, respectively. To examine the antimicrobial activity of the extracts, gram positive bacteria (Bacillus subtilis, Staphylococcus aureus), gram negative bacteria (Escherichia coli, Salmonella typhimurium), yeast (Candida albicans), mold (Aspergillus flavus) and antibiotic resistant bacteria (E. coli, S. aureus) were measured to form a clear zone. Results: The antibacterial activity of the fruit extract was higher than that of the stem extract, and the antibacterial activity of the 100% MeOH (FM) extract and the 70% EtOH (FE) extract of the palm cactus fruit was suitable for the surface disinfectant overall, and clear-zone formed even at relatively low concentrations (250 mg/mL). The FM and FE have antibacterial activity against various bacteria, therefore they can be used as disinfectants. Conclusion: The antimicrobial activity of Opuntia ficus-indica extract from Jeju Island by stem and fruit was determined at relatively low concentrations (250 mg/mL). The Opuntia ficus-indica extract will be useful for surface disinfectants dissolving the patients' bath, oral gargling, and powdered powder in water or ethanol in appropriate concentrations.

코로나바이러스감염증-19와 일부 가축전염병 방역소독제품의 함유성분 및 유해물질 조사 (Investigation of Ingredients and Hazardous Substances in Disinfectants Used against COVID-19 and Some Livestock Diseases)

  • Objectives: The Coronavirus Disease 2019 (COVID-19) pandemic has caused the death of 740,000 people around the world as of August 12, 2020. Foot-and-Mouth Disease, Avian Influenza, and African Swine Fever are serious livestock diseases. Government agencies in Korea have provided ingredient information and usage instructions for disinfectants used to counter those infectious diseases. The purpose of this study was to provide information on the chemical ingredients in disinfectant products used against COVID-19 and certain livestock diseases. Methods: We collected information from the Korean government. The Central Disaster Management Headquarters and Central Disease Control Headquarters provided information on disinfectant products used against COVID-19. The Animal and Plant Quarantine Agency of Korea provided information on efficacy-certified disinfectant products for use against selected livestock diseases. Health hazard and environmental hazard information on the ingredients in the disinfectants was collected from the Korea Occupational Safety & Health Agency's Material Safety Data Sheets, and toxicity value information was collected from United States Environmental Protection Agency's CompTox Chemicals Dashboard. Results: There were 76 COVID-19 disinfectant products in use, and the most common ingredients were benzalkonium chloride (51%), alkylbenzyl dimethyl ammonium (30%), and ethanol (3%). There were 216 livestock disease disinfectant products comprised of 89 acidic, 88 oxidic, 30 aldehydic, three alkaline, and six other products. Among the 49 active ingredients used in the disinfectants that were investigated, health and environmental hazard information was provided for many of them, but only 20 chemicals had official toxicological information. Conclusion: Since the disinfectants included numerous chemicals, an understanding of their chemical characteristics could be critical to prevent unintended human or environmental exposure.

  • 유통식품에서 분리한 E. coli 및 S. aureus의 차아염소산나트륨과 염화벤잘코늄에 대한 감수성을 현탁액시험법으로 청정조건과 오염조건에서 각각 조사한 결과 차아염소산나트륨의 오염조건에서 S. aureus가 5 log 감소의 살균소독력이 나타나지 않았을 뿐, 모든 균주가 사용범위 내에서 살균소독력을 나타내었으며, 항생제 내성균과 감수성균에 대한 살균소독제 감수성의 차이는 발견되지 않았다(p>0.05).

  • Yeung Nam University Medical Center, Department Of Central medical Supply Background: The adverse effect of intravenous therapy combined with various complications. Because sterilization technique and appropriate nursing care can prevent various complications, it is important to use appropriate sponge in intravenous therapy. The purpose of study was to identy sterilization effect and detection of bacteria and to provide basic data for use of appropriate disinfectants. Methods: From May 15 1995 to Aug. 3. 1995, disinfectants that were used in Yeung Nam University Medical Center were tested by bacteria culture. To test sterilization effect of disinfectants of intravenous injection sites after disinfection, 10 subjects were used and were tested by bacteria culture for the study. Results: 1) By sterilization effect of disinfectants, bacteria were increased from 103 to 10 from 48 hours in both 2% Zephanon and 2% zephanon that was sterilized by steam, from 10 to 10 from 72 hours and 10 from 48 hours in 70% lsoprophyl alchol. Also, bacterias were detected in 70% lsoprophyl alchol on 48 hours. 2) By stenlization effect of sponge that were used in nursing unit, bacterias were detected in 2% Zephanon on 2 hours, 70% lsoprophyl alchol on 2 hours, 70% lsoprophyl alchol on 8 hours and 70% lsoprophyl alchol on 48 hours. 3) By sterilization effect and detection of bacteria of intravenous site after disinfection, bacterias were detected in 10 of 10 control groups, 8 of 10 sites that were disinfected by steam sterilized 2% zephanon sponge, 6 of 10 sites that were disinfected by 70% lsoprophyl alchol and 4 of 10 sites that were disinfected by 10% Batadine. Conclusions : it is conclued that 70% lsoprophyl alchol is appropriate for intravenous therapy and 10% Betadine is appropriate in ward that were polluted the air and in immunodeficient patients.

Synergistic antibacterial effect of disinfectants and microbubble water to Salmonella Typhimurium

  • Salmonella is a pathogenic bacterium that has long been important industrially because it has a wide host range and can be transmitted to humans through direct contact as well as indirect contact such as food contaminated with animal waste. Understanding how to reduce Salmonella contamination in pig farms is important for public health and the livestock industry from an economic perspective. In the swine industry, high concentrations of disinfectants have been applied because it is difficult to effectively control Salmonella in environments contaminated with organic substances. In order to evaluate the synergetic effect of disinfectants, the efficacy of two commercial disinfectants diluted in hard water and microbubble water (MBW) were compared under the laboratory condition. Different concentrations of both disinfectants combined with 1% detergent diluted in the two diluents were evaluated for their antibacterial effect. In the case of monopersulfate-based disinfectant groups, the growth of Salmonella was not observed at 1:200 dilution with both the hard water and MBW combined with 1% detergent. In the case of citric acid-based disinfectant, the bacterial growth was not observed at 1:800 dilution with MBW combined with 1% detergent. Our results show that the use of MBW as a diluent might improve the biological activities of acid-based disinfectant.

  • Once released into the air, humidifier disinfectants became tiny nano-size particles, and resulted in chemical bronchoalveolitis. Families had lost their most beloved members, and even some of them became broken. Based on an estimate of two million potential victims who had experienced adverse effects from the use of humidifier disinfectants, we can say that what we have observed was only the tip of the iceberg. Problems of entire airways, as well as other systemic effects, should be examined, as we know these nano-size particles can irritate cell membranes and migrate into systemic circulation. The story of humidifier disinfectant is not finished yet.

가축방역용 소독제 처리가 액비내 미생물 변화에 미치는 영향 (Effect of the Disinfectants on Microbes Changes in Piggery Slurry)

  • 1. 돈슬러리의 초기 pH는 8.2로 약알칼리를 띠고 있었으며, 수분 함량은 $98.6\%$, 유기물 함량은 $0.4\%$였다. 돈슬러리내에 존재하는 총미생물수는 $6.3\times10^5cfu/ml$로 조사되었으며 Escherichia coli, Staphylococcus aureus 및 Bacillus cereus는 각각 $1.2\times10^4,\;4.1\times10^4,\;1.7\times10^4 cfu/ml,$였다. 2. 소독제 종류, 살포농도에 따른 처리별 수분, 유기물 함량은 큰 변화를 보이지 않았으며, pH는 기간이 경과함에 따라 높아지는 경향이었다. 3. 처리구별 초기 총미생물수는 $2.2\times10^5,\;9.9\times10^5\;cfu/ml,$의 분포를 나타냈으며, 소독제 살포후에는 모든 처리구에서 총미생물수가 점차적으로 감소하기 시작하여 2주째에 가장 낮은 수준인 $0.1\times10^5\~0.8\times10^5\;cfu/ml$으로 조사되었고 14일후에는 모든 처리구에서 다시 증가하는 경향을 나타내었다. 4. 병원성미생물인 Escherichia coli와 Staphylococcus aureus는 초기에는 각각 $1.0\times10^4\~4.8\times10^4,\;3.2\times10^4\~9.9\times10^4\;cfu/ml$의 분포를 나타내었으나, 소독제 살포당일부터 급격하게 감소하기 시작하여 살포 7일후에는 모든 처리 구에서 사멸되는 것으로 조사되었으며 살포 30일 후부터 서서히 증가하는 경향을 보였다. 5. 돈슬러리내 미생물중 부숙에 관여하는 미생물인 Bacillus cereus의 초기 미생물수는 $0.7\times10^4\~5.8\times10^4\;cfu/ml$였으며 소독제 살포당일에는 뚜렷한 감소효과를 보이지 않았으나 살포 3일 후부터 서서히 감소하기 시작하여 살포 14일 후까지 초기 수준에 비해 $8.6\~85.75\%$까지 감소하다가 그 이후부터는 서서히 증가하기 시작하여 시험초기와 비슷한 수준을 보였다. 6. 소독제 살포가 초기에는 돈슬러리 부숙에 약간의 영향을 미칠 수는 있으나, 기간이 경과함에 따라 그러한 영향은 거의 없어지는 것으로 나타났다.

화학소독제 처리 후 가타파차 콘의 멸균 효과 및 표면 성상의 변화 평가 (Assessment of decontamination of gutta-percha cone and the change of surface texture after rapid chemical disinfection)

  • 본 연구의 목적은 첫째 임상에서 진료실에 노출된 가타파차 콘 표면의 오염 균종을 중합효소연쇄반응법 (polymerase chain reaction, PCR)을 이용해 동정하고, 둘째 이들 세균으로 오염시킨 가타파차 콘에 대해 2종의 소독제의 rapid sterilization 효과를 비교하였다. 또한 이들 소독제에 5분간 처리된 가타파차 콘 표면 성상의 변화를 주사전자현미경으로 관찰하였다. 진료실에 수 개월간 노출된 가타파차 콘 100개를 수거하여 배양지에 넣어 배양 후 universial primer를 사용한 PCR assay 를 통해 오염 균종을 동정하였다. 실험실 상에서 이 균종을 다시 배양하여 소독된 가타파차 콘에 접종하고 1주일간 배양한 후 2종의 소독제(5% NaOCl, 2% Chlorhexidine)에 1, 3, 5, 10 분간 담근 후 각 소독제의 종류와 적용시간에 따른 멸균 효과를 turbidity test 와 subculture 를 이용하여 평가하였다. 또한 각 소독제에 5분간 처리된 가타파차 콘 표면 성상의 변화를 주사전자현미경으로 관찰하였다. 중합효소연쇄반응법의 분석결과 17개의 가타 파차 콘이 오염된 것으로 나타났고 대부분이 Staphylococcus 계통이었으며, 2종의 소독제 모두 이들 균종에 대해 1분내에 멸균 효과를 나타냈다. 주사전자현미경상 NaOCl로 소독된 가타파차 콘 표면에는 cuboidal crystal 의 침전물이 전반적으로 관찰되었다. 본 연구 결과 2종의 소독제 모두 근관충전 전 가타파차 콘의 rapid sterilization 을 위해 유용하였으나 클로헥시딘으로 처리된 가타파차 콘이 크리스탈 침전물이 없는 좀더 깨끗한 표면을 갖는 것으로 나타났다.

가습기 살균제 노출 실태와 피해규모 추산 (An Estimation of Population at Risk of Exposure to Humidifier Disinfectant and Associated Health Effects)

  • Objective: This study was designed to estimate the precise nationwide number of those who used humidifier disinfectants (HDs) and experienced specific health effects, including death, due to exposure to HDs in Korea between 1994 and 2011. Methods: A total of 5,000 households (15,472 people) were surveyed by stratifying the sample with a design that is equivalent to nationally approved household systematic sampling and face-to-face interviews from October 16, 2019 to December 30, 2019. Results: The proportion of use of humidifier disinfectants at home was 18.4% (2,844/15,472 people) and the proportion of those who experienced health effects among those who were exposed to HDs was 10.7% (303/2,844 people). Based on these factors, it was estimated that 8.94 million people (95% CI=8.25-9.63 million) were exposed to HDs and 0.95 million people (95% CI=0.87-1.02 million) experienced health effects, which means almost one in five and one in fifty Koreans respectively. The estimated number of deaths from HDs were 20,366 people (95% CI=18,801-21,931). Conclusion: The survey was large and analyzed a representative sample across the country, so this estimation of the nationwide population exposed and at risk to HDs is more accurate and reliable than previous studies. However, it is necessary to confirm whether results similar to those of this study can be reproduced through a survey using a cross-sectional survey at the national level. In particular, a survey focused on death cases is needed.
