System Analysis of Disease Classification of Oriental Medicine Diagnosis and Study for Improvement Method (한방진단명의 질병분류체계 분석과 개선방안 연구)

  • Background : To examine the difference between ICD-10 and The Korean standard classification of disease(oriental medicine), and to aim at improve the practical use as statistical data. It is one of the reason of disease classification. On that account we convert the many to many correspondence presenting classification of oriental medicine into many to one correspondence. Method : The study tracked out 155 patients discharged from the university hospital which is located in Gyeonggi Province and managing hospital and oriental medicine hospital from July to October this year. The period of this study was from August 1 to November 18. We compared correspondence between the two services' diagnosis(hospital services and oriental medicine hospital services) at the same time and attempted many to one correspondence classification. That is for production of statistical data. Result : We investigated the group which have had medical treatment experience of two kinds of services at the same time. The result of this investigation was that the same oriental medicine diagnosis used differently in western medicine diagnosis. 44.5% was accorded with western medicine diagnosis. Correspondence of the western medicine diagnose with the top of the Korean standard classification of disease(oriental medicine) list's western medicine diagnosis was 13.5%. For many to one correspondence classification for statistics, one western medicine diagnosis was selected for one oriental medicine diagnosis. In case of the main diagnosis(I sign) was not enough to explain oriental medicine diagnosis' characteristic, we chose multiple other diagnosis, so other diagnosis(II sign) about patient's cause of disease could be selected for supplement after we examined the patient's records. The statistics was possible with this many to one correspondence. Conclusion : The result of this study about correspondence between western medicine diagnoses and those of oriental medicine confirms that The Korean standard classification of disease(oriental medicine) is hard to be standardized with western medicine diagnosis. Therefore, according to this study, we use new many to one correspondence classification, multiple oriental medicine diagnoses with one ICD-10, which can be used by statistical data.

  • Children with abnormal liver function can often be seen in outpatient clinics or inpatients wards. Most of them have respiratory disease, or gastroenteritis by virus infection, accompanying fever. Occasionally, hepatitis by the viruses causing systemic infection may occur, and screening tests are required. In patients with jaundice, the tests for differential diagnosis and appropriate treatment are important. In the case of a child with hepatitis B virus infection vertically from a hepatitis B surface antigen positive mother, the importance of the recognition of immune clearance can't be overstressed, for the decision of time to begin treatment. Early diagnosis changes the fate of a child with Wilson disease. So, screening test for the disease should not be omitted. Non-alcoholic fatty liver disease, which is mainly discovered in obese children, is a new strong candidate triggering abnormal liver function. Muscular dystrophy is a representative disease mimicking liver dysfunction. Although muscular dystrophy is a progressive disorder, and early diagnosis can't change the fate of patients, it will be better to avoid parent's blame for delayed diagnosis.

  • Several authors suggested that the clinical characteristics of incomplete presentation of Kawasaki disease are similar to those of complete presentation and that the 2 forms of presentation are not separate entities. Based on this suggestion, a diagnosis of incomplete Kawasaki disease in analogy to the findings of complete presentation is reasonable. Currently, the diagnosis of incomplete Kawasaki disease might be made in cases with fewer classical diagnostic criteria and with several compatible clinical, laboratory or echocardiographic findings on the exclusion of other febrile illness. Definition of incomplete presentation in which coronary artery abnormalities are included as a necessary condition, is restrictive and specific. The validity of the diagnostic criteria of incomplete presentation by the American Heart Association should be thoroughly tested in the immediate future.

  • While the population of pet dogs and pet-related markets are increasing, there is no convenient and reliable tool for pet health monitoring for pet owners/caregivers. In this paper, we propose a mobile platform-based pre-diagnosis system that pet owners can use for pre-diagnosis and obtaining information on coping strategies based on their observations of the pet dog's abnormal behavior. The proposed system constructs symptom-disease association databases for 100 frequently observed diseases under veterinarian guidance. Then, we apply the possibilistic fuzzy C-means algorithm to form the "probable disease" set and the "doubtable disease" set from the database. In the experiment, we found that the proposed system found almost all diseases correctly, with an average of 4.5 input symptoms and outputs 1.5 probable and one doubtable disease on average. The utility of this system is to alert the owner's attention to the pet dog's abnormal behavior and obtain an appropriate coping strategy before consult a veterinarian.

  • The diagnostic requirements were suggested and explained regarding the systems of differentiation of symptoms and signs in the third year study of standardization and unification of the terms and conditions used for diagnosis in oriental medicine. The systems were as follows : - analysing and differentiating of epidemic febrile disease - analysing and differentiating in accordance with the Sasang constitutional medicine baced on four-type recognition - differentiation of diease according to pathological changes of Chong and Ren channels - standards for diagnosis of women's disease - standards for diagnosis of children's disease - standards for diagnosis of motor and sensor disturbance(-muscle born joint etc.) - standards for diagnosis of neuropsychiatric diease - standards for diagnosis of five sense organ diease - standards for diagnosis of external disease. Correlation of Ultrasound and Scintigraphic Findings in Scrotal Disease (음낭 질환의 초음파 및 씬티그라피에 관한 방사선학적 연구)

  • Recently ultrasound and scintigraphic study are most useful modality for diagnosis of scrotal diseases. They are able to determine the nature, anatomic extent of the lesion, and possible to differential diagnosis of the scrotal lesion. The ultrasound and scintigraphic findings were analyses in 59 patients of confirmed scrotal disease at Seoul Red Cross Hospital from June 1986 to Aug. 1989. The results were as follows. 1) Most patients were below the 30 years old age (69.4%), and most prevalent age was $11\sim30$ Yrs. in trauma, $31\sim40$ Yrs. in inflammation, and $51\sim60$ Yrs. in mass lesion of scrotum. 2) The diagnostic accuracy of scrotal disease was 94.9% in ultrasound and 72% in scintigraphic study. The ultrasound study showed high accuracy in all kind of scrotat disease, but scintigraphy showed only high detection rate in inflammatory disease. 3) Scrotal ultrasound is first choice of study for the diagnosis of scrotal lesion, and useful determine the cystic or solid lesion and anatomic extent of disease. However it can't be differentiate the acute epididymitis with acute testicular torsion. 4) Scrotal scintigraphy is useful study for the diagnosis of acute painful scrotal disease, and showed specific findings in acute epididymitis and acute torsion for differential diagnosis. 5) In consideration of age, history and clinical findings of patient, appropriate study will be chosen firstly in ultrasound and scintigraphy, and combined all of findings of these studies will be helpful for the accurate diagnosis and adequate treatment of scrotal disease.

  • In this paper, we proposed a methodology for portable disease diagnosis platform using high performance computing. The proposed methodology consists of gathering clinical data, diagnosis and feature selection algorithm, implementation of diagnosis platform. For the algorithm verification, a clinical data which is obtained from 401 people(314 normal subjects and 87 liver cancer patients) using a microarray consists of 1,146 aptamers were used. As the result, we could diagnosis liver cancer with 97.5% accuracy using the 32 selected aptamers. Based on these results, we designed and implemented a portable disease diagnosis platform which has 32 bio-signals as inputs.

A Study on the Reliability of Oriental OB & GY Diagnosis Questionnaires (한방부인과 진단설문지의 신뢰도연구)

  • Objective: This study was undertaken to examine the reliability of disease mechanism diagnosis, estimate the disease mechanism items of questionnaires and inquire about the relation of disease mechanisms to the oriental OB & GY diagnosis program. Questionnaires and abridged questionnaires were used for the object diagnosis of oriental medicine in the dept of Oriental OB & GY, Oriental Medical Hospital of Dong-Eui University. Methods: We analyzed the results of questionnaires about 1706 outpatients who had OB & GY disease in the Oriental Medical Hospital of Dong-Eui University from April 2000 to March 2004. Results: 1. The reliability of Oriental OB & GY questionnaires between $90\%$ and $95\%$ were 9 cases, between $85\%$ and $90\%$ were 3 cases, and under $85\%$were 3 cases. Abridged questionnaires were lower than original questionnaires, but 12 cases of a total 15 cases of disease mechanism were above $85\%$. Therefore, both abridged questionnaires and original questionnaires had similar results. 2. Abridged questionnaires were usually lower than existing questionnaires in the comparison of disease mechanism output frequency and that of disease mechanism average score. Therefore, the results of abridged questionnaires seemed to be poorer than those of existing questionnaires, but a great difference wasn't seen. 3. Disease mechanism that was over $50\%$ in the rate of pure question per disease mechanism was 10 cases $(66.7\%)$. Disease mechanism that contributed to producing disease mechanism result and in which pure question was over relevance calculation 0.9 was also 10 cases $(66.7\%)$. In abridged questionnaires, the duplication of questions per disease mechanism Was decreased, the rate of pure questions was increased, and the number of related disease mechanisms was decreased by abridgment of the questionnaires' questions. 4. The calculation of disease mechanism went with the increase of the duplication of questions in many cases, but Tam­Umhe, Kihe-Hyule, Kihe-Umhe, and Shin-Tam went with disease mechanism in many cases despite no duplication of questions. Conclusions: About the reliability of Oriental OB & GY questionnaires, 12 of a total 15 cases of disease mechanism were above $85\%$; therefore both abridged questionnaires and original questionnaires had similar results.

  • Objectives : This study examined the reliability of disease mechanism diagnosis, to evaluate items of questionnaires and inquire about the relationships between disease mechanisms and 'diagnosis program' questionnaires used for the objective diagnosis of Oriental medicine in the department of Oriental OB&GYN, Oriental Medical Hospital of Dongeui University. Method : We analyzed the results of questionnaires from 3504 outpatients of OB&GYN disease at the Oriental Medical Hospital of Dongeui University from April 2000 to April 2005. Results & Conclusions : 1. The research questionnaire had 188 questions, the summary questionnaire 137, and the diagnosis questionnaire 80. 2. The reliability of all questionnaires shows above 90% in deficiency of qi, deficiency of Yin, insufficiency of Yang coldness heat syndrome liver and spleen kidney in all, 8 case disease mechanisms. These are higher in the diagnosis questionnaires than in the research questionnaires and the summary questionnaires, except for kidney disease mechanism. 3. Cronbach a of the questionnaires decreased, especially blood deficiency, phlegm, heat syndrome, and insufficiency of Yang; these 4 case disease mechanisms were lower than 0.6. 4. For degree of correspondence of meeting points, both. the diagnosis and the summary questionnaires were above 80% with the exception of the 2 case disease mechanisms heart and blood deficiency. The meeting points of both the diagnosis and research questionnaires were above 80% in the to case disease mechanisms deficiency of qi blood stasis deficiency of Yim insufficiency of Yang damp dryness liver spleen kidney phlegm. 5. The change in the result values of questionnaires was a decreased level of deficiency of qi heat syndrome phlegm damp kidney and raised level of coldness heart disorder of qi dryness 6. The computation degree of disease mechanism in DSOM(r) D.1.1 was much lower on phlegm deficiency of qi heat syndrome disorder of blood, somewhat lower on insufficiency of Yang and higher on coldness than in the two different questionnaires.

  • Onbyeong(溫病) is called an acute epidemic febrile disease caused by warm pathogen, a major symptom of Onbyeong is high fever. Doctrine of Onbyeong is a study of an occurrence, progress and treatments of an acute epidemic febrile disease. Doctrine of Onbyeong is valid in the Cheong Dynasty at China. Now, a theory of doctrine of Onbyeong at China and Korea is being applied in not only an acute febrile disease but also many other lifestyle diseases. Onbyeongjobyeon is a book written by Oguktong(吳鞠通). Oguktong was influenced by Jangjung-gyeong(張仲景) "Sanghanron(傷寒論)". Oguktong had organized Seopcheonsa(葉天士)'s medical thoughts and Oguktong's medical experiences. A Samcho(三焦) deteriorated case is divided into three groups - Upper, Middle and Lower-energizer - that is discussed of a vertical progress of Onbyeong. And a Wigiyeonghyeol(衛氣營血) deteriorated case is divided into four groups - Wi, Gi, Yeong and Hyeol - that is discussed of a horizontal progress of Onbyeong. In Korean medicine, there are four types of diagnosis which are watching, listening, asking and taking. Informations, got by four types diagnosis are synthesized and classified for medical treatments. A pulse diagnosis belongs to a method by taking a wrist among four diagnosis. A Korean Medicine doctor makes a conclusion of cause, region and condition of disease by taking a pulse. Because all organs in human body are connected by a meridian system. organs conditions are reflected in a meridian system. So by taking a pulse, a progress and a prognosis of disease is diagnosed In this thesis, by taking a pulse on "Onbyeongjobyeon(溫病條辨)", a location and a feature of disease's cause with weakness and strength of a vital force are examined, and a character of a pulse diagnosis of Onbyeong is examined.
