Predictive Value of Serum Insulin-like Growth Factor-1 in Hepatocellular Carcinoma

  • Background: Hepatocellular carcinoma (HCC) is the commonest primary malignant cancer of the liver in the world. Insulin-like growth factor-1 (IGF-1) levels reflect hepatic function and are inversely correlated with the severity of background chronic liver disease. Objective: This study evaluated whether basal serum IGF-1 levels can predict prognosis of HCC patients according to different risks of disease progression. Materials and Methods: A total of 89 patients with hepatocellular carcinoma (HCC) were recruited in 3 groups: Group I, 30 HCC patients receiving sorafinib; Group II, 30 HCC patients with best supportive care; and Group III include 29 patients undergoing transcatheter arterial chemoembolization (TACE). All patients were investigated for serum levels of AST, ALP, Bb, Cr, BUN, AFP and IGF-I. Results: Patients with disease control had significantly higher baseline IGF-1 levels 210 (185-232.5) ng/mL (p value<0.01) than did patients without disease control. Low basal IGF-1 levels were associated with advanced HCC, such as multiple tumors and advanced stage, and low IGF-1 levels predicted shorter TTP and overall survival in patients treated with TACE. Conclusions: The levels of serum IGF-1, expressed as continuous values, may be helpful for accurately assessing hepatic function and the prognostic stratification of patients with HCC.

유치원·학교 구성원의 코로나19 신속항원검사 결과(2022년 3월 2일부터 5월 1일까지) (COVID-19 Rapid Antigen Test Results in Preschool and School (March 2 to May 1, 2022))

  • 목적: 코로나19 오미크론 변이 바이러스 확진자 급증에 따라 질병관리청과 교육부는 유치원·학교 구성원을 대상으로 선제검사를 시행하였다. 정상적인 학교생활을 영위하기 위해 자가진단용 신속항원검사 (rapid antigen test, RAT)를 이용하여 코로나19 확진자를 교내 전파가 이루어지기 전에 발견하는데 목적이 있다. 방법: 2022년 3월 2일부터 5월 1일까지전국의유치원·학교의구성원을대상으로검사인원 19,458,575명, 검사건수 50,675,386건을 수집하였다. 건강상태 자가진단 앱에 입력된 결과를 코로나19정보관리시스템 확진자 현황 정보와 병합하여 RAT 양성률과 양성예측도를 분석하였다. 결과: 주간 RAT 양성률은 1.11-5.90%, 양성예측도는 86.42-93.18%였다. 신분별 RAT 양성률은 학생이 1.13-6.16%, 양성예측도는 87.19-94.03%, 교직원 RAT 양성률 0.99-3.93%, 양성예측도는 77.55-83.10%였다. 증상 유무에 따른 RAT 양성률은 의심 증상이 있는 경우 76.32-88.02%, 의심 증상이 없는 경우 0.34-1.11%, 양성예측도는 의심 증상이 있는 경우 79.54-95.49%, 의심 증상이 없는 경우 64.66-87.84%이었다. 이번 유치원·학교 구성원의 RAT 선제 검사를 통하여 943,342명의 확진자를 교내 전파 이전에 신속하게 발견하였다. 결론: 자가진단용 RAT를 잘 활용하면 확진자를 신속하게 발견하는데 도움이 되어 유치원·학교에서는 교육 공백을 최소화할 수 있고, 전파의 위험도 줄일 수 있을 것이다. 장시간 동안 정해진 공간에서 동일한 사람들과 반복적으로 노출되는 위험을 최소화하여 안전한 환경에서 학교생활이 유지될 수 있도록 노력해야 할 필요가 있다. 이러한 점에서 선제검사의 필요성은 더 중요하게 되었다. 이번 연구에서 자가진단용 RAT 검사는 선제적 선별검사 도구로써 유용성과 확진자를 신속하게 발견하는 데 활용될 수 있다는 것을 보여준다.

Circulating Levels of Adipocytokines as Potential Biomarkers for Early Detection of Colorectal Carcinoma in Egyptian Patients

  • Background: Early detection of various kinds of cancers nowadays is needed including colorectal cancer due to the highly significant effects in improving cancer treatment. The aim of this study was to evaluate the potential value of adiponectin, visfatin and resistin as early biomarkers for colorectal cancer patients. Materials and Methods: Serum levels of adiponectin, visfatin and resistin were measured by a sandwich-enzyme-linked (ELISA) assay technique in 114 serum samples comprising 34 patients with colorectal cancer (CRC), 27 with colonic polyps (CP), 24 with inflammatory bowel disease (IBD) and 29 healthy controls. The diagnostic accuracy of each serum marker was evaluated using receiver-operating characteristic (ROC) curve analysis. Results: The mean concentration of adiponectin was significantly higher in CRC and CP groups than IBD and control groups (P-value <0.05). Also the mean concentration of serum resistin was significantly elevated in the IBD and control groups compared to CRC and CP groups (P-value = 0.014). However, no significant difference was noted in patients of the CRC and CP groups. On the other hand, the mean concentration of visfatin was significantly elevated in CRC and control groups compared to CP and IBD groups (P-value = 0.03). ROC analysis curves for the studied markers revealed that between CRC and IBD groups serum level of adiponectin had a sensitivity of 76.7% and a specificity of 76% at a cut off value of 3940, +LR being 3.2 and -LR 0.31 with AUC 0.852, while serum level of adiponectin between CP and IBD had a sensitivity of 77.8% and a specificity of 75% at a cut off value of 3300, with +LR=3.11 and -LR = 0.3 with AUC 0.852. On the other hand the serum level of visfatin between CRC and CP groups had a sensitivity of 65.5% and a specificity of 66.7 at a cut off value of 2.4, +LR being 1.67 and -LR 0.52 with AUC 0.698. Also the serum level of resistin had a sensitivity of 62.5% and a specificity of 70.3% at a cut off value of 24500, with +LR=2.1 and -LR = 0.53 with AUC 0.685 between control and other groups. On the other hand by comparing control vs CP groups resistin had a sensitivity of 81.8% and a specificity of 70.8% at a cut off value of 17700, with +LR=2.8 and -LR = 0.26 with AUC 0.763 while visfatin had a sensitivity of 68.2% and a specificity of 70.8% at a cut off value of 2.7, with +LR=2.34 and -LR = 0.0.45 with AUC 0.812. Conclusions: These findings support potential roles of adiponectin, visfatin and resistin in early detection of CRC and discrimination of different groups of CRC, CP or IBD patients from normal healthy individuals.

각종(各種) 신질환(腎疾患)에서의 혈청(血淸) $\beta_2-microglobulin$ 측정(測定)의 의의(意義) (The Significance of Serum $Beta_2-Microglobulin$ Measurement in Various Renal Diseases)

  • To evaluate change of serum $beta_2-microglobulin$ concentration$(s\beta_2-MG)$ and the usefulness of $s\beta_2-MG$ and $s\beta_2-MG/serum$ creatinine concentration(sCr) ratio in various renal diseases, $s\beta_2-MG$ and sCr were measured in 25 normal controls and 90 patients of various renal diseases(16 cases of glomerulonephritis, 12 cases of acute renal failure, 8 cases of chronic renal failure, 24 cases of nephrotic syndrome, 15 cases of tubulointerstitial diseases and 15 cases of lupus nephritis) using $Phadebas^\circledR$ $Beta_2-Micro$ Test kits. The results were as follows; 1) In normal control, the mean value of $s\beta_2-MG$ was $1.65{\pm}0.41mg/l$ and the mean value of $s\beta_2-MG/sCr$ ratio was $0.14{\pm}0.05$. 2) In various renal diseases, the mean value of $s\beta_2-MG$ was $6.74{\pm}5.47mg/l$. The mean value of $s\beta_2-MG/sCr$ ratio was $0.24{\pm}0.11$ and significantly elevated than that of normal control. (p<0.05) 3) The correlation between $s\beta_2-MG$ and sCr in glomerular and tubulointerstitial disease was log $s\beta_2-MG-0.90$ log sCr-0.48 and its correlation coefficient was 0.78(p<0.05). 4) In glomerular disease, the correlation between $s\beta_2-MG$ and sCr was log $s\beta_2-MG-0.89$ log sCr-0.46(r - 0.76) and in tubulointerstitial disease, it was log, $s\beta2-MG-0.95$ log sCr-0.59 (r-0.87). There was no significant difference between the two groups(p<0.05). 5) Among 32 cases of glomerular and tubulointerstitial disease patients, whose sCr was within normal range, 17 cases showed elevated $s\beta_2-MG$. The mean values of $s\beta_2-MG/sCr$ ratio in these patients was $0.30{\pm}0.14$ and significantly elevated than that of normal control(p<0.05). 6) In 15 cases of lupus nephritis, 12 cases showed elevated $s\beta_2-MG$ with normal sCr and 12 cases showed elevated $s\beta_2-MG/sCr$ ratio. With above results, it was found that the $s\beta_2MG$ can be used as an index of glomerular filtration rate as in the case of sCr and that $s\beta_2-MG/sCr$ ratio can be used as a tool in early detection of slightly decreased glomerular filtration rate and in detection of the renal disease of increased $\beta_2-MG$ production.

미생물 제제와 살균제에 의한 인삼 점무늬병의 방제 (Control of Alternaria Leaf Blight of Ginseng by Microbial Agent and Fungicides)

  • Bacillus subtilis QST713(BS QST713)의 단독, 살균제와 혼합 및 교호처리하여 인삼 점무늬병의 방제효과를 검정하였다. BS QST713를 10일 간격으로 단독으로 처리했을 때, 인삼 점무늬병에 대한 방제가는 83.3%, azoxystrobin, chlorothalonil copper sulfate basic, copper sulfate basic, kresoxim-methyl, difenoconazole, mancozeb 단독 처리구는 $80.4{\sim}83.7%$로 나타났다. BS QST713와 mancozeb를 혼합하여 14일 간격으로 처리하였을 때의 방제가는 83.6%, BS QST713와 azoxystrobin 혼합 처리구는 82.6%이었으나, difenoconazole, kresoxim-methyl, copper sulfate basic 및 chlorothalonil copper sulfate basic와 혼합 처리구는 $61.1{\sim}76.4%$로 방제효과가 약간 감소하였다. 대전에서 BS QST713와 copper sulfate basic를 10일 간격으로 교호처리시 방제가가 85.9%이었으나, 다른 살균제와의 교호처리 시는 $55.5{\sim}78.2%$로 낮아 졌다. 그러나 무주에서 모든 교호처리에서 방제가가 90% 이상으로 나타났다. 이상의 결과에 의하면 BS QST713의 단독, 혼합 및 교호처리는 살균제의 사용량을 줄일 수 있는 방제법으로 이용될 것이다.

  • Disease associated with microorganisms are far from resolved by current therapeutics. One of effective approach to health maintenance and disease control is the use of dietary bacterial and carbohydrate supplements. This comprises use of probiotics and prebiotics. Probiotics mean the live microorganisms, which when administered in adequate amounts confer a health benefit on the host. Prebiotics mean a nondigestible food ingredient that beneficially affects the host by selectively stimulating the growth and/or activity of one or a limited number of bacteria that can Improve the host health. Especially, probiotics has the relation which is close with innate immunity and adaptive immunity. And probiotics has the clinical value with many disease like lactose intolerance, constipation, acute gastroenteritis, food hypersensitivity and allergy, atopic dermatitis, crohn's disease, rheumatoid arthritis, pelvic radiotherapy, intestinal inflammation and chemical exposure, colon cancer, inhibitory effect of Helicobacter pylori and lowering the level of cholesterol. We use jointly korean medicine and probiotics and it has the more therapeutic effect in the many disease.

벚나무 빗자루병균(Taphrina wiesneri)에 대한 살균제의 방제효과 (Control Efficacy of Fungicides on Cherry Witches' Broom Caused by Taphrina wiesneri)

  • 2008년 4월 제주도의 왕벚나무 빗자루병 이병옆으로부터 5개의 균을 분리하여 생리학적 방법과 유전학적 방법을 이용해 동정한 결과 Taphrina wiesneri로 동정되었다. 21개의 농약을 이용하여 실내 항균력을 검정한 결과 tebuconazole, difenoconazole, propiconazole과 같은 triazole계의 농약이 우수한 항균효과를 나타내었다. Triazole계의 농약을 처리한 야외 실험 결과 $25.7{\sim}52.8%$의 빗자루병 방제효과를 나타내었다.

Mulberry popcorn disease occurrence in Korea region and development of integrative control method

  • Mulberry fruits also have tremendous potential for providing various valuable industrial products of very high economic value for human beings. Nevertheless, through global warming, the popcorn disease caused by sclerotia forming fungi reduces the productivity of mulberry fruits in worldwide. So, in this study, we investigated damage ratio of mulberry popcorn disease in mulberry fruit production farm (Buan, Jeongueb, Sangju, Gochang in Korea). In Jeonbuk Buan, popcorn disease rate was the highest about 30%, on the other hand, in case of Gyungbuk Sangju and Jeonbuk Gochang, not damage. Also, we investigated about popcorn disease prevention by various of chemical treatment methods.

Microbe-Mediated Control of Mycotoxigenic Grain Fungi in Stored Rice with Focus on Aflatoxin Biodegradation and Biosynthesis Inhibition

  • Rice contaminated with fungal species during storage is not only of poor quality and low economic value, but may also have harmful effects on human and animal health. The predominant fungal species isolated from rice grains during storage belong to the genera Aspergillus and Penicillium. Some of these fungal species produce mycotoxins; they are responsible for adverse health effects in humans and animals, particularly Aspergillus flavus, which produces the extremely carcinogenic aflatoxins. Not surprisingly, there have been numerous attempts to devise safety procedure for the control of such harmful fungi and production of mycotoxins, including aflatoxins. This review provides information about fungal and mycotoxin contamination of stored rice grains, and microbe-based (biological) strategies to control grain fungi and mycotoxins. The latter will include information regarding attempts undertaken for mycotoxin (especially aflatoxin) bio-detoxification and microbial interference with the aflatoxin-biosynthetic pathway in the toxin-producing fungi.

법정감염병 발생자료와 감염병관리사업 평가지표와의 관계 실증분석 (Actual Analysis of the Interrelationship between Evaluation Indicators of Communicable Disease Control and Prevention Activities and Communicable Disease Incidence Data)

  • 이 연구는 감염병 발생자료와 감염병 관리사업 평가지표와의 관계를 실증적으로 분석함으로써 감염병 관리사업의 평가지표의 타당성을 실증적으로 분석함을 목적으로 하였다. 자료는 2004년과 2005년 2개년간의 시군구(보건소) 감염병 환자 발생 수 합계와 2005년도 감염병관리사업 평가지표 등 이었다. 자료의 분석은 빈도분석, 분산분석, 다중회귀분석 등을 이용하였다. 보건소 유형 및 법정감염병 각 군별로 각기 상이한 지표들이 도출되었으며, 각 군로도 특이한 지표 보다는 다양한 분야의 지표들이 혼재되어 있는 양상으로 도출되었다. 특히, 교육실적 등이 발생건수와 유의한 관계를 보이는 경우가 많아 발생건수는 신고건수 즉, 사업의 성과의 결과로 판단하는 것이 더욱 타당할 것으로 판단된다. 전체적인 지표의 개선이 필요하거나 사업의 투입시간 및 추이를 본 후 재평가가 필요할 것으로 생각되며, 각 사업별로 분리하여 특이한 평가지표를 생산할 필요가 있어 보인다. 감염병관리사업의 평가지표 개발은 기초자치단체별 감염병관리사업 종합평가체계를 구축하는데 핵심적인 역할을 할 것이며, 감염병관련업무 표준화를 촉진하고 관련지침을 개정에 활용될 것이며, 향후 보건사업 및 보건의료조직의 계량적인 성과 평가에 활용될 것이다. 또한 시군구 보건소에서 수행한 성공적인 감염병 관리 사업의 사례를 발굴, 제시함으로써 보다 성공적인 감염병관시업의 접근이 가능케 할 것이다.
