재난의료지원팀 내 1급 응급구조사의 재난대비·대응역량에 관한 연구 (Disaster preparedness and response competency of emergency medical technician-paramedics in the disaster medical assistant team)

  • Purpose: This study aimed to investigate disaster preparedness competence and disaster response competence of paramedics working in emergency medical centers operating a disaster medical assistance teams. Methods: Data of 174 emergency medical technician(EMT)-paramedics were collected from July 15 to August 14, 2018 at regional and local emergency medical centers that operate disaster medical assistant team. Analysis of the data was carried out with IBM SPSS statistics 24.0 software (IBM, Armonk, NY, USA). Results: The mean disaster preparedness competence score was $3.57{\pm}0.63$ (out of five). Participants' disaster preparedness competence significantly differed according to type of emergency medical center (p<.000), disaster education experience (p<.000), and education frequency (p=.001). The mean disaster response competence score was $4.09{\pm}0.57$ (out of five). Participants' disaster response competence significantly differed according to disaster education experience (p<.000) and medical assistance experience (p=.045). Conclusion: Emergency medical technician-paramedics without disaster training should first be provided with this training. Further, it is important for EMT-paramedics to know their disaster preparedness and response capacities and strengthen their shortcomings. It is also important to develop education and training programs that properly equip EMT-paramedics with practical competencies.

전남지역 재난의료대응 유관기관 재난의료대응 지식, 인식 및 교육 요구도 차이 분석 (The difference in knowledge, awareness, and educational demand about disaster medical response-related institutions in Jeollanam-do)

  • Purpose: This descriptive research study aimed to investigate the knowledge and perception of the natural disaster medical system by relevant disaster medical response teams in Jeonnam region, and provide baseline data for a disaster education program based on analysis of priorities of educational demand. Methods: Online questionnaires were distributed to 200 research participants including paramedics from five fire stations in J province, 22 public health centers, two disaster base hospitals, ERU (Emergency Response Units), and DMAT (Disaster Medical Assistance Team). The questionnaires elicited basic information about respondents, their knowledge and perception on disaster preparation and response, cooperation system, and educational and training needs. Results: The top priority items selected were: other disasters for paramedics, first aid for the rapid response team, and command system for DMAT. Conclusion: Customized education and training programs must be developed to suit each organizational need. Detailed operational guidelines must be established and with them a unified educational curriculum should be put into practice.

  • Purpose : This study aimed to determine the experiences of the disaster response team 9 participants who participated in the disaster after the fire incident occurred in Jecheon Sports Center and their meaning and essence. Methods : Nine disaster response teams were the subjects of the study, and Individual in-depth interviews were conducted. Data were collected online between January 6 and April 30, 2021, from the software ZoomTM. This study was conducted as a qualitative study by applying Giorgi's phenomenological experiential research method, which has an advantage in revealing the essential structure and meaning of experience. Results : Three major themes were derived from the study results: the stimulus through unfamiliar experiences, the grievances due to work environment and regional characteristics, and dullness resulting from hiding and enduring. The following 17 sub-themes were identified: learning through unfamiliar experiences, frustration due to unexpected circumstances, shock from unfamiliar experiences, doing my best in the present, confidence due to increased experience, disunified system, intervention of various interests, the atmosphere that puts responsibility on others, inactive help, unforgettable regional characteristics, working without time to settle, tolerating it in my own way, memories left in the unconsciousness, sudden suffering from memories that come to mind, movement in anxiety and tension, dullness, work with colleagues in the same situation. Conclusion : This study is meaningful in that it attempted to provide basic data in preparing a long-term strategy for effective policy direction and institutional protection based on the systematic mental health management of the disaster response team. Additionally, this study's results can be used as primary data for future research among Disaster Response Team.

국가재난의료매뉴얼의 개발 (Development of the Disaster Medical Manual in Korea)

  • 세월호 참사 등 각종 재난에 있어 재난의료대응체계의 문제점이 노출되었는데, 이 중 현장응급의료지원팀의 현장대응 불가, 현장에서의 소방, 의료진의 협업 곤란, 현장의료지원 및 부상자의 수용, 진료를 위한 체계적이고 세부적인 매뉴얼 미비가 중요한 사항이었다. 따라서 2014년 5월부터 중앙응급의료센터 내 재난응급의료상황실 설치 운용에 따른 기존의 비공식적으로 흩어져 있던 재난의료대응 매뉴얼 수집, 수정 및 이에 대한 교육이 필요한 상황이 되었으며, 재난응급의료상황실을 중심으로 한 재난 초기 의료대응체계를 확립할 필요성이 대두되었다. 중앙응급의료센터의 재난의료대응 개선 방안은 신속대응반 중심의 빠른 출동 체계 수립이나, 실제 적용할 체계가 미흡하여, 2014년 2월 응급의료법 개정 시행규칙에 따라 국가와 지자체에 재난대응 매뉴얼을 개발하고 기본안을 마련하기로 하여, 본 연구진은 학술적, 실무적으로 적절한 국가재난의료 매뉴얼을 국내 최초로 개발하였으며, 이를 훈련 및 시뮬레이션을 통하여 적용하였고 그 과정을 정리 분석하였다.

다수사상자사고 대응 실습교육 프로토콜 개발 및 효과성 검증 (Development and evaluation of training protocols for mass casualty incidents during disaster response)

  • Purpose: The purpose of this study was to develop a training protocol to standardize the management of mass casualties as part of the disaster response, and to verify the effectiveness of the training protocol. Methods: The study was conducted as a quasi-experimental study with a non-equivalent control group and pretest-posttest design. The protocol was divided into 5 parts, the first for the advance party, the second for the rescue team, the third for the paramedic team, the fourth for the ambulance team, and the fifth for the 119 EMS team. This study was conducted on November 15, 2021 and consisted of 21 subjects in the final experimental group and 23 subjects in the control group. In this study, the prior homogeneity test was analyzed using the χ2-test, intragroup comparisons were analyzed using the paired t-test, and intragroup comparisons were analyzed using the independent t-test. Results: The protocol was developed in five parts: advance party, rescue team, paramedics team, ambulance team, and 119 EMS team. In verifying the effectiveness of the protocol, it was found that there were significant differences in self-efficacy (t=-0.941, p=0.001) and self confidence within the group (t=-0.025, p=0.001) after the implementation of the mass casualty incident response training program. However, there was no significant difference between the experimental and control groups. Conclusion: Based on the findings of this study, it is believed that disaster response personnel can experience lower levels of anxiety and tension in disaster situations if they receive practical and realistic education and training. In the future, it is necessary to enhance protocol based practical education that can improve the knowledge and skills of each team and individual.

Development of Earthquake Damage Estimation System and its Result Transmission by Engineering Test Satellite for Supporting Emergency

  • Drawing on its extensive experience with natural disasters, Japan has been dispatching Japan Disaster Relief (JDR) team to disaster-stricken countries to provide specialist assistance in rescue and medical operations. The JDR team has assisted in the wake of disasters including the 2004 Indian Ocean Earthquake and the 2008 Sichuan Earthquake in China. Information about the affected area is essential for a rapid disaster response. However, it can be difficult to gather information on damages in the immediate post-disaster period. To help overcome this problem, we have built on an Earthquake Damage Estimation System. This system makes it possible to produce distributions of the earthquake's seismic intensity and structural damage based on pre-calculated data such as landform and site amplification factors for Peak Ground Velocity, which are estimated from a Digital Elevation Model, as well as population distribution. The estimation result can be shared with the JDR team and with other international organizations through communications satellite or the Internet, enabling more effective rapid relief operations.

Research on disaster management personnel improvement measure of local government

  • The purpose of this study is to examine the disaster management system of local governments by diagnosing the current status of disaster management experts in Korea, deriving problems and improvement measures, and looking at local governments' total labor cost system. The status and status of disaster management personnel were compared and analyzed to find ways to improve the local government's disaster management response system under the Framework Act on Disaster and Safety Management in Korea. Considering Korea's reality, because national security and disaster management exist together, we devised a plan to improve on-site response capabilities and seek solutions by deriving the problems of the currently operating security and safety-related organizations.

A new approach to quantify safety benefits of disaster robots

  • Remote response technology has advanced to the extent that a robot system, if properly designed and deployed, may greatly help respond to beyond-design-basis accidents at nuclear power plants. Particularly in the aftermath of the Fukushima accident, there is increasing interest in developing disaster robots that can be deployed in lieu of a human operator to the field to perform mitigating actions in the harsh environment caused by extreme natural hazards. The nuclear robotics team of the Korea Atomic Energy Research Institute (KAERI) is also endeavoring to construct disaster robots and, first of all, is interested in finding out to what extent safety benefits can be achieved by such a disaster robotic system. This paper discusses a new approach based on the probabilistic risk assessment (PRA) technique, which can be used to quantify safety benefits associated with disaster robots, along with a case study for seismic-induced station blackout condition. The results indicate that to avoid core damage in this special case a robot system with reliability > 0.65 is needed because otherwise core damage is inevitable. Therefore, considerable efforts are needed to improve the reliability of disaster robots, because without assurance of high reliability, remote response techniques will not be practically used.

다수 환자 발생 시 안전 증진 요인에 관한 연구 (A Study on the Factors of Safety Enhancement in Mass Casualty Incidents)

  • 우리나라는 자연재난 및 인위적인 재난 등 다양한 재난을 겪고 있다. 본 연구는 재난관리 단계 중 대응 단계에서 활동하는 소방 구급 대원의 재난대응 능력에 대한 인식을 파악하기 위한 서술적 조사연구이다. 연구 대상자는 G도 소재 소방서 구급 대원 중 현장 경험 2년 이상의 1급 응급구조사를 대상으로 하였으며, 현장대응 능력의 하위 요소에 대한 문항이 포함된 설문지를 작성하여 최종 수집된 설문지 161건을 SPSS Program을 이용하여 분석하였다. 연구 결과 대상자의 환자분류 능력, 환자처치 능력, 환자이송 능력, 재난지원 능력 및 재난대응 신속성의 평균은 각각 3.53점, 3.68점, 3.66점, 2.95점 및 3.44점이었으며, 다중회귀 분석의 결과로 재난대응 신속성에 영향을 미치는 변수는 환자처치 능력, 환자이송 능력, 재난 지원 능력 순으로 나타났다. 결론적으로, 소방 구급 대원은 재난대응 신속성을 향상시키기 위해서는 환자처치 능력, 환자이송 능력, 재난지원 능력을 우선적으로 고려해야 하겠으며, 궁극적으로 재난대응 능력을 향상시키기 위한 재난대응 지침을 마련해야 하겠다.

효율적인 재난 상황관리를 위한 재난현장 필수정보 연구: 제천 스포츠 센터 화재사건을 중심으로 (A Study on the Essential Information to Collect Disaster Sites for Effective Disaster Management : Focused on Jecheon Sports Center Fire case)

  • 연구목적: 재난이 발생했을 때, 보통 재난안전상황실로 정보들이 무분별하게 수집되고 전달되고 있어 신속하고 올바른 의사결정에 어려움을 겪고 있다. 따라서 본 연구에서는 현장대응팀과 상황실 모두 최적의 재난 상황인지 및 대응이 가능토록 재난관리 필수정보 요소를 도출하고자 하였다. 연구방법: 본 연구에서는 다중밀집시설 대형화재 재난 사례를 바탕으로 실제 재난 대응과정을 확인하고, 다중밀집시설 대형화재 위기관리 표준매뉴얼과 당시 재난발생시 작성된 화재상황보고 서식을 분석함으로써 현장과 상황실에서 필요로 하는 재난관리 필수정보를 도출하였다. 연구결과: 재난관리 필수정보는 상황접수 및 보고·전파, 총력대응·지역긴급구조통제단(지통단)·지역재난안전대책본부(지대본) 단계, 중앙 긴급구조 통제단(중통단)·중앙재난안전대책본부(중대본) 운영단계, 수습복구 단계의 4단계로 구분하였으며, 각 단계별로 수집되어야할 정보와 핵심 업무에 해당하는 정보를 중심으로 제시하였다. 결론: 이렇게 도출된 재난관리 필수정보는 현장에서 정보를 수집하는 담당자는 효율적으로 정보를 수집할 수 있도록 가이드의 역할이 가능하며, 재난대응의 의사결정을 담당하는 상황실에서는 의사결에 필요로 한 핵심정보를 전달받게 됨으로써 신속한 상황판단이 가능해지므로 더욱 효율적인 재난 상황관리가 가능해질 것으로 기대된다.
