Converged Factors Influencing Depression of Community Dwelling Elderly (지역사회 거주 노인의 우울에 미치는 융합적 영향요인)

  • The purpose of this research is to analyze the depression, happiness on aging, self-esteem, life satisfaction and disability of daily living activity level of the elderly people, and to analyze the variables to impact on their depression. The study data were analyzed descriptive analysis, t-test, ANOVA, Pearson's Correlation Coefficient, and multiple regression analysis. The most influential variables on the elderly depression were self-esteem with 27.9% of explanatory power (${\beta}=-.318$, p<.001), followed by life satisfaction with 5.5%(${\beta}=-.248$, p<.001), happiness on aging 4.1%((${\beta}=-.200$, p<.001), disability of daily living activity 1.6%(${\beta}=.128$, p=.026), and 4 variables explaining 39.1% of the elderly depression. Therefore it is necessary to develop specific education programs to reduce depression and to improve self-esteem and happiness on aging of the elderly.

Trends and Factors Affecting Participation Rate in Korean National Health Screening among People with Disabilities (우리나라 20세 이상 장애인 건강검진 수검률 추이 및 관련 요인분석)

  • Background: People with disabilities have higher prevalence rates and earlier onset of chronic disease than the non-disabled; therefore, their participation in health screening is important. This study evaluates the participation rate and trends in health screening of people with disabilities, and examines the association between their participation rate and disability characteristics, and socioeconomic status. Methods: Data on disability-related characteristics were collected from the National Disability Registry, and participants' corresponding health examination data were taken from the Korean National Health Insurance Corporation between 2002 and 2011. A total of 873,819 participants aged ${\geq}20$ years were analyzed in this study. Results: The rate of participation of people with disabilities in health screening has increased each year, but their participation rate is lower than that of the total population. The participation rate was lower in females than in males; the elderly group than in the younger group; those who live in city areas than rural areas; self-employed for health insurance than employees; those with an internal organ disability than those with an external physical disability; those with a severe disability than those with a mild disability; and those with a short-term disability than for those with a long-term disability. Conclusion: The factors associated with participation rate are age, sex, socioeconomic status, and disability characteristics. These findings indicate that health check-ups of people with disabilities should be promoted using an approach that takes into account the large individual differences in socioeconomic status and disability characteristics in this population.

  • The usual reasons for elderly safety accidents include lack of care for disaster, physical disability, and an error of judgment and so on. But it is true that accidents will occur continuously with the afore-mentioned causes. Against the elderly safety accidents, safety is the main prerequisite for leading the successful and easy life in the elderly. Therefore, this study is purposed to provide the way of thinking about elderly safety issues by suggesting the methods and contents of education program. This study is specifically focused on developing audio-visual materials involved in all the safety accidents and researching the effect of safety education ability through materials with a view to identify the substantial problems. Education program should make a continuous and long term study so that the curricula of the safety accidents prevention education can be done systematically.

The Prevalence and Association Factors of Unmet Medical Needs by Age Group in the Elderly (노년기에 따른 미충족 의료 현황 및 관련 요인)

  • This study investigated the prevalence of unmet needs for health care among Korean adults and related factors. Using data from the 2012 Korea Health Panel, 1,896 adults aged 65 and over and two age groups(Young-Old(66-74) and Old-Old($65{\leq}$)) were analyzed to identify these factors. Logistic regression analysis was used to examine the main factors associated with unmet medical needs. According to the results of this study, the factors influencing unmet medical needs with regard to economic factors were Subjective Health Status(young-old), Disability(young-old), Activity Limitation(young-old), Education Level(old) and Economic Activity(old). The factors influencing unmet medical needs were factors of attitude, information, and disease. factors were Disability(young-old), Activity Limitation(young-old), Education Level(old) and Economic Activity(old). Therefore, further research that investigates unmet needs depending on age group in the elderly would suggest helpful policy implications.

Mediating Effect of Self-esteem on the Relationship between Economic, Health Characteristics and Life Satisfaction of Elderly People with Disability (고령장애인의 경제적 및 건강 특성과 삶의 만족도 관계에 대한 자아존중감의 매개효과)

  • This study is aimed at finding plans to enhance life satisfaction of elderly people with disability. For this, it analyzed the influence of economic characteristics and health characteristics upon life satisfaction, as well as a mediating effect of self-esteem. This study utilized data from the $7^th$ Panel Survey of Employment for the Disabled(PSED). The results are as follows, First, life satisfaction was high in the case of being employed and when old-age preparation was ready, current status of health, whether or not they need help in their daily life, chronic disease. Second, the mediate effect of self-esteem was analyzed for the influence of the plan for old age, good health condition and other's help in daily life upon life satisfaction. That is, those enhanced the self-esteem and it played an important role to improve life satisfaction. Based on such study results, this study recommends a number of the policy suggestions.

The effects of Moxibustion Therapy on Chronic Low Back Pain, Daily Living Disability and Sleep Pattern in Elderly Women (뜸요법이 여성노인의 만성요통, 일상생활기능장애 및 수면양상에 미치는 효과)

  • This is a nonequivalent control group non-synchronized experiment research to verify the effect of moxibustion therapy on efficient management of daily living disabilities and sleep patterns in elderly women with chronic low back pain. Total 12 sessions of moxibustion therapy were applied to elderly women(30 subjects in an experimental group and 30 subjects in a control group) in G City once a week for 12 weeks from March 15th to May $31^{st}$ 2015. Then the follow-up study was conducted 2 weeks after the post survey. For moxibustion, was applied to Shen-shu, Ashi point, Ta chang shu, Yang kuan and Yosu. 5 sheets of moxibustion were applied to 7 acupuncture points of Mugeukboyang moxibustion; Zu san li & Chu chi, Chung wan, Chung chi & Shui tao, Fei shu, Kao hung and Tien shu, Wijung respectively and experimental treatment was conducted. For assessing the degree of back pain, Visual Analog Scale(VAS) developed by Scott & Huskisson(1979) was used and for measuring the degree of daily living disability, Oswestry Disability Index, which was developed by Fairbank et al(1980) and translated and revised by Yim, Hyeon-sul et al.(1998), was used. For measuring sleep patterns, the sleep pattern measurement tool developed by Oh, Jin-su, Song, Mi-soon and Kim, Sin-mi(1998) was used. For data analysis, SPSS/WIN 18.0 was employed and Chi-square test, t-test and repeated measures ANOVA were performed. There were significant differences in pain score(F=2510.32, p<.001), daily living disability score(F=1937.82, p<.001) and sleep pattern score((F=15.54, p<.001) of elderly women that were provided with moxibustion therapy, compared to the control group. Therefore, it was found that moxibustion therapy made a positive contribution to reduction in pain and daily living disabilities and improvement in sleep quality. Based on this study finding, there is a need to apply moxibustion therapy to elderly people with chronic low back pain as a nursing intervention in future.

Care and future expectations of families with severe disabilities (중증장애 가족의 돌봄과 미래기대)

  • Care of people with severe disabilities requires care throughout their life cycle compared to non-disabled families. For this reason, carers of severely handicapped families are highly likely to have negative feelings throughout the family as well as daily stress. Disabled families also have a high rate of experiencing difficulties between non-disabled children or married couples in life centered on the disabled. In particular, the rapid aging in Korea affects the lives of the elderly disabilities along with the aging of the caregivers. The study explored alternatives to realistic support through the past and present experiences of caregivers with disabilities through qualitative research methods, and derived hopes and expectations for the future as follows.First, after confirming the disability for infants and toddlers with disabilities, information about the support system or system is provided from the rating agency. Second, providing "customized care support" suitable for the type of disability or individual characteristics at the social, political, institutional, and economic levels. Third, the provision of programs for non-disabled children and the provision of healing programs only for families with disabilities or those with disabilities. Fourth, the provision of spaces and programs that provide rest and rest for adults with disabilities. Fifth, the application of a long-term care system for the elderly reflecting the aging age of the disabled and institutionalization of community care for the disabled. The research is meaningful in that it presents discussion points for improving the quality of life of adults and the elderly with disabilities.

Correlation between the Oswestry Disability Index, Berg Balance Scale, and Kinematic Data during Gait Analysis in Elderly People with Chronic Back Pain (만성 허리 통증 노인의 오스웨스트리 장애지수, 버그 균형 척도, 보행 시 운동학적 움직임과의 상관관계)

  • PURPOSE: Chronic low back pain is one of the main factors that affect the quality of life and cause problems of gait and balance in elderly people. This study investigated the correlation between the Oswestry disability index (ODI), Berg balance scale (BBS), and kinematic data measured while analyzing the gait of elderly people with chronic back pain. METHODS: A total of 29 subjects participated in this study. The ODI, BBS, and kinematic data of lower extremities were measured while walking. All data were analyzed using Pearson's correlation coefficients and the significance was measured at .05. RESULTS: ODI had a significant correlation with 1, 13, and 14 items of BBS (p < .05), and left hip external rotation and right ankle abduction respectively in the stance and swing phase of gait (p < .01). 13 items of BBS had a significant correlation with the right ankle abduction in the stance phase of gait (p < .01). In addition, 14 items of BBS had a significant correlation with right and left ankle abduction in the stance and swing phase of gait (p < .01). CONCLUSION: While attempting to predict chronic low back pain and balance issues, it may be useful to check the right ankle abduction in the stance and swing phase of gait. In the future, it would be helpful if some simple tests could be designed to assess balance in elderly people with chronic low back pain.

Determinants analysis of affecting to Health screening among Persons with Disabilities (장애인의 건강검진 수검에 영향을 미치는 요인 분석)

  • Purpose: The purpose of this study was to examine the factors affecting of health screening among persons with disabilities. Method: The study examines the factors affecting of health screening using over 20 years of age who participated in the 2017 National survey of the disabled data. Subjects were 6,332 person with disabilities over 20 years of age. Data were analyzed using descriptive statistics, X2-test, multiple logistic regression with the SPSS win 21.0 program. Results: Health screening among persons with disabilities is differentiated with education level, marital status, Subjective House economic status, Health insurance, Economic activity, Type of disability, Grade of disability, Stress recognition, Depressive symptom, Suicidal thinking, Suicidal attempt, and grade of disability. The significant predictors of the health screening were age, Subjective House economic status, Economic activity, and Grade of disability. Conclusion: We should consider age, disability level, and economic level when developing a screening program for people with disabilities. Especially, the development and promotion of the health screening program for persons with disabilities and related education are required.

A Study of the Relationship among Family Support, Public Support and the Depression of the Functional Disabled Elderly (기능손상노인의 우울감과 가족지지, 공적지지 간의 관계)

  • The number of the functionally disabled elderly has been increasing in Korea and their families should have been in charge of supporting and taking care of them. This study was designed to grope for devices which were helpful to improve the psychological welfare of the functionally disabled elderly in the present situation. The results of this study were as follows: First, considering the level of psychological welfare of the functionally disabled elderly, the depression degree of the subjects was 52.9 which was higher than the middle point, 50. And the degree of family support the functionally disabled elderly were aware of, it also was high ; the score was 34.6 compared to the middle point, 33. On the other hand, the degree of public support that the functionally disabled elderly received was 2.01 which was lower than the middle point, 4.5. Second, the level of the depression of the functionally disabled elderly followed by background variables, the degree of agreement with family support, and public support represented significant differences among group according to religion, the type of previous occupation, the ownership of house, the type of residence, the presence of spouse, education level, and the source of income. Third, the relative influence toward the depression of the functionally disabled elderly was shown in the order of importance as follows: family support, the degree of functional disability, the ownership of house, public support, the presence of spouse, the source of income, the number of daughters, the degree of functional disablity.
