  • Objects : The purpose of this study is to value measuring of sagital, horizontal, frontal aspects of DITI as scale of low back pain. Methods : We investigated 16 patients with low back pain who had visited in the H Korean medicine hospital in Gimpo city, Gyeonggi Province from January 1st to, August 31th, 2016. We evaluated the change of them by using Digital Infrared Thermographic Imaging. Results : There is meaningful differences in values for CV12 in pretest and posttest which were observed by DITI. In the Correlation analysis of values of meridian points in pretest and posttest, there is relation ΔSagital04(ST25/BL52), ΔSagital04(CV4/GV3) with low back pain. Conclusions : These results suggest that ΔSagital04(ST25/BL52), ΔSagital04(CV4/GV3) can be used as value of Low back pain.

Clinical Study with Thermography on Shoulder Hand Syndrome after Stroke (뇌졸중후(腦卒中後) 견관절(肩關節) 수부(手部) 증후군(症候群)의 적외선(赤外線) 체열(體熱) 촬영(撮影)을 이용(利用)한 임상적(臨床的) 관찰(觀察))

  • Shoulder hand syndrome is characterized by pain, vasomotor instability, and tenderness, mainly in the distal upper extremity. The pathophysiologic mechanism of this syndrome is not yet proved. The purpose of this study is to evaluate the usefulness of thermographic imaging on shoulder hand syndrome after stroke for early diagnosis and its clinical pattern analysis including acupuncture and electroacupuncture therapy. This study was performed from June to September in 1996 on 46 stroke patients who were admitted at Oriental hospital of Kyung Hee Medical Center. The study group were 23 patients with shoulder hand syndrome. The control group were 23 patients without shoulder hand syndrome. Skin temperatures on the both upper extremities were measured by Digital Infrared Thermographic Imaging(D.I.T.I.) before the study and 3 weeks later again. The results were as follows; 1. The shoulder hand syndrome group were significantly more restricted in shoulder passive range of motion than the control group. 2. The shoulder hand syndrome group showed significant temperature difference of both dorsal hands. 3. The electroacupunture therapy group were significantly more improved on the temperature difference of both dorsal hands than acupuncture therapy group in 3 weeks later. 4. Both posterior arms showed the biggest temperature difference from 11 to 30 days in shoulder hand syndrome group. 5. The lesser passive ROM(range of motion) of shoulder group showed significantly increased temperature difference of both hands. The above results show that measurement of shoulder passive range of motion and D.I.T.I. is a useful method for early diagnosis on shoulder hand syndrome and its clinical pattern analysis including evaluation of acupuncture and electroacupuncture therapy. Continuous study will be needed for more clinical application and evaluation on shoulder hand syndrome.

Expansile Duraplasty for Posttraumatic Syringomyelia - A Case Report - (외상후 척수공동증에 대한 확장성 경막성형술 - 증례보고 -)

  • A 35-year-old female patient who had been paraplegic since her upper thoracic cord injury one and a half years before was presented with a syringomyelia involving C7 to T2 cord segments on follow-up magnetic resonance imaging(MRI). Her pain on the chest and bilateral axilla worsened since about 2 years after initial injury, and MRI demonstrated a progressively enlarging cystic syrinx cavity of the cervicothoracic cord. The posttraumatic syringomyelia was managed operatively by lysis of the arachnoid adhesions, myelotomy, and expansile duraplasty. Post-operatively her pain was relieved, and MRI showed decrease in size of the syrinx, and digital infrared thermographic imaging(DITI) showed diminution of the hyperthermic area on the anterior chest. In conclusion, the posttraumatic syringomyelia can be managed successfully without shunt operation. It seems apparent that expansile duraplasty is effective in controlling the posttraumatic syrinx.

  • Background : Monoplegia is the paralysis of a limb. It is commonly caused by an injury to the cerebral cortex, and rarely caused by injury to the internal capsule, brain stem, or spinal cord. Most problems with cerebral cortex is derived from the occlusion of a brain cortex blood vessel due to thrombus or embolus. Objectives : This study is to see if there is a significance in thermal differences of acupoints in diagnosis and treatment of monoplegia on an upper extremity to test the validity of acupuncture and herbal treatment for it. Methods : By using Digital Infrared Thermographic Imaging(DITI), thermal differences$({\Delta}T)$ of acupoints on the upper extremity in a patient with monoplegia on the right upper extremity were measured after an attack of the disease. By giving Mangeum-tang(萬金湯) and treating the patient with acupuncture. the temperature changes of the upper extremity were examined through DITI and improvement was observed. Results : Compared with the left arm which suffered no such injury, the right recovered about 80% of sensation, and the grade of monoplegia improved from Grade O to Grade V. Also, the temperatures of right palmar-dorsal hand and the region of Weiguan(外關, Waiguan, TE5) were $1^{\circ}C$ and $1.45^{\circ}C$ higher than the same left region on admission day, but the thermal differences$({\Delta}T)$ narrowed to $0.5^{\circ}C$ by the last day. Conclusions : Results suggest that DITI screening is a reliable method of prognosis and that the time required for treatment can be estimated through this method in cases of monoplegia to an upper extremity. Also, progress in treatment is reflected in thermal differences of acupoints of the monoplegic upper extremity in accordance with the theory of meridian. This supports a role for acupuncture and herbal treatment for monoplegia.

Videothoracoscopic Sympathectomy in Hyperhidrosis (다한증의 흉강경을 이용한 교감신경절 절제술)

  • Exessive sweating of the palms and soles, is a psychologically and occupationally distressing and sometimes disabling condition. Hyperhidrosis is one of the common abnormalities in autonomic nervous system. There were no specific treatment on hyperhidrosis, so invasive thoracic sympathectomy via axillary thoracotomy or cervical approach had been used. Video-assisted thoracic surgery(VATS) is now mostly performed for treating of the palmar and axillary hyperhidrosis. From March 1996 to March 1997, 15 patients with bilateral palmar hyperhidrosis had been treated by the bilateral thoracic sympathectomy(T2, T3, T4) with thoracoscopic resection. The patient were evaluated preoperative and postoperative Digital Infrared Thermographic Imaging (DITI) at Kyung-Hee University Hospital. There were no case of the thoracotomy conversion. There were 3 complications ; pulmonary edema in 1 case, Horner's syndrome in 1 case, and gustatory hyperhidrosis in 1 case. More than half of the patients also had compensatory sweating in the lower abdomen, the buttocks, the back and the thighs. In conclusion, most of the patients were satisfied with the postoperative results of the thoracoscopic sympathectomy, including no more palmar and axillary sweating, less pain, better cosmetic appearances, decreased sweating of the face and soles. In addition, intraoperative temperature monitoring of the hands could estimate the successful thoracoscopic sympathectomy and the preoperative and postoperative Digital infrared thermographic imaging(DITI) could especially be the technique for the objective manifestation of the successful results of the thoracoscopic sympathectomy.

The clinical study of Digital Infrared Thermographic Imaging on Depressed patients (전신체열촬영(全身體熱撮影)에 의한 우울증(憂鬱症)과 안면마비(顔面麻痺)의 객관적(客觀的) 진단방법(診斷方法) 모색(摸索))

  • Depression is a psychiatric syndrom consisting of dejected mood, psychomotor retardation, insomnia and weight loss. sometimes associated with irrational quilt feeling. And it is also similiar to Hwa-byung(火病) symptom in oriental medicine. But it is difficult to diagnose with objective method. Digital Infrared Thermographic Imaging(D.I.T.I.) is one of diagnotic method that measure the changes of skin temperature in body.Specially we checked the skin temperature on depressed patients by using D.I.T.I. and compared with Bell's palsy patients and normal persons group.The results are as follows:Average body temperature of the depressed patient group is 36.62${\cdot}$0.58$^{\circ}C$ , Bell's palsy patients group is 36.59${\cdot}$0.34$^{\circ}C$ and that of the control group is 36.71${\cdot}$0.43$^{\circ}C$ . So there is no meaningful difference. The depressed patient group has higher temperature than the Bell's palsy patients group and control group by $\delta$T〉1.0$^{\circ}C$ at the following acupuncture points in these body parts - upper and lower, left and right, anterior and posterior. When acupuncture points temperature was compared superior and inferior part of the body, depressed patient group have meaningful difference at the GV-4 (Myung-moon) and also in the Bell's palsy patients group and control group.When acupuncture points temperature was compared left and right part of the body. ddpressed patient group have no meaningful difference and also in the Bell's palsy patients group and control group.When acupuncture points temperature was compared in the anterior and posterior part of the body, depressed patient group have meaningful difference at the Gv-4(Myung-moon) and also in the bell's palsy patients group and control group.From this study. we think that D.I.T.I. could be used to diagnose objectively on the depressed patients and useful to another psychoneurogenic diagnosis in oriental medicine.

  • Objective : This study designed to find out the effects of deep acupuncture in cerebrovascular hemiplegia. Methods : This study was performed on 30 patients with cerebrovascular hemiplegia to observe difference in temperature of skin surface between general acupuncture and deep acupuncture at Hapkok using digital infrared thermographic imaging(D.I.T.I.) Results : 1. Temperature of paralytic side is sigficantly higer $0.39{\pm}0.78^{\circ}C$ than that of contralateral side. 2. Thermal difference(${\Delta}T$) of paralytic side-contralateral side decreased $0.08{\pm}0.53^{\circ}C$ after general acupuncture, but there is no significant. $0.20{\pm}0.50^{\circ}C$ after deep acupuncture decreased significantly. So deep acupuncture is more effective than general acupuncture. 3. Thermal difference of paralytic side-paralytic side and contralateral side-contralateral side decreased significantly after acupuncture, and thermal difference of deep acupuncture on paralytic side-paralytic side decreased $0.42{\pm}1.07^{\circ}C$ more than that of general acupuncture. So deep acupuncture is more effective than general acupuncture. Conclusions: This study showed that deep acupuncture is more effective than general acupuncture In cerebrovascular hemiplegia, and further study is needed on clinical trials.

A Clinical case study for the diagnosis and assessment of abdominal cold hypersensitivity treatment by DITI (적외선 체열검사(DITI)로 복부냉증의 진단과 치료 경과를 평가한 단일 임상례)

  • Purpose: This study was performed to evaluate DITI (Digital Infrared Thermographic Imaging)'s potential for diagnosis and indicator of abdominal cold hypersensitivity treatment from the clinical case study. Methods: The patient was a 35-year-old woman who was suffered by general coldness, sweating and general pain. The patient was treated by Herb medicine, Acupuncture treatment, Moxa treatment and Physical Treatment. The progress of symptoms was evaluated by checking the change of VAS, and inspecting the DITI. We studied the statistical significance from the difference of DITI results before and after treatment by paired T-test using SPSS windows (version 12.0). Results: The general coldness, sweating and general pain of this patient have declined, especially the abdominal coldness improvement showing. As a results, the temperature of four abdominal sites(GV4,6,12,17) after treatment were statistically higher than those before treatment. With comparison of Indang, DITI difference of abdominal sites before and after treatment were statistically significant from paired T-tests. Conclusion: The results suggest that DITI can be used for the diagnosis and assessment of abdominal cold hypersensitivity treatment.

The Study About the Correlation between Assessment Instruments of Knee OA and DITI (퇴행성 슬관절염 평가도구와 적외선 체열촬영과의 상관관계 연구)

  • Objectives : The aim of this study is to validate the correlation between Assessment Instruments of Knee OA and digital infrared thermographic imaging(DITI) and to find out the methods to assess knee OA by DITI. Methods : 60 subjects were evaluated. They were asked to answer VAS pain scale, WOMAC, KHAQ and LFI. Correlation was assessed by examining the spearman's rank correlation coefficients. Results : VAS, WOMAC and LFI was correlated with not only the thermal difference between Ant. knee and Post. knee, but also the lowest temperature of Ant. knee. Conclusion: DITI value was useful for evaluating the degree of OA. And we should find out that decrease of pain is correlated with change of DITI value.

A Case of Peripheral Neuropathy after Acupuncture (의인성(醫因性) 말초신경병증(末梢神經病症) 치험(治驗) 1례(例))

  • Objective : The purpose of this study is to report the patient with peripheral neuropathy, who improved by Oriental medical treatment. Methods : The patient was managed by acupuncture, moxibustion, physical treatment and herbal medicine. We checked digital infrared thermographic imaging(D.I.T.I.), electromyography(E.M.G.), nerve conduction velocity(N.C.V.), deep tendon reflex(D.T.R.), sensory recover area and range of motion(R.O.M.) of knee & ankle. Results : After 6 week treatment, clinical sign(the movement and sense of leg) of improvement was appeared. Also E.M.G., N.C.V., D.T.R. and D.I.T.I. was recovered to nearly normal range. Conclusion : The results suggest that combination of acupunture, moxibustion and herbal medicine is good method for treatment of peripheral neuropathy. But further studies may be required to concretely prove the effectiveness of this methods for treating peripheral neuropathy.
