Concentration of Heavy Metals, Residual Pesticides and Sulfur Dioxide of before/after a Decoction - In Prescription of Digestive System - (전탕 전과 후의 중금속, 잔류농약 및 잔류이산화황의 농도변화 - 소화기계 약을 중심으로 -)

  • Objectives : To compare the contents of hazardous substances before/after a decoction. Methods : The heavy metal contents before/after a decoction were measured by Inductively Coupled Plasma Atomic Emission Spectrometer (ICP-AES) and mercury analyzer. In order to analyze pesticides in 6 samples we used simultaneous multi-residue analysis of pesticides by GC/ECD, which was followed by GC/MSD analysis to confirm the identity of the detected pesticide in each sample. In addition, the contents of sulfur dioxide (SO2) were performed by Monier-Williams distillation method. Results : 1. The mean values of heavy metal contents (mg/kg) for the samples were as follows: Samchulkunbi-tang (before decoction - Pb; 1.592, Cd; 0.155, As; 0.055 and Hg; 0.014, after decoction - Pb; 0.036, Cd; 0.002, As; not detected and Hg; 0.001), Yijin-tang (before decoction - Pb; 0.830, Cd; 0.077, As; 0.045 and Hg; 0.015, after decoction - Pb; 0.193, Cd; 0.010, As; not detected and Hg; 0.002), Banhabaikchulcheunma-tang (before decoction - Pb; 0.976, Cd; 0.164, As; 0.167 and Hg; 0.019, after decoction - Pb; 0.031, Cd; 0.003, As; 0.006 and Hg; 0.005), Pyungwi-san (before decoction - Pb; 2.162, Cd; 0.128, As; 0.061 and Hg; 0.018, after decoction - Pb; 0.080, Cd; 0.006, As; not detected and Hg; 0.005), Leejung-tang (before decoction - Pb; 1.480, Cd; 0.294, As; 0.034 and Hg; 0.012, after decoction - Pb; 0.064, Cd; 0.007, As; 0.007 and Hg; 0.002) and Kwibi-tang (before decoction - Pb; 0.907, Cd; 0.193, As; 0.085 and Hg; 0.020, after decoction - Pb; 0.072, Cd; 0.006, As; 0.004 and Hg; 0.002). 2. Contents (mg/kg) of sulfur dioxide ($SO_2$) before a decoction in Banhabaikchulcheunma-tang, Pyungwi-san, Leejung-tang and Kwibi-tang exhibited 3.5, 3.4, 3.8 and 12.4, respectively. However, contents of sulfur dioxide after a decoction in all samples were not detected. 3. Contents (mg/kg) of residual pesticides before/after a decoction in all samples were not detected. Conclusions : These results will be used to establish a criterion of heavy metals, residual pesticides and sulfur dioxide.

Agarwood's Domestic and International Research Trends and Literature Review of Herbal Medicine (침향(沈香)의 국내외 연구동향과 본초학에 대한 문헌고찰)

  • Objectives : To investigate the modern research achievements of agarwood and its association with the efficacy of herbal medicine based on the in vivo and in vitro activities of volatile compounds detected in agarwood. Methods : Databases such as PubMed and ScienceOn were searched for medicinal in vivo and in vitro activity studies on agarwood. They were categorized into "medicine and pharmacy" and "others not related to medicine and pharmacy," and the studies on medicine and pharmacy were organized according to active efficacy. The efficacy and virtue of agarwood as identified in the book (or herbal medicine/herbology) corresponded to modern medical terms and diseases in reference to the . Separately, the Gas Chromatography & Mass spectrometer (GC-MS) analysis results of agarwood from a total of 5 production areas of Aquilaria crassna from Vietnam, Indonesia, Malaysia, Myanmar, and Cambodia as identified in previous studies were consulted to search for research papers on the medicinal activity of the 13 compounds of the aromatic sesquiterpene family detected in agarwood, and they were categorized according to topic. Results : There were 123 studies on the medicinal activity of agarwood. Overall, 46 studies on single extracts of agarwood reported activities such as improving mental health, including anti-anxiety and stability, and antiallergic, antioxidant, antibacterial, and digestive system protective effects. In total, 59 papers on the activities of single compounds isolated from agarwood reported anti-inflammatory and mental health benefits. Separately, 13 compounds detected in agarwood, such as α-agarofuran and β-caryophyllene, were reported by 110 studies to have anticancer, stabilizing, antibacterial, and anti-inflammatory activities. There were also papers on the cultivation methods and resin formation conditions of agarwood trees unrelated to the efficacy of herbal medicine. Conclusion : Among the pharmacological papers, a total of 57 papers corresponded to the effects of agarwood in traditional herbal medicine, including 33 papers related to agarwood extracts and 24 papers analyzing 8 types of sesquiterpenes, such as β-caryophyllene and cedrol, from previous studies. Based on the research findings of each paper, it was possible to correlate the effects of agarwood in traditional herbal medicine with the achievements of modern pharmacological research. In addition, further research is anticipated in new areas related to traditional herbal medicine, including the improvement of mental health such as anti-depression, as well as activities related to anticancer, antioxidant, and hair growth.

A Study on the Comparison of One Step Method and Two Step Method to Improve Reporting of CA 19-9 Results (CA 19-9 결과보고 개선을 위한 One step 방식과 Two step 방식의 비교에 관한 연구)

  • Purpose: CA 19-9 is the most widely used tumor marker for the diagnosis of digestive system tumor, especially pancreatic and biliary tract cancer. This study was conducted to improve the result value near reference range by comparing the reagents of CA 19-9 one step method and two step method. In addition, it was intended to establish a standard for selecting reagents. Material and Methods: 120 patients who visited the National Cancer Center in 2023 were selected as subjects for this study. The reagents used in the study were CA 19-9 IRMA kits (Shinjin, Korea) and three types of reagents were compared. Two step method reagent that is currently being used (A), one step method reagent (B) and two step method reagent improved by request (C) were compared and regression analysis was performed on their data. And we also performed recovery test, linearity test and hook effect test for each reagent. Result: There were 46 cases of reagent B in which the concentration value was lower than the result measured in reagent A that was previously used, and 77 cases of reagent C. As a result of regression analysis of reagents A, B, and C, the coefficients of determination of reagents A and B, reagents A and C, and reagents B and C were 0.653, 0.577, and 0.875. In the recovery rate test and the linearity test, the results of all reagents were good, and in the hook effect test, reagent B showed a hook effect at a low value. Conclusion: The improved reagent C appears to have been improved based on the concentration value of reagent B, which the manufacturer judged to be more stable at low concentrations. The hook effect in reagent B can be a fatal reason for disqualification when selecting reagents in general patient samples which high-concentration samples appear frequently. The first improved reagent C will be able to be used once it is confirmed that it has more stability for various concentration values.

Clinical Analysis of the Pediatric Patients Seen in the Emergency Medical Center (응급의료센터 소아 환자들의 경향 분석)

  • Purpose : We studied a clinical analysis of pediatric patients who visited the emergency medical center of Yeungnam University Hospital to compare the characteristics of pediatric emergency patients after year 2000 with the previous studies. Methods : We reviewed 7,034 children under the age of 15 years who visited the emergency medical center of Yeungnam University Hospital during the 2 year period from January 2001 to December 2002, and then we performed a clinical and statistical analysis. We analyzed the pediatric patients according to gender, age, season, day of the week, time of the visit, the disease classification and the final disposition of the patients. Results : Among the patients who visited the emergency room, 15.6% of the total emergency patients were under the age of 15. The male to female ratio was 1.6 : 1. Among the 7,034 pediatric patients, the most common age group was between 1 year and under 3 years of age(26.9%). The peak seasonal incidence was early summer and spring, especially during June(11.2%) and May(10.6 %). The peak incidence day of the week was Sunday(24.8%) and the peak time when the emergency pediatric patients visited the emergency room was between 20 and 24 o'clock(28.8%). The distribution of diseases, according to ICD-10 system, were injury and poisoning(30.4%), diseases of the respiratory system(22.8%), and diseases of the digestive system(14.6%). 30% of total pediatric patients were admitted to the hospital. Conclusion : After year 2000, as compared with the previous studies, the proportions of emergency pediatric patients has decreased. The distribution of diseases was not much different from the previous studies and the proportions of non-urgent diseases, such as acute nasopharyngitis or acute gastroenteritis, were still high. These result have come about due to the declining birth rate and changes of the medical system in Korea.

A study on the degree of need of the knowledge of pathophysiology, clinical microbiology and mechanisms and effects of drugs in clinical nurses (기초간호자연과학의 병태생리학, 병원미생물, 약물의 기전과 효과 내용별 필요도에 대한 연구)

  • The purpose of this study was to define the content of the requisite knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs needed for clinical knowledge for nursing practice. Contents of knowlege on pathological physiology, clinical microbiology, and mechanisms and effects of drugs were constructed from syllabus of basic nursing subjects in 4 colleges of nursing, and textbooks. The degree of need of 72 items was measured with a 4 point scale. The subjects of this study were college-graduated 136 nurses from seven university hospital in Seoul and three in Chonnam Province, Kyungbook Province, and Inchon. They have been working at internal medicine ward, surgical ward, intensive care unit, obstetrics and gynecology ward, pediatrics ward, opthalmology ward, ear, nose, and throat ward, emergency room, rehabilitation ward, cancer ward, and hospice ward. The results were as follows : 1. The highest scored items of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice were side effects of drugs, anticoagulants, mechanisms of drugs, antihypertensive drugs, tolerance and addiction of drugs, interactions among drugs, hospital infection in the order of importance. The lowest scored item was structure of microorganisms. 2. The highest order of need according to unit was repair in tissue injury unit, definition etiology classification of inflammation in inflammation unit, transplantation and immunologic response in alterations in immunity unit, thrombus and thrombosis in disorders of cardiovascular function unit, gene disorders in genetic disorders unit, hospital infection in infection unit, virus in microorganisms unit, side reactions of drugs in introduction unit, anticonvulsants in drugs for central nervous system unit, local anesthesia in anesthesia unit, anticoagulants in drugs for cardiovascular system unit, anti-inflammatory drugs in antibiotics unit, anti-ulcer drugs in drugs for digestive system unit, and bronchodilators in drugs for respiratory system unit. 3. The common content of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs needed for all clinical areas in nursing were side effects of drugs, anticoagulants, interactions among drugs, and hospital infection. However, the degree of need of each pathological physiology, clinical microbiology, clinical microbiology, and mechanisms and effects of drugs was different depending on clinical areas. 4. Significant differences in the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice such as tissue changes due to injurious stimuli, degenerative changes of tissue, alterations in metabolism of carbohydrates, ischemia, hyperemia and congestion, hospital infection, structure of microorganism, classification of microorganism, bacteria, virus, antidepressants, antipsychotic drugs, antiemetic drugs, antiparkinsonism drugs, antianxiety drugs, antibiotics, tuberculostatics, antiviral drugs, antifungal drugs, parasiticides, antiulcer drugs, antidiarrheais, and anti constipation drugs were shown according to the work area. 5. Significant differences in the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice such as transplantation and immunologic response, alterations in the metabolism of uric acid, structure of microorganism, classification of microorganism, immunosuppressants, drugs for congestive heart failure were demonstrated according to the duration of work. Based on these findings, all the 72 items constructed by Korean Academic Society of Basic Nursing science should be included as contents of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs.

Comparison of School Nursing Phenomena at Elementary, Middle and High Schools by Applying ICNP (ICNP를 적용한 학교간호현상 및 특성과 초.중.고등학교의 학교간호현상 비교)

  • Purpose : this study purposed to investigate school nursing phenomena in Korea by applying ICNP, the international standard nursing classification system developed by ICN, and to compare school nursing phenomena at elementary, middle and high schools. Method : The subjects of this study are 110 nursing teachers from 82 elementary schools, 16 middle schools, and 12 high schools. In the survey, subjects were asked to score the degree to which phenomena and characteristics of Korean school nursing, identified in previous research, on a 5 point scale. Questionnaires were distributed and recovered by mail and email. The period of data collection was 6 months from July to December 2003. The general characteristics of schools and nursing teachers were represented with frequencies and percentages, the phenomena and characteristics of school nursing with the mean score of the questions, and the phenomena school nursing by school grade with ANOVA and Duncan's posterior analysis. Results : 1) As for the characteristics of schools according to school nursing phenomena related to human behavior, the mean score of questions on inadequate stress management was highest at 3.24 points followed by the score on inadequate weight control (3.23), inadequate eating habits (3.22), the risk of spine disorders (2.68), inadequate emergency management (2.62), inadequate response to sex -related problems (2.19), and smoking and drug use (1.85). 2) As for the characteristics of schools according to school nursing phenomena related to human function, the mean score of questions on oral health management was highest at 3.11 points followed by the score on the risk of digestive system disorder (2.87), improper eyesight management (2.81), the risk of respiratory system disorders (2.75), lack of sexual identity (2.52), and inadequate contagious disease control (2.12). 3) As for the mean score according to school nursing phenomena related to environment, the score of the risk of accidents in classroom was highest as 2.68 points followed by the score of the risk of accidents around the school (2.65), maladjustment to school (2.62), the risk of accidents outside the classroom (2.43), inadequate learning environment (1.83), the risk of exposure to socially and physically harmful environment factors (1.82), and inadequate waste disposal (1.77). 4) This study tested the mean scores of questions corresponding to each school nursing phenomenon in order to see if there is a difference in the school nursing phenomenon among elementary, middle and high schools, and performed Duncan's posterior comparison for school nursing phenomena. A significant difference was found at p<.1. According to the results, school nursing phenomena found to be significantly different among elementary, middle and high schools was smoking and drug use (p<.05), which appeared more problematic in high school than in elementary school. phenomena such as inadequate eating habits, inadequate weight control, inadequate response to sex-related problems and inadequate waste disposal were also found to be statistically different at p<.1; however, according to the result of Duncan's posterior comparison, no difference was found among groups in improper eating habits and improper response to sex-related problems, and a significant difference was found between middle and high schools in inadequate weight control and inadequate waste disposal. Conclusion: Based on the results of this study, it is necessary to plan school health projects focused on the characteristic school nursing phenomena that had high scores and to develop and execute school healthe projects in accordance with the characteristics of elementary, middle and high schools. Considering that the scores of school nursing phenomena related to human behavior are high, it is necessary to introduce school health promotion projects in a systematic way.

Morbidity Pattern of Residents in Urban Poor Area by Health Screening (도시 영세지역 주민의 건강진단 결과)

  • The purpose of the this study was to assess the morbidity pattern of urban residents in the poor area by health screening for the community diagnosis. The items of health screening were history taking and physical examination by medical doctor and hearing test, check blood pressure, test for hematocrit, liver function(sGOT, sGPT), urine sugar and protein, and chest X-ray. The examinee in health screening were 437 persons and they occupied 16.9% of total residents in the poor area. Male examinee were 129 persons(9.9% of total residents) and female examinee were 308 persons(23.9% of total residents). Age group of above sixty years old, 42.0% of total residents in the poor area were participated, but only 5.9% were participated in age group of 10 to 19 years old. Among the 437 examinee, 191 persons(43.7%) had one or more abnormal findings in health screening. In male 38.7% had abnormal findings, and some what lower than that of female(45.8%). Age group of above sixty years had most high rate of abnormal findings(69.8%), in contrast to age group of 10 to 19 years old (10.9%). Diseases of the digestive system was the most common and which occupies 23.7% of total abnormal findings. And diseases of the circulatory system occupied 19.7%. Low hematocrit(14.6% of total participants of 437 persons) occupies the most common abnormal findings for screening test(hematocrit, blood pressure, hearing test, sGOT/sGPT, urine protein and urine sugar, chest X-ray) and high blood pressure(10.1%) occupied second, third; hearing impairment (5.5%), fourth ; abnormal liver function (4.1%), fifth ; sugar in urine (2.3%), sixth ; protein in urine(1.4%) and lastly abnormal chest X-ray (0.9%). The positive rate of abnormal findings in health screening was very high compared with morbidity rate by health interview. It is supposed that some portion of this high rate is by selection bias in examinee in health screening specially high participating rate in older age, and the other portion is due to the low socioecomic status and bad environment of the residents of the poor area. These findings will be good information for the research and development of health care system in the urban poor area.

Effect of Dietary Supplementation of Plum or Omija on Growth Performance, Blood Biochemical Profiles and Antioxidant Defense System in Broiler Chickens (육계 사료 내 매실 및 오미자 첨가가 성장, 혈액 생화학 성분 및 항산화 작용에 미치는 영향)

  • To investigate the effect of dietary supplementation of freeze-dried plum (Prunus mume Siebold and Zucc., PMS) or omija meal (Schizandra chinensis Baill.; SCB) on growth performance, organ weights, blood biochemical profiles and antioxidant defense system, a total of 96, 3-day-old male broiler chickens were assigned to three dietary groups: (1) control diet, (2) control diet supplemented with PMS at 0.2%, (3) control diet supplemented with SCB at 0.2%. In vitro antioxidant activity, plum and omija extracts showed a significantly higher radical scavenging activity (RSA). In particular, omija extract showed much higher RSA than plum extract. Dietary addition of plum or omija did not affect body weight, feed intake, feed conversion and the relative weight of digestive organ in birds. Plasma triglyceride significantly (P<0.05) increased in birds fed the diet supplemented with omija compared with those fed control diet without affecting the other blood biochemical components. Furthermore, reduced form of glutathione (GSH) in the liver or muscle significantly (P<0.05) increased in birds fed the diet fortified with plum and omija. However, the specific activities of superoxide dismutase (SOD), glutathione peroxidase (GPX) and glutathione-S-transferase (GST) and MDA (malondealdehyde) in the intestine, liver and muscle were not altered by dietary antioxidant sources. In conclusion, dietary plum and omija resulted in a positive effect on some antioxidant indicators such as increased in vitro RAS in extracts and in vivo GSH level in the liver and muscle without affecting growth performance. Therefore, dietary addition of 0.2% of plum or omija could be applicable as potential antioxidant sources in broiler chick production.

A Comparative Analysis of the Level of Occupational Health : Before and After the Subsidiary Program on Health Care Management of Small Scale Industries (영세사업장 보건관리 지원사업 실시 전후의 산업보건수준 비교 분석)

  • The small scale industries which have less than 30 employees occupy 86.5% of total number of industries in Korea. And though they have higher accident rate and lower environmental condition than big industries, it has been not mandatory to appointing health care manager at factory. So, from 1993, government subsidizes to the health care management of small industries. The purpose of this study is to identify the real feature of health care status in small industries, and to evaluate the level of health care management, before and after the subsidiary program. 65 small plating industries which have been managed by the same health care management support institution in 1993 were selected for study. Of the 65 industries, 3 which have not taken both environmental evaluation and health screening in 1994, and 9 which have closed were excluded from study sample. And the remaining 53 were analyzed by using the results of environmental evaluation and health screening, reported to the Ministry of Labor, before and after the subsidiary program, the analysis was done by the comparison of the two year paired data of the same industry. Over-permissible-limit rate, health screening implementation rate, above grade C rate were calculated and compared. The status of health care management ; 1. Of the sample industries, 96.9% provide protective equipment and 80.0% set up ventilating system. Protective gloves (89.2%) and protective clothing (80.0%) are widely provided, but ear plugs (4.6%) are rarely provided. 21.5% of the protective equipment are well put on, and 40.4% of the ventilating systems function well. 2. In 1993, 35 industries, 53.8% of the sample, checked working environment twice. Over-permissible-limit rates of heavy metal (12.2%), suspended particle (11.1%), noise (5.5%) were high. To put on protective equipment and to set up local ventilating system were pointed out by the examiners. 3. General health screening was done at 63.1% of the sample industries and 35.3% of total workers were examined. Specific health screening was done at 93.8% of the sample industries and 75.4% of workers were examined. 15.5% of workers was provided to be above grade C and to have digestive system disease (43.3%), circulatory disease (18.9%), and hematopoietic disease (14.2%), etc. 4. In 1993, the subsidiary program of health care management was provided in forms of health education, health counseling, and rounding check of working field. And 61.5%, 83.0%, 55.4% of sample industries respectively received it. The average visit per industry was 1.8. Comparisons of the level of occupational health before and after the subsidiary program ; 1. Over-permissible-limit rates of hazardous factors of 1993 and that of 1994 were compared. The rates of suspended particle, noise, organic solvent of 1994 (37.5%, 13.4%, 24.2% respectively) were higher than that of 1993 (25.0%, 6.0%, 6.3% respectively). In the case of acid, there was no difference between the rate of 1993 and that of 1994. Only the rate of heavy metal decreased from 12.9% in 1993 to 3.0% in 1994. 2. General health screening was done at 38.7% of the sample industries in 1993 and at 44.6% in 1994. But the implementation rate of specific health screening decreased from 72.4% in 1993 to 64.6% in 1994. 3. The implementation rate of specific health screening was analyzed by some health factors. The rate of suspended particle increased from 61.8% in 1993 to 91.2% in 1994. But the rates of the others-noise, organic solvent, heavy metal, specific chemical substances-decreased. 4. Above grade C rate in health screening increased from 27.8% in 1993 to 35.5% in 1994. But that of endocrine disorders and pulmonary disease decreased.

Studies on Cropping System for Year-Round Cultivation of Forage Crops in Gyeongnam Province (경남지방에서 조사료 주년생산 작부체계에 관한 연구)

  • Present experiment was conducted at the field of Gyeongnam Agricultural Research and Extension Services in Jinju city for two continuous cropping seasons to develop several adaptable and valuable year-round forage-producing system for elevating self-sufficiency and dollar-saving by reduced importing of crude forage. Twenty cropping systems were tested in experiment using whole crop barley (WCB), oat, rye, Italian ryegrass (IRG), and triticale in winter season and com, sorghum, sorghum ${\times}$ sudangrass hybrid, and oat in summer time. Sorghum ${\times}$ sudangrass hybrid showed highest fresh forage yield among experimented summer season crops, and followed com. Com produced the most dry matter yield, and followed sorghum${\times}$sudangrass hybrid, sorghum and oat in order. There was no significant effect of former winter crops on fresh and dry matter production succeeding summer time crops. Among winter season forage crops tested, oat showed the highest fresh and dry matter when clipped on mid-May, and followed triticale, IRG, rye and WCB. Winter-time cultivated crops showed no clear effect on the growth and forage (fresh and dry matter) producing ability of following summer crops. There was the most protein content in oat plant among summer season planted crops, and in sorghum for acid detergent fiber (ADF) and in sorghum ${\times}$ sudangrass hybrid for neutral detergent fiber (NDF), respectively. While, com showed highest value of relative feed value (RFV) and total digestive nutrients (TDN) among those crops. Among winter crops, the highest crude protein was in oat plant showing no significant differences of ADF and NDF, while, relatively higher value of RFV was recognized with rye and triticale. Also, triticale contained more TDN as compare to other forage crops. The cropping combinations such as com followed by (fb) rye and maize fb triticale were regarded as promising systems having higher dry matter producing ability among tested combinations. Considering TDN producing potential, the combinations with sorghum ${\times}$ sudangrass hybrid fb triticale andlor rye were would be suitable ones, coincidently. There was a tendency which elevating pH, electric conductivity (EC) and organic matter (OM) contents in soil after experiment comparing to before planting. More crude protein content in plant was shown at mid-May clipping as compared to the forage at April cut in all winter season grown crops. ADF and NDF contents were increased by delayed clipping showing decreased tendency of RFV and TDN in plant. In conclusion, many cropping systems would be available using above mentioned forage crops according to farmer's conditions and scale, etc.
