일부 질환들에서 진료량-진료비, 재원일수와의 관계에 대한 연구 (The Study on Volume Relationships in Several Diseases)

  • Investigating the existence of volume-outcome relationships for specific disease groups relates directly to the policy issue of whether, and how, specific inpatient services should be regionalized. This study examined whether medical costs and lengths of stay as outcomes were affected by changes in volume within hospitals. Based on the claims data obtained from National Federation of Medical Associations, each six disease categories from medical and surgical conditions were selected and 29,720 cases from 1,266 hospitals were analyzed. Main findings of the research can be summarized as follows: 1. Analyzing volume and cost per case relationship, tonsillectomy class 1, hernia procedure class 0, appendectomy and cesarean section class 0,1 in surgical conditions showed negative relationship significantly. In cases of medical conditions, costs per case in respiratory neoplasm class 2, COPD class 1, 2, digestive malignancy were also related to volume negtively. 2. Comparing volume with length of stay per case, lens procedure class 0, hernia procedure class 0, appendectomy class 0,1, cesarean section class 1 in surgical conditions showed negative relationships significantly. In medical conditions, volume of respiratory neoplasm class 2, COPD class 1,2, digestive malignancy class 0 were associated with negatively. 3. Within same disease categories, changes in cost and length of stay per case to volume were more remarkable in severe cases. These results suggested a significant inverse relationship between disease cases and cost, length of stay per case as outcome variables.

  • Objectives : C V A(Cerebral Vascular Attack) is the severest and most important disease that makes doctors difficult in treating, also makes patients difficult to recover. I intended to research for imformations and standards of C V A patients of Sasang Constitutional Medicine through this clinical study. Methods : 106 C V A patients(61 men and 45 women) who had been inpatients from October 19, 1996 to August 19, 1999 in Conmaul Oriental Hospital were investigated and analysed the characteristics, past histories, symtoms, progress, relationship with other diseases for example DM and HTN, efficacy of oriental medical treatments. Results and Conclusions : The results of constitutional analysis showed 51 Taeumin(48.1%) 13 Soyangin(12.3%) 12 Soeumin(11.3%) 7 Taeyangin(6.6%). In the frequency of C V A, age of $60{\sim}69(43.4%)$ is major, both $50{\sim}59\;and\;70{\sim}79$ (17.9%) are next. and in pre-disease on the past, C V A is major, digestive disease and musuloskeletal disease are next. In chief complaint, kinetic disorder is major dyslogia is next. In efficacy of treatments, Slightly Improved(52.9%) is the most occupied part of that.

Combination Therapy with a PI3K/mTOR Dual Inhibitor and Chloroquine Enhances Synergistic Apoptotic Cell Death in Epstein-Barr Virus-Infected Gastric Cancer Cells

  • The phosphoinositide 3-kinase/protein kinase B/mammalian target of rapamycin (PI3K/AKT/mTOR) signaling pathway is a promising target for gastric cancer (GC) treatment; however the efficacy of PI3K/mTOR dual inhibitors in GC has not yet been maximized. Additionally, the effect of autophagy regulation by PI3K/mTOR dual inhibitors has not been clearly elucidated in GC treatment. We aimed to show that our newly developed PI3K/mTOR dual inhibitor, CMG002, when combined with an autophagy inhibitor, chloroquine (CQ), potently induces effective cancer cell death in Epstein-Barr virus (EBV)-associated gastric cancer (EBVaGC) cells, where both the PI3K/AKT/mTOR and autophagy pathways play important roles in disease pathogenesis. EBV- and mock-infected AGS and NUGC3 GC cell lines were treated with CMG002 +/- CQ. PI3K/AKT/mTOR signaling pathway mediators, cellular apoptosis and autophagy markers were confirmed by Western blot assay. Cell viability was assessed using the Cell Counting Kit-8 (CCK-8) assay. CMG002 effectively blocked the PI3K/AKT/mTOR pathway by markedly decreasing phosphorylation of AKT and its downstream mediator S6. CMG002 induced G0/G1 cell cycle arrest and enhanced apoptotic cell death in AGS and NUGC3 cells, particularly EBV-infected cells compared with mock-infected cells, as confirmed by flow cytometric analyses and TUNEL (terminal deoxynucleotidyl transferase dUTP nick end labeling) assays. The combination of CMG002 plus CQ synergistically increased apoptotic cell death in EBV-infected GC cell lines when compared with CMG002 alone (P < 0.05). Our results suggest that the new PI3K/mTOR dual inhibitor, CMG002, when used in combination with the autophagy inhibitor, CQ, provides enhanced therapeutic efficacy against EBVaGC.

Expansion of Indication for Endoscopic SD in Early Gastric Cancer

  • Endoscopic resection is now accepted as curative treatment modalities for early gastric cancer without lymph node metastasis. However, based on large-scaled data about the risk of lymph node metastasis in early gastric cancer and as a result of the technical development of endoscopic devices, it was suggested that the criteria for endoscopic resection should be extended. According to the treatment guidelines for gastric cancer in Japan, the extended indications include the following: differentiated-type mucosal cancer without ulceration and greater than 2 cm in diameter, differentiated-type mucosal cancer with ulceration and up to 3 cm in diameter, undifferentiated-type mucosal cancer without ulceration and up to 2 cm in diameter, and, in the absence of lymphovascular invasion, a tumor not deeper than submucosal level 1 (less than $500\;{\mu}m$). In this review, we discuss the evidence of the application of expanded endoscopic indication based on analysis of biologic behavior and data of endoscopic resection.

Endoscopic Resection for the Treatment of Superficial Esophageal Neoplasms

  • Superficial esophageal neoplasms (SENs) are being diagnosed increasingly frequently due to the screening endoscopy and advances in endoscopic techniques. Endoscopic resection (ER) is a relatively noninvasive treatment method with low morbidity and mortality that provides excellent oncologic outcomes. Endoscopic submucosal dissection is associated with higher rates of en bloc, complete and curative resections and lower rates of local recurrence than endoscopic mucosal resection. The most serious complication of ER is stricture, the treatment and prevention of which are crucial to maintain the patient's quality of life. ER for SEN is feasible, effective, and safe and can be considered a first-line treatment for SENs in which it is technically feasible.

한방클리닉에 내원(來院)한 감기환자에 대한 조사연구 (A Study on Common Cold Patients utilizated of Korean Medical Clinic)

  • Objectives : A common cold is a viral disease. There exists only allopathy for it depending on symptoms because of no cure. Korean medicine has taken a serious view of common cold patients with digestive problems as well as patients of the common cold. Accordingly, this study is aimed at finding out the traits of patients with digestive problems among common cold patients. Methods : A survey was conducted with 62 cold patients who got medical treatment in Korean medicine clinics located in small cities and towns including metropolitan cities. The SPSS for Windows( 19.0 Version) was used for statistical analysis. Results : Depending on which clinic common cold patients decided to go to, there was a significant difference in the number of sick days. The group of patients who decided to go straight to oriental medicine clinics had fewer sick days. Only 9.7% of the group of patients who got treatment in Koean medicine clinics had more than 6 sick days. The group of common cold patients with digestive problems had more sick days than the group with no digestive problems, which showed that there was a significant correlation between digestive problems and sick days. When it came to digestive problems depending which clinic common cold patients decided to go to, there was no significant value because of little data collection. However, while 41.9% of the group who went to hospital at first and then to Koean medicine clinic had digestive problems, 24.2% of the group who directly went to Koean medicine clinic had digestive problems. On the whole, 66.1% of the entire common cold patients were shown to have digestive problems. Conclusion : There are relatively many patients with digestive problems among common cold patients, and there is a significant correlation between digestive problems and sick days on common cold.

소아과(小兒科) 외래(外來) 환자(患者)의 주소증(主訴症)에 관(關)한 연구(硏究)(동국분당한방병원 : 1997년 1월 1일 ${\sim}$ 1997년 12월 31일) (An Observation of the Chief Complaints of Pediatric Outpatients)

  • BACKGROUND : The purpose of this study was to investigate the new effective oriental medical treatments in pediatric diseases and its clinical applicability. METHOD : The study was composed of 1725 new patients who had been treated at the pediatric unit in the Dongguk Bun-Dang Oriental Medicine Hospital for 1 year, from 1 January 1997 to 31 December 1997, and had ages between 0 and 18 years. The chief complaint was mainly categorized by oral examination on patients and their care-givers. RESULT 1. The ratio of male to female was 1.35:1 and the number of each age group and its percentage distribution was 228 patients (13.2%) for the age group between 0 and 1 year, 746 patients (43.2%) between 2 and 6 years, and 751 patients (43.6%) for the age group over 7 years. 2. The chief complaint of the new outpatients was weakness, asthma, common cold, sinustis, skeletal disease, atopic dermatitis, stomach-ache, short stature, diarrhea, nasal blooding, night terror, allergic rhinitis, gastric disorder, enuresis, dermatitis, strabismus in the order of majority.3. The number of patients and its percentage for the most three complaints was 494 patients (28.6%) for weakness, 647 patients (37.4%) for respiratory disease, 144 patients (8.3%) for digestive disease. 4. The digestive disease tended to increase in summer, and appeared mostly in the age group between 0 and 2 years. The respiratory disease increased In inter-season and winter, but decreased significantly in summer. The age group between 1 and 5 years was the largest group of respiratory disease. CONCLUSION 1. The chief complaint in pediatric diseases that needed an oriental medical treatment was mainly the disease that tends to take long time and the weakness, and appeared frequently in digestive and respiratory diseases. 2. The oriental medical treatment was still preferred as a way to improve the weakness by patients, rather than a way to overcome their disease. In particular, the study shows that the oriental medical treatment should be emphasized in terms of preventing the disease. 3. The new diseases, which were developed with the change of human life and environment, (atopic dermatitis, enuresis, short stature, obesity, dysthymic disorder, strabismus), should be investigated as a new field of oriental medical treatment.

Eosinophilic Esophagitis and Eosinophilic Gastroenteritis: Similarities and Differences

  • Eosinophilic gastrointestinal disease (EGID), a chronic allergic condition characterized by dense infiltration of eosinophils in the digestive tract, is classified into two types, eosinophilic esophagitis (EoE), which features dense infiltration of eosinophils in the esophageal epithelial layer, and eosinophilic gastroenteritis (EGE), in which the entire digestive tract including the esophagus may be involved. Patients with EoE only have esophageal symptoms, since the other parts of the digestive tract are not involved. On the other hand, 80% of EGE patients have lesions in the small intestine. The esophageal epithelial layer in healthy individuals has no or negligible infiltration by eosinophils, while the small intestinal mucosal layer, especially the distal small intestinal mucosa, can show dense eosinophil infiltration even in the absence of disease. Therefore, histological changes observed in cases of EGE are not qualitative but rather quantitative, as compared to EoE, which has qualitative histopathological changes, indicating important pathogenetic differences between the types. Comparisons of clinical, laboratory, and morphological characteristics between EoE and EGE have revealed several interesting differences. Both EoE and EGE patients are frequently affected by atopic diseases, such as bronchial asthma and allergic rhinitis, and elevated plasma levels of Th2 type cytokines and chemokines are also similarly seen in both. On the other hand, age at diagnosis differs, as the former is generally found in individuals from 30 to 50 years old, while the latter appears in all age groups. Additionally, 80% of patients with EoE are male as compared to only 50% of those with EGE. Furthermore, approximately 60% of patients with EoE respond favorably to proton pump inhibitor (PPI) administration, whereas EGE patients rarely show a response to PPIs. Nevertheless, both diseases show a similarly favorable response to a six foods elimination diet and glucocorticoid administration. These similarities and differences of EoE and EGE provide important clues for understanding the pathogenesis of these EGID types.

이진탕가미방(二陳湯加味方)을 투여한 불응성 위식도역류질환의 증례군 연구 (Case Series of Refractory Gastroesophageal Reflux Disease Treated with Lijin-tang-gamibang)

  • Objectives: The purpose of this study was to investigate the efficacy of Lijin-tang-gamibang on refractory gastroesophageal reflux disease (GERD). Methods: This before-and-after study compared the first medical examination and examination at the end of medical treatment. Twelve refractory GERD patients who visited the Department of Digestive Diseases of Kyung Hee University Korean Medicine Hospital from August 14th, 2017 to October 14th, 2017, were treated with Lijin-tang-gamibang for four consecutive weeks. The clinical characteristics of refractory GERD and the efficacy of herbal medical treatment was assessed by questionnaires, the Nepean Dyspepsia Index-Korean version (NDI-K), the Korean Gastrointestinal Symptom Rating Scale (KGSRS), the Numerical Rating Scale (NRS), the Korean version of the Perceived Stress Scale (KPSS-10), the Qi Stagnation Questionnaire (QSQ), the Spleen Qi Deficiency Questionnaire (SQDQ), and the Patient Global Impression of Change (PGIC). Prolonged effects were reported upon the follow-up telephone survey two weeks after treatment. Results: After treatment with the herbal medicine Lijin-tang-gamibang, the clinical characteristics of 12 refractory GERD patients were improved, especially in terms of the most common symptoms of acid regurgitation and heartburn. Each symptom score of NDI-K, KGSRS, NRS, SQDQ, and PGIC showed significant advances. Prolonged effects were reported in NDI-K, NRS and PGIC questionnaires two weeks after treatment. Conclusions: These results suggest that Lijin-tang-gamibang is an effective treatment for refractory GERD.
