Clinical Study of the Pediatric Patients Who Had Visited Emergency Room(ER) at Oriental Medical Hospital (한방병원 응급실에 내원한 소아 환자에 관한 임상적 고찰)

  • Objectives: By analyzing data of pediatric patients who had visited the emergency room of an oriental medical hospital, we can understand their characteristics and diseases. The purpose of this study was to introduce the excellence of Oriental medicine, to develop various treatments and to revitalize pediatric emergency care at oriental medical hospital. Methods: The study was composed of 371 pediatric patients who had visited the emergency room of the $\bigcirc\bigcirc$ university oriental hospital from January 2008 to December 2009. Results: 1. Average age of the pediatric patients was 4.28 years old, and it has showed that 1 to 3 years old patients (36.7%) were the most common age. 2. The number of pediatric patients was increased in June. According to the weekly distribution data, the number of pediatric patients who had visited on Sunday was the most (29.1%). Also, the number of pediatric patients who had visited ER for 18 to 21 hours(35.6%) were the most common. 3. The major problems of hospitalization were digestive symptoms and nervous symptoms. The nervous symptoms were the most at infancy. The musculoskeletal symptoms were the most common in adolescence. The digestive symptoms were common in other stages of development. 4. The time interval between arrival and onset; within 6 hours were the most(48.0%). Acupuncture and herbal medication treatment(75.2%) were the most common medical treatments. Most of the pediatric patients(97.3%) were discharged after medical treatments. Conclusions: Pediatric patients who had visited emergency room at the oriental medical hospital were mostly not due to acute form of serious diseases. The most common disease states that have preferred to treat with oriental medicine were dyspepsia, crying, febrile fit, and ankle sprain. We have to introduce the excellence of oriental medicine, and we need to try to develop other treatments such as magnetic acupuncture, moxa therapy, aroma therapy and revitalize pediatric emergency care at oriental medical hospital.

  • Foods and related substances from diverse sources known to have a potential for disease risk reduction are called functional foods, while nutraceuticals are bioactive compounds isolated from food and sold in dosage form. Nutraceutical and functional food industries are rapidly growing in recent years and most of the cases development of these functional materials involves certain biotransformation processes. A number of bioactive compounds has been identified up to date and isolated from seafood related products through enzyme-mediated hydrolysis. The enzymatic bioconversion process require suitable biocatalysts and appropriate bioreactor systems to incubate byproducts with digestive enzymes. Membrane bioreactor technology is recently emerging for the development of bioactive compounds from seafood processing byproducts.

Retrospective Survey on the Mortality of Non-human Primates at Everland Zoological Gardens(1976-1999) (에버랜드 동물원에서 사육중인 영장류의 폐사원인 분석(1976-1999))

  • The mortalities of the monkeys that have been raised in Everland Zoological Gardens within a designated period (1976-1999) were retrospectively analyzed based on the clinical charts and/or autopsy reports. During that period, a total of 161monkeys from 8 species were died. Noninfectious and traumatic factors, respiratory and digestive disorders account for the majority of cause of death. 62% of the death was concentrated in the winter season. The mortality of female and male was 53 and 47%, respectively. The mortality of adult monkey(62%) due to neoplasia and senile changes.

A Case Report of VulvoVaginal Candidiasis treated by Dahuanghuanglianxiexin-tang based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 대황황련사심탕(大黃黃蓮瀉心湯)을 투여한 칸디다성 질염 치험 1례)

  • Objective : This case report show the effect of Dahuanghuanglianxiexin-tang on VulvoVaginal Candidiasis. Methods : According to disease pattern identification diagnostic system based on shanghanlun provisions, The author diagnosed Taeyang-byung kyeolhyung. The symptoms of VulvoVaginal Candidiasis were worsen when the patient had long sitting position and digestive problems. Therefore the patient treated with Dahuanghuanglianxiexin-tang for 3 months. The severity of Candida Vaginitis was evaluated by patient's subjective statements(Vaginal pruritus, Vaginal secretion, Burning sensation). Results : After the treatment, Level of syptoms decreased from Severe to Trace and Eliminated. Conclusions : Dahuanghuanglianxiexin-tang improved the symptoms of VulvoVaginal Candidiasis on this study. Although Dahuanghuanglianxiexin-tang is not an usual treatment for VulvoVaginal Candidiasis, the author could prescribe the medicine due to shanghanlun six meridian patterns diagnostic system.

  • The mortalities of exotic felids that have been raised in Everland Zoological Gardens within a designated period (1976∼2001) were retrospectively analyzed based on the clinical charts and/or autopsy reports. During that period, a total of 154 exotic folios from 5 species were died. Noninfectious and traumatic factors, respiratory and digestive disorders account for the majority of cause of death. 40% of the death was concentrated in the winter season. The mortality of female and male was 55% and 45% respectively. The mortality of newborn folios(57%) were significantly higher than that of adult(32% and juvenile felids(11%) due to neonatal problems.

  • Objectives: In traditional medicine, Wolgukwhan has been used for the treatment of digestive system disease, such as indigestion, brash, ructation, nausea and vomiting. This study was purposed to investigate the effects of Wolgukwhan methnol extract (WGWM) on oxidative liver cell injury. Methods: In vivo assay, we administerated acetaminophen(500mg/kg, i.p.) to starved mice 24hrs after pretreatment of WGWM for 6days. In the liver homogenates, lipid peroxide and glutathione(GSH) levels were measured. In addition, activities of hepatic enzyme, such as catalase, glutathione peroxidase(GPX), glutathione S-transferase(GST) were measured in the hepatic mitochondrial and cytosolic fractions. Results: In vivo administeration of WGWM showed effective inhibition of acetaminophen induced lipid peroxidation and elevations of glutathione level. The acetaminophen treatment resulted in a decrease of catalase, GPX and GST activities. By contrast, WGWM pretreatment increased compare to those of untreated groups. Conclusions: These results suggested that WGWM might protect against lipid peroxidation by free radicals, destruction of hepatic cell membranes.

Position Detection of a Capsule-type Endoscope by Magnetic Field Sensors (자계 센서를 이용한 캡슐형 내시경의 위치 측정)

  • Development of a locomotive mechanism for the capsule type endoscopes will largely enhance their ability to diagnose disease of digestive organs. As a part of it, there should be provided a detection device of their position in human organs for the purpose of observation and motion control. In this paper, a permanent magnet outside human body was employed to project magnetic field on a capsule type endoscope, while its position dependent flux density was measured by three hall-effect sensors which were orthogonally installed inside the capsule. In order to detect the 2-D position data of the capsule with three hall-effect sensors including the roll, pitch and yaw angle, the permanent magnet was extra translated during the measurement. In this way, the 2-D coordinates and three rotation angles of a capsule endoscope on the same motion plane with the permanent magnet could be detected. The working principle and performance test results of the capsule position detection device were introduced in this paper showing that they could be also applied to 6-DOF position detection.

A Case of Small Bowel GIST Initially Suspected as Peritoneal Seeding of Gastric Cancer

  • Gastrointestinal stromal tumors (GISTs) constitute the most common primary mesenchymal tumors of the digestive tract and characteristically express c-kit (CD117). GISTs are the most common non-epithelial tumor of the GI tract and frequently originate from the stomach and small bowel. Specifically, the synchronous occurrence of a GIST with other epithelial tumors is rarely reported. Recently, we discovered one case of a concurrent gastric cancer and a small bowel GIST that was initially suspected to be peritoneal seeding from gastric cancer. The patient was initially admitted with epigastric pain. Gastric cancer with peritoneal seeding was suspected after an evaluation. Following a laparoscopic examination, a distal gastrectomy with D2 lymph node dissection and small-intestine segmental resection was performed. The final pathologic diagnosis was early gastric cancer and high-risk small bowel GIST. The patient refused adjuvant therapy for the GIST, and currently shows no other marked indisposition. He has been disease-free for 14 months.

The Clinical Observation on Cases of Chronic Paranasal sinusitis (鼻淵에 應用되는 補中益氣湯 加減方의 治驗例)

  • We know paranasal sinusitis is one of the most common disease in the field of otolaryngology. Especially in the case of chronic paranasal sinusitis, patient is sufferred from recurrence. So we prescribed Bojungikgitang(補中益氣湯) to the patients who were suffered from chronic paranasal sinusitis, and we witness some noticeable improvement. Here are the findings of my experience 1. The addition and subtraction temperament drugs of Bojungikgitang(補中益氣湯加減方) are effective to the patient of chronic and feeble paranasal sinusitis by the effect of removing wind(祛風), unnecessary metabolite(祛痰), and draining pus(排膿) and strengthening digestive organs(健碑) and enriching the lung yin(益肺陰) and alleviation of pain(止痛). 2. In the symptom of chronic paranasal siunutitis, nasal obstruction, postnasal drip, sputum, headache were quick to disappear, and dysosmia, internasal polyp, etc got better slower. 3. The most quick improvement appeared within two weeks and most symptoms improved within one month, and disappeared within two months.

Small Bowel Tumors and Polyposis: How to Approach and Manage? (소장 종양과 용종증: 접근 방법과 관리)

  • Although small bowel the mainly occupies the most part of the gastrointestinal tract, small intestine tumors are rare, insidious in clinical presentation, and frequently represent a diagnostic and management challenge. Small bowel tumors are generally classified as epithelial, mesenchymal, lymphoproliferative, or metastatic. Familial adenomatous polyposis and Peutz-Jeghers syndrome are the most common inherited intestinal polyposis syndromes. Until the advent of capsule endoscopy (CE) and device-assisted enteroscopy (DAE) coupled with the advances in radiology, physicians had limited diagnostic examination for small bowel examination. CE and new radiologic imaging techniques have made it easier to detect small bowel tumors. DAE allows more diagnosis and deeper reach in small intestine. CT enteroclysis/CT enterography (CTE) provides information about adjacent organs as well as pictures of the intestinal lumen side. Compared to CTE, Magnetic resonance enteroclysis/enterography provides the advantage of soft tissue contrast and multiplane imaging without radiation exposure. Treatment and prognosis are tailored to each histological subtype of tumors.
