체계적인 식품 위해 정보 수집.공유 체계 구축 방안 (Strategies for Establishing of the Systemic Collecting and Sharing of Food Risk Information)

  • Food safety has been a growing consumer concern over the last few decades, and remains a priority for consumers, the food industry, and regulatory agencies alike. Although consumer concern for food safety has increased, consumer confidence has decreased. The emphasis on food safety is related to that of preliminary risk management. The ability to collect and provide food risk information is a key element in enhancing the way food safety authorities protect consumers from risk. This review aims to investigate the current situations of international organizations, as well as several countries' systems for collecting and providing food risk information. Through the comparison and analysis of each system, this review proposes strategies to establish a systematic collecting of information and provision of infrastructure in Korea. To develop an information collection system suited to Korea's situation, it is necessary for Korea to strengthen interactions and cooperation with other trade partners through the enlargement of international networks. Such efforts on food risk communication should be made by providing high quality and clear information.

Estimation of dietary intake and human health risk of hexachlorobenzene by marine organism consumption in Korea

  • Hexachlorobenzene (HCB) was analyzed in various marine organisms of Korea. HCB was detected in all organism samples with residual concentrations from 0.51 to 222 pg/g wet weight. HCB residue was the highest content in crustacean, and followed by bivalves, fish, cephalopods and gastropods. The residues were comparable to or lower than those in marine organisms of other countries. Daily dietary intake of HCB from seafood was estimated to be 13.4 pg/kg body weight/day. The relative contribution of taxonomic group to the total dietary intake of HCB were in the order of crustaceans $(40.1\%)$, bivalves $(34.2\%)$, fish $(23.1\%)$, cephalopods $(2.22\%)$, and gastropods $(0.38\%)$. Daily dietary intake of HCB expressed as toxic equivalent (TEQ) value was estimated to be $1.34\times10^{-3}pg$ TEQ/kg body weight/day. This value did not exceed tolerable daily intake (TDI) proposed by the WHO, the UK toxicity committee and the KFDA. Cancer risk and target hazard quotient (THQ) due to the consumption of the marine organism in Korean adult population were evaluated using the exposure equation of food ingestion. This result suggests that dietary intake of HCB by the consumption of Korean seafood seems to be safe for human health with negligible cancer and non-cancer risks so far.

담석 형성에 영향을 주는 식생활 관련 요인 연구 (Study on Dietary Risk Factors in Gallstone Formation)

  • It has been reported frequently that clinical features of gallstone disease in Korean were similarly changing to those of Occidentals. This changing was thought to be due to Westernized lifestyle and dietary patterns in Korean. The purpose of this study was to investigate the nutritional risk factor among patients with gallstone. The subjects of this study were 90 gallstone group with biliary stone as confirmed by cholecystectomy and 111 control group with no biliary stone as confirmed by ultrasonography adjusted according to age and gender. Anthropometric indices and biomarkers were measured and dietary habit as well as nutrient consumption pattern were investigated using a structured checklist of health-related eating behavior and a semi-quantitative food frequency questionnaire. The mean age was higher in GG (gallbladder stone patient group) than CG (control group). The average BMI and WHR (waisthip ratio) tended to be higher than those in CG. WC (waist circumference) and WHR of women in GG were significantly higher than in CG. GG had lower levels in TC (p < 0.01), LDL-C (p < 0.05), and HDL-C (p < 0.05), but higher FBG levels (p < 0.001). GG tended to be associated with lower physical activity and more frequent consumption of meat, meats with high fat and high sugar content food. The consumption levels of fiber (p < 0.05), vitamin C (p < 0.05), calcium (p < 0.01) in GG were significantly lower than in CG. These findings showed that the association with incidence of gallbladder stone and anthrophometric indices and dietary consumption patterns. Further study may be necessary to elucidate the dietary risk factors in the changing patterns of gallstone disease.

코로나19로 인한 가정경제 변화와 청소년 식습관의 연관성 - 제17차 청소년건강행태조사 이용 - (The Association between Household Economic Changes by COVID-19 and Dietary Habits in Adolescents - Based on the 17th Korea Youth Risk Behavior Survey -)

  • The purpose of this study was to investigate the association between household economic changes by COVID-19 and dietary habits in Korean adolescents. The study analyzed data from the 17th Korea Youth Risk Behavior Survey in 2021. A total of 54,848 middle and high school students, 28,401 males and 26,447 females, participated in this study. COVID-19 lead to deterioration of household economy in the lower level of household economic status (p<0.001). The deterioration of household economic status by COVID-19 was associated with dietary habits such as lower breakfast intake, and higher fast food, soda drinks, and sweet drinks consumption (p<0.001). Logistic regression analysis showed that deterioration of household economic status was significantly decreased in frequency of breakfast intake (OR: 0.73, 95% CI: 0.67~0.80). The deterioration of household economic status by COVID-19 was also significantly associated with increased frequency of fast food (OR: 1.28, 95% CI: 1.16~1.41), soda drinks (OR: 1.42, 95% CI: 1.30~1.56) and sweet drinks (OR: 1.35, 95% CI: 1.24~1.46) consumption (p<0.001). In conclusion, it was confirmed that the change in household economy caused by COVID-19 was associated with dietary habits in adolescents. As the household economy deteriorated, there was an increase in undesirable dietary habits including skipping breakfast and fast food consumption.

부산시교육청 영양교육체험센터 식생활 진단 시스템을 활용한 초등학생의 체중급별에 따른 영양·식생활 위험요인 비교 (Examining Nutritional and Dietary Risk Factors Across Weight Classes in Elementary School Students using Busan Office of Nutrition Education Center's Dietary Diagnosis System)

  • This study was undertaken to analyze the growth, nutritional, and dietary risk factors of elementary school students belonging to the Busan Metropolitan City Office of Education and provide the basic data needed to develop an underweight and obesity prevention program. In 2021, BMI and Dietary Screening Test (DST) data of 4,046 children surveyed by the Nutrition Education Experience Center's "Diagnosis System" of the Busan Regional Office of Education were analyzed. The DST consists of 36 questions about lifestyle habits, meal quality, meal regularity, snack quality, and eating behavior. Of the children included, 6.8% were underweight, 65.4% were normal weight, 13.4% were overweight, and 14.4% were obese. Children in the obesity group had shorter sleep and meal times (P<0.001), lower vegetable and fruit consumption frequencies (P<0.001), higher fast food consumption frequencies (P<0.001), higher rates of skipping meals (P<0.01) and breakfast (P<0.001), and more frequently used smartphones and watched TV during meals (P<0.001). The underweight group had the highest scores for all eating development factors but more frequently had chewing and swallowing difficulties (P<0.001). The study confirms underweightedness and obesity are present different problems and indicates that nutrition teachers should conduct accurate studies on the eating habits and behaviors of obese and underweight students and provide individually tailored nutritional counseling.

Associations between food consumption/dietary habits and the risks of obesity, type 2 diabetes, and hypertension: a cross-sectional study in Jakarta, Indonesia

  • BACKGROUND/OBJECTIVES: This study aimed to assess the current mean daily intake of 10 food groups, analyze the sociodemographic factors associated with food consumption, and determine the associations between food consumption/dietary intake and the prevalence rates of obesity, type 2 diabetes (T2D), and hypertension (HTN) in Jakarta, Indonesia. SUBJECTS/METHODS: A total of 600 participants aged 20-85 yrs were included in this cross-sectional study. Food consumption and dietary habits were assessed using a food frequency questionnaire. To determine the association between food consumption/dietary habits and the abovementioned diseases, logistic regression analysis was performed. RESULTS: The average vegetable and fruit intake was lower, while sugar and salt consumption were higher than that recommended by Indonesia's national dietary guidelines. A high intake of ultra-processed foods (UPFs) was associated with young age, men, "single" status, a high education level, and employment with a high monthly income. Obesity and T2D were positively correlated with high intakes of cereals and tubers, UPFs, sugars, fats, and oils. Conversely, an inverse association was found between legume, vegetable, and fruit consumption and obesity risk. An inverse correlation was also observed between vegetable consumption and T2D risk. Moreover, a high salt intake was inversely correlated with fruit consumption in terms of HTN risk. Non-indulgence in habitual late-night snacking and refrainment from consuming more than one dish at each meal were also negatively related to the prevalence of obesity, T2D, and HTN. Inverse correlations were also observed between the prevalence rates of T2D and HTN and abstaining from adding sugar to beverages. CONCLUSION: Foods high in fat, sugar, and sodium were strongly associated with the risks of obesity, T2D, and HTN. Additionally, poor eating habits were also associated with disease development.

Dietary Factors and Risk of Pancreatic Cancer: a Multi-Centre Case-Control Study in China

  • Background: Pancreatic cancer is the sixth leading cause of cancer death with an increasing trend in China. Dietary intake is believed to play an important role in pancreatic cancer carcinogenesis. The aim of this paper was to evaluate associations between some dietary factors and risk of pancreatic cancer in a multi-centre case-control study conducted in China. Materials and Methods: Cases (n=323) were ascertained from four provincial cancer hospitals. Controls (n=323) were randomly selected from the family members of patients without pancreatic cancer in the same hospitals, 1:1 matched to cases by gender, age and study center. Data were collected with a questionnaire by personal interview. Odds ratios (OR) and 95% confidence intervals (95%CI) were estimated using conditional logistic regression. Results: Tea intake (OR =0.49; 95%CI: 0.30-0.80) was associated with a half reduction in risk of pancreatic cancer. Reduced vegetable consumption (P trend: 0.04) was significant related to pancreatic cancer. Although no significant association was found for meat and fruit, ORs were all above or below the reference group. A protective effect was found for fruit (OR=1.73 for consumption of 1-2 times/week vs more than 3 times/week; 95%CI: 1.05-2.86). A high intake of meat was associated to a higher risk of pancreatic cancer (OR=0.59 for consumption of 1-2 times/week vs. more than 3 times/week; 95%CI: 0.35-0.97). Conclusions: The present study supports fruit consumption to reduce pancreatic cancer risk and indicates that high consumption of meat is related to an elevated risk. Direct inverse relations with tea and vegetable intake were also confirmed.

심리측정 패러다임을 이용한 대구 지역 주부들의 식품 위해요소에 대한 위험 지각 수준 및 우려도에 영향을 미치는 요인 분석 (Analysis of Levels of Risk Perception using Psychometric Paradigm and Factors Affecting Concerns about Food Risk Elements of Housewives in Daegu)

  • This study examined the levels of risk perception of food risk elements by adopting a psychometric paradigm and analyzed factors affecting concerns about food risk elements to obtain basic materials for food safety policy. The data were collected from 296 housewives in Daegu, Korea, by a self-administered questionnaire. Frequency distributions, Pearson's correlation coefficient analysis, factor analysis, Cronbach's ${alpha}$, and multiple regression analyses were conducted by SPSS 21.0. The mean level of concern for food safety was 3.75/5.00 points, and the mean percentage of correct answers about heavy metal contamination was highest among food risk elements. The respondents perceived radioactive contaminated foods, GM foods, and endocrine disruptors as a new, delayed, scientifically unknown, involuntary, serious, and uncontrollable risk in risk perception. According to the result of factor analysis for risk perception, two factors such as non-controllability and dread were categorized. In the risk perception map, radioactive contaminated foods and GM foods were considered as an uncontrollable and dreaded risk, heavy metal contamination, endocrine disruptors, and pesticide residues as a controllable and dreaded risk, and foodborne illness and food additives as a controllable and less dreaded risk. On the other hand, the levels of concerns about food risk elements were higher in order of radioactive contaminated foods, GM foods, and endocrine disruptors. The results of multiple regression analysis showed that age, concern for food safety, percentage of correct answers about food risk elements, non-controllability, and dread influenced the concerns about food risk elements. These results imply that food safety policies should consider differences in consumer's risk perception of food risk elements.

고식이섬유 및 고탄수화물 섭취와 대사질환과의 연관성 (High fiber and high carbohydrate intake and its association with the metabolic disease using the data of KNHANES 2013 ~ 2017)

  • 본 연구는 2013 ~ 2017년 국민건강영양조사의 검진조사에 참여한 만 20세 이상 성인을 대상으로 고식이섬유 섭취 및 고탄수화물 섭취와 대사질환 간의 연관성을 확인하였다. 한국 성인의 평균 식이섬유 섭취량은 24.1 g/일이었으며, 탄수화물 섭취 수준이 증가할수록 식이섬유 섭취량도 증가하는 경향을 보였다. 고식이섬유 섭취와 대사증후군과의 연관성을 확인하기 위해 식이섬유 섭취량을 5분위수로 분류하였을 때, 충분섭취량 이상의 고식이섬유 섭취는 대사증후군 위험 감소와 연관이 있었다. 또한, 식이섬유 섭취가 탄수화물 섭취와 밀접한 관련이 있다는 점을 반영하여 고탄수화물 섭취와 대사질환과의 연관성을 식이섬유 섭취수준에 따라 확인하고자 하였다. 이에 식이섬유 충분섭취량을 기준으로 하여 성별로 저식이섬유 섭취 그룹과 고식이섬유 섭취 그룹으로 분류한 후, 탄수화물 에너지 섭취비율과 대사질환과의 연관성을 분석한 결과 식이섬유 섭취수준에 따라 고탄수화물 섭취와 대사질환과의 연관성이 다름을 확인하였다. 저식이섬유 섭취 그룹에서는 남자의 경우 탄수화물 섭취가 삼분위군 일 때 일분위군에 비해 대사증후군과 죽상경화성 이상지질혈증의 교차비가 유의적으로 증가하고, 여자의 경우 탄수화물 섭취가 이분위군일 때 일분위군에 비해 고콜레스테롤혈증의 교차비가 유의적으로 증가하였다. 그리고 고식이섬유 섭취 그룹에서는 남자의 경우 탄수화물 섭취비율이 증가할수록 고중성 지방혈증의 위험도가 증가하는 경향을 보였다. 본 연구의 결과는 한국 성인의 고식이섬유 섭취가 대사증후군의 낮은 위험도와 관련이 있다는 근거를 제시하였으며, 특히 남자의 경우 고탄수화물 섭취와 대사지표와의 연관성을 살펴볼 때 식이섬유 섭취 수준을 살펴보는 것이 필요함을 제시했다는데 의의가 있다. 따라서, 향후 우리나라 사람들을 대상으로 전향적인 연구를 실시하여 식이섬유, 탄수화물 섭취와 대사질환과의 인과관계를 검증할 필요가 있으며, 이를 통해 탄수화물의 질적 섭취에 대한 실질적인 섭취 지침을 마련할 필요가 있다.

우리나라 성인의 대사증후군 유병과 관련된 식습관 및 식행동 요인 (Dietary Behaviors Related to Metabolic Syndrome in Korean Adults)

  • This study was conducted to investigate the cross-sectional associations between dietary factors and the risk of metabolic syndrome (MetS) in 12,755 subjects (males 5,146, females 7,609) aged 19 years or above using data from the 4th (2007-2009) Korea National Health and Nutrition Examination Survey (KNHANES). The prevalence of MetS in Korean adults was 23.6% (males 26.1%, females 20.9%) with the criteria for modified National Cholesterol Education Program Adult Treatment Panel III. While males had a higher prevalence of abdominal obesity, hyperglycemia, hypertriglyceridemia, and high blood pressure than females, the prevalence of low HDL-cholesterol level was higher in females than in males. Among dietary guidelines, the response of 'yes' for asking practice of 'avoiding salty foods', and 'eating moderately and increasing physical activity for healthy weight' were significantly associated with the decreased risk of MetS in both males and in females. Especially, the risk of MetS was significantly lower in the subjects that responded the practice of all items of Korean Dietary Guidelines. Significantly negative associations with MetS were also found in the responding for practice of 'limiting consumption of alcoholic beverages' in males, and taking dietary supplements in females. Skipping breakfast was positively associated with the risk of MetS. In conclusion, dietary behaviors such as having breakfast, practice of dietary guidelines, and food consumption in moderation could modify the prevalence of MetS, and our findings could be useful for establishing guidelines for preventing MetS.
