한국 남자의 연령별 골밀도에 영향을 미치는 영양요인 분석 (Nutritional Factors Related to Bone Mineral Density in the Different Age Groups of Korean Men)

  • Nutritional factors affecting bone mineral density (BMD) in the different age groups of Korean men were investigated to obtain baseline data for maintaining bone health. Information on diet and anthropometry was collected in 80 elementary school children, 83 high school students, 87 adults aged 25 to 35 years and 98 elderly people over 60 years of age. Data for food and nutrient intake were obtained by 24-hour recall method. BMDs of lumbar spine (L$_2$- L$_4$) and femoral neck were measured by dual energy x-ray absorptiometry. The relationship between BMD and nutritional factors were analyzed. In the femoral neck, 5.7% of adults was classified as osteopenia and 47.9% and 37.8% of the elderly were classified as osteopenia and osteoporosis. It was shown that plant protein, Ca, p, Fe, thiamin, riboflavin and vitamin C intakes were related with BMD in all age groups. As for the mean adequacy ratio (MAR) of nutrients, the lowest quartile group of BMD showed significantly lower MAR among children. The RDA percent of nutrients was a strong influential factor on BMD. Subjects who consumed below 75% of Korean RDA in energy, vitamin A, thiamin, and vitamin C showed lower BMD. Stepwise multiple regression analysis revealed that MAR in children, vegetable Ca in adolescents, and vitamin C in adults and elderly people were the highest influential factor on BMD. Therefore, the above results demonstrated that not only calcium but also other nutrients such as protein, iron, vitamin A, riboflavin, and vitamin C were necessary in order to keep the healthy bone status. In addition, although there were various dietary factors that influenced bone density, MAR was identified as the major factor that affected bone density. 강원 지역 일부 여대생 중 골밀도 정상군과 위험군의 식품섭취빈도법을 이용한 식품과 영양소 섭취 상태 비교 (Evaluation of Food and Nutrient Intake by Food Frequency Questionnaire between Normal and Risk Groups according to the Bone Mineral Density of Female College Students Residing in Gangwon Area)

  • The purpose of this study was to examine the relationship between bone density and dietary intake for college women in their twenties. This study was performed on 160 female college students residing in Gangwon-do. It was conducted using ultrasound measurement of calcaneus bone density, anthropometric checkup and food-frequency questionnaires (FFQ) comprising 94 kinds of commonly consumed foods. Subjects were divided into two groups according to the T-score of bone density: a normal group (n = 113 persons, T-score ${\geq}1$) and a risk group (n = 47, T-score < 1.0). The average age of the subjects was 20.17 years and there was no significant difference between the two groups. Body weight and body fat percentage of the normal group were significantly higher than those of the risk group. The mean daily energy intake of the normal group was significantly higher than that of the risk group. Also, protein, fat, vitamin A, niacin, vitamin B6, folate, calcium, phosphorus, sodium, potassium, iron and zinc intake for the normal group were significantly higher than for the risk group. For the intake of the commonly consumed foods (or dishes) listed in FFQ, the mean daily intake amount of loaf bread, rice cake, potatoes, spicy beef soup, cucumber, seasoned spinach perilla leaves, crown daisy, stir-fried mushroom, sea mustard, beef rib, ham, chicken, mackerel, common squid, drink type curd yogurt, oriental melon and chocolate in the normal group was significantly higher than in the risk group. While, the mean daily intake of ramyun (instant noodle) and carbonated beverage by the normal group was significantly lower than that of the risk group. In conclusion, 20 something female college students showed a higher rate (26.9%) of the bone mineral density risk group (osteopenia or osteoporosis). For the risk group, the levels of nutrient and food intake were lower than in the normal group. Therefore, the bone density risk group needs to increase their nutrient intake and diet quality by increasing the intake of various foods. In addition, they should decrease the intake of foods, which are negative for skeletal health such as instant noodles and carbonated beverages.

대학생의 골초음파 상태에 영향을 미치는 영양요인 분석 (Relation Between Nutritional Factors and Bone Status by Broadband Ultrasound Attenuation among College Students)

  • 본 연구는 최대 골질량에 도달하는 20대의 대학생 1039명(남학생 532명, 여학생 507명)을 대상으로 골초음파 상태와 영양소 섭취 및 생화학적 영양 상태를 나타내는 지표들과의 관계를 분석하였다. 대상자의 골밀도 수치에 따라 t-score$\underline{\geq}$-1인 정상 골밀도군과 t-score<-1 골감소군으로 분류하여 비교한 결과 남학생의 경우 정상 골밀도군이 90.4%, 골감소군이 9.6%이며 여학생의 경우 정상 골밀도군이 94.1%, 골감소군이 5.9%의 분포를 보였다. 체적지표인 BMI를 아시아-태평양 비만기준에 의거하여 분리하였을 때는 남학생은 저체중 3.4%, 정상+과체중 70.7%, 비만 25.9%, 여학생은 저체중 16.2%, 정상+과체중 76.5%, 비만 7.3%의 분포를 보였다. 정상 골밀도군과 골감소군의 골밀도와 생화학적 영양 상태는 남학생 여학생 모두 통계적으로 유의한 차이가 없었다. 전체대상자들의 영양소 섭취상태를 살펴 본 결과, 남학생의 경우 정상 골밀도군과 골감소군 간의 유의적인 차이가 나타나지 않았으나 여학생의 경우 동물성 단백질과 동물성 철분 섭취량이 정상 골밀도군에서 골감소군보다 유의하게 높았다. 평균 에너지 섭취량은 동일한 연령대의 한국인영양섭취기준(KDRIs)과 비교하였을 때, 각 남학생 정상골밀도군 약 74%, 골감소군 67%, 여학생 정상골밀도군 약 89%, 골감소군 81%의 섭취수준을 나타내었다. 평균 단백질섭취량은 남녀 두 군 모두 100%를 상회하는 섭취 양상을 나타냈으며, 강한 뼈를 유지하거나 각종 신진대사에 필수적인 영양소이기 때문에 골격건강에 가장 큰 영향을 미치는 칼슘의 경우 남학생과 여학생 모두 KDRIs와 비교하여 권장섭취기준의 약 63%정도로 부족한 것으로 나타났다. 골 건강 상태에 영향을 미치는 영양인자의 로지스틱 회귀분석 결과 연령, 성별 및 BMI를 교란변수로 통제하였을 경우 동물성 단백질, 동물성 철분 및 아연섭취는 골감소를 예방하는 보호 작용이 있는 것으로 나타났다. 동물성 단백질과 이들 미량 영양소의 섭취정도는 높은 식사의 질을 반영하므로 질적으로 우수한 식생활이 최대 골질량에 도달하는 시기인 20대의 골격형성과 건강을 위해 필수적인 것으로 사료된다.

Do types of snacks, sleep hours, and eating places affect nutritional intakes and its adequacy in adolescents?

  • BACKGROUND/OBJECTIVES: High-quality meal intake, including snacks, is necessary for optimal development during adolescence. The purpose of this study was to evaluate the nutritional intake of adolescents and the quality of their diet according to snack type, eating location, and sleep hours. SUBJECTS/METHODS: A survey of middle school students living in Seoul and Gyeonggi province was conducted using questionnaires and the 24-h recall method to collect data on the 1-day dietary intake of the students from March to May, 2018. The data were analyzed using SPSS 23.0 and the CAN program. RESULTS: The average nutritional intake status among the respondents showed that the energy intake was lower than the recommended, showing 1,914.8 kcal for middle school boys and 1,752.7 kcal for girls. In terms of the nutritional intake status by gender, only the consumption of protein and niacin were significantly higher in boys than girls (P < 0.05). According to the sleep amounts, vitamin C intake was significantly higher in the sleep-deprived group than in the sleep-moderate or sleep-recommended group (P < 0.05), but the intake did not meet the recommendation in any group. The lower density nutrients found in the index of nutritional quality according to eating places were vitamin C, calcium, iron, and folic acid. School meals showed a higher nutritional density in protein, vitamin B1, and niacin compared to convenience store meals (P < 0.05). CONCLUSIONS: This study suggested that sleep amount, and eating place affected the dietary quality of adolescents.

폐경 후 여성의 골밀도에 따른 영양소 섭취상태와 혈청 구리, 아연, 망간 함량에 관한 연구 (A Study on Nutrient Intakes and Serum Levels of Copper, Zinc and Manganese in Korean Postmenopausal Women with Different Bone Mineral Density)

  • The purpose of this study was to investigate the relationships among nutrient intakes, BMD, serum copper, zinc and manganese contents in Korean postmenopausal women residing in Seoul and Kyunggi-do. Nutrient intakes analyses and anthropometric measurements were performed for 64 postmenopausal women. Also, fasting blood samples were collected. The BMDs of the lumbar spine (L2-L4) and femoral neck were measured by dual energy X-ray absorptiometry (DEXA). Subjects were classified into three groups: osteoporosis (T-score ${\leq}$ -2.5, n = 20), osteopenia (-2.5 < T-score ${\leq}$ -1, n = 24), and normal (T-score) -1, n = 20). The results are summarized as follows; the mean age, height and weight were 62.1 yrs, 153.8 em, and 56.1 kg, respectively, with no significant difference among three groups. On the other hand, the mean BMI $(23.7\;g/m^2)$ of normal group was significantly higher than those of other two groups. The mean energy intake was 1,395.3 kcal (77.5% of Dietary referance intakes for koreans (KDRls)). The average daily intakes of vitamin $B_2$, calcium and zinc did not reach KDRIs. The mean intakes of iron, copper, zinc and manganese were 10.9 mg, 1.6 mg, 7.8 mg and 5.1 mg respectively, with no significant difference among three groups, The mean serum levels of copper, zinc and manganese were 96.4 mg/dl, 91.5 mg/dl and 39.5 ng/dl, respectively with no significant differences among three groups. The BMD of lumbar spine showed a significantly negative correlation intakes of animal protein, fat and iron intakes after adjusting for age, Bill. However, the BMD of lumbar spine was not significantly correlated with animal iron intakes after adjusting for animal protein, and fat intakes, indicating animal food intake is a possible confounding factor. Proper intakes of vitamin $B_2$, vitamin C are required to protect osteoporosis in postmenopausal women. These results suggest that excessive animal food intakes may have adverse effect on bone mineral density. Further studies arc necessary to elucidate the role of serum minerals in BMD.

Nutrient intake and bone health status of Korean male college students as related to smoking situations

  • The purpose of this study was to compare the dietary habits, nutrient intake, bone mineral density(BMD) and bone metabolism in Korean male collegians as related to smoking situation. One hundred sixty one young adult males at the age of 20-26 participated in this study. The subjects were divided into four groups: non smoker(n=42), light smoker(n=34), moderate smoker(n=49) and heavy smoker(n=36). The anthropometric characteristics, smoking situations, dietary habits and nutrient intakes were observed. Bone status of the calcaneus was measured by using quantitative ultrasound(QUS). Bone metabolism markers including serum alkaline phosphatase activity(ALP) and N-mid osteocalcin(OC) were analyzed. There were no significant differences in height, weight, BMI, energy and calcium intake among the four groups. Iron intake of moderate and heavy smoker was significantly lower than that of light smoker. Heavy smokers consumed significantly lower vitamin C than moderate smokers, and their coffee consumption and lifetime alcohol consumption were significantly highest among the 4 groups. QUS parameters and serum ALP were not significantly different among the four groups. Serum OC levels were significantly lower in heavy and non smoker group compared to the moderate smoker group. In conclusion, heavy smokers in young male collegians had undesirable lifestyle and dietary habits, like as high consumption of coffee and alcohol, and low intake of Fe and vitamin C. Although, there was no significant difference in their current bone status from the other groups, these undesirable factors with heavy smoking may affect their bone health in the long term.

맞벌이 가정과 전업주부 가정 미취학 자녀의 신체발달과 영양섭취상태 비교연구 (A Comparison of Growth Development and Nutrient Intakes between Double Income Families' and Full-Time Housewives' Preschool Children)

  • The purpose of this study was to examine the comparison of growth development, bone mineral density and nutrient intakes between double income families' preschool children(DIFPC) and full-time housewives' preschool children(FHPC). Subjects were 111 preschool children. Anthropometric characteristics and bone mineral density in right forearm were measured. The questionnaire was composed of health status, life style, dietary behaviors, and dietary intakes and was completed by the children's mothers. The average age of the DIFPC(n=60) was 53.02 months and that of the FHPC(n=51) was 54.80 months. The birth height and weight of the subjects were 50.47㎝ and 3.27㎏ for DIFPC and 50.85㎝ and 3.36㎏ for FHPC, respectively. The average height, weight, % body fat, and obesity index were 108.50㎝, 18.35㎏, 15.35%, 96.71% in DIFPC and 111.46㎝, 19.64㎏, 16.80%, 97.31% in FHPC, respectively. The bone mineral density in forearm of two groups were 0.24g/㎠ in all. The infant feeding method was significantly different between DIFPC and FHPC; 58.9% of DIFPC was fed formula, while 44.4% of FHPC was fed breast milk(p<0.05). Proportions of children for their regular meal were 59.4%, 89.6%, and 61.0% for breakfast, lunch, and dinner, respectively. The major reasons for irregular meal were lack of time and poor appetite for breakfast and snacks for lunch and dinner. Most of the children answered they have snack over once a day, and 60.0% have unbalanced diet. The intakes of energy, calcium, iron, zinc, vitamin A, vitamin B1, niacin, and vitamin C did not meet the Korean RDAs. The intakes of K and vitamin A for DIFPC were significantly higher than those of FHPC(p<0.05, p<0.05). In conclusion, double income families' preschool children more have a low frequency of breast feeding and low intakes of micro nutrients, such as K and vitamin A than full-time housewives' ones.

과거의 우유 및 유제품의 섭취가 한국노인의 골밀도에 미치는 영향 (The effect of Nutrient Intake and Past Dairy Products Consumption on Bone Mineral Density of Postmenopausal of Korean Women)

  • 본 연구는 한국노인의 식습관에 근거한 영양섭취 패턴을 분석하고 요추 골밀도를 측정하여 식이 인자와 과거의 우유 및 유제품의 섭취경력이 골밀도에 미치는 영향을 분석하였다. 노인들을 대상으로 과거의 식습관을 조사하는 것은 매우 어려우므로 본 연구에서는 인지회상법(cognitive food frequency recall method)을 우리나라의 식습관과 생활여건에 맞게 수정하여 사용하였다. 38~67세 사이의 여성을 대상으로 골밀도 135명을, 식이섭취조사, 인체계측, 혈액, 과거의 우유제품 섭취조사에 임한 조사대상자는 170명이었다. 연구결과를 요약하면 다음과 같다. 1. 평균연령은 53.4세, 평균체중은 57.9kg, 평균신장은 157.4cm, 체질량지수는(BMI)는 36.73, 섭취열량은 2,101.3㎉, 칼슘의 섭취량은 638.7mg, 단백질 섭취량은 70.3g이었고, RDA에 근접되거나 약간 상회하는 정도의 영양섭취와 신체소견을 나타내었고, 50~54세군에서 가장 높은 BMI를 보였으나 각 연령군간의 통계적 유의성은 없었다. 2. 요추의 골밀도 측정치는 평균(L2-L4) 0.912g/$ extrm{cm}^2$로, 50세 이하군에서 1.02g/$\textrm{cm}^2$, 50~54세군에서 0.92g/$\textrm{cm}^2$, 55~59세군에서 0.85g/$\textrm{cm}^2$, 60세 이상에서 0.81g/$\textrm{cm}^2$으로 연령의 증가에 따라 현격한 감소를 나타내었다. 3. 열량섭취와 식이 단백질량, 칼슘, 인은 상호 높은 양의 상관을 보였고, 요추의 골밀도에는 연령, 체중이 식이 요인보다 유의한 상관이 있었다. 4. 유제품의 과거의 섭취 경험은 탈지유의 섭취가 가장 많았고, 탈지유 이외의 다른 유제품의 섭취 경험이 없는 대상자가 94% 이상이었다. 유제품의 상용섭취를 시작한 시기는 4~5년전부터로 대다수가 청장년기의 유제품 섭취경험이 없었다. 5. 유제품의 섭취 빈도와 골밀도는 섭취시기가 10대 이전이 가장 상관이 높았고, 특히 60세 이상군에서 현저한 유의적 상관이 있었으며, 50~59세 사이와 20~30대에서도 유의적인 상관이 높았으며, 50세 이하군에서는 유의한 상관이 없었다. 이상의 결과를 통하여 볼 때, 여성의 골격 건강을 위해 열량, 단백질, 칼슘, 인 등의 식이 섭취량이 척추 골밀도와 밀접한 관련성을 나타냈고 과거의 유제품의 섭취, 어린 시절에 섭취한 경험자일수록 일생동안 충실한 골질량과 높은 골밀도치를 유지할 수 있었다. 그러나 아직 우리나라의 일상 식사에서 칼슘을 권장량 이상으로 섭취하는 여성이 적으므로 칼슘 섭취량에 대한 강조가 필요하며, 폐경 후의 급격한 골손실을 고려해 볼 때 30대 이전에 어느 정도의 칼슘 섭취량이 바람직할 것인가에 대해서는 계속적인 연구가 요구된다. 또한 일상생활에서 우유 및 우유제품의 섭취를 증가시키기 위해 꾸준한 국민적 홍보가 필요할 것이다.

폐경 여성의 골밀도 상태와 이에 영향을 미치는 요인에 관한 연구 (A Study on the Bone Mineral Density and Related Factors in Korean Postmenopausal Women)

  • A total of 216 women of natural menopause over 40 years were interviewed to find the relationships among general environmental fctors, anthropometric measurements, daily nutrient intake, and BMD. The average age of the subjects was 54.0 years. Average ages of menarche and menopause were 16.7 years and 47.7 years, respectively. In general, energy and nutrients intake of the subjects were relatively low compared to the Korean recommended dietary allowances(RDA). Specifically, intakes of energy, protein, clcium, iron, and vitamin A were lower than Korean RDA, BMD levels of lumbar spin (LS), femoral neck(FN), ward's triangle(WT), and trochanter(TC) were significantly decreased wit age in descending order of LS, FN, TC, and WT BMD, accordingly. BMD levels of FN and TC of the subjects were higher compared to those of the same age but BMD levels of LS and WT were lower than those of the same age from the general population. For factors related to BMD levels, hight, weight, BMI, and hip circumference had positive correlations with BMD. On the other hand, menarche, and WHR had negative correlations with BMD. Protein, lipid, iron, vitamin B1, vitamin B2, and niacin intake among factors related nutrient intakes were positively correlated with BMD levels. Based on the stepwise regression analysis, the factors and vitamin B1 intakes. Overall, BMD was decreased with age as can be expected. For those being tall, moderately weighed, and smaller waist circumference, BMD levels were high when the relationships of body characteristics and BMD levels were considered. The higher BMD levels were found for those with high intake of protein and vitamin B1. From the findings, it is suggested that the women after menopause keep the optimum body weight and good eating habits. Specially intakes of good quality protein, vitamin B1 seemed to be important.

한국 여대생의 골밀도에 영향을 미치는 식이 요인 분석 (Effect of Dietary Factors on Bone Mineral Density in Korean College Women)

  • 본 연구에서는 건강한 여대생들을 대상으로 평상시 식이 섭취 상태 및 연령, 초경 나이 등의 기본 변수 및 신체활동 요인들이 골밀도에 어떤 영향을 주는 지에 대하여 알아보았다. 1) 연구 시작시 대상자들의 평균 연령은 22.1세였고 초경의 나이는 12.6세이었으며, BMI는 20.3이었다. 2) 대상자들의 요추와 대퇴 골밀도 수치는 비슷한 연령을 대상으로 한 다른 연구들과 비슷하였고, 미국과 북유럽 여성들과 비교해보면 미국, 북유럽, 우리 나라 여성 순서대로 나타났다. 3) 대상자들의 8번의 24시간 회상법에 의해 수집한 평균 일일 섭취열량은 1,790 kcal이었고, 7차 영양권장량과 비교해보면 칼슘과 철분 섭취가 권장량의 70% 수준으로 가장 빈약한 것으로 나타났다. 4) 칼슘 대 인의 섭취비율은 0.53으로 한 다른 연구와 비교했을 때 도시여성이 0.66으로 본 대상자의 섭취비율이 낮았다. 5) 식품 섭취를 보면 우리 나라 성인 20대 여성들에 비하여 총 식품 섭취량이 더 많았으며 그 중에서도 동물성 식품 섭취 량이 더 많은 것으로 나타났다. 6) 연령은 대퇴 경부, 와드삼각에서 유의적인 음의 상관성을 보였고, BMI는 요추와 대퇴 모든 부위에서 유의적인 양의 상관관계를 보였다. 초경, 초경후의 기간, 신체활동 정도와 골밀도에는 아무런 관련성이 없었다. 7) 골밀도와 영양소 섭취량과의 상관분석에서는 요추 골밀도에는 비타민 C 섭취가 유의적인 관련성을 나타냈다. 대퇴의 경우는 대퇴경부는 지방섭취와 유의적인 음의 관계를 탄수화물 섭취와 비타민 B, 섭취는 유의적인 양의 관계를 보였다. 와드삼각과 대퇴전자부 또한 탄수화물 섭취와 유의적인 양의 관련성을 나타내, 대퇴골밀도 유지에는 탄수화물 섭취가 긍정적 인 영향을 줌을 알 수 있었다. 8) 골밀도와 식품 섭취량과의 상관분석에서는 요추의 경우 감자류, 과일류, 총 식물성 식품섭취량이 양의 관련성을 대퇴경부는 감자류, 과일류, 식품성 식품섭취 비율과 양의 관련성을 와드삼각은 채소류, 과일류. 식물성 식품 섭취비율과 양의 관련성을 대퇴전자부는 과일류 식물성 식품섭취 비율과 양의 관련성을 나타내. 과일섭취가 모든 골밀도에 중요한 변수로 나타났다. 9) 골밀도와 제반의 요인들과의 다중회귀분석에서는 요추는 BMI와 과일류 섭취가 대퇴 세 부위에서는 연령과 과일류 섭취가 의미 있는 변수로 나타났으며 모델의 결정력은 각각 37%, 35%, 43%, 24%로 높게 나타나서 모든 부위의 골밀도에서 높은 과일류 섭취가 중요한 요인임을 알 수 있었다. 이상의 결과를 종합해 볼 때, 우리 나라 여대생들의 골밀도는 미국이나 북유럽 젊은 여성들과 비교해 볼 때 약간 낮지만 비슷한 수준으로 나타났고, 과일류의 높은 섭취가 요추 및 대퇴 모든 부위 골밀도에 긍정적인 효과를 보였으므로 이 시기 과일류 섭취에 신경써야 하며 앞으로 과일류 섭취와 골밀도에 대한 많은 연구가 이루어져야 한다고 사료된다.
