A study on the isolation of Salmonella spp from Patients with diarrhea in Inchon(1992~1997) (인천지역에서 발생한 설사환자 가검물중 식중독균인 Salmonella spp 분리에 관한 연구(1992~1997))

  • In order to investigate the prevalence of Salmonella spp. from 1992 to 1997 in Inchon, 173 strains of Salmonella spp. were isolated from patients with diarrhea and analyzed them epidemiologically. The results obtained were as follows; 1. The yearly isolation rates were 34.68% (60 strains) in 1996, 20.80%(36 strains) in 1994, 18.50%(32strains) in 1997, 10.40%(18 strains) in 1992 and 9.83%(17 strains) in 1995 and 5.78%(10 strains) in 1993, in order. 2. The isolation rates based on specimen were 84.97%(147 strains) from feces, 14.45%(25 strains) from blood and 0.58%(1 strain) from pus. 3. The highest isolation rate based on age group was at twenties(25.43%) and four-ties(19.65%), fifties(19.08%), teens(15.03%) and thirties(10.98%), in order. 4. Based on the regional distribution, Nam-gu showed the highest isolation rate (20.80%) and followed by Namdong-gu(20.23%), Dong-gu(19.65%) and Yeansu-gu(0.58%), in order 5. Seasonally, the highest isolation rate was from May to September during the investigation. 6. The isolation rate in male(58.96%) was higher than that in female(41.04%).

Prevalence of Cryptosporidium-Associated Diarrhea in a High Altitude-Community of Saudi Arabia Detected by Conventional and Molecular Methods

  • Cryptosporidium diarrhea represents a relevant clinical problem in developing countries. In Al-Taif, a city of Saudi Arabia that lies at an altitude of an around 2 km above the sea level, Cryptosporidium infection seems to be undiagnosed in nearly all clinical laboratories. Furthermore, nothing was published regarding Cryptosporidium-associated diarrhea in this area. The objectives of this research were to (1) determine the Cryptosporidium prevalence among patients with diarrhea and (2) to estimate the performances of 3 different diagnostic methods. Total 180 diarrheal fecal samples, 1 sample per patient, were collected between January and August 2013. Samples were screened for Cryptosporidium with modified Zeihl Neelsen (ZN) microscopy, $RIDA^{(R)}$ Quick lateral flow (LF) immunotest, and a previously published PCR. The Cryptosporidium prevalence rate was 9.4% (17/180), 10% (18/180), and 11.6% (21/180) by microscopy, LF, and PCR test, respectively. Infection was significantly (P=0.004) predominant among children <5 years (22%) followed by children 5-9 years (11.1%). Although infection was higher in males than in females (16.2% males and 8.5% females), the difference was not statistically significant (P=0.11). Compared to PCR, the sensitivity of microscopy and the LF test were 80.9%, 85.7%, respectively. To conclude, high Cryptosporidium-associated diarrhea was found in this area especially in children ${\leq}9$ years. The PCR test showed the best performance followed by the LF test and ZN staining microscopy. The primary health care providers in Al-Taif need to be aware of and do testing for this protozoon, particularly for children seen with diarrhea.

  • In patients with functional gastrointestinal disorders, dietary factors have an important effect on the development or worsening of gastrointestinal symptoms. Therefore, nutritional advice is often needed for the treatment of these patients. Although no firm conclusions can be drawn from current studies, the use of low-fat diets can be recommended in patients with functional dyspepsia. Constipation can be treated with a high-fiber diet. There are few studies in irritable bowel syndrome patients with diarrhea. The diet recommendations must be applied to each patient depending on symptoms.

Study on the Identification Methods of the Non-agglutinating Vibrio (NAG Vibrio의 검사에 관한 소고)

  • The genus Vibrio contains some of the most important intestinal pathogens of humans, including Vibrio cholerae, the cause of epidemic Asiatic cholera. A group of organisms which have been reffered to as the non-agglutinating vibrio (NAG) do not agglutinate in the Vibrio cholerae 0 group 1 antisera, but are indistinguishable from the 0-1 group both chemically and genetically. Non-O-l Vibrio cholerae can cause isolated as well as focal outbreaks of diarrhea, but the volume of fluid loss does not approach that of classic cholera, and the disease is usually self-limiting. These free-living organisms are found world-widely distributed in the environment including sewage, contaminated water, estuaries, seafood and animals. These strains involved in several cases were isolated from the environment and some patients of diarrhea, and a few epidemiologic reports indicated the wide distribution of the strains throughout the country, giving an attention to the role the organisms may play in an outbreak of diarrhea in Korea. More research on the epidemiology, serologic typing and virulence of the group of organisms, should be, therefore, done to obtain a complete understanding of their role in human disease.

  • Transient symptomatic zinc deficiency (TSZD) is clinically similar to acrodermatitis enteropathica. The patients are experienced dermatitis on periorifice and extremities, alopecia, diarrhea, growth retardation, anemia, delayed wound healing and infection. They showed a characteristically rapid response to zinc supplement without recurrence. We report 3 cases of TSZD infants, 2 premature infants with inadequate zinc supply and 1 mature infant with chronic diarrhea.

A Study of Nutritional Improvement in the Patients with Neurologic Disorders by Changing Enteral Feeding Methods (경장영양 공급방식 변경에 따른 신경계질환자의 영양개선 효과 연구)

  • Protein-calories malnutrition is common among patients in the hospital. In particular, elderly patients with neurologic disorders has more risk of nutritional deficiency due to swallowing difficulty. Enteral tube feeding is more economical, physiological and immunological than parenteral nutrition for patients who have adequate gastrointestinal function. This study was conducted patients with neurologic disorders who received enteral nutrition at Asan Medical Center from February 1 to October 10, 2002. The control group (48 patients) were given traditional feeding methods 4 times a day while the treatment group (45 patients) were given improved feeding methods 3 times a day. We assessed nutritional status of patients and compared to both groups. We investigated body weight, serum albumin, hemoglobin, total lymphocyte count by means of nutrition markers. The objectives of this study is to reduce the time needed for nutritional requirement of patients without an increase in gastrointestinal intolerances. The results of this study are as follows: 1. Nutritional status of many patients in both groups were either malnourished or at risk for malnutrition. 2. The time to arrive to the nutritional requirements were 6.21 $\pm$ 0.35 days for the control group and 4.24 $\pm$ 0.52 days for the treatment group. The treatment group showed a significantly shorter amount of time. 3. The changes of the nutritional marker in the control group showed a significant drop in body weight, serum albumin and serum hemoglobin while the treatment group experienced a significant increase in body weight, serum albumin and total lymphocyte count. 4. Feeding intolerane such as diarrhea, high residual volume, ileus, nausea and vomiting were investigated. Diarrhea found in 25.1% (12 patients) of the control group and 22.2% (10 patients) of the treatment group and these findings are not significant.

  • The genus Sarcocystis is not usually considered as an important enteric pathogen in immune compromised patients. It might be expected that species for which humans are the final host (Sarcocystis hominis and Sarcocystis suihominis as well as possibly others) would be encountered increasingly often in immunodeficient persons. This study aimed to address how to detect and differentiate Sarcocystis oocysts and/or sporocysts from enteric protozoans in the diarrheal samples of immunodeficient patients in Shiraz, Iran. Diarrheal samples of 741 immunodeficient patients with recurrent persistent or chronic diarrhea were examined by microscopy and molecular biological analysis. Oocysts-positive samples were 68 Cryptosporidium spp., 9 Cystoisospora belli (syn. Isospora belli), 2 Cyclospora cayetanensis, and 15 microsporidia (Enterocytozoon bieneusi). Sarcocystis-like sporocysts found from a woman were identified as Sarcocystis cruzi through 18S rDNA amplification and phylogenetic analysis. To the best of our knowledge, this is the first report of S. cruzi from a human.

  • 1. Objectives : The aim of this study was to survey the Original symptoms of Soyangin's and to compare the tendency of their symptoms between Soyangin Gyolhyungjeung and Soyangin Heat-related diarrhea accompanied by headache in patients of indigestion. 2. Methods : We recruited 76 Soyangin subjects in the Korean Medicine clinic and researched their original symptoms. Subjects were diagnosed by the Sasang Constitutional specialists and Sasang Constitutional Medication. Subjects were two groups, confirmed to Soyangin Gyolhyungjeung and Soyangin Heat-related diarrhea accompanied by headache. All subjects answered the questionnaire about their Original symptoms. We found the differences between 2 groups using the Mann-Whitney U Test. Analysis was conducted using SPSS 18.0 for Korean. 3. Results : In Original symptoms, there was no difference between 2 groups in sleep, appetite, digestion, eating habits, water temperature, function of bowel movement, urination, sweating, sensitivity to coldness and sensitivity to warmness. But there was difference between 2 groups in frequency of defecation & hardness of feces(p=0.027). 4. Conclusions : We found the differences of heat and cold in symptoms between two Soyangin syndromes.

Antibiotics-Associated Hemorrhagic Colitis Caused by Klebsiella oxytoca: Two Case Reports

  • Nowadays, Klebsiella oxytoca is described as a causative organism for antibiotic-associated hemorrhagic colitis (AAHC). Here we report two cases of pediatric AAHC, from which K. oxytoca was cultured after starting amoxicillin-clavulanate or amoxicillin treatment. The patients developed severe abdominal pain and a large amount of bloody diarrhea. K. oxytoca was obtained in intestinal fluid culture of a boy through the colonoscopy. On the other hand, colonic tissue culture and intestinal fluid culture were negative of the other patient. K. oxytoca was detected in stool culture when he was admitted. These cases showed characteristic endoscopic findings of segmental hemorrhagic colitis, and both boys recovered spontaneously within 2-3 days after they stopped taking the antibiotics. Therefore, in children who develop relatively large amount of bloody diarrhea after antibiotic treatment, we should consider AAHC caused by K. oxytoca.

  • 1. Objectives We introduced a case of postoperation stomach cancer patient with metastasis to peritoneum and colon who suffered from repetitive diarrhea and abdominal pain. 2. Methods Stomach cancet is the most common cancer in Korea. Eventhough the 5-year survival rate of stomach cancer is increasing recently, metastasized stomach cancer does not show good prognosis, especially when the surgical operation is not applicable. After partial resection of stomach, most of patients experienced gastrointestinal disorders like dyspepsia, reduced meal size, abdominal discomfort, loose stool, and constipation. 3. Results and Conclusions Taeumjowi-tang, a Sasang Constitutional Medicine, improved the diarrhea and abdominal pain.
