Improvement in Diagnostic Accuracy of Hemangioma and Vascular Malformation through Multidisciplinary Clinic (협력진료를 통한 혈관종 및 혈관기형 진단 정확도 향상)

  • Purpose: It is difficult to provide accurate diagnosis and proper treatment for vascular anomalies, because vascular anomalies have a wide spectrum of lesion with various symptoms. Furthermore, vascular anomalies often require the services of a wide range of specialists. So, many patients have been discouraged by the lack of expertise and treatment options available to them. In this study, we aimed to describe the scope of the Vascular Malformation clinic (VAM clinic), a multidisciplinary clinic for hemangioma and vascular malformation patients and investigate of diagnostic accuracy of VAM clinic. Methods: 68 patients have included in a retrospective, medical record review study in the VAM clinic from may 2002 to august 2009. Data included the types of diagnoses of patients seen in the clinic, locations of lesions, comparison of accuracy of diagnoses at other clinic and initial diagnoses at VAM clinic and types of recommended treatments. Results: Initial diagnosis at VAM clinic showed 94.2% accuracy, while diagnosis at other clinic only showed 12.5% accuracy. Head and neck is most frequently occurred lesions in patients of VAM clinic. And VAM clinic is able to provide various and proper treatments to patients. Conclusion: Hemangiomas and vascular malformations can present with various medical problems. VAM clinic, the multidisciplinary clinic provided accurate diagnoses and proper treatments to patients.

A Novel Fundus Image Reading Tool for Efficient Generation of a Multi-dimensional Categorical Image Database for Machine Learning Algorithm Training

  • Background: We described a novel multi-step retinal fundus image reading system for providing high-quality large data for machine learning algorithms, and assessed the grader variability in the large-scale dataset generated with this system. Methods: A 5-step retinal fundus image reading tool was developed that rates image quality, presence of abnormality, findings with location information, diagnoses, and clinical significance. Each image was evaluated by 3 different graders. Agreements among graders for each decision were evaluated. Results: The 234,242 readings of 79,458 images were collected from 55 licensed ophthalmologists during 6 months. The 34,364 images were graded as abnormal by at-least one rater. Of these, all three raters agreed in 46.6% in abnormality, while 69.9% of the images were rated as abnormal by two or more raters. Agreement rate of at-least two raters on a certain finding was 26.7%-65.2%, and complete agreement rate of all-three raters was 5.7%-43.3%. As for diagnoses, agreement of at-least two raters was 35.6%-65.6%, and complete agreement rate was 11.0%-40.0%. Agreement of findings and diagnoses were higher when restricted to images with prior complete agreement on abnormality. Retinal/glaucoma specialists showed higher agreements on findings and diagnoses of their corresponding subspecialties. Conclusion: This novel reading tool for retinal fundus images generated a large-scale dataset with high level of information, which can be utilized in future development of machine learning-based algorithms for automated identification of abnormal conditions and clinical decision supporting system. These results emphasize the importance of addressing grader variability in algorithm developments.

  • The aim of this article is to introduce articles of the special issue on 'Revitalization Conditions on Crisis on Industrial Regions: Experience from Europe and the Republic of Korea, and to explore policy alternatives to crisis on industrial regions on the basis of policy diagnoses and prescriptions. In the existing research, diagnoses of such Korean regions are quantitatively focused upon industry, employment, plant, consumption, investment, real estate, and labour market, and are qualitatively based upon external environment and functional, structural and spatial characteristics. Prescriptions of such regions emphasize the establishment of a law and an institutional fix, financial supports, jobs' creation, industrial diversification, the intensification of urban foundation throughout the urban revitalizaion. In the policy development for these regions, it is required to link the industrial sector to social, educational, political and welfare sectors, and furthermore the collaboration of inter- and intra-ministry and the active participation of provincial and local governments are needed.

Evaluation of ultrasonography as a diagnostic tool in the management of periapical cysts and granulomas: A clinical study

  • Purpose: The aim of this study was, firstly, to determine the concordance of ultrasonographic and histopathological diagnoses in patients in whom apical resection was already indicated. Secondly, this study aimed to determine whether lesions were periapical granulomas or cysts, and to compare them after root canal treatment using ultrasonography and periapical radiographs. Materials and Methods: In the first stage of the study, ultrasonographic and histopathologic diagnoses of 10 lesions were compared. Secondly, the periapical radiographs and ultrasonographic images of 44 lesions were measured. The presence of internal vascularity was determined by ultrasonographic color and power Doppler modes. Follow-up examinations of healing after root canal treatment were performed using ultrasonography and periapical radiographs, and these modalities were compared. Results: In the periapical granuloma and cyst groups, the measurement values decreased for all variables. There was no significant difference in the dimensional changes of lesions between the 2 groups, and ultrasonography and periapical radiographs were compatible. The pre-diagnoses were compared with histopathological diagnoses and were found to be compatible. Conclusion: The ultrasonographic color and power Doppler techniques could be an effective method for diagnosing periapical lesions as cysts or granulomas. After root canal treatment of granulomas and cysts, the dimensional shrinkage and healing patterns appear to be similar. Ultrasonography and periapical radiographs were consistent in terms of dimensional comparisons, and ultrasonography may be an alternative method for follow-up of the healing of periapical lesions.

A Study for Diagnostic Agreement between Web-based Diagnosis Support System and Korean Medical Doctors' Diagnosis (웹기반 진단 보조 시스템의 진단 일치도 연구)

  • Objectives: This study aims to evaluate the clinical validity of the system by conducting a clinical study to assess the diagnostic agreement between the system and Korean medical doctors. Methods: This study was conducted from September 7, 2023, to December 7, 2023, across five Korean medicine institutions, involving 100 adult participants aged 20-64 who consented to participate. Participants first entered their symptoms into a web-based program, which utilized an AI-based algorithm to diagnose 36 types of pattern differentiation. Subsequently, Korean medical doctors conducted face-to-face diagnoses using the same 36 types. The diagnostic agreement between the system and the doctors' diagnoses was analyzed using descriptive statistical analysis, and the results were expressed as a percentage agreement. Results: Analysis of the diagnostic data from 100 participants revealed that the web-based diagnosis support system identified an average of 7.76±0.79 patterns per patient, while Korean medical doctors identified an average of 7.99±0.10 patterns per patient. The diagnostic agreement between the system and the doctors showed an average of 7.08±1.08 patterns per patient, with an overall diagnostic agreement rate of 88.57±13.31%. Conclusion: This study developed a web-based diagnosis support system for traditional Korean medicine and evaluated its clinical validity by assessing diagnostic agreement. Comparing the diagnoses of the system with those of Korean medical doctors for 100 patients, the system showed an approximately 89% agreement rate with the clinical diagnoses. The system holds potential for aiding Korean medical doctors in pattern differentiation diagnosis in clinical practice.

Identification of Major Nursing Diagnosis, Nursing Outcomes, and Nursing Interventions (NNN) Linkage for Cancer Patients Undergoing Chemotherapy (항암화학요법 환자에게 적용된 주요 간호진단, 간호결과 및 간호중재의 연계성 확인)

  • Purpose: This study was aimed to identify NANDA-NOC-NIC linkage in cancer patients receiving chemotherapy. Methods: This study was a descriptive study conducted in three steps. First, nursing diagnoses were identified from the electronic nursing records. Second, content validity of nursing diagnoses and outcomes were evaluated. Third, major nursing interventions associated with expected nursing outcomes were collected from 97 nurses who worked in the oncology unit. Data were analyzed using descriptive statistics. Results: Four major nursing diagnoses were identified: acute pain, knowledge deficit, health seeking behaviors, and ineffective protection. Associated with each respective diagnosis, 3 major outcomes (pain level, pain control, and comfort state) for acute pain, 8 major nursing outcomes (diet, disease process, treatment regimen, illness, ostomy care, prescribed activity, health behavior, and infection management) for knowledge deficit, 4 major outcomes (health promoting behavior, health promotion, health belief, and knowledge: health resource) for health seeking behaviors, and 3 major outcomes (fatigue level, immune status, and nutritional status) for ineffective protection were identified. In addition, nursing interventions frequently used in clinical practice for each major nursing outcome were identified. Conclusion: The identified NANDA-NOC-NIC linkage can contribute to improving the applications of nursing process and care plans.

Study to Develop Linkages between Nursing Diagnoses and Interventions (간호진단과 간호중재 연계를 위한 연구)

  • Purpose: This study was performed to validate the linkage between nursing diagnoses and nursing interventions by identifying performance and importance of nursing interventions linked to five NANDA nursing diagnoses. Method: Data was collected from 153 staff and head nurses working in 4 hospitals in K city. The results were analyzed using mean, SD and spearman correlation for ranking correlation. Result: The most importantly considered interventions were Medication Administration (IV) for pain, Pain Management for Constipation, Intravenous (IV) Insertion for Diarrhea, treatment, Vital Sign Monitoring for Hyperthermia, and Vital Sign Monitoring for Infection risk. The most frequently performed interventions was Medication Administration (IV) for Pain, Fluid Management for Constipation, Intravenous (IV) Insertion for Diarrhea, Vital Sign Monitoring for Hyperthermia, and Vital Sign Monitoring for Infection: Risk for. The rank correlations between importance and performance were highest in Diarrhea and lowest in Constipation. Conclusion: The above findings can be used to develop a nursing information system which can be used to facilitate documenting the nursing process, and a nursing information system developed by this research process will ultimately contribute to identifying nurses contribution to patient health.

Development of a Nursing Diagnosis System Using a Neural Network Model (인공지능을 도입한 간호정보시스템개발)

  • Neural networks have recently attracted considerable attention in the field of classification and other areas. The purpose of this study was to demonstrate an experiment using back-propagation neural network model applied to nursing diagnosis. The network's structure has three layers ; one input layer for representing signs and symptoms and one output layer for nursing diagnosis as well as one hidden layer. The first prototype of a nursing diagnosis system for patients with stomach cancer was developed with 254 nodes for the input layer and 20 nodes for the output layer of 20 nursing diagnoses, by utilizing learning data set collected from 118 patients with stomach cancer. It showed a hitting ratio of .93 when the model was developed with 20,000 times of learning, 6 nodes of hidden layer, 0.5 of momentum and 0.5 of learning coefficient. The system was primarily designed to be an aid in the clinical reasoning process. It was intended to simplify the use of nursing diagnoses for clinical practitioners. In order to validate the developed model, a set of test data from 20 patients with stomach cancer was applied to the diagnosis system. The data for 17 patients were concurrent with the result produced from the nursing diagnosis system which shows the hitting ratio of 85%. Future research is needed to develop a system with more nursing diagnoses and an evaluation process, and to expand the system to be applicable to other groups of patients.

  • Number of dairy cow patients treated at Hyup Doug Large Animal Clinic during the year July 1,1965, through June 30, 1966, were amounted to 485 cases and the result of analysis of disease incidence during one year period were us follows; 1. In the etiologic diagnostic categories, diseases caused by static mechanical abnormality were most productive and 28.1% of all diagnoses involved in this category. 2. In the topographic diagnostic categories, 39.4% of all diagnoses involved the urogenital system and were obeserved most productive. 3. As a result of each topographic diagnostic categories were subdivided into disease entities, 13.4% of all diagnoses involved the retained placenta and this were observed most productive in the miscellaneous disease enties. Next productive diseases were miscellaneous ovarian disease (10.9%), indigestion (10.3%), mastitis (8.0%) and metritis (6.8%) chronologically.

An analysis of using trend and relationship among DRGs, Nursing Diagnoses and Nursing Interventions (DRG, 간호진단, 간호중재의 활용경향 및 관계분석;미국의 일 지역을 중심으로)

  • The purposes of this research were to: a) define the changing trends of DRGs in comparison to the National Data, b) define the changing trends of Nursing Diagnoses and Nursing Interventions for the 5 most frequently occurring Diagnostic Related Groups (DRGs) across 3 years, and c) define the relationships between nursing diagnoses and nursing Interventions for the 5 most frequently occurring DRGs across the 3 years. This study was a secondary data analysis of medical and nursing data based on the United States Nursing Minimum Data Set and the Uniform Hospital Discharge Data Set retrieved from a Midwestern USA medical center. The results showed interesting comparisons with national statistics as well as practice relevant trends within the nursing data. Additionally, the results showed the possibility that nursing data can be extracted from the medical data, so they can used in the nursing productivity and cost issues etc. In conclusion, this study supports the power of minimum data sets and nursing classifications to begin to describe a more global perspective the inter-relationships and trends of nursing data within the medical diagnosis context.
