젊은 당뇨 환자에서 신경병성 관절병증 종골 견열 골절의 자연 경과(1예보고) (Natural History of the Calcaneal Avulsion Fracture in Neuropathic Arthropathy in a Young Diabetic Patient (A Case Report))

  • The avulsion fracture of the calcaneal tuberosity is rare injury. Usually, it occurs from indirect trauma in old patients with osteoporosis or in patients with diabetic neuropathy. Especially, the bone and joint damage occurred in active patient with severe sensory loss or arthropathy related to nerve damage regardless of the cause is referred to neuropathic arthropathy. Generally, a patient with nondisplacement or minimally displacement is treated by conservative therapy and a patient with severe displacement is treated by open reduction and internal fixation. We experienced a 33 years-old woman with diabetes mellitus who had the displaced avulsion fracture of the calcaneal tuberosity without significant trauma and did not treat. We report upon this case at the 2 years follow-up.

전화추후관리가 우울성향을 가진 당뇨병 환자의 혈당개선에 미치는 효과 (Effect of Telephone Follow-up on Blood Glucose Improvement of Diabetes Mellitus Patients with Depressive Symptoms)

  • The aim of this study was to examine the effect of telephone follow-up on glycemic control, lipid profiles. and treatment method in diabetic patients with depressive symptoms over one year follow-up period. Twenty-two diabetic patients attended the structured diabetes education program between March 2004 and February 2005 and completed a questionnaire using the Beck Depression Inventory(BDI) to measure depressive symptoms. The patients showed depressive symptoms with the criterion being a BDI score at least 16. The intervention was applied to the telephone follow-up for one year. Data were analyzed using Wilcoxon signed rank test and McNemar test. The patients decreased their mean glycosylated hemoglobin(HbAlc) levels by 1.9%(p=0.001). There was a significant mean change in the fasting blood sugar(FBS) level, with a mean change of -61mg/$d{\ell}$(p=0.008). These findings indicated that the telephone follow-up may be effective in the HbAlc and FBS control.

  • Type 2 Diabetes Mellitus, a chronic metabolic disorder which results from a high blood glucose level, is one of the most prevalent and costly diseases of our time. Considering increasing rates of obesity and the aging population in Korea, the number of diabetic patients is likely to rise rapidly in the future. There are five conventional diabetic drugs which work through different mechanisms; sulfonylureas, biguanide, meglitinide, alpha-glucosidase inhibitors, and thiazolidinedione. Although they all have antidiabetic effects, some side effects such as hypoglycemia, weight gain and gastrointestinal intolerance are associated with them. Incretin based therapies, utilizing glucagon-like peptide-1 (GLP-1) and dipeptidyl peptidase-4 (DPP-4) inhibitors, which have a lower risk of adverse side effects, have recently been introduced. At present PPAR-targeting drugs are being actively developed. In this research review, particular emphasis has been placed on the current trends and possible biological targets for the new generation of antidiabetic drugs.

The Effect of Diabetes, Gestational Diabetes or Pre-eclampsia on Urinary Protein and Mineral Excretion during Pregnancy

  • Thirteen healthy control, 13 pre-eclamptic, 7 diabetic(DM) and 12 gestational diabetic(GDM) pregnant women participated in a study ofthe interrelationships between the levels of protein, calcium, magnesium, phosphorus, zinc and copper in urine. Urinary protein, magnesium and copper levels were significantly higher (p<0.0005, p<0.0003, p<0.005 respectively) in pre-eclamptic women than those of control, DM and GDM women. Urinary zinc excretion in pre-eclamptic women (1.61 mg/g creatinine) was higher than that of DM women (0.81mg/g creatinine); urinary zinc losses of control and GDM women were wre between the other two rups. The GDM women excreted significantly ore phosphorus in their urine in comparison to control and preeclamptic women (p<0.02), but this was not seen in DM women. Among the DM women, urinary protein excretion was positively correlated with glycosylated hemoglobin(r=0.940) and fasting blood glucose concentration (r=0.889). Urinary zinc excretion also was correlated with glycosylated hemoglobin (r=0.853) and fasting blood glucose (r=0.956). In the GDM and pre-eclamptic women there were also significant correlations between urinar calcium and magnesium (r=0.857, r=0.749 respectively) and between urinary protein and copper(r=0.638, r=0.778 respectively).

Development of a Zebrafish Larvae Model for Diabetic Heart Failure With Reduced Ejection Fraction

  • Background and Objectives: Diabetes mellitus (DM)-associated heart failure (HF) causes high morbidity and mortality. In this study, we established a zebrafish larvae model for in vivo research on diabetic HF. Methods: DM-like phenotypes were induced by treating zebrafish larvae with a combination of D-glucose (GLU) and streptozotocin (STZ). HF was induced by treatment with terfenadine (TER), a potassium channel blocker. Additionally, myocardial contractility, motility, and viability were evaluated. Results: The zebrafish larvae treated with a combination of GLU and STZ showed significantly higher whole-body glucose concentrations, lower insulin levels, and higher phosphoenolpyruvate carboxykinase levels, which are markers of abnormal glucose homeostasis, than the group treated with only GLU, with no effect on viability. When treated with TER, DM zebrafish showed significantly less myocardial fractional shortening and more irregular contractions than the non-DM zebrafish. Furthermore, in DM-HF with reduced ejection fraction (rEF) zebrafish, a significant increase in the levels of natriuretic peptide B, a HF biomarker, markedly reduced motility, and reduced survival rates were observed. Conclusions: We established a DM-HFrEF zebrafish model by sequentially treating zebrafish larvae with GLU, STZ, and TER. Our findings indicate the potential utility of the developed zebrafish larvae model not only in screening studies of new drug candidates for DM-HFrEF but also in mechanistic studies to understand the pathophysiology of DM-HFrEF.

Modulation of Pro-inflammatory and Anti-inflammatory Cytokines in the Fat by an Aloe Gel-based Formula, QDMC, Is Correlated with Altered Gut Microbiota

  • Abnormal inflammatory responses are closely associated with intestinal microbial dysbiosis. Oral administration of Qmatrix-diabetes-mellitus complex (QDMC), an Aloe gel-based formula, has been reported to improve inflammation in type 2 diabetic mice; however, the role of the gut microbiota in ameliorating efficacy of QDMC remains unclear. We investigated the effect of QDMC on the gut microbiota in a type 2 diabetic aged mouse model that was administered a high-fat diet. Proinflammatory (TNF-α and IL-6) and anti-inflammatory (IL-4 and IL-10) cytokine levels in the fat were normalized via oral administration of QDMC, and relative abundances of Bacteroides, Butyricimonas, Ruminococcus, and Mucispirillum were simultaneously significantly increased. The abundance of these bacteria was correlated to the expression levels of cytokines. Our findings suggest that the immunomodulatory activity of QDMC is partly mediated by the altered gut microbiota composition.

비타민 C 보충이 제 2형 당뇨병 환자의 혈당 및 항산화상태에 미치는 효과 (Effect of Vitamin C Supplementation on Blood Sugar and Antioxidative Status in Types II Diabetes Mellitus Patients)

  • Purpose: This study was to determine the effect of oral vitamin C supplements on blood sugar and antioxidative status in Types II diabetes mellitus patients. Method: Data for the study were collected from June 24 to August 31, 2001. Participants(31) took 1g/day vitamin C for 4 weeks, after a 1 - week taking no Vitamin C, followed by Vitamin C 3g/day for 4 weeks. A baseline blood sample was obtained following a 12hour overnight fast and at the end of each 4week Vitamin C administration. Blood samples were taken for plasma vitamin C concentration, fasting blood sugar, HbA1c, superoxide scavenging activity and hydrogen peroxide scavenging activity. The data were analyzed by SPSS for repeated measures ANOVA. Result: Plasma vitamin C concentration was significantly increased over dose(F=3.316, p=.043). Fasting blood sugar and HbA1c was significantly decreased over dose(F=13.192, p=.000; F=11.995, p=.000). Superoxide scavenging activity and hydrogen peroxide scavenging activity was significantly increased over dose(F=486.138, p=.000; F=177.704, p= .000). Conclusion: The results suggest that megadose vitamin C supplementation may have a beneficial effect in diabetes mellitus patients on both glycemic control and antioxidant status. Thus dietary measures to increase plasma vitamin C may be an important health strategy for reducing the compliance of diabetic patients

소갈(消渴)로 진단된 2형 당뇨병 환자 치험(治驗) 2례 (A Case Study of Two Type II Diabetic Patients Diagnosed with Sogal)

  • Objectives: This study aimed to explore and describe how two patients diagnosed with type II diabetes mellitus were successfully cured solely through the use of herbal medicine. Methods: The patients were administered the herbal medicine to reduce symptoms of stress-induced fever with dryness. The prescribed herbal medicines included Yangkyuksanwha-tang, Hyungbangsabaek-san, Hyungbangjihwang-tang, and Hyungbangdojeock-san. Results: The therapeutic outcomes showed that all numerical levels of symptoms such as thirst, polyuria, weight loss, blood sugar, and glycated hemoglobin (HbA1c) recovered to within normal levels. Conclusions: The results of this study suggest that our medical treatment of type II diabetes mellitus diagnosed as Sogal was a valid treatment. We hope that our team's new evidence and findings will shed positive light on the treatment of type II diabetes mellitus using herbal medicine.

알록산으로 유도된 당뇨병 토끼에서 겐타마이신의 약물동태 변화 (Pharmacokinetic Changes of Gentamycin After Intravenous Administration to Rabbits with Alloxan-Induced Diabetes Mellitus)

  • Many diabetic patients develop serious complications during the course of the disease, including cardiovascalar disorders, nepropathy, neuropathy and retinopathy. Because some physiological changes occurring in diabetes mellitus patients could alter the pharmacokinetics of drugs used to treat the disease, the pharmacokinetics of gentamycin was investigated after intravenous administration (2 mg/kg) to control rabbits and acute or chronic alloxan-induced diabetes mellitus rabbits (AIDRs). After intravenous administration, the serum concentrations of gentamycin were significantly higher between 6 and 12 hr in chronic AIDRs compared with those in control rabbits. The AUC was significant greater in chronic ($31.91\;{\pm}\;3.76\;{\mu}g/ml{\cdot}hr$) AIDRs than that in control ($21.60\;{\pm}\;2.45\;{\mu}g/ml{\cdot}hr$) rabbits. Total body clearance (CLt) in AIDRs were significantly decreased compared with that in control rabbits. Cumulative urinary excretion of gentamycin was decreased, although not significantly, in AIDRs compared with that in control rabbits.

Antioxidant Activity of Aqueous Extract of Coscinium fenestratum in STZ-Nicotinamide Induced Diabetic Rats

  • The aqueous extract of Coscinium fenestratum was studied for its antioxidant status in STZ-nicotinamide induced type 2 diabetic rats at two dose levels of 250 mg/kg and 500 mg/kg. At the end of the experimental period, diabetic rats treated with aqueous extract at both dose levels showed a significant increase in the levels of enzymatic antioxidants such as glutathione peroxidase, glutathione synthetase, peroxidase, superoxide dismutase and catalase as compared to the untreated control. Similarly, a significant increase was also observed in the levels of the non enzymatic antioxidants ceruloplasmin, ascorbic acid and tocopherol. The results suggest that the aqueous stem extract of C. fenestratum prevents type 2 diabetes mellitus induced oxidative stress.
