일 농촌지역 성인의 고혈압과 당뇨병 관리 행태 (Management Behavior in Hypertension and Diabetes of Rural Adults in Korea)

  • 연구의 목적은 일 농촌지역의 전수조사를 통해 고혈압과 당뇨병 환자들의 관리 행태를 분석하기 위해 시도되었다. 조사는 2014년 5월 19일부터 9월 21까지 구조화된 설문지를 이용한 자기기입식으로 진행되었다. 연구결과, 고혈압으로 진단 받은 사람 355명(36.6%) 중 현재 치료를 받고 있는 사람은 337명(94.9%)이었으며, 당뇨병으로 진단을 받은 사람 109명(11.2%) 중에서 현재 치료를 받고 있는 사람은 96명(88.1%)이었다. 고혈압 잠재위험군 총 211명 중 고혈압 진단을 받은 사람은 89명(42.2%)이었으며, 당뇨병 잠재위험군 356명 중에서 당뇨병 진단을 받은 사람은 78명(22.0%)이었다. 위 결과를 바탕으로 고혈압과 당뇨병 예방을 포함한 효과적인 관리 전략을 국가 차원에서 수립하고 실행하는 방안을 제안하고자 한다.

당뇨병 환자의 성별 우울, 불안, 스트레스 반응과 자가관리의 관계연구 (A Study of Depression, Anxiety, Stress Response and Self-care by Gender in Diabetic Patients)

  • Purpose: The purpose of this study was to investigate the depression, anxiety, stress response and self-care, to analyze the correlation among depression, anxiety, stress response and self-care by gender, and to determine factors associated with self-care in diabetic patients. Method: The subjects of this study were 103 participants with diabetes mellitus. Data were analyzed by chi-square test, t-test, Pearson correlation coefficient and multiple regression analysis by using SAS program. Result: Items for self-care evaluation by gender were significant differences in hospital visit, hypoglycemia preparation, proper hygiene, taking a rest, foot injury check, drinking, and smoking. The female patients are more likely to have higher self-care score than the male patients. However, there were no differences in depression, anxiety and stress response by gender. In male patients, there were positive correlations between the degree of depression and stress response, the degree of anxiety and stress response. In female patients, there were positive correlations between the degree of depression and stress response, the degree of anxiety and stress response. In multiple regression analysis, gender and experienced admission is associated with self-care. Conclusion: We should consider integrated approaches for psychological problems in the management of diabetic patients.

제1형 당뇨병 아동의 긍정적 심리요인에 대한 탐색적 연구: 주관적 행복감과 삶의 만족감을 중심으로 (Exploring Positive Psychology of Children with Type 1 Diabetes Focusing on Subjective Happiness and Satisfaction with Life)

  • Purpose: The purpose of this study was to describe the subjective happiness and satisfaction with life of children with type 1 diabetes and explore related factors. Methods: A descriptive cross-sectional study design was used and the study was conducted with children at a diabetes camp. Data were collected using a self-report questionnaire to explore positive and negative psychological factors. The questionnaire included Subjective Happiness Scale, Satisfaction with Life Scale, Perceived Stress Scale and the Center for Epidemiological Studies Depression Scale for Children (CES-DC), Multidimensional Scale of Perceived Social Support, and General Self-Efficacy Scale. Results: Data from 15 children were used for data analysis. The correlational analysis results showed that subjective happiness was positively correlated satisfaction with life, social support, and self-efficacy, and was negatively correlated with perceived stress. Satisfaction with life was positively correlated social support, and was negatively correlated with perceived stress. Conclusion: Results indicate that the positive psychology factors were closely related with social support and self-efficacy and may alleviate perceived stress and depressive feelings. Therefore, researchers and clinicians should include positive psychological factors in their health management model for children with chronic illness.

  • 당뇨병은 전 세계적으로도 발병률 및 유병률이 매년 증가하는 사회적 중대한 문제이다. 혈중 포도당의 농도가 높아지는 고혈당을 특징으로 한 당뇨병은 여러 증상 및 징후를 일으키고 있다. 그로 인해 환자들이 일상생활에서의 자가관리 강화를 인지하고 실천해야 한다. 하지만 환자들이 자가관리에 있어서 실패하는 가장 큰 요인을 식이요법 실패로 꼽고 있다. 이에 본 논문에서는 사진 촬영 또는 이미지 업로드를 통해 식단을 입력하고 섭취지표와 혈당 통계를 통해 사용자들의 식단을 분석해주는 애플리케이션을 설계 및 구현하였다.

Diabetes Management and Hyperglycemia in Safety Sensitive Jobs

  • The chronic and acute effects of hyperglycemia affecting cognition and work are as important as those of hypoglycemia. Its impact, considering that majority of diabetic patients fail to reach therapeutic targets, would be potentially significant. Self monitoring of blood glucose, recognition of body cues and management interventions should be geared not only towards avoidance of disabling hypoglycemia, but also towards unwanted hyperglycemia. Over the long term, chronic hyperglycemia is a risk for cognitive decline. Acute episodes of hyperglycemia, above 15 mmol/L have also been shown to affect cognitive motor tasks. Maintaining blood sugar to avoid hyperglycemia in diabetic workers will help promote safety at work.

당뇨병환자에게 적용한 중재프로그램 연구논문 분석 (Analysis of Reported Study on Intervention Programs for Patients with Diabetes Mellitus)

  • 본 연구는 당뇨병 환자에게 적용한 중재프로그램의 효과를 분석한 국내 연구논문의 내용을 분석한 연구이며, 검색은 Riss4u, 국회도서관, 국립 중앙도서관 등을 이용하였다. 검색어는 "당뇨병 환자", "당뇨병환자와 교육", "당뇨병환자와 운동", "당뇨병환자와 중재", "당뇨병환자와 교육 또는 운동"이었다. 선정기준에 적합한 53편의 논문을 중재프로그램의 적용방법 및 효과변수로 구분하여 분석하였고, 유형은 총 8가지이며 사용된 효과변수는 113가지이다. 가장 많이 사용된 유형은 교육과 상담을 동시 적용한 경우 이며 당 대사, 자가 간호, 자기 효능감이 자주 사용되는 종속변수였다. 중재프로그램의 적용방법은 다양하였으며, 효과변수의 결과는 효과가 없거나 상이한 경우도 있었다. 향후 중재프로그램의 기대효과를 높이기 위해서는 디지털 융합 시대에 적합한 체계적인 내용 구성 및 운동이 적극적으로 포함되어야 할 것이며, 또한 사회 심리적 변수를 포함한 효과변수의 선정 및 효과변수 측정법에 대한 고려가 필요하다.

영양상담이 제2형 당뇨병 환자의 혈당과 식사요법에 대한 지식과 실천에 미치는 영향 (Effects of Nutrition Counseling on Diabetes Management in Type 2 Diabetes Mellitus Patients)

  • The purpose of this study was to investigate the effects of nutrition counseling on diabetes management by determining changes in anthropometry and blood components as well as knowledge and practice of diet therapy and nutrient intake in 34 (male 11, female 23) type 2 diabetes patients. The knowledge and the practice of diet therapy, drinking, smoking and exercise were analyzed by questionnaires. Dietary nutrient intake were obtained from the patients by the 1 day 24-hr recall. Blood glucose level and blood pressure were measured before and 3 months after the treatment. The results are summarized as follows: Average weight (p<0.05) and body mass index (p<0.05) were significantly lower post-counseling. Fasting blood glucose levels (p<0.01) and postprandial-2hour blood glucose levels (p<0.01) were also significantly lower post-counseling. In lifestyle changes for self-management the patients showed significantly higher exercise habits post-counseling (p<0.01). Regarding their level of diet knowledge, they showed significantly higher levels post-counseling in six items such as importance of diet therapy for diabetes (p<0.001), principles of diet therapy (p<0.001), nutrient composition of foods (p<0.01), carbohydrate composition of foods (p<0.001), the prescribed calories (p<0.001) understanding food item and exchange units of cereals, grains (p<0.001) and fruits, juices (p<0.001). Regarding their diet practices, the patients showed significantly higher levels of practice post-counseling in keeping within permitted meal size (p<0.001), using food exchange lists (p<0.001), keeping exact meal times (p<0.01), and controling sweet foods (p<0.001). Protein (p<0.05), animal lipid (p<0.05), and vitamin C (p<0.05) intakes were significantly higher post-counseling.

감정상태가 국내 성인당뇨 혈당조절에 미치는 영향에 관한 실증연구 (Investigating Effects of Emotional States on the Glucose Control of Diabetes in Korean Adults)

  • 당뇨병 유병율이 증가함에 따라 당뇨병은 사회경제적 및 보건학적 이슈이다. 당뇨병 조절은 질병의 치료라기보다 평생에 걸쳐 관리해야 하는 것으로 자기간호가 수행되어야 한다. 본 연구는 성인 당뇨병을 혈당 조절군과 비조절군으로 구분하여 당뇨병 유병 위험 요인의 차이에 대해 입증하고자 한다. 나아가 당뇨병 환자들이 지각하는 감정요인과 당뇨병 유병과의 관련성을 밝히고자 한다. 이를 위해 5년치 국민건강영양조사 자료의 당뇨병 유병 성인들의 혈당조절 수준 영향요인을 분석하였다. 결과, 당뇨병 유병자인 혈당비조절군과 혈당조절군에 대한 유병률의 요인에 대해 입증하였으며, 특히 우울증이나 스트레스와 같은 부정감정이 당뇨병 유병 및 혈당조절에 유의한 연관이 있음을 확인하였다. 따라서 당뇨병에 대한 진단은 우울증과 스트레스 같은 부정감정을 고려하여 병행치료가 필요하다고 여겨진다.

당뇨병 적정관리를 위한 교육 실태 분석: 우리나라 보건소를 중심으로 (Focused on Diabetes Education Practice of Community Health Centers in Korea)

  • Objectives: The purpose of this article was to investigate the current practice of diabetes education along with the specific interventions, process, and outcomes in community health centers in Korea. Methods: Data were collected by a mail questionnaire from September 20, 2012 to December 20, 2012. Among 253, a total of 161 responded, constituting a 63.3% return rate. Results: Primary staff of diabetes education was the nurse and respondents recognized their role largely as a director. More than half of respondents provided education to people with type 2 diabetes by group. Most common service offered was nutrition therapy and the majority of respondents used printed materials. Among 4 criteria of outcomes, eating (nutrition), knowledge scores, blood pressure, and patients' survey on satisfaction were collected most frequently. Nearly three quarters of respondents were not participated in activities for quality improvement and outcomes were not reported properly. Conclusions: The results are able to draw ideas for organizing diabetes education programs and evaluating outcomes in community heath centers. This article has significance that it is the first comprehensive survey of diabetes education practice in community health centers and provides a baseline for establishing national standards of diabetes self-management education.

IT-기반의 임신성 당뇨병 영양관리 프로그램 개발을 위한 요구도 조사 (Needs for Development of IT-based Nutritional Management Program for Women with Gestational Diabetes Mellitus)

  • Objectives: The aim of this study was to examine self-management status, nutritional knowledge, barrier factors in dietary management and needs of nutritional management program for women with Gestational Diabetes Mellitus (GDM). Methods: A total of 100 women with GDM were recruited from secondary and tertiary hospitals in Seoul. The questionnaire composed of general characteristics, status of self-management, dietary habits, nutrition knowledge, barrier factors in dietary management, needs for nutrition information contents and nutritional management programs. Data were collected by a self-administered questionnaire. All data were statistically analyzed using student's t-test and chi-square test using SAS 9.3. Results: About 35% of the subjects reported that they practiced medical nutrition and exercise therapy for GDM control. The main sources of nutrition information were 'internet (50.0%)' and 'expert advice (45.0%)'. More than 70% of the subjects experienced nutrition education. The mean score of nutrition knowledge was 7.5 point out of 10, and only about half of the subjects were reported to be correctly aware of some questions such as 'the cause of ketosis', 'the goal of nutrition management for GDM', 'the importance of sugar restriction on breakfast'. The major obstructive factors in dietary management were 'eating more than planned when dining out', 'finding the appropriate menu when dining out'. The preferred nutrition information contents in developing management program were 'nutritional information of food', 'recommended food by major nutrients', 'the relationship between blood glucose and food', 'tips on menu selection at eating out'. The subjects reported that they need management program such as 'example of menu by calorie prescription', 'recommended weight gain guide', 'meal recording and dietary assessment', 'expert recommendation', 'sharing know-how'. Conclusions: Based on the results of this study, it is necessary to develop a program that provide personalized information by identifying the individual characteristics of the subjects and expert feedback function through various information and nutrition information contents that can be used in real life.
