COMPARISON OF RECOLONIZATION OF THE SUBGINGIVAL MICROFLORA AFTER SCALING AND ROOT PLANING ON SINGLE AND MULTIROOT PERIODONTAL POCKETS (치석 제거술과 치근면활택술후 다근치와 단근치의 치은연하 세균 재군락에 대한 비교연구)

  • The purpose of this study was to assess the recolonization of the subgingival microflora following scaling and root planing on single and multiroot teeth with periodontal pockets which were above 5mm. 7 patients with deep pockets were selected for this study. They had not taken antibiotics for 6 months and no history of dental treatment for 6 months before the study. After initial clinical(plaque index, gingival index, probing pocket depth), microbiological and BANA test were determined, each subject received a single session of scaling and root planing, but they were not received oral hygiene instructions. Clinical indices, microbial parameters and BANA test were reassessed 1, 2, and 4 weeks after treatment. The results were as follows : 1. Plaue index, gingival index and pocket depth were not significantly when compared single root group with multiroot group, both groups were siginficantly reduced at 2weeks in plaque index and 2, 4 weeks in gingival index(P<0.05), probing pocket depth was siginificantly changed at 2, 4weeks in multiroot teeth group and 4 weeks in single root teeth group(P<0.05). 2. Percentage of cocci was significantly increased at 4weeks in single root teeth group(P<0.05), motile rod was significantly changed at 4weeks in both group(P<0.05), spirochetes and nonmotile rods were not significantly changed. 3. BANA test was significantly reduced at 1 and 2 weeks (P<0.05) in single root teeth group, multiroot teeth group was not significantly all weeks. This results were suggested that clinical and microbiological effect following scaling and root planing on periodontal disease.

EFFECT OF CHLORHEXIDINE MOUTH RINSE ON SUBGINGIVAL BACTERIA (Chlorhxidine 구강 양치용액이 치은연하 세균에 미치는 영향에 대한 연구)

  • The purpose of this study was to assess the clinical and microbiological effect of chlorhexidine rinse following scaling and root planing on periodontits. 10 patients with periodontal disease were selected for the study. They had not taken antibiotics for months and no history of dental treatment for 6 months before the study. They were good in general health. Patients received a scaling and root planing under local infiltration anesthesia, chlorhexidine rise group were subjected to twice a day 0.1% chlorhexidine rinse for a period 2 week. After initial clinical(plaque index, gingival index, probing pocket depth), microbiological and BANA tests were determined, each subject received a single session of scaling and root planing but no oral hygiene instructions. Clinical indices were measured, microbial parameters and BANA test were reassessed 1, 2 and 4 weeks after treatment. The results were as follows : 1. Plaque index, gingival index and pocket depth in chlorhexidine rinse group and control group were not significantly reduce during all weeks when compared chlorhexidine rinse group with control groups. Plaque index in chlorhexidine rinse group and control group were siginificantly reduced at 1, 2, 4weeks(P<0.05), gingival index and pocket depth wee ignificantly reduced at 2, 4weeks in both groups(P<0.05). 2. Perecntage of cocci and motile rods was significantly changed at 1, 2, 4weeks in chlorhexidine rinse group(P<0.05), control group was significantly changed at 4weeks in control group(P<0.05), intergroup difference was significantly at 2weeks in cocci and 4weeks in motile rods(P<0.05). 3. Percentage of non-motile rods in all group were not significantly changed when compared with those of baseline. 4. Percentage of spirochetes was significantly reduced at 4 week(P<0.05), control group was not significantly reduced during all weeks. 5. BANA test scores was significantly reduced during all weeks in chlorhexidine rinse group(P<0.05), control group was not significantly reduced during all weeks. The result showed that clinical and microbiological effect following scaling, root palning and chlorhexidine on periodontal disease.

Qualitative study on the scaling experience through the application of comprehensive dental hygiene care : A grounded theory approach (포괄치위생관리 과정을 적용한 스케일링 수행 경험에 관한 질적 연구 : 근거이론적 접근)

  • Objectives: This study was performed in order to provide evidence-based data for the expected professional impact of dental hygienists, and to apply and disclose the comprehensive dental hygiene care process through an in-depth analysis of their scaling experience and investigation of the importance of an evidence-based scaling work performance. Methods: The data were collected from June 3, 2019 to October 3, 2019 by conducting in-depth individual interviews on 10 dental hygienists who are working in dental clinics and hospitals by region. The data were analyzed by using the grounded theory methodology, which is a field of qualitative research method. Results: Study results showed that the core category derived from the paradigm model and change process in this study was 'a process of becoming a mature professional outside practical work'. Conclusions: In this study, the participants were able to gain a sense of occupational accomplishment as dental hygienists by performing scaling based on the comprehensive dental hygiene care (CDHC) process, and to advance into professionals through continuous efforts and research in order to enhance their job competencies.

The Clinical and Microbiological Study of the Effect of Minocycline Strip Locally Administrated on Adult Periodontitis (성인성 치주질환에서 미노클린 국소송달제 효과에 대한 연구)

  • 20 Subjects with clinical diagnosis of adult periodontitis with sites having average pocket depth of 4mm were selected for the study. After scaling and root planing of those sites, 30% minocycline-HCI contained in biodegradable device were inserted in one group, scaling and root planing was carried out in another group and the last group was without any periodontal treatment. the difference between the group was detemined by pocket dept, bleeding on prbing, attachment level, distribution of subgingval plaque bacteria. Conclusion was made for the comparisons between baseline and 4 week and the groups. 1. In analysis of pocket depth, there was significant sifference in scaling and root planing group only. 2. Both the scaling and root planing group and minocycline group showed significant decrease in bleeding on probing. 3. There was significant increase in the attachment level in scaling and root planing group, but no difference was foundbetween the groups. 4. There was significant decrease in the total number of subgingival bacteria in all groups and the number of motile bacteria decreased significantly in the minocycline group. These result indicate that insertion of Minocycline-HCI at the base of periodntal pocket was useful as an additional aid of mechanical trarment at the point of periodontal pocket, bleeding on probing, attachment level, microbial distribution.

Clinical and Microbiological Study about Efficacy of Air-polishing and Scaling and Root-planing

  • The efficacy of air-polishing on subgingival debridement, as compared to scaling and root planning (SRP), was evaluated clinically and microbiologically. Fifteen patients diagnosed as chronic periodontitis, and having single-root tooth over 5 mm of pocket depth symmetrically in the left and right quadrant, were investigated. Subgingival debridement was performed by SRP and air-polishing. The results were evaluated and compared clinically and microbiologically. Probing pocket depth (PPD), bleeding on probing (BOP), relative attachment level (RAL) and change of gingival crevicular fluid (GCF) were assessed before treatment, and at 14 and 60 days after treatment. Microbial analysis was done pre-treatment, post-treatment, and at 14 and 60 days after treatment. Results of air polishing showed that post treatment, the PPD and BOP decreased, and attachment gain was observed. There was no clinical difference when compared to SRP. The volume of GCF decreased at 14 days, and increased again at 60 days. Compared to SRP, there was a statistical significance of the volume of GCF at 60 days in air-polishing. In the microbial analysis, high-risk bacteria that cause periodontal disease were remarkably reduced. They decreased immediately after treatment, but increased again with the passage of time. Thus, our results show that subgingival debridement by air-polishing was effective for decrease of pocket depth, attachment gain, decrease of GCF and inhibition of pathogens. Further studies are required to compare air-polishing and SRP, considering factors such as degree of pocket depth and calculus existence.

Musculoskeletal pain and discomfort of dental hygiene students during scaling (일부 치위생학과 학생들의 스케일링 실습 과정에서의 근골격계 통증과 불편감)

  • The purpose of this study was to investigate the association between wrong postures and pain during scaling and encourage dental hygienists and students to exercise scaling in a good position. After obtaining informed consent, 107 students (3rd and 4th grade students) who had an experience with scaling practice were enrolled. The questionnaire included three general items, four items related to the posture during scaling, and nine items related to pain management (total 16 items), for which the five-point Likert scale was used. Through the questionnaire, we examined the preference of posture during scaling, posture education during scaling, pain in each part during scaling, pain management, and pain management method. In the scaling exercise, 86.3% of the subjects were instructed on the correct posture, and 87.9% of the subjects perceived the possibility of inducing musculoskeletal disorders based on the scaling posture. The percentage of subjects who responded that they performed scaling in the correct posture was 33.6% and that of subjects who answered that they bowed or turned their head by more than 15° was 64.4%. Further, 45.7% of the subjects answered that they bent their shoulders, and 29.9% of the subjects answered that their postures were not parallel to the floor. Pain during scaling was still higher when they bent their head, they bent their waist, and they bent their wrist (p<0.05). During scaling, pain was most frequent in the fingers and hands (15%), followed by the neck (14%), shoulders (11.2%), waist (9.3%), and feet and legs (2.8%). The percentage of subjects who performed regular exercise (or stretching) to prevent pain was 29.9% and that of subjects who managed pain after scaling was 12.1%. Further, exercise (24.6%) and self-massage (20.3%) were highly used as the pain management methods, and the school practice was preferred to education media for pain management (79.4%). In the scaling practice, there was a training on pain management, but the frequency of practicing in the wrong posture was high. Moreover, pain increased upon practicing in an incorrect posture. Therefore, more in-depth and systematic education on the necessity and method of musculoskeletal disease management during scaling is required.

  • 양안시차를 이용한 입체 디스플레이는 입체감과 편안함이 상충하는 특징이 있다. 본 논문에서는 이를 해결하기 위해 영상 특성에 따른 길이 조절을 통해 인지되는 입체감을 최대한 떨어뜨리지 않으면서 불편함을 최소화하는 방법을 연구하였다. 인간이 입체를 인지하는 기저는 상당히 복잡하며 영상의 여러 형태에 따라 동일한 깊이(양안시차, disparity)를 주는데도 입체감과 편안함이 다르게 인지된다. 따라서 영상 형태에 따라 다르게 인지되는 영향 인자를 선정하고 이에 대한 깊이감과 편안함의 관계를 실험을 통해 분석하여 적합한 값을 얻었다. 이를 표시되는 입체 디스플레이에 적용하여 들어오는 영상을 처리하여 영향 인자의 정도를 구하고 이에 따라 깊이가 조절(Depth Scaling)되도록 하여 편안한 입체 영상을 구현하였다.

  • The development of image technologies in such area as broadcasting and movies has recently increased our attention to 3D stereoscopic images. In addition, the development of stereoscopic image representation technologies in 3D contents becomes more active over time due to the representational limitations of 2D images. Without limitation to the above-mentioned area, stereoscopic image technologies have been developed and studied so that they can be widely accessed in diverse areas including medical services and education. Due to the refined production, however, required to represent a three dimensional effects and the fatigue caused by the perception of a three dimensional effects, the stereoscopic image technologies are not combined into real time systems such as games where environments change unforeseeably. In this study we design a technique to adjust the depth scaling that will enable efficient management of a three dimensional effects and to relieve fatigue through automatic view point interval adjustment in accordance with situations based on the geometrical structure of the DirectX SDK graphic pipeline. Through this, we would like to suggest a new alternative idea to activate the production of games combined with stereoscopic image technologies.

Clinical and microbiologic effects of the subantimicrobial dose of doxycycline on the chronic periodontitis (저용량 독시싸이클린 투여가 만성 치주염에 미치는 임상적 미생물학적 효과)

  • Purpose: Tetracycline and its chemically modified non-antibacterial analogues can inhibit certain host-derived tissue destructive collagenases such as matrix metalloproteinases. The purpose of this study was to evaluate clinical and microbiologic effects of the subantimicrobial dose of doxycycline(SDD) in conjunction with scaling and root planing. Materials and methods: A total of 30 patients with chronic periodontitis who were going to receive scaling and root planing were randomly allocated to receive either a doxycycline hyclate for 3 months or nothing. Clinical probing depth, clinical attachment level, gingival recession, and bleeding on probing were measured by one periodontist. After a periodontal examination, microbial samples were collected using sterile paper points. The effect of SDD in conjunction with scaling and root planing on alterations of the periodontal pathogens (Aggregatibacter actinomycetemcomitans, Tannerella forsythia, Porphyromonas gingivalis) were also assessed using l6S rRNA polymerase chain reaction. Results: During the treatment period, clinical parameters for both treatment group and control group were improved. After 3 months, reductions in probing depth and gains in clinical attachment level were significantly greater for the SDD group than control group. Microbial analysis showed that there was no alteration of the periodontal pathogens and no difference between the groups. Conclusion: This study suggested that the subantimicrobial dose of doxycycline as an adjunct therapy with scaling and root planing might be effective and safe in the management of chronic periodontitis.

A Conceptual Two-Layer Model of Thermohaline Circulation in a Pie-Shaped $\beta$-Plane Basin

  • The three dimensional structure of thermohaline circulation in a D-plane is investigated using a conceptual two layer model and a scaling argument. In this simple model, the water mass formation region is excluded. The upper layer represents the oceans above the main thermocline. The lower layer represents the deep ocean below the thermocline and is much thicker than the upper layer. In each layer, geostrophy and the linear vorticity balance are assumed. The cross interfacial velocity that compensates for the deep water mass formation balances downward heat diffusion from the top. From the above relations, we can determine the thickness of the upper layer, which is the same as thermocline depth. The results we get is basically the same as that we get for an f-plane ocean or the classical thermocline theory. Mass budget using the velocity scales from the scaling argument shows that western boundary and interior transports are much larger than the net meridional transport. Therefore in the thermohaline circulation, horizontal circulation is much stronger than the vertical circulation occuring on a meridional plane.
