주요우울장애에서의 전기경련요법 (Electroconvulsive Therapy in Major Depressive Disorder)

  • Major depressive disorder(MDD) is one of the most common diseases with serious health consequences such as increased morbidity, disability, and mortality. Electroconvulsive therapy(ECT) has been used as a treatment for mental disorder since the 1930s. A growing number of recent publications support the conclusions that ECT is an effective and safe treatment for depressed patients. Dosing strategies, frequency, safety, side effects and efficacy of ECT in MDD will be considered. ECT may be an alternative to treatment with antidepressants.

초기 성인기 우울증에 대한 유전적, 환경적 요인의 영향 (Effects of Genetic and Environmental Factors on the Depression in Early Adulthood)

  • 우울 장애는 유전적 요인과 함께 환경적 요인이 복합적으로 작용하는 정신 장애이다. 본 연구에서는 세로토닌 체계와 연관된 유전적 요인과 생활사건과 같은 환경적 요인이 초기 성인기 우울 장애 발현에 미치는 영향을 조사함으로써 우울 장애의 원인론에 있어 유전자${\times}$환경 상호작용을 설명할 수 있는 자료를 제시하고자 하였다. 534명의 대학 신입생을 종적으로 추적 조사하여 생활사건 빈도와 중요도, 우울 척도와 불안 척도를 조사하였으며 전화 면담을 통해 DSM-IV 우울 장애 여부를 확인하였다. 최종적으로 150명이 연구에 포함되었으며 이전 연구로 확인된 TPH1 유전형과 함께 보관되어 있던 대상군의 DNA를 이용하여 SNaPshot$^{TM}$ 방식으로 TPH2, 5HTR2A 유전자를 추가 분석하였다. 유전자 정보와 생활사건 특성이 우울 증상에 미치는 영향을 확인하기 위해 로지스틱 회귀 분석과 상관 분석, 카이 자승 분석을 사용하였다. TPH1 유전형 중 C 대립 유전자가 존재하지 않는 집단과 달리 C 대립 유전자가 존재하는 집단에서는 생활사건 빈도가 우울 장애 유발에 유의한 영향을 미쳤다. 이러한 영향은 다른 대립 유전자나 유전형을 보이는 집단에서는 관찰되지 않았다. 본 연구의 결과는 TPH1 유전형은 생활사건 이후 우울 장애 발생의 유의미한 예측 요인임을 시사한다. 이는 우울 장애의 유전${\times}$환경 상호작용에 TPH1 유전자가 작용하고 있음을 제시한다.

우울증 노인을 위한 현실치료 프로그램의 효과 (The Effects of a Reality Therapy Program for the Elderly with Depressive Disorder)

  • Purpose: The purpose of this study is to develop a Reality Therapy program for the elderly with depressive disorder, and then examine the effects of the developed program. Methods: This study used 'non-equivalence control group pretest-posttest design'. The participants were forty-eight elderly who were on medication treatment with depressive disorder. Twenty-two participants were assigned to the experimental group and twenty-six ones were assigned to the control group. A 10-session Reality Therapy program was provided to the experimental group. The data were collected from September to December in 2012. Dependent variables were self-esteem, internal control, and depression. Results: The scores of self-esteem for the experimental group were significantly increased compared to the control group (t=-4.87, p<.001). The scores of depression for the experimental group were significantly decreased compared to the control group (t=3.61, p=.001). However, there was no significant change on the scores of internal control between the two groups. Conclusion: The results of this study indicate that the Reality Therapy program is effective in increasing self-esteem and decreasing depression for the elderly with depressive disorder.

주요우울장애 환자의 증상 심각도에 따른 신경인지적 특성 (Neurocognitive Characteristics According to Depression Severity in Patients with Major Depressive Disorder)

  • Objectives This study investigated associations between symptom severity and neurocognitive functions in patients with major depressive disorder using comprehensive neuropsychological tests. Methods Ten patients with low depression (LD) and 22 patients with high depression (HD) participated. Symptom severity was determined by both the Beck Depression Inventory and the Hamilton Depression Rating Scale. Additionally, anxiety was measured by the Korean version of the State-Trait Anxiety Inventory (STAI-KYZ). Ten subtests of the Korean-Wechsler Adult Intelligence scale (K-WAIS), Stroop test, the word/design fluency tests were administered to assess cognition. Results The LD and HD groups did not differ in any of cognitive measures but anxiety level. Namely, the HD group obtained significantly higher scores on the state (U = 29.50, p < 0.01) and trait (U = 28.50, p < 0.001) anxiety scales than the LD group. A significant interaction effect between trait anxiety and depression severity was observed on the picture arrangement subtest of the K-WAIS [F (1, 27) = 5.09, p < 0.05]. Conclusions Cognitive deficits observed in patients with major depressive disorder may be related to other factors rather than current symptom severity. Trait anxiety possibly moderates the effect of current symptom severity on social judgment in these patients.

  • Object : This study was designed to assess the change of heart rate variability (HRV) during stimulation test among the patients with major depressive disorder. Methods : 15 patients with major depressive disorder (MDD) and 15 normal controls were enrolled in this study. We sequentially measured HRV at baseline, during cognitive stimuli and emotional stimuli. Results : There are significant differences between the two groups in HRV index, TINN on baseline state and under cognitive stimulus. Conclusion : Stimulation protocol using HRV can be useful in estimating autonomic nervous function.

노년기 우울증 환자의 수면의 질 저하가 삶의 질에 미치는 영향 (Poor Sleep Quality and Its Effect on Quality of Life in the Elderly with Late Life Depression)

  • Objectives More than half of the elders suffer from chronic sleep disturbances. Moreover, sleep disturbances are more prevalent in patients with depressive disorder than in community dwelling elderly. In this study, we aim to estimate the risk factors of poor sleep quality and its effect on quality of life in patients with late life depression. Methods This study included 159 depressive patients aged 65 years or older who completed Pittsburgh Sleep Quality Index (PSQI). A global PSQI score of 5 or greater indicates a poor sleeper. Structural diagnostic interviews were performed using the Korean version of Mini International Neuropsychiatric Interview (MINI). Depression was evaluated by the Korean form of Geriatric Depression Scale (KGDS). Global cognition was assessed by Mini-Mental State Examination in the Korean version of the Consortium to Establish a Registry for Alzheimer's Disease assessment packet. Quality of life was evaluated by the Korean version of Short-Form 36-Item Health Survey (SF-36). Results The frequencies of poor sleepers were 90.5% in major depressive disorder, 71.8% in minor depressive disorder, 47.1% in subsyndromal depressive disorder, and 73.0% in all types of depressive disorders. Multivariate logistic regression analysis indicated that female [odds ratio (OR) = 2.83, 95% confidence interval (CI) = 1.20-6.67] and higher KGDS score (OR = 1.13, 95% CI = 1.05-1.21) were risk factors of poor sleep quality in patients with late life depression. In the analysis of ANCOVA adjusted for age, gender, education and KGDS score, the mean scores of vitality mental health and mental component summary of SF-36 were lower in poor sleepers than in good sleepers. Conclusions Poor sleepers among patients with late life depression are very common and are associated with female and higher KGDS scores. Poor sleep quality causes a significant negative effect on mental health quality. So researchers and clinicians should be more vigilant in the evaluation and treatment of sleep disturbances in patients with late life depression.

우울장애 한약치료에 대한 전향적 관찰연구 (A Prospective Observational Study of Herbal Medicines on Depressive Disorder)

  • Objectives: The purpose of this study was to verify the clinical effects of herbal medicines on patients with depressive disorder. Methods: A prospective observational study on patients with depressive disorder was conducted. The patients were treated with herbal medicines for eight weeks. The Korean Hamilton Depression Rating Scale (K-HDRS) was evaluated as the primary outcome and the Korean Beck's Depression Inventory (K-BDI), Instrument on Pattern Identification for Depression, Korean Symptom Checklist-95 (KSCL-95), Insomnia Severity Index (ISI), Korean State-Trait Anxiety Inventory (STAI-K), Korean State-Trait Anger Expression Inventory (STAXI-K), EuroQol five-dimensional Questionnaire (EQ-5D), Minnesota Multiphasic Personality Inventory-2 (MMPI-2), Heart Rate Variability (HRV) were measured as the secondary outcome. Adverse events were assessed for safety. Results: A total of 35 participants were observed for 12 weeks. The K-HDRS improved significantly in four, eight, and 12 weeks and the K-BDI showed significant differences in four, and eight weeks. There were no significant improvements in other evaluation scales. Gyejibongnyeong-hwan was the most frequently and continuously prescribed, showing significant improvements of K-HDRS after eight weeks of treatment. Conclusions: In our observational study, herbal medicine was effective for relieving the depression of patients with depressive disorder and Gyejibongnyeong-hwan showed the most significant effectiveness.

제2형 양극성 장애의 주요우울 삽화에 대한 복합 한방 치료 사례보고: 대인관계 및 사회적 리듬 치료와 변증 치료를 중심으로 (A Case Study on a Patient with Major Depressive Episode of Bipolar II Disorder Treated with a Combination of Interpersonal and Social Rhythm Therapy and Korean Medicine)

  • 본 논문은 제2형 양극성 장애 환자에게 한의학적 변증체계와 함께 IPSRT의 심리 치료 기법을 적용한 증례이다. 제2형 양극성 장애 환자에게 한의치료와 IPSRT를 4개월간 병용한 결과, 환자의 사회적 리듬, 우울증 및 대인관계 능력이 개선되었으며 4개월후 추적조사에서도 호전상태가 유지되었다. 향후 양극성 장애 환자에 대한 높은 근거수준의 한의 임상연구 수행과 한의표준임상진료지침의 개발 등을 통해 양극성장애에 한의치료가 보다 활발히 활용되기를 기대한다.

  • 소아 ${\cdot}$ 청소년기에 나타나는 우울장애는 복잡한 임상양상을 나타낸다. 이 장애의 역학, 임상 양상, 원인 등에 대하여 더욱 많은 논문들이 발표가 되고 이재가 되고 있으나, 치료에 대한것은 아직 잘 확립 되지 않았다 많은 수의 논문들에서 소아 ${\cdot}$ 청소년기의 우울장애는 성 인과는 다르게 치료를 해야 한다는 발표를 하고 있다. 이 논문에서는 이 질환에 대하여 가능한 다양한 치료 방법에 대하여 검토를 하였으며, 임상가들을 위하여 체계적인 접근방식을 제공하였다. 또한 우울증의 치료지침을 제시하였다. 결론적으로 소아 ${\cdot}$ 청소년기의 우울장애의 치료를 성공적으로 하기 위하여는 포괄적이고 전체적인 접근 방식을 필요로 한다. 또한 우울증을 가진 개개인의 아동과 그의 가족들이 필요로 하는 치료방식이 서로 다르기 때문에 임상가는 가능하면 여러가지의 다양한 치료방식에 익숙해야 한다.

경도인지장애에서 우울증 유병률과 관련요인 (Prevalence and Associated Factors of Depression in Mild Cognitive Impairment)

  • Objective : The aims of this study were to investigate the prevalence of co-morbid depression and related factors of depression in mild cognitive impairment (MCI) patients. Methods : Nine hundred and six MCI individuals were included in this study. Depression was defined as major and minor depressive disorder according to the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV). Depression-related demographic and clinical factors were also explored through multivariate logistic regression analysis. Results : In MCI patients, the prevalence of depression as defined according to the DSM-IV diagnostic criteria was 1.8% for major depressive disorder, 11.4% for minor depressive disorder, and overall 13.2% for both. Multivariate logistic regression showed that increased prevalence of overall depression was associated with female gender and non-amnestic subtype of MCI. Conclusion : Our findings based on a large number of MCI subjects who visited memory clinic indicated that more than ten percent of MCI patients have comorbid depression. In addition, female and non-amnestic subtype of MCI patients seems to be more vulnerable to depression.
