The relationship between dental prosthesis status, prosthesis need and mastication function in the Korean elderly population - The 5th Korea National Health and Nutrition Examination Survey(2010-2012)- (한국 노인의 치과보철물 상태 및 필요도와 저작기능의 관련성 연구 -제5기 국민건강영양조사자료 분석(2010-2012)-)

  • Objectives : The objective of this study was to investigate the relationship between the status and necessity of dental prosthesis and mastication function in the Korean elderly. Methods : This study investigated 4,557 elderly people aged over 65 in the 5th Korea National Health and Nutrition Examination Survey(2010-2012). Frequency analysis, ${\chi}^2$ test, and logistic regression analysis were used. Results : Those aged over 75 complained of greater discomfort in mastication function than those from 65 to 74, and it was statistically significant(p<0.001). Mastication discomfort was not so significant in those with higher education and higher income(p<0.001). Those who did not use prosthesis tended to have better mastication function(p<0.001). Denture users complained of mastication discomfort when they used it(p<0.001). Conclusions : This study verified the relationship between the discomfort in mastication and related factors, and will provide the basic data for the access to dental clinic services.

Rehabilitation using short implants in the mandibular fully edentulous patient with severe alveolar bone loss: a case report (치조골 흡수가 심한 하악 완전 무치악 환자에서 짧은 임플란트를 이용한 고정성 보철 수복 증례)

  • A short dental implant is considered as possible solution in difficult clinical situations for the placement of a regular length implant. Using a short implant avoiding more invasive surgical procedures simplifies the treatment plan and shortenes the duration of treatment. In this case, 71-year-old female came up with discomfort from her old mandibular denture. As she had fully edentulous mandible and got a negative feedback from removable denture, implant-supported fixed prosthesis was planned. Six short implants were placed on her mandible with severe alveolar bone loss. After 1 year follow up period, implants were well retained with any other abnormal findings. The patient was satisfied with her prostheses and satisfactory outcomes were attained in terms of both esthetic and functional clinical results.

  • Occlusal disharmony is frequently observed among edentulous patients. When artificial teeth come into contact, the unfavorable displacing force may lead to the discomfort, mucosal trauma, or even neuromuscular alterations and emotional disturbances. An optimal occlusal scheme is a critical factor for successful complete dentures. For this case, an edentulous patient with significant interarch size discrepancy due to mandibular prognathism contributing to inadequate function of dentures was treated with complete dentures. The posterior cross-bite tooth setup for compensating the abnormal jaw relations provided a stable and retentive complete denture prosthesis, which was considered adequate by both patient and dentist.

  • Severely absorbed edentulous ridge cannot bear mechanical stress, causes undesired transformation of oral environment and makes patients difficult to adapt to dentures. Nowadays implant overdenture can be a treatment of choice in order to relieve patients' discomfort and improve stability and retention of the denture. Placement of implant on maxilla is difficult because of its bone quality and anatomic structure. It also has wide supportive tissue and convenience of border sealing, which provides sufficient support and stabilization with conventional complete denture. Mandible, on the other hand, is difficult to obtain sufficient support, retention and stabilization with conventional complete denture. Therefore, implant overdenture is recommended on mandible. Locator attachment has been improved for convenience of use and male parts of various retention enabled it to replace ball type attachment clinically. In this study, we restored maxillary arch with conventional denture, and mandibular arch with implant and tissue-supported overdenture and Locator attachment system.

The Effects of National Health Insurance Denture Coverage Policies for the Elderly on the Unmet Dental Needs of the Edentulous Elderly

  • As the elderly population increases, they are increasingly affected by oral health problems. Therefore, efforts are being made to improve the oral health of older people, alleviate mental discomfort, and reduce unmet dental needs. This study was conducted to confirm the relationship between the National Health Insurance Elderly Denture Coverage and the unmet dental need for the edentulous elderly, as part of the protection policy. We analyzed the 2011 and 2013 Community Health Survey data of the edentulous elderly, aged 75 years or older, before 2012. In order to more precisely confirm the effects of the denture donation policy on unmet dental care, basic life recipients who were subject to the free elderly prosthetic project were excluded from the analysis. The final analysis included 20,400 subjects. According to our investigation of the factors that affect the unmet dental needs of the elderly, the National Health Insurance Elderly Denture Coverage did not affect unmet dental needs. The statistically significant variables that affected the unmet dental needs of the elderly were education and income levels, which are representative socioeconomic status variables. The lower the level of education, the unhealthier the dental care experience, and income levels showed a similar tendency. The elderly who have a low socioeconomic status are more likely to experience unmet dental needs because they lack the knowledge and socioeconomic ability to pay for dental care. Therefore, the policy for health protection of the entire elderly population should be continuously expanded. In addition, the socioeconomically vulnerable groups may have health problems due to the restriction of medical use, which may lead to quality of life deterioration.

The relationship of snack habits, oral health behavior and oral health status in psychiatric patients (일부 정신병원 입원환자의 구강건강상태와 구강건강행위 및 간식습관과의 관련성)

  • Objectives : The purpose of the study is to investigate the relationship between variables associated with oral health status in psychiatric patients and to provide basic information on oral health education program development for the psychiatric patients. Methods : Subjects were 151 psychiatric patients who filled out the structured questionnaire. The data were analyzed for independent t-test, one-way ANOVA and hierarchical multiple regression by using SPSS Win Program 18.0 version. Results : Higher DMFT index was shown in the older age(p<0.001) and low perceived oral health group(p<0.001). Higher DMFT index included no tooth brushing after breakfast(p=0.045), wrong tooth brushing method(p<0.001). Chewing discomfort(p=0.027) and denture(p=0.027) were closely related to higher DMFT index. Bread(p=0.033) and snack(p<0.001) increased DMFT index. Factors affecting oral health status were age, denture, snack and bread. The explanation power of the final model was 29.4%. Conclusions : Dietary control for the psychiatric patients is very important to prevent dental caries. Tooth brushing and use of oral hygiene devices can improve good oral health care. Furthermore, this result can improve the oral health care for the disabled people.

Implant-assisted full denture using digital guide: a case report (무치악 환자에서 디지털 가이드를 이용한 임플란트 수복증례)

  • By classifying temporary denture production for surgical guides, digital guide-based surgery, and final prosthesis production, the problems of each process were assessed in advance and the factors that could be improved were confirmed in this study. The manufacturing process of fusion dental prosthesis uses virtual programs and computed tomography images to manufacture devices using the latest technologies of computer-aided design/computer-aided manufacturing and three-dimensional printing, which enables implants to be placed in the desired location in advance. Moreover, implant placement is not dependent on the skill and condition of the dentist, and because it uses a computer system, it can always be performed at a constant and optimal position. This can reduce the remanufacturing rate compared with the general method, shorten the treatment period, and eliminate patient discomfort. Unlike the traditional method of using impression materials and plaster models, digital fusion dental prostheses would be evaluated as a technology for producing prosthesis through professional design technology and communication.

Implant-assisted removable partial denture using digital guide surgery in partially edentulous mandible: A case report (하악 부분 무치악 환자에서 디지털 가이드 수술을 이용한 임플란트 융합 가철성 국소의치 수복 증례)

  • Fixed implant prostheses or removable partial dentures are common treatment for partially dentulous patients. Recently, an implant-assisted removable partial denture (IARPD) has been introduced and is widely used. In the case of removable partial dentures using implant attachment and surveyed fixed prostheses, the consideration about parallelism of insertion path between implants and surveyed fixed prostheses is an important factor for success of IARPDs. In this case, she complained of discomfort while using the mandibular removable partial denture. Thus, an IARPD using implant surveyed fixed prostheses was fabricated. Thereafter, more implants were placed by a digital guide surgery to have the same insertion path as the existing surveyed fixed prostheses. Locator attachments were installed to the left and right premolar implants, and the left molar was able to obtain support of the removable partial denture with healing abutment. The clinical results were satisfactory on the aspect of aesthetic and masticatory function.

  • This report describes two cases of complete arch implant-supported restorations. The first patient had seven dental implants in each arch with monolithic zirconia frameworks. At four weeks' follow-up, the one-piece maxillary framework was fractured, which was re-designed and re-fabricated using laser-sintered cobalt-chrome alloy. The second patient had four implants in the mandible only. A mandibular monolithic zirconia framework and a maxillary conventional complete denture were fabricated and delivered. At five years' follow-up, the patient reported no significant discomfort. Careful consideration and monitoring of the status of antagonistic arches and stress distribution on zirconia frameworks were suggested for complete arch implant-supported fixed restorations.

  • In a case of class III skeletal patients with severe alveolar bone resorption, it must be a complete denture treatment plan provided stable and durable occlusion. Despite a markedly increased tooth mobility and unfavorable crown-to-root ration due to periodontal tissue breakdown, if the inflammatory process is controlled and an adequate oral hygiene performed, fixed splints will be considered. Patient's discomfort will be eliminated by achievement better clinical tooth mobility using fixed splints. So it can be a cost and time effective treatment option. In this case, it used a T-Scan System for confirmation a bilateral balanced occlusion and a occlusal force distribution reflected a patient's functional mandibular movement.
