Maxillary overdenture restoration using functionally generated path technique: a case report (Functionally generated path technique을 이용한 편악 피개의치 수복 증례)

  • The functionally generated path (FGP) technique, first described by Meyer in 1933, is a method in the oral cavity to use the movement pathway formed by the opposing cusps within the border movement of the mandible. Using this method, an appropriate occlusal shape can be given to the patient. In this case, the FGP technique was selected to provide a bilateral balanced occlusion when restoring the edentulous maxilla that opposes the natural mandibular teeth with irregular arrangement with overdentures. In addition, in order to precisely form the occlusal surface of the posterior region with the FGP technique and to reduce the attrition of denture teeth, zirconia denture teeth, not conventional resin artificial teeth, were individually manufactured. After treatment with these materials and methods, satisfactory results were obtained for both the operator and the patient.

  • Stable posterior occlusal support is crucial for adequate masticatory function and facial aesthetics. In elderly patients over the age of 65, masticatory ability has a significant impact on nutritional intake and overall health. This case report presents a prosthetic treatment of an elderly patient with edentulous maxilla and bilateral posterior edentulous mandible. The upper jaw was restored with a complete denture to establish an ideal occlusal plane, and the three-dimensional positions of the mandibular implants were determined accordingly. The implants were placed through computer-guided implant surgery and were immediately loaded with fixed provisional restorations. The implant-fixed zirconia final restorations were inserted in the lower posterior region, and the occlusal surface of posterior artificial teeth in the upper denture was substituted with cobalt-chrome alloy to resist occlusal wear. The patient's posterior occlusal support and masticatory function were promptly restored through this treatment process, and long-term stability against wear was also obtained.

Effect of universal primer on shear bond strength between resin cement and restorative materials (다용도 프라이머가 레진 시멘트와 수복재의 전단 결합 강도에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate the difference in shear bonding strength between resin cements to dental materials when a universal primer (Monobond plus) was applied in place of a conventional primer. Materials and methods: Four groups of testing materials: gold alloy (Argedent Euro, n = 16), non precious metal (T-4, n = 20), zirconia (Cercon, n = 20) and glass ceramic (IPS e.max press, n = 20), were fabricated into discs, which were embedded in an acrylic resin matrix. The gold alloy specimens were airborne-particle abraded, 8 of the specimens were coated with Metal primer II, while the remaining 8 specimens were coated with Monobond plus. The non precious and zirconia specimen were airborne-particle abraded then, the control group received Alloy primer coating, while the other was coated with Monobond plus. Glass ceramic specimens were etched. 10 specimens were coated with Monobond-S and the remaining specimens were coated using Monobond plus. On top of the surface, Multilink N was polymerized in a disc shape. All of the specimens were thermal cycled before the shear bonding strength was measured. Statistical analysis was done with Two sample $t$-test or Mann-Whitney U test (${\alpha}$=.05). Results: There were no significant differences in bonding strength depending on the type of primer used in the gold alloy and glass ceramic groups ($P$>.05), however, the bonding strengths of resin cements to non precious metal and zirconia groups, were significantly higher when the alloy primer was used ($P$<.05). Conclusion: Within the limitations of this study, improvement of universal primers which can be applied to all types of restorations is recommended to precious metals and zirconia ceramics. But, the bond strengths of non precious metals and zirconia ceramics were significantly lower when compared to a 10-MDP primer. More research is needed to apply universal primers to all types of restorations.

The Effect of Surface Treatment on the Shear Bond Strength of Resin Cement to Zirconia Ceramics (표면처리가 지르코니아와 레진 시멘트의 전단결합강도에 미치는 효과)

  • The aim of this study was to investigate the shear bond strength between zirconia ceramic and resin cement according to various surface treatments. The surface of each zirconia ceramic was subjected to one of the following treatments and then bonded Rely X Unicem or Rely X ARC resin cement; (1) Rocatec system and $50{\mu}m$ surface polishing, (2) No treatment and $50{\mu}m$ surface polishing, (3) Rocatec system and $1{\mu}m$ surface polishing, (4) No treatment and $1{\mu}m$ surface polishing. Each of eight bonding group was tested in shear bond strengths by universal testing machine(Z020, Zwick, Ulm, Germany) with crosshead speed of 1mm/min. The results were as follows; 1. Rocatec treatment groups showed greater bonding strengths than No Rocatec groups. There was significant difference of among groups(P<0.001) 2. For Rocatec groups, $50{\mu}m$ surface roughness groups showed greater bonding strengths than $1{\mu}m$ surface roughness groups.(P<0.001) But for No Rocatec groups, There was no significant difference of among groups(P>0.05) 3. Rely X Unicem groups showed greater bonding strengths than Rely X ARC groups. There was significant difference of among groups(P<0.01) Within the conditions of this study, Rocatec treatment was an effective way of increasing zirconia bonds to a resin cement, even in the case of self-adhesive resin cement.

Effect of Glass Frit on the Sintering Behavior and Mechanical Properties of 3Y-ZrO2 (3Y-지르코니아 조성에서 Glass Frit의 첨가량에 따른 소결 거동 및 기계적 특성의 변화)

  • The effect of glass frit on the sintering behavior and mechanical properties of 3 mol% $Y_2O_3$-doped zirconia($3Y-ZrO_2$) have been studied. Up to 30 wt% of glass frit was added to $3Y-ZrO_2$. Sintering was performed in a box furnace up to $1300^{\circ}C$ for specimens with glass frit and $1600^{\circ}C$ for specimens without glass frit in air for 1h. Relative density and mechanical properties were measured to investigate the effect of glass frit. The addition of glass frit enhanced both sintered density and mechanical properties of $3Y-ZrO_2$ which is suitable for dental applications. Maximum sintered density 93.3% of theoretical density was obtained with the specimens containing 30 wt% frit sintered at $1300^{\circ}C$, whereas the optimum amount of frit addition for mechanical properties was determined as 10 wt%. Maximum value of strength, fracture toughness, and Vickers microhardness for specimens with glass frit were 206 MPa, $3.4\;MPa\;m^{1/2}$, and 5.3 GPa, respectively.

  • Atypical teratoid rhabdoidal tumor(AT/RT) is a rare and fast-growing tumor usually diagnosed in childhood. Routine magnetic resonance imaging(MRI) assessment of AT/RT patients is done to detect metastatic tumors and recurrent lesions. The purpose of this case report is to present a case about caries treatment of a 3-year-8-month old female patient with AT/RT under general anesthesia. A 3-year-8-month old patient with AT/RT visited Seoul National University Dental Hospital for caries treatment. At the first dental visit, she was receiving chemotherapy. Multiple caries were observed from clinical and radiographic examination. The dental procedure was successfully performed under general anesthesia. Patients with brain tumor may take routine MRI to detect metastatic tumors and recurrent lesions. Dentists should take into consideration of possible dislodgement of restoration by ferromagnetic properties and artifact formation on MRI image. Ready-made zirconia crowns can be a good restoration option for such patients.

Prosthetic treatment for Down's syndrome patient with dental cross bite problem using maxillary double crown denture (교차교합을 가진 다운증후군 환자의 상악 이중관 의치를 이용한 수복 증례)

  • Patients with Down's syndrome have several dental complications such as small teeth caused by underdevelopment of dentin and enamel, periodontitis, agenesis of teeth, prolonged retention of primary teeth and malocclusion due to narrow palate. Removable denture with maxillary double crowns would be a good treatment option to solve the problems of the patient with Down's syndrome. Double crowns compensate the insufficient support and retention of denture and easily solve the cross bite problem. Double crowns also allow easy repair of denture in case of abutment teeth extraction. In this case, 26-year-old female patient with Down's syndrome and dental phobia had small number of teeth with enamel hypoplasia, prolonged retention of primary teeth and dental cross bite. Prosthetic treatment was done using removable denture with double crowns in the maxilla. In the mandible, teeth preparation was done on enamel margin without anesthesia. Anterior laminate and posterior complete zirconia crown restorations were performed. As a result, the cross bite was effectively corrected by denture with double crowns. Pronunciation and appearance were also improved without extraction of teeth and dental anesthesia.

Mini-implant with additional retentive structure by using digital method (부가적인 유지구조를 가진 미니 임플란트의 디지털 수복 증례)

  • Mini-implant has been reported as an alternative method to conventional diameter implants in the treatment of mandibular anterior region with narrow ridge. However, one-piece type mini-implant showed biological and technical complications, such as peri-implantitis by residual cement and prosthetic detachment. Recently, one-piece type mini-implant with additional retentive structure has been introduced to increase the retention and stability of the mini-implant. The mini-implant is based on spreading the upper retentive structure with a spreader and connected it with the inner surface of the zirconia prosthesis. In two cases, we tried to reduce these complications through the mini-implant on the narrow mandibular anterior region, and it was possible to reduce the overall treatment period through a digital system. In this case report, restoration using the digital mini-implant system showed improvement of patient satisfaction on the aspect of function and esthetic in the narrow mandibular anterior region.

Influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with a lingual slot

  • PURPOSE. Cement-retained implant prostheses can lack proper retrievability during repair, and residual cement can cause peri-implantitis. The purpose of this in vitro study was to evaluate the influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with lingual slots, known as retrievable cement-type slots (RCS). MATERIALS AND METHODS. We fabricated six types of titanium abutments (10 of each type) with two different heights (4 mm and 6 mm), three different convergence angles ($8^{\circ}$, $10^{\circ}$, and $12^{\circ}$), a sloped shoulder margin (0.6 mm depth), a rectangular shape ($6mm{\times}6.5mm$) with rounded edges, and a rectangular ledge ($2mm{\times}1mm$) for the RCS. One monolithic zirconia crown was fabricated for each abutment using a dental computer-aided design/computer-aided manufacturing system. The abutments and crowns were permanently cemented together with dual-curing resin cement, followed by 24 hours in demineralized water at room temperature. Using a custom-made device with a slot driver and torque gauge, we recorded the torque ($N{\cdot}cm$) required to remove the crowns. Statistical analysis was conducted using multiple regression analysis and Mann-Whitney U tests (${\alpha}=.05$). RESULTS. Removal torques significantly decreased as convergence angles increased. Multiple regression analysis showed no significant interaction between the abutment height and the convergence angle (Durbin-Watson ratio: 2.186). CONCLUSION. Within the limitations of this in vitro study, we suggest that the retrievability of cement-retained implant prostheses with RCS can be maintained by adjusting the abutment height and convergence angle, even when they are permanently cemented together.

Maxillary cement retained implant supported fixed prosthesis using the millingable Pd-Ag alloy generated by CAD/CAM system: clinical report after two years in service (상악 전부 무치악 환자에서 CAD/CAM 시스템으로 제작되는 Pd-Ag 합금을 이용한 고정성 임플란트 전악 수복 증례)

  • Full-mouth reconstruction of a patient using dental implants is a challenge if there is vertical and horizontal bone resorption. Therefore, it is should be cautious in making the fixed prostheses that restore the function and the esthetics of the gingiva and teeth. In full mouth rehabilitation, CAD/CAM system makes it possible to fabricate restorations with high precision, regardless of span of the restoration. Recently, Palladium-silver (Pd-Ag) alloy which is highly biocompatible and millingable has been developed to compensate for the shortcomings of the titanium or zirconia. This clinical report presents the reconstruction of a maxillary arch with a cement retained implant supported fixed prosthesis using a Pd-Ag alloy generated by CAD/CAM system on eleven osseointegrated implants. The occluding surfaces were made of Pd-Ag alloy, to decrease the risk of chipping or fracture. The prostheses were esthetically pleasing, and no clinical complications have been reported after two years.
