Oral Manifestation of Unknown Hematopoietic Malignancy

  • Burkitt lymphoma is a highly aggressive type of hematopoietic malignancy that is comparatively common in children and young people. It is important that Burkitt lymphoma be diagnosed as early as possible for prompt intervention due to its rapidly progressive, high-grade malignant nature. Dentists, especially maxillofacial surgeons, can play a life-saving role in patients with such unknown malignancy as the first clinical or radiological manifestation might occur in the oral region.

Simplified nonsurgical treatment of peri-implantitis using chlorhexidine and minocycline hydrochloride

  • Purpose: The present study investigated the outcomes of a newly-developed, simple, and practical nonsurgical treatment modality suitable for most forms of intrabony defects around failing dental implants using intrasulcular delivery of chlorhexidine solution and minocycline hydrochloride (HCl). Methods: Forty-five dental implants in 20 patients diagnosed with peri-implantitis were included. At baseline and the study endpoint, the probing pocket depth (PPD), clinical attachment level (CAL), and the presence of bleeding on probing (BOP) at 6 sites around each implant were recorded. The radiographic osseous defect morphology at the mesial or distal proximal aspect of each implant was classified as 1) narrow or wide and 2) shallow or deep. For a comparative analysis of bone changes according to the defect morphology, the distance from the implant shoulder to the most coronal bone-to-implant contact point (DIB) at the mesial and distal aspects of each implant was measured at baseline and the endpoint. Patients were scheduled to visit the clinic every 2-4 weeks for intrasulcular irrigation of chlorhexidine and delivery of minocycline HCl. Results: We observed statistically significant decreases in PPD, CAL, and BOP after treatment. At the endpoint, bone levels increased in all defects, regardless of the osseous morphology of the intrabony defect. The mean DIB change in deep defects was significantly greater than that in shallow defects. Although the mean bone gain in narrow defects was greater than in wide defects, the difference was not statistically significant. Conclusions: We propose that significant and sustainable improvements in both clinical and radiographic parameters can be expected when intrabony defects around dental implants are managed through a simple nonsurgical approach involving combined intrasulcular chlorhexidine irrigation and local delivery of minocycline HCl.

A Survey of Dentists and Dental Hygienists on Dental Team Composition and Work Assignments (치과의사 및 치과위생사의 치과팀 구성 및 업무 분장에 대한 견해)

  • The purpose of this study was to investigate the status and views on the dental team composition and works, the experience of a dental manpower shortage and opinions on the solution. The subjects were 313 dentists and dental hygienists. The mobile questionnaires were conducted on the status of dental team composition and works, the experience of a dental manpower shortage and opinions on the solution. There was no big difference between the current dental team composition and ideal composition ratio, and dentists answered that it would be more ideal to reduce dental hygiene and increase nurse assistant when the working dentist was 3~5. 85.6% of the dentists and 56.4% of the dental hygienists had difficulty in employing the dental auxiliary personnel. The difficulty level was 8 out of 10 points among dentists and 5.7 points among dental hygienists. 91.1% of the dentists and 95.5% of the dental hygienists agreed on the necessity of expanding the dental hygienist's work. The dental works were temporary crown making and setting, surgical operation assistance, and overall dental assistance. Based on the results of this study, it is necessary to continuously make efforts to communicate with the dental personnel in Korea to solve a lot of problems such as the job assignment, the dental personnel policy and system, and the work environment.

Trend analysis of new health technology in dentistry (치의학 분야의 신의료기술 동향 분석)

  • Objectives : Currently, it is being reported that new health technology (HT) is introduced and spread throughout Korea at a very fast rate. However, the current status of new HT related to dentistry has not been known or studied. Therefore, this study aims to understand the present state and awareness on dentistry-related domestic new HT via surveys and research, and thus apply this as a reference to construct an effective new HT-related system in the field of dentistry. Methods : The assessment status of New Health Technology Assessment (nHTA) committee was searched on the website for any updates on new HT5). The cases were searched in the range between Jan 1 2012 and Jul 31 2016 by submission date and were analyzed by year, division of technology, progress and major operating practitioner. Among those, technologies utilized by dentists were additionally analyzed. The survey result was obtained by combining responses of 25 participants from the workshop for new HT application and 11 members of Korean Association of Oral and Maxillofacial Surgeons (KAOMS). The workshop was co-hosted by KAOMS and Clinical Translational Research Center for Dental Science (CTRC) under Seoul National University Dental Hospital (SNUDH). Results : Most of the respondents to the survey have heard of new health technology but were unaware of the application procedure. 'Safety and effectiveness' was selected the most for things to consider when introducing new HT, followed by 'scientific evidence of the technology.' For new dental HT status, submissions between 2012 and Jul 31 2016 were analyzed by major operating practitioner, including cases where there are multiple practitioners. Yet, only 19 cases (17 %) of dentists were reported. Among 385 cases approved by nHTA, only 2 cases included dentists. Conclusions : Active publicity by the government on new HT and its application procedure, and continued interest and research in the dental and medical community is likely to be required for the development of dentistry.

Evaluation of Clinically Effective Doses of Triamcinolone Acetonide for Intralesional Injection in Oral Lichen Planus

  • Purpose: The aim of this study was to evaluate the optimal doses of intralesional triamcinolone acetonide (TA) in the treatment of oral lichen planus (OLP). Methods: A randomized clinical trial was performed. Sixty-two lesions of OLP were received 12 mg (group A) or 20 mg (group B) of TA intralesionally weekly for 2 weeks. Subjective symptoms, lesion size, favorable conversion of clinical subtypes, and clinical response were evaluated at weeks 0, 1, 2, and 4. Results: After two consecutive injections of TA, group B showed significant reduction in burning sensation and reticular area (p<0.01). Favorable conversion and complete response were greater in group B. Mild oral candidiasis was developed in group B (10.7%). Conclusions: A 20-mg injection of TA was much more effective compared with 12-mg injection of TA in the treatment of OLP.

  • Objectives: The purpose of the study was to analyze papers published in the Journal of Korean Society of Dental Hygiene (JKSDH) and to identify the current state of dental hygiene research and recommend directions for future research. Methods: A total of 315 articles published between 2016 to 2018 were reviewed using analysis criteria. Results: The number of grant research and experimental studies during 2016-2018 was higher than that before 2015. Quantitative studies were dominant and oral health was the most common research topics. The number of published papers, the proportion of reported reliability of instrument studies, reported ethical consideration and studies that described criteria for sample size had increased. The most common sampling of quantitative studies were convenient sampling and questionnaire and big data of data collection methods were the most. Conclusions: Findings of this study indicate that the recent trends in dental hygiene research and the direction of dental hygiene research and will improve the quality of papers and promote the reputation of JKSDH as an international journal.

Propofol protects against lipopolysaccharide-induced inflammatory response in human amnion-derived WISH cells

  • Background: Nonobstetric surgery is sometimes required during pregnancy, and neck abscess or facial bone fracture surgery cannot be postponed in pregnant women. However, dental surgery can be stressful and can cause inflammation, and the inflammatory response is a well-known major cause of preterm labor. Propofol is an intravenous anesthetic commonly used for general anesthesia and sedation. Studies investigating the effect of propofol on human amnion are rare. The current study investigated the effects of propofol on lipopolysaccharide (LPS)-induced inflammatory responses in human amnion-derived WISH cells. Methods: WISH cells were exposed to LPS for 24 h and co-treated with various concentrations of propofol (0.01-1 ㎍/ml). Cell viability was measured using the MTT assay. Nitric oxide (NO) production was analyzed using a microassay based on the Griess reaction. The protein expression of cyclooxygenase-2 (COX-2), prostaglandin E2 (PGE 2), p38, and phospho-p38 was analyzed using western blotting. Results: Propofol did not affect the viability and NO production of WISH cells. Co-treatment with LPS and propofol reduced COX-2 and PGE2 protein expression and inhibited p38 phosphorylation in WISH cells. Conclusion: Propofol does not affect the viability of WISH cells and inhibits LPS-induced expression of inflammatory factors. The inhibitory effect of propofol on inflammatory factor expression is likely mediated by the inhibition of p38 activation.

Effect of non-surgical periodontal therapy with the application of a comprehensive dental hygiene care process for periodontal disease patients: using mixed methods research (포괄치위생관리 과정을 적용한 치주질환자의 비외과적 치주처치 효과 : 혼합연구방법 적용)

  • Objectives: This study aims to conduct in-depth research on the effect of non-surgical periodontal therapy (NSPT) with the application of a comprehensive dental hygiene care (CDHC) process, and provide basic data for the wide application of CDHC. Methods: From May 8, 2021 to September 24, 2021, mixed-methods research was conducted in 36 patients with periodontal diseases. A paired samples t-test was used to analyze the quantitative research data using IBM SPSS program(ver. 22.0; IBM Corp., Armonk, NY, USA) and qualitative research data were analyzed using the thematic analysis method. Results: With NSPT applying the CDHC process, the perception of periodontal health and self-efficacy of periodontal healthcare were increased (p<0.001). Presence of gingivitis, probing pocket depth, bleeding on probing rate, presence of subgingival calculus, and dental plaque index were reduced (p<0.001). Based on 195 meaningful statements, 26 concepts, 12 sub-themes, and 5 themes , , , and were drawn. Conclusions: The perception of periodontal health and the self-efficacy were improved, and substantial change in the clinical index. The CDHC application allowed the study participants to perceive the importance of dental care and professionalism of dental hygienists.
